1.Analyses of the epidemiological characteristics of injury-related deaths among the elderly in Yuyao City of Zhejiang Province from 2013 to 2022

ObjectiveTo analyze the causes and trends of injury-related mortality among elderly residents in Yuyao City, and to provide a scientific basis for formulating prevention and control strategies for injury-related deaths among the elderly. MethodsData on injury-related deaths among elderly residents aged 60 years old and above in Yuyao City from 2013 to 2022 were collected. The epidemiological characteristics were analyzed, including the crude mortality rates, standardized mortality rates, age-specific mortality rates, annual percent change (APC), and average annual percent change (AAPC). ResultsThe crude mortality rate of injury among elderly people aged 60 years old and above in Yuyao City was 263.90/100 000, with a standardized mortality rate of 131.21/100 000. A statistically significant decreasing trend in standardized mortality was observed from 2013 to 2020 (APC=-3.17%, P=0.017). The standardized mortality rates of injury for elderly males and females were 154.74/100 000 and 134.46/100 000, respectively. Among elderly females, the standardized mortality rate of injury initially decreased and then increased (APC2013—2020=-5.14%, APC2021—2022=22.86%, both P<0.05). The top five causes for injury-related deaths among the elderly were accidental falls, traffic injuries from motor vehicles, drowning, accidental asphyxia, and suicide, accounting for 74.78% of all injury-related deaths among the elderly. Among these top five causes, mortality rates due to accidental falls, motor vehicle traffic injuries, and accidental asphyxia all increased significantly with age (χtrend2=8 315.40, 25.12, and 37.26, respectively; all P<0.001). Analyses of the annual trends showed a significant increase in deaths caused by accidental falls (AAPC=4.97%, P=0.013) and a significant decrease in deaths caused by suicide (AAPC=-14.87%, P<0.001). ConclusionFrom 2013 to 2022, the injury-related mortality burden among adults aged 60 years old and above in Yuyao City remained considerable, primarily caused by accidental falls, motor vehicle traffic injuries, drowning, accidental asphyxia, and suicide. There is an urgent need to develop targeted injury prevention strategies and measures to reduce the risk of unintentional injury deaths among the elderly.

3.A comparative study of color Doppler ultrasound and CT angiography for preoperative evaluation of perforator vessels in free posterior interosseous artery flap.

OBJECTIVE:
To investigate the accuracy of color Doppler ultrasound (CDU) and CT angiography (CTA) in the preoperative evaluation of perforator vessels in free posterior interosseous artery perforator (PIAP) flaps.
METHODS:
Between January 2020 and December 2023, 19 patients with hand skin and soft tissue defects caused by trauma were admitted. There were 11 males and 8 females, with a median age of 45 years (range, 26-54 years). The interval between injury and admission was 5-11 days (mean, 7.2 days). The skin and soft tissue defects were located on the dorsum of the hand in 8 cases and on the fingers in 11 cases. The size of defect ranged from 4.0 cm×2.5 cm to 7.5 cm×3.5 cm. After locating the perforator vessels through CDU and CTA before operation, the free PIAP flaps were designed to repair hand defects, with the size of 4.5 cm×3.0 cm-7.5 cm×4.0 cm. The defects of donor sites were directly sutured. The number and diameter of perforator vessels in the posterior interosseous artery detected by CDU and CTA were compared. The differences in localization of perforator vessels using CDU and CTA and their clinical effects were also compared to calculate the accuracy and recognition rate. During follow-up, the survival of the skin flap was observed, and the Vancouver scar scale (VSS) score was used to evaluate the healing of the donor site, while the visual analogue scale (VAS) score was used to evaluate the patient's satisfaction with the appearance of the skin flap.
RESULTS:
The number and the diameter of PIAP vessels was 5.8±1.2 and (0.62±0.08) mm assessed by CDU and 5.2±1.0 and (0.60±0.07) mm by CTA, showing no significant difference between the two methods ( P>0.05). The number, course, and distribution of perforator vessels of the PIAP vessels observed during operation were basically consistent with those detected by preoperative CDU and CTA. Compared with intraoperative observation results, the recognition rates of dominant perforating vessels by CDU and CTA were 95.0% (18/19) and 89.5% (17/19), respectively, and the accuracy rates were 100% (19/19) and 84.2% (16/19), with no significant difference between the two methods ( P>0.05). All flaps survived after operation, and all wounds and incisions at donor sites healed by first intention. All patients were followed up 6-13 months (mean, 8.2 months). At last follow-up, the skin flaps had elasticity and soft texture,with the patient satisfaction VAS score of 9.2±0.8. The donor sites had no obvious scar hyperplasia with the VSS score of 11.7±0.9.
CONCLUSION
CDU and CTA accurately identify the dominant perforator vessels and provide reliable information for vessel localization, facilitating precise flap harvesting and minimizing donor site injury. However, CDU offers superior visualization of distal end of perforator vessels in the forearm compared to CTA.

