1.Experience of YU Guoyong in Treating Type 2 Diabetes Mellitus with Acupuncture and Medicine Based on the Theory of"Five Evolutive Phases and Six Climatic Factors"

[Objective]To summarize Chief Physician YU Guoyong's clinical experience in treating type-2 diabetes mellitus(T2DM)by using the Yunqi Decoction combined with the six climate acupuncture method.[Methods]The experience of treating T2DM by using the Yunqi Decoction combined with the six climate acupuncture method is explained through clinical summarization and collation of case data,and is supported by clinical test cases.[Results]T2DM is caused by the lack of innate endowment,the deficiency of Zangfu and the environmental effects of the acquired,its pathogenesis is the failure of Qi transformation and harm from hyperactivity,the development of the disease and the regression of the disease are affected by the innate endowment and changes of five evolutive phases and six climatic factors.Chief YU seizes the essence of biased constitution and takes into account the symptoms of the disease by using the traditional Chinese medicine syndrome differentiation system of"nature factor-human body-disease".The Yunqi Decoction combined with the six climate acupuncture method is based on the theory of"Five Evolutive Phases and Six Climatic Factors".In the case cited,the patient sought medical advice because of the aggravation of T2DM.During the first consultation,Chief YU diagnosed the patient as having spleen deficiency and liver hyperfunction based on the patient's innate endowment and the characteristics of five evolutive phases and six climatic factors at the onset of the disease,as well as the symptoms of the disease,so the treatment was to strengthen the spleen and dispel dampness,soothe the liver and promote circulation,and was given Baizhu Houpo Decoction and Jingshun Decoction,then activated blood circulation,eliminated blood stasis and paid attention to the deficiency in the later stage,and was also adjusted with the six climate acupuncture methods.[Conclusion]Chief YU recognizes T2DM from the changes of five evolutive phases and six climatic factors,physical endowment and deficiency or excess of Zangfu and uses the Yunqi Decoction combined with the six climate acupuncture methods,which has the characteristics of the division of heaven and body,specializing in specific diseases,and is effective in clinical use.

For middle-aged and elderly patients with conditions such as spinal fractures and degenerative spinal diseases, spinal internal fixation is a core surgical procedure for reconstructing spinal stability, heavily relying on the biomechanical stability provided by pedicle screw systems. Whereas, these patients are often complicated by osteoporosis that can significantly compromise the stability of the bone-pedicle screw interface, leading to a marked increase in pedicle screw loosening and surgical failure rates. The bone cement-augmented pedicle screw technique, which involves injecting bone cement into the vertebral body or screw trajectory to optimize the mechanical properties of the bone-pedicle screw composite, has been proven to significantly enhance fixation strength and effectively prevent screw-related failures, thereby reducing the incidence of internal fixation failure in high-risk populations undergoing spinal fusion. However, the widespread clinical application of this technique has faced challenges such as inaccurate clinical decision-making (indication and contraindication selection), non-standardized operative practices, and insufficient awareness of complication prevention, resulting in considerable variability in clinical outcomes and even severe complications. To address this, Prof. Luo Fei from First Affiliated Hospital of Army Medical University initiated the project and the Chinese Association Orthopaedic Surgeons organized relevant experts to develop the Evidence-based clinical practice guideline for bone cement-augmented pedicle screw technique ( version 2025), based on current evidence. The guidelines put forward 8 recommendations regarding the clinical value, scope of application, and operational standards of the technique, aiming to provide evidence-based medical support and technical standardization for clinical decision-making.
3.Analysis on new-type drugs use and related factors of high-risk behaviors in online-dating men who have sex with men in Shandong Province

Objective:To understand the related factors of new-type drugs use and high-risk behaviors in online-dating men who have sex with men (MSM) in Shandong Province, and provide evidence for the development of targeted HIV prevention and intervention strategies.Methods:A cross-sectional study was conducted from April to July 2023 in eight sentinel surveillance sites across cities in Shandong, recruiting MSM participants. Each city enrolled a sample of 400 individuals. Face-to-face questionnaire surveys were conducted to collect data on socio-demographic characteristics, drug use and sexual behaviors, risk perception, and online-dating practices of the MSM, and χ2 test was used to compare the differences between online-dating MSM who used new-type drugs and those who didn't use. Logistic regression model was used to analyze the related factors of group sex behavior and the lack of HIV detection before sexual behavior in the online-dating MSM. Statistical analysis was performed by using software SPSS 29.0. Results:A total of 3 235 MSM were surveyed, in whom 2 787 (86.15%) used online-dating platforms to find partners in the past six months. The majority were those who were aged 25-34 years (39.18%, 1 092/2 787), unmarried/divorced/widowed (71.51%, 1 993/2 787), had an education level of college or above (69.36%, 1 933/2 787), and primarily identified as homosexual gays (77.97%, 2 173/2 787), 37.78% (1 053/2 787) believed that they were at low risk for HIV infection and 10.88% (303/2 786) believed that using new-type drugs was less harmful and 57.59% (1 605/2 787) reported new-type drugs use. Multivariate logistic regression analysis indicated that MSM who used new-type drugs had higher risk for group sex in the past six months (a OR=1.31, 95% CI: 1.06-1.63) and sexual activity without pre-HIV testing (a OR=1.57, 95% CI:1.14-2.15) compared with those who didn't use new-type drugs.MSM who believed that the risk of using new-type drugs was general, higher and very high had higher risks for group sex in the past six months (a OR=1.54, 95% CI: 1.09-2.18) and sexual activity withou pre-HIV testing (a OR=2.02, 95% CI:1.34-3.05) compared with those who believed using new-type drugs was less harmful. Conclusions:The use of new-type drugs was relatively common in the online-dating MSM in Shandong, with widespread high-risk behaviors and poor awareness of risks to health. A gap between knowledge awareness and behavior exists. It is necessary to strengthen the health education and promotion of HIV informed dating practices in MSM.

