Results: At the 1-year follow-up, significant improvements were observed in all hand functional parameters across all 3 groups. However, the incomplete recovery rates of the mild, moderate, severe groups were 26.67%, 46.88%, and 57.50%, respectively (p < 0.05). Multivariate regression revealed that preoperative 10s-G&R test result, age, Hoffmann sign, duration of symptom, and mJOA Upper score serve as significant predictors for postoperative 10s-G&R test outcomes. Patients with a preoperative 10s-G&R test < 15 cycles have a 1.9 times higher risk of incomplete recovery of hand function (p = 0.005). Conclusion Most patients, regardless of their preoperative hand function, exhibit potential for improvement in hand dexterity. However, worse initial hand dexterity correlates with poorer outcomes. Surgical treatment is recommended before the 10s-G&R test drops below 15 cycles.

Objectives:To elucidate the temporal pattern of hand dexterity changes in patients with cervical spondylotic myelopathy(CSM)one year after decompression surgery and to develop and validate a predictive model for hand dexterity improvement.Methods:A total of 136 adult patients with CSM(70 males and 66 females;mean age 56.37±11.12 years)treated in our hospital were prospectively enrolled between October 2021 and October 2022.Hand dexterity was assessed using the 10-second grip and release(10-s G&R)test,the modified Japanese Orthopaedic Association(mJOA)upper extremity motor function score,and the JO A cer-vical myelopathy evaluation questionnaire(JOACMEQ)upper extremity function before operation,and at 6 and 12 months after operation.A one-way ANOVA was conducted to examine the temporal pattern of hand func-tion improvement.Logistic regression analysis was performed to identify risk factors and develop a predictive model for postoperative hand dexterity impairment.The model's discrimination was assessed using receiver op-erating characteristic(ROC)curve and area under curve(AUC)analysis.Results:At 6 months after operation,significant improvements were observed in mJOA score(from 13.09±2.42 to 16.64±1.42 分),mJOA upper extremity motor function score(from 3.90±0.89 to 4.77±0.48),JOACMEQ upper extremity function score(from 76.97±19.30 to 90.45±14.70),and 10-s G&R test result(from 14.41±3.14 cycles to 18.16±4.63 cycles)(all P＜0.001);At 12 months after operation,the 10-s G&R test result further increased to 18.16±4.63 cycles(P=0.038),though no significant changes were noted in the subjective scores.The cycles of preoperative 10-s G&R test,as well as age and smoking status,were identified as the risk factors for hand dexterity impairment one year after surgery.The prediction model was validated with an AUC of 0.780(95％CI=0.665-0.894).Conclusions:Regarding the hand function of CSM patients after decompression surgery,subjective measurements typically stabilize at 6 months,while objective measurements continue to improve at 1 year.Based on the cycles of preoperative 10-s G&R test,age,and smoking status,the improvement in the cycles of the 10-s G&R test one year post-surgery can be predicted.

ObjectiveTo adapt the Stanford Expectations of Treatment Scale（SETS） into Chinese（C-SETS） and test the feasibility， validity and reliability of its application in patients with diarrhea-predominant irritable bowel syndrome（IBS-D） with liver-constraint and spleen-deficiency syndrome treated with traditional Chinese medicine（TCM）. MethodsWe obtained authorisation from the developer of the SETS， and followed the principle of "two-way translation" to translate the SETS by literal translation and back translation to form the C-SETS. Ninety-six IBS-D patients with liver-constraint and spleen-deficiency syndrome were enrolled as respondents and filled out C-SETS before receiving treatment； the feasibility was assessed by the recall rate， completion rate and the duration of filling out the scale； the reliability was assessed by Cronbach's α； the structural validity was assessed by exploratory and confirmatory factor analysis， and the content validity was assessed by correlation analysis. ResultsThe C-SETS consists of 10 items， with the 1st， 3rd， and 5th rating items constituting the Positive Expectations subscale， and the 2nd， 4th， and 6th rating items constituting the Negative Expectations subscale， each of which is rated on a 7-point Likert Scale. The recall of C-SETS was 100%（96/96）， the completion rate was 89.58%（86/96）； Cronbach's α for the Positive and Negative Treatment Expectations subscales were 0.845 and 0.854， respectively； exploratory factor analysis showed that the coefficient of commonality for all six entries was larger than 0.4， and that the six entries could be used by both factors to explain 77.092% of the total variance； validation factor analysis showed that the goodness-of-fit index， comparative fit index， root mean square of approximation error， canonical fit coefficient， and chi-square degrees of freedom ratio took the values of 0.943， 1.003， 0， 0.943， and 0.626， respectively； and the results of Spearman's analysis suggested that the C-SETS had good content validity. ConclusionThe C-SETS has well feasibility， reliability， and validity， which initially proves that it can be used as a tool to assess the treatment expectation of patients with IBS-D with liver-constraint and spleen-deficiency syndrome before receiving TCM treatment.

