The growing prevalence of outpatient chemotherapy highlights the vital role that community pharmacists have in patient care. An online interaction meeting was held annually over a three-year period that aimed to foster mutual understanding between patients with cancer and community pharmacists. Each session involved six pharmacists and seven to eight cancer patients, with the same pharmacists participating each year. These meetings highlighted the potential of community pharmacists to reduce patient anxiety and provide treatment guidance, underscoring the importance of connecting patients with pharmacists.
2.Associations of serum carotene levels and decline for the ability of attention: a longitudinal study in the Japanese general population.

BACKGROUND:
Although serum carotene may contribute to dementia prevention, there is a lack of longitudinal evidence for early cognitive decline before dementia symptoms. The aim of this study was to examine whether serum carotene levels were associated with annually evaluated cognitive trajectories among the Japanese general population.
METHODS:
Among 581 baseline participants, 199 individuals (83 males; mean age [min, max], 62.7 [39, 90] years) who underwent cognitive assessments more than twice after baseline were analyzed. "Attention" levels were assessed using one- and three-target Digit Cancellation Tests (D-CAT1 and D-CAT3). "General cognitive ability" was assessed by the short version of Mini-Mental State Examination (SMMSE). Serum carotenes (α-carotene, β-carotene and lycopene) were measured by high-performance liquid chromatography. After the measurements, we calculated total carotene levels by summing up the levels of all measured carotene. Carotene levels were categorized into three groups for analysis (low: 0%-25%, middle: 25%-75%, and high: 75%-100%). A linear mixed model was used to estimate the slope of the D-CAT score trajectory and to compare it between three categories.
RESULTS:
Compared with the middle carotene group, decline of attention was faster in the D-CAT1 for low β-carotene (β = -3.48, p = 0.035), lycopene (β = -3.10, p = 0.062), and total carotene (β = -4.75, p = 0.003), but not for α-carotene (β = -2.60, p = 0.111). For the D-CAT3, decline of attention was faster in the group of low lycopene (β = -3.17, p = 0.002) and total carotene (β = -2.17, p = 0.037) compared with the middle carotene group, while no clear association for α-carotene (β = -0.67, p = 0.521) and β-carotene (β = -0.64, p = 0.639). There were no clear associations between serum carotene and the SMMSE score.
CONCLUSIONS
These findings suggest low levels of serum lycopene are associated with a decline of attention in the setting of the general population.

3.Caspase-11 mediated inflammasome activation in macrophages by systemic infection of A.actinomycetemcomitans exacerbates arthritis

Clinical studies have shown that Aggregatibacter actinomycetemcomitans(A.actinomycetemcomitans)is associated with aggressive periodontitis and can potentially trigger or exacerbate rheumatoid arthritis(RA).However,the mechanism is poorly understood.Here,we show that systemic infection with A.actinomycetemcomitans triggers the progression of arthritis in mice anti-collagen antibody-induced arthritis(CAIA)model following IL-1β secretion and cell infiltration in paws in a manner that is dependent on caspase-11-mediated inflammasome activation in macrophages.The administration of polymyxin B(PMB),chloroquine,and anti-CD11b antibody suppressed inflammasome activation in macrophages and arthritis in mice,suggesting that the recognition of lipopolysaccharide(LPS)in the cytosol after bacterial degradation by lysosomes and invasion via CD11b are needed to trigger arthritis following inflammasome activation in macrophages.These data reveal that the inhibition of caspase-11-mediated inflammasome activation potentiates aggravation of RA induced by infection with A.actinomycetemcomitans.This work highlights how RA can be progressed by inflammasome activation as a result of periodontitis-associated bacterial infection and discusses the mechanism of inflammasome activation in response to infection with A.actinomycetemcomitans.
4.A Case of Duodenitis Suspected to Be Associated With Legionella Infection

In infectious enteritis, the affected site differs depending on the organism causing the disease, and based on this feature, it is possible to estimate the causative organisms of infectious enteritis. However, there have been few reports of infectious enteritis mainly affecting the duodenum. This report describes a case of duodenal enteritis that was suspected to be associated with Legionella infection. The patient was a 66-year-old man. He was febrile for 5 days and had watery diarrhea for 3 days before admission, and he presented to our hospital when the symptoms did not improve. He was hypoxemic at presentation, and a CT scan on admission revealed nonsegmental ground-glass opacities in both lung fields and thickening of the intestinal wall, mainly in the duodenum. Blood tests and chest imaging findings led to suspicion of Legionella pneumonia. Urine specimens were submitted for Legionella antigen testing and were found to be positive, confirming the diagnosis. Subsequent antimicrobial therapy resulted in improvement of the diarrhea symptoms and duodenal wall thickening, and the patient was discharged on hospital day 19. This case shows that Legionella pneumonia can manifest as abdominal symptoms. The possibility of Legionella pneumonia should thus be considered in patients with duodenitis.
5.Laparoscopic Surgery for Ascending Colon Lipoma Causing Recurrent Intussusception: A Case Report and Review of 47 Cases in Japan

