Objective:
This study aimed to elucidate the hand function recovery capacity of degenerative cervical myelopathy (DCM) patients with different severities of hand dexterity impairment.
Methods:
Hand functional outcome measures such as the 10-second grip and release (10s-G&R) test, modified Japanese Orthopaedic Association (mJOA) upper extremity score and Japanese Orthopaedic Association Cervical Myelopathy Evaluation Questionnaire (JOACMEQ) upper extremity function were collected before surgery and at the 1-year follow-up. A total of 102 DCM patients were categorized into mild, moderate and severe group based on the preoperative 10s-G&R test result. Hand functional parameters were compared across the 3 groups. Multivariate linear regression was conducted to explore predictive factors. Receiver operating characteristic curve analysis was performed to assess the predictive efficacy of the preoperative 10s-G&R test and establish the cutoff value for incomplete recovery of hand dexterity.
Results:
At the 1-year follow-up, significant improvements were observed in all hand functional parameters across all 3 groups. However, the incomplete recovery rates of the mild, moderate, severe groups were 26.67%, 46.88%, and 57.50%, respectively (p < 0.05). Multivariate regression revealed that preoperative 10s-G&R test result, age, Hoffmann sign, duration of symptom, and mJOA Upper score serve as significant predictors for postoperative 10s-G&R test outcomes. Patients with a preoperative 10s-G&R test < 15 cycles have a 1.9 times higher risk of incomplete recovery of hand function (p = 0.005).
Conclusion
Most patients, regardless of their preoperative hand function, exhibit potential for improvement in hand dexterity. However, worse initial hand dexterity correlates with poorer outcomes. Surgical treatment is recommended before the 10s-G&R test drops below 15 cycles.

6.Temporal changes of hand dexterity in patients with cervical spondylotic myelopathy after decompression surgery

Objectives:To elucidate the temporal pattern of hand dexterity changes in patients with cervical spondylotic myelopathy(CSM)one year after decompression surgery and to develop and validate a predictive model for hand dexterity improvement.Methods:A total of 136 adult patients with CSM(70 males and 66 females;mean age 56.37±11.12 years)treated in our hospital were prospectively enrolled between October 2021 and October 2022.Hand dexterity was assessed using the 10-second grip and release(10-s G&R)test,the modified Japanese Orthopaedic Association(mJOA)upper extremity motor function score,and the JO A cer-vical myelopathy evaluation questionnaire(JOACMEQ)upper extremity function before operation,and at 6 and 12 months after operation.A one-way ANOVA was conducted to examine the temporal pattern of hand func-tion improvement.Logistic regression analysis was performed to identify risk factors and develop a predictive model for postoperative hand dexterity impairment.The model's discrimination was assessed using receiver op-erating characteristic(ROC)curve and area under curve(AUC)analysis.Results:At 6 months after operation,significant improvements were observed in mJOA score(from 13.09±2.42 to 16.64±1.42 分),mJOA upper extremity motor function score(from 3.90±0.89 to 4.77±0.48),JOACMEQ upper extremity function score(from 76.97±19.30 to 90.45±14.70),and 10-s G&R test result(from 14.41±3.14 cycles to 18.16±4.63 cycles)(all P<0.001);At 12 months after operation,the 10-s G&R test result further increased to 18.16±4.63 cycles(P=0.038),though no significant changes were noted in the subjective scores.The cycles of preoperative 10-s G&R test,as well as age and smoking status,were identified as the risk factors for hand dexterity impairment one year after surgery.The prediction model was validated with an AUC of 0.780(95%CI=0.665-0.894).Conclusions:Regarding the hand function of CSM patients after decompression surgery,subjective measurements typically stabilize at 6 months,while objective measurements continue to improve at 1 year.Based on the cycles of preoperative 10-s G&R test,age,and smoking status,the improvement in the cycles of the 10-s G&R test one year post-surgery can be predicted.