4.Application of assisted reproductive technology in fertility pre-servation and promotion for women with malignant tumors.

OBJECTIVES:
To evaluate the strategies and outcomes of assisted reproductive technology (ART) for fertility preservation and promotion in women with malignant tumors, and to analyze ART outcomes across different tumor types.
METHODS:
We conducted a retrospective analysis of female patients who underwent ART for fertility preservation or treatment at the Reproductive Center of the Women's Hospital, Zhejiang University School of Medicine, between January 1, 2018, and December 31, 2023. A total of 163 ART-aided pregnancy patients with malignant tumors were included in the case group, among which 6 patients underwent embryo cryopreservation for fertility preservation before radiotherapy or chemotherapy. Additionally, 11 unmarried women underwent oocyte cryopreservation due to borderline ovarian tumors, ovarian cancer, breast cancer, or hematological malignancies. The control group was selected from women without a history of malignant tumors who received ART treatment during the same period, using propensity score matching at a ratio of 1∶2, resulting in 326 cases. Data were collected through the reproductive medical record system and telephone follow-up (as of October 31, 2024). Baseline characteristics, controlled ovarian hyperstimulation parameters, laboratory indicators, and pregnancy outcomes were compared between case and control groups and among patients with different tumor types, and the tumor recurrence of the patients was followed up.
RESULTS:
Patients in the case group had significantly lower ovarian reserve (AMH, AFC) and a higher proportion of diminished ovarian reserve compared to the control group (all P<0.01). Regarding the ovulation induction protocol, the proportion of patients using the minimal stimulation protocol in the case group was significantly higher than that in the control group (29.45% vs. 12.88%, P<0.01), and the total dosage of gonadotropins used was lower (P<0.01). In terms of assisted reproductive outcomes, there were no statistically significant differences between the two groups in the number of retrieved oocytes, number of high-quality embryos, fertilization rate, cumulative pregnancy rate, cumulative live birth rate, or miscarriage rate (all P>0.05). However, the number of oocyte retrieval cycles and embryo transfer cycles required to achieve a live birth outcome in the case group were significantly higher than those in the control group (both P<0.05). Subgroup analysis showed that there were no significant differences in cumulative pregnancy rate and live birth rate among patients with different tumor types (thyroid cancer, reproductive system tumors, breast cancer, lung cancer). Nevertheless, lung cancer patients had the lowest ovarian reserve and required the most oocyte retrieval cycles due to their older age; breast cancer patients had a relatively lower fertilization rate partially because some of them were complicated with male factors. A follow-up of 154 tumor patients (with a follow-up rate of 88.5%) revealed that 6 patients (4.20%) had tumor recurrence, and 1 breast cancer patient died due to tumor recurrence. None of the 11 unmarried patients who had undergone oocyte cryopreservation had used the cryopreserved oocytes for assisted pregnancy yet, and 1 patient who had undergone fertility preservation died due to tumor recurrence.
CONCLUSIONS
Women of reproductive age with malignant tumors are at risk of diminished fertility. ART can effectively preserve and promote fertility, enabling favorable pregnancy and live birth outcomes. It is recommended to initiate a multidisciplinary assessment promptly prior to radiotherapy/chemotherapy and formulate an individualized ART regimen for fertility preservation or promotion, so as to achieve reproductive goals or safeguard future fertility potential.
5.Application Value of 18F-FDG PET/CT Metabolic Parameters in Prognosis of Nasopharyngeal Carcinoma