Objective:To conduct a visual analysis of the research hotspots and development frontiers in-hospital stroke from both Chinese and English literature.Methods:Relevant literature on in-hospital stroke was retrieved from the Web of Science Core Collection, China National Knowledge Infrastructure, and Wanfang Database, with the search period extending from the inception of each database to June 2024. CiteSpace 6.2R3 was used to analyze the cooperation networks of authors, countries/regions, and institutions, and to conduct co-occurrence, clustering, and emerging keyword analyses.Results:A total of 465 English papers and 72 Chinese papers were included, showing a year-on-year increase in the overall number of publications. The research in English literature was concentrated in United States, with close cooperation between European and American countries, and comprehensive universities as the main research institutions. In Chinese literature, major research institutions were located in teaching hospitals, with limited collaboration between institutions. The research hotspots and frontiers can be categorized into three themes: risk factors, reperfusion therapy, and quality improvement.Conclusions:Research in in-hospital stroke in both Chinese and English literature is at a stable development stage, with both commonalities and differences in research directions. Future studies should focus on enhancing cooperation between authors, disciplines, institutions, and countries, further exploring the risk factors and mechanisms of in-hospital stroke, implementing precision treatment, building prevention and treatment systems, and expanding the breadth and depth of research in this field.

The incidence of osteoporotic thoracolumbar vertebral fracture (OTLVF) in the elderly is gradually increasing. The kyphotic deformity caused by various factors has become an important characteristic of OTLVF and has received increasing attention. Its clinical manifestations include pain, delayed nerve damage, sagittal imbalance, etc. Currently, the definition and diagnosis of OTLVF with kyphotic deformity in the elderly are still unclear. Although there are many treatment options, they are controversial. Existing guidelines or consensuses pay little attention to this type of fracture with kyphotic deformity. To this end, the Lumbar Education Working Group of the Spine Branch of the Chinese Medicine Education Association and Editorial Committee of Chinese Journal of Trauma organized the experts in the relevant fields to jointly develop Clinical guidelines for the diagnosis and treatment of osteoporotic thoracolumbar vertebral fractures with kyphotic deformity in the elderly ( version 2024), based on evidence-based medical advancements and the principles of scientificity, practicality, and advanced nature, which provided 18 recommendations to standardize the clinical diagnosis and treatment.
10.Effect of puerarin on myocardial inflammation in rats with heart failure through TLR4/NF-κB signaling pathway

Objective:To investigate the effect of puerarin on myocardial inflammation in rats with heart failure from the Toll-like receptor 4(TLR4)/nuclear transcription factor-κB(NF-κB)inflammatory pathway.Methods:The rat model of heart failure was established by subcutaneous injection of isoproterenol(5 mg/kg for 7 days),and was divided into model group,puerarin group,TLR4 activator group(RS09,25 μg/kg),TLR4 inhibitor group(TAK-242,0.5 mg/kg),puerarin+TLR4 activator group,with 20 rats in each group.Another 20 rats were selected as normal control group.Echocardiography was used to detect heart function;HE staining was used to observe pathological damage of myocardial tissue;TUNEL method was used to detect apoptosis of myocardial tissue cells;ELISA method was used to detect the levels of inflammatory factors IL-6,IL-1β and tumor necrosis factor-α(TNF-α)in myocardial tissue;immunohistochemical method was used to detect the expression of TLR4 and macrophage activation marker-F4/80;Western blot was used to detect the protein expressions of TLR4/NF-κB pathway,MyD88,IL-1 receptor-associated kinase(IRAK)and Galec-tin-3.Results:Compared with the normal control group,the necrosis,inflammatory infiltration and apoptosis of rat myocardial cells were severe in the model group,the heart function was decreased,the inflammatory factor release,macrophage activation,and TLR4/NF-κB pathway protein expression were increased(P<0.05).Puerarin and TLR4 inhibitors could improve the pathological damage of heart failure rats such as myocardial cell necrosis,inflammatory infiltration and apoptosis to the same extent,improve heart function,reduce inflammatory response,inhibit macrophage activation and reduce TLR4/NF-κB pathway activation(P<0.05).TLR4 activator could promote the activation of TLR4/NF-κB pathway,aggravate myocardial cell damage,apoptosis and cardiac function damage in rats with heart failure,and weaken the above-mentioned effects of puerarin(P<0.05).Conclusion:Puerarin may inhibit the activation of TLR4/NF-κB pathway,reduce myocardial inflammation,and play an anti-heart failure effect.