8.Distribution and drug resistance of the bacterial strains isolated from urine in a tertiary Traditional Chinese Medicine hospital in Shanghai from 2010 to 2021

Objective To investigate the distribution and antibiotic resistance profiles of the bacterial strains isolated from urine samples in a tertiary Traditional Chinese Medicine hospital in Shanghai in the 12-year period from 2010 through 2021 for better empirical antibiotic use in clinical practice.Methods The clinical data of patients with urinary tract infection,including the species and antibiotic resistance of the bacterial strains isolated from urine samples from January 2010 to December 2021 were retrospectively analyzed.Results A total of 5 231 nondupliate bacterial strains were isolated,among which E.coli was the most common(52.8%),followed by Enterococcus spp(19.1%)and Klebsiella spp(11.1%).Most of the urinary isolates(76.0%)were isolated from the elderly aged 60-89,and only 3.1%of the strains were isolated from the young people aged under 44.Most of the bacterial strains were isolated from female patients(75.8%),however,more P.aeruginosa and A.baumannii were isolated from male patients compared to female patients(55.3%vs 44.7%and 67.7%vs 32.3%).About 13.7%of the strains were collected from the Department of Nephrology,more than the strains from any other clincial department.In intensive care unit(ICU),the proportion of E.coli was the lowest,while the proportion of Enterococcus spp was the highest.Enterobacterales showed lower resistance raets to carbapenems,cephamycin,amikacin,cefepime,and β-lactam/β-lactamase inhibitor combinations.P.aeruginosa showed higher susceptibility rates to carbapenems,aminoglycosides and β-lactam/β-lactamase inhibitor combinations compared to A.baumannii(≥54.6%vs≤39.8%).Gram-positive cocci were highly sensitive to glycopeptide antibiotics.No SS.aureus or E.faecalis isolates were found resistant to vancomycin.About 2.7%of the E.faecium isolates were resistant to vancomycin.The prevalence of drug-resistant bacteria was the highest in elderly patients,and in the strains isolated from ICU and emergency department.Conclusions Compared with general hospitals,a high proportion of elderly patients were treated in this hospital.It should be more cautious in the treatment of patients with urinary tract infection.The major bacterial species isolated from urine were E.coli,Enterococcus,and K.pneumoniae.Empirical treatment should be prescribed considering patient age and gender as well as the species and distribution of pathogenic bacteria.Urine culture and antibiotic susceptibility testing should be performed proactively.Appropriate antimicrobial agents should be selected according to the results of antimicrobial susceptibility testing.Antimicrobial ressitance surveillance should also be strengthened.
9.Deep Learning-Enhanced Hand Grip and Release Test for Degenerative Cervical Myelopathy: Shortening Assessment Duration to 6 Seconds

Hand clumsiness and reduced hand dexterity can signal early signs of degenerative cervical myelopathy (DCM). While the 10-second grip and release (10-s G&R) test is a common clinical tool for evaluating hand function, a more accessible method is warranted. This study explores the use of deep learning-enhanced hand grip and release test (DL-HGRT) for predicting DCM and evaluates its capability to reduce the duration of the 10-s G&R test.
Methods:
The retrospective study included 508 DCM patients and 1,194 control subjects. Propensity score matching (PSM) was utilized to minimize the confounding effects related to age and sex. Videos of the 10-s G&R test were captured using a smartphone application. The 3D-MobileNetV2 was utilized for analysis, generating a series of parameters. Additionally, receiver operating characteristic curves were employed to assess the performance of the 10-s G&R test in predicting DCM and to evaluate the effectiveness of a shortened testing duration.
Results:
Patients with DCM exhibited impairments in most 10-s G&R test parameters. Before PSM, the number of cycles achieved the best diagnostic performance (area under the curve [AUC], 0.85; sensitivity, 80.12%; specificity, 74.29% at 20 cycles), followed by average grip time. Following PSM for age and gender, the AUC remained above 0.80. The average grip time achieved the highest AUC of 0.83 after 6 seconds, plateauing with no significant improvement in extending the duration to 10 seconds, indicating that 6 seconds is an adequate timeframe to efficiently evaluate hand motor dysfunction in DCM based on DL-HGRT.
Conclusion
DL-HGRT demonstrates potential as a promising supplementary tool for predicting DCM. Notably, a testing duration of 6 seconds appears to be sufficient for accurate assessment, enhancing the test more feasible and practical without compromising diagnostic performance.