Objective To investigate the value of 18F-FDG PET/CT metabolic parameters in the prognostic assessment of nasopharyngeal cancer patients. Methods The clinical data and PET/CT metabolic parameters of 185 nasopharyngeal cancer patients were retrospectively analyzed. The collected parameters were SUVmax, MTV, TLG, total metabolic tumor volume (TMTV) and whole-body total lesion glycolysis (WTLG). The ROC curve was used to determine the optimal cut-off values of PET/CT metabolic parameters. Univariate and multivariate Cox regression models were used to screen the independent prognostic factors. Kaplan–Meier curves were used to analyze the survival differences. Results The results of univariate Cox regression analysis showed that age, pathologic type, WTLG, TMTV, MTV, and TLG were closely associated with OS and PFS; and SUVmax was associated with PFS (P<0.05). Multivariate Cox regression analysis results showed that age, TMTV, and WTLG were the independent prognostic factors for OS and PFS (P<0.05). The combination of WTLG with T/N staging (AUC=0.781 and 0.781) and TMTV with T/N staging (AUC=0.800 and 0.790) yielded greater predictive accuracy than that of WTLG and TMTV alone (AUC=0.724 and 0.719) or T/N staging (AUC=0.593 and 0.575). Conclusion TMTV and WTLG are important prognostic predictors of nasopharyngeal carcinoma. TLG and MTV of primary lesions are prognostic factors for patients’ PFS and OS. SUVmax has limited prognostic value. Systemic metabolic indexes (TMTV and WTLG), when combined with T/N staging, can optimize prognostic stratification.
6.Exploration of the comprehensive management practice pathway for long-term prescription medications in psychiatry

OBJECTIVE To explore comprehensive management and potential issues associated with long-term prescriptions medications of psychiatry, in order to provide a reference for the comprehensive management of long-term prescriptions of psychiatry in psychiatric hospitals and other medical institutions’ pharmacies. METHODS Starting from the applicable principles for long-term prescriptions of psychiatry, this study introduced the standardized assessment and precautions before issuing long-term prescriptions, the formulation and adjustment of the drug list, as well as the rational management of the long-term prescriptions. It also analyzed potential issues that may arise in the comprehensive management of long-term prescription medications and proposed corresponding countermeasures and suggestions. RESULTS & CONCLUSIONS Prior to initiating long-term prescriptions, a standardized assessment should be conducted on patients from the aspects of their psychiatric condition and long-term potential risk factors, pharmacological treatment plans and other non-pharmacological therapies, physical illnesses. Additionally, healthcare providers should fulfill their obligation to inform patients or their family members. The comprehensive management of long-term prescription medications should be jointly established and improved by multiple departments, and the formulation of drug catalogs should avoid including drugs with potential social harm or medication risks while complying with policy requirements. Furthermore, measures such as adding special identifiers to long-term prescriptions, providing patients with reminders about (No.YGLX202537) prescription expiration, or offering online consultations can also effectively enhance the rationality of medication use under long-term prescriptions. Currently, the implementation of long-term prescriptions in psychiatry remains challenged by inconsistencies in prescription duration, incomplete coverage of diagnostic categories, poor patient adherence, and the risk of deviation in clinical assessments. In this regard, measures such as collaborating with multiple departments to strengthen long-term prescription information management, providing matching pharmaceutical services, ensuring the quality and rationality of long-term prescription implementation, and using modern methods to screen high-risk patients can be taken to improve patient medication compliance and safety.
7.Research progresses on Keap1-Nrf2 pathway in inflammatory diseases

The Keap1-Nrf2 pathway has been shown to be an important defense mechanism against oxidative stress, which may be an effective therapeutic strategy for many diseases. The research progresses on Keap1-Nrf2 pathway in inflammatory diseases were mainly reviewed. The basic components and activation mechanism of Keap1-Nrf2 pathway were introduced. The relationship between Keap1-Nrf2 pathway and the crosstalk between NF-κB pathway and HO-1 pathway, the expression of inflammatory mediators and enzymes, and inflammatory bodies were expounded. Natural product-derived inhibitors, small molecule inhibitors targeting Keap1-Nrf2 pathway and their clinical progress were introduced, and the potential application value of Keap1-Nrf2 pathway in the treatment of inflammation was discussed.
8.Association between ambient ozone exposure during pregnancy and risk of preterm birth in Guangdong Province