The patient was a 66-year-old man who presented to our hospital with intermittent abdominal pain. Abdominal ultrasound showed a mass presenting as a target sign in the right abdomen, suggesting intussusception. Subsequent computed tomography (CT) and lower gastrointestinal endoscopy revealed a 5 cm lipoma in the ascending colon. The intussusception had resolved spontaneously, so the patient was followed up on an outpatient basis. Although endoscopic treatment was considered for the lipoma, due to its large size, the patient was referred to our department for surgery 1 month later. A follow-up CT scan revealed a recurrence of the intussusception. Because the patient tolerated oral intake, had normal bowel movements without symptoms of bowel obstruction, and experienced only mild abdominal pain, conservative outpatient observation was continued until laparoscopic-assisted ileocecal resection was performed. The postoperative course was uneventful, and the patient was discharged on postoperative day 8 without any complications. Colonic lipoma causing intussusception is relatively rare. Since lipoma is a benign disease, it is a good candidate for laparoscopic surgery, which offers excellent cosmetic outcomes and rapid postoperative recovery. Even in cases with intussusception, when there is no bowel obstruction and the abdominal pain is mild, elective surgery within 1-3 months can be considered a viable option.
6.A Case of Metronidazole-Induced Encephalopathy During the Treatment of Pelvic Dead Space Infection

Metronidazole is an effective antibacterial agent against anaerobic bacteria, and its use in the gastrointestinal field, particularly in patients with intra-abdominal infections, has increased with the availability of injectable formulations. However, a known side effect of metronidazole is central nervous system impairment, namely, metronidazole-induced encephalopathy. Here, we present a case of a 68-year-old man who underwent laparoscopic abdominoperineal resection for rectal cancer in March of year X. Postoperatively, metronidazole was administered for pelvic dead space infection and perineal wound infection, and the patient continued the oral medication after discharge. In May of year X, the patient was referred to our institution near his home for postoperative adjuvant chemotherapy. In February of year X+1, the patient developed dysarthria, and a FLAIR (fluid-attenuated inversion recovery) image on head magnetic resonance imaging revealed bilaterally symmetric hyperintense lesions in the dentate nuclei of the cerebellum. Suspecting metronidazole-induced encephalopathy based on the medication history, metronidazole was discontinued, leading to symptom improvement within approximately 3 days. We diagnosed metronidazole-induced encephalopathy based on the imaging findings and clinical observations. When using metronidazole, we should pay attention to the dosage and duration of administration and make efforts to detect side effects early.
7.Improvement of Health Awareness among Pharmacy Students through Holding a Women's Health Care Cafe

In Japan, there are insufficient measures to ensure that women can maintain good health while working. In addition, many women have limited health literacy, making it difficult for them to access information on physical and mental health problems that are specific to them. In this context, it is essential for pharmacists to become the first point of contact for women seeking information about their physical health, as they are familiar with healthcare providers in the community. To address this issue, we organized a Women's Health Care Cafe for pharmacy students to provide them with an opportunity to learn about women's health.
8.Intralobar Nephroblastomatosis Mimicking Wilms Tumor Treated with Chemotherapy after Removal of the Affected Kidney

Nephroblastomatosis (NBM) is a precursor of Wilms tumor. We herein report a case in which Wilms tumor was initially suspected and the affected kidney was removed.The tumor was subsequently diagnosed as intralobar NBM and a favorable outcome was achieved with postoperative chemotherapy. A 2-year-old boy who presented with gross hematuria was found to have an enlarged left kidney with hydronephrosis.Needle biopsy of the left kidney suggested Wilms tumor and left nephrectomy was performed. The tumor was histopathologically diagnosed as intralobar NBM.Although NBM is regarded as a precancerous lesion, a definite treatment plan has not yet been established. In the present case, we used a similar chemotherapy regimen to that for Wilms tumor. Eight years after the completion of chemotherapy, Wilms tumor has not developed or recurred. Appropriate management plans need to be developed by accumulating similar cases.
9.Intrapleural Urokinase and Antibiotic Therapy for Empyema after In Situ Reconstruction of the Aorta and Lobectomy for Aortopulmonary Fistula due to Thoracic Aortic Aneurysm Rupture

A 62-year-old man presented with hemoptysis and hoarseness. He was diagnosed with an aortopulmonary fistula due to a thoracic aortic aneurysm rupture and was referred to our department. Emergency in-situ reconstruction of the aorta and pulmonary lobectomy were performed. Nine days postoperative, he developed empyema. Intrapleural urokinase and antibiotic therapy were selected as management instead of a video-assisted thoracoscopic debridement and decortication due to his worsening condition. The treatment was successful, and he was discharged from the hospital without any further complications. This study highlights the benefit of intrapleural administration of urokinase and antibiotics in patients with acute empyema, when surgical treatment is inappropriate.
10.Comparison of postoperative nausea and vomiting between remimazolam and propofol: a propensity score-matched, retrospective, observational, single-center cohort study

Background:
Remimazolam is a novel ultrashort-acting benzodiazepine that has recently become available for general anesthesia. However, the incidence of postoperative nausea and vomiting (PONV) associated with remimazolam remains unknown. In this propensity score-matched, retrospective, observational study, we compared the rates of PONV between remimazolam and propofol.
Methods:
In this retrospective observational study, propensity score-matching was performed to minimize selection bias. Patients who received total intravenous anesthesia with remimazolam or propofol at the Hamamatsu University Hospital between August 2020 and July 2021 were enrolled in the study. Data on patient demographics, anesthetic agents, and PONV within the first 24 h were collected and analyzed.
Results:
Of the 1,239 patients who met the study selection criteria, 585 received remimazolam and 684 received propofol. After propensity score matching, 333 matched pairs were further analyzed. Patient demographics and the anesthetic agents used were comparable between the matched cohorts. The incidence of PONV was significantly higher in the remimazolam group than in the propofol group (35% vs. 21%, P < 0.001).
Conclusions
The incidence of PONV is higher with remimazolam anesthesia than with propofol anesthesia. The findings of this study require confirmation in larger prospective randomized controlled trials.