Background Air pollution exposure has a significant impact on maternal and child health. However, the research on the association between ambient ozone (O3) exposure during pregnancy and the risk of premature birth in newborns is limited, and the conclusions are inconsistent. Objective To investigate the association of ambient O3 exposure during pregnancy with the risk of preterm birth in Guangdong Province. Methods Data of pregnant women in Guangzhou from 2013 to 2019 and Foshan from 2018 to 2023 were collected, and O3 concentrations during different trimesters were assessed according to maternal residential addresses. Bilinear interpolation was used to evaluate the concentrations of air pollution. A cohort study design was adopted in our study. Restricted cubic spline curves were used to evaluate the exposure-response relationship between O3 exposure and preterm birth risk and explore potential exposure threshold of O3. Logistic regression models were used to evaluate the association of O3 exposure with preterm birth. Results A total of 702 924 pregnant women were included in this study, of whom 43 051 (6.12%) were preterm. The average O3 exposure concentrations of pregnant women during the first, second, third, and whole trimesters were 95.51, 97.51, 100.60, and 97.87 μg·m−3, respectively. We observed J-shaped associations between O3 exposure and preterm birth risk during the second, third, and whole trimesters of pregnancy using restricted cubic spline curves. This study found that there were threshold concentrations between O3 exposure and preterm birth risk during different gestational periods, and the threshold concentrations in the first, second, third, and whole trimesters were 112.32, 99.83, 111.74, and 112.46 μg·m−3, respectively. During the second, third, and whole trimesters of pregnancy, after adjusting for maternal age, baby sex, pre-pregnancy body mass index, mode of delivery, baby birth weight, gestational diabetes, and gestational hypertension, the odds ratios (OR) of preterm birth were 1.02 (95%CI: 1.01, 1.04), 1.02 (95%CI: 1.00, 1.03), and 1.17 (95%CI: 1.13, 1.21) for each 10 μg·m−3 increase in O3 concentration above the O3 threshold. No significant association was found between O3 exposure and the risk of preterm birth during the first trimester. Conclusion There is a nonlinear association between the risk of preterm birth and O3 exposure during pregnancy, and higher concentrations of O3 exposure during pregnancy are associated with the risk of preterm birth. Above the O3 threshold concentration during pregnancy, especially during the second, third, and whole trimesters, the risk of preterm birth elevates with the increase of O3 exposure concentrations.
9.Study on the effect of berberine combined with fluconazole on fluconazole-tolerant Candida albcians strains

Objective To investigate the combined effect of berberine (BBR) and fluconazole (FLC) on FLC-tolerant Candida albicans in vitro. Methods The sensitivity of 8 strains of Candida albicans to FLC was assessed by determining their minimal inhibitory concentration (MIC) using broth microdilution method. FLC-tolerant strains were screened from FLC-sensitive strains by disk diffusion assay. The effect of BBR combined with FLC on FLC-tolerant Candida albicans was investigated by disk diffusion assay. Results All eight strains of Candida albicans exhibited sensitivity to FLC, with minimal inhibitory concentration (MIC50) values below 0.5 μg/ml. Strains Y0109, 9821, 7879, 7654, and 9296 displayed colony growth in the inhibition zone after 48 h of constant temperature incubation, indicating FLC tolerance. When strains Y0109 and 9821 were subjected to a combination of BBR and FLC, the number of colonies within the inhibition zone decreased progressively with the increase of BBR concentration following a 48 h constant temperature culture. The inhibition zone became clear with the increasing of BBR concentration and increased with the increase of FLC loading, which showed a dose-dependent relationship. Conclusion The BBR combined with FLC demonstrated efficacy against FLC-tolerant strains.
10.Correlation between the change track of metabolic syndrome index and thyroid nodules in individuals receiving health examinations

Objective:To analyze the correlation between metabolic syndrome indexes and thyroid nodules in individuals receiving health examinations.Methods:It′s a retrospective cohort study. A total of 2 678 individuals who received health examinations in the Health Management Center of the First Hospital of Shanxi Medical University for four consecutive years and met the admission criteria were selected as the research objects. According to the metabolic syndrome index data of health examination, according to the different duration of metabolic syndrome during the observation period, the group-based trajectory model, Bayesian information criterion and average posterior grouping probability were used to determine the best trajectory groups, and the objects were divided into three different metabolic syndrome index trajectory groups: normal, abnormal and recovery group. During the physical examination in 2022 and 2023, the detection of thyroid nodules in each group was followed up, and the difference of detection rate of thyroid nodules in different metabolic syndrome trajectory groups was compared by Log-rank test, and the correlation between different metabolic syndrome index change trajectories and thyroid nodules was analyzed by logistic regression model.Results:The cumulative detection rate of thyroid nodules in normal group, abnormal group and recovery group was 18.8% (77/410), 27.5% (327/1 190) and 24.7% (266/1 078), respectively ( χ2=19.482, P<0.001). In model 4, after adjusting for age, gender, smoking, drinking, staying up late, insomnia, physical activity, family history and other confounding factors, the risk of thyroid nodules in abnormal group and recovery group was still 2.011 times (95% CI: 1.457-2.776) and 2.006 times (95% CI: 1.389-2.897) of that in normal group. Conclusion:There is a positive correlation between metabolic syndrome index and thyroid nodules in individuals receiving health examinations, and metabolic syndrome index can be used as a predictive index of thyroid nodules.