Objective: To provide high-quality clinical evidence of the efficacy of Tibetan medicine Honghua Ruyi (HHRY) pills for endometriosis-associated dysmenorrhea. Methods: This study constitutes a multicenter, randomized, double-blind, placebo-controlled trial encompassing a three-menstrual cycle intervention followed by a three-menstrual cycle follow-up period. A total of 164 eligible females with endometriosis-associated dysmenorrhea were randomly divided into HHRY pills and placebo groups in a 1:1 ratio. The primary outcome included dysmenorrhea symptoms assessed using Visual Analog Scale (VAS) scores and quality of life, whereas the secondary outcome measures included the maximum VAS for non-menstrual pelvic pain, duration of pain episodes (in days), frequency and quantity of the consumption of ibuprofen sustained-release capsules (or other non-steroidal anti-inflammatory drugs), and days off work/study for staff/student due to dysmenorrhea, ovarian cyst, and/or pelvic nodule size. The safety was monitored throughout the treatment period. All the analyses were based on the intention-to-treat principle. For continuous outcomes, simple or multiple linear regressions were used to estimate the differences between the HHRY pills and placebo groups, with categorical data expressed as the number and percentage of occurrences. Differences were compared using the chi-square test or Fisher's exact test. The predefined analysis was adjusted for concomitant treatment, a variable considered to be associated with outcomes but unaffected by treatment allocation. Estimates of treatment effects were reported with 95% confidence intervals. Two-tailed P values ≤ .05 were considered statistically significant. Conclusion Positive results from this trial, upon completion would provide robust evidence for the efficacy and safety of HHRY pills in treating dysmenorrhea in patients with endometriosis.

Objective To explore the efficacy and safety of medical thoracoscopic bulla volume reduction for the treatment of chronic obstructive pulmonary disease(COPD)combined with giant emphysematous bullae(GEB).Methods A total of 66 patients with COPD combined with GEB were enrolled in the study.All the subjects received treatment at Zhengzhou Central Hospital affiliated with Zhengzhou University between March 2021 and December 2022.The subjects were divided into two groups,a medical thoracoscope group consisting of 30 cases treated with medical thoracoscopic bulla volume reduction and a surgical thoracoscope group consisting of 36 cases treated by video-assisted thoracoscopic surgery.All patients were followed up before discharge and 3 months and 6 months after discharge.The preoperative and postoperative levels of the pulmonary function,6-minute walk distance(6MWD),and St.George's Respiratory Questionnaire(SGRQ)scores and differences in postoperative complications were compared between the two groups.The operative duration,postoperative length-of-stay,and surgical costs and hospitalization bills,and the maximum visual analog scale(VAS)pain scores at 24 h after the procedure were assessed.Results The baseline data of the two groups were comparable,showing no statistically significant difference.The forced expiratory volume in 1 second(FEV1)6 months after the procedures improved in both the medical thoracoscopy group([0.78±0.29]L vs.[1.02±0.31]L,P＜0.001)and the surgical thoracoscopy group([0.80±0.21]L vs.[1.03±0.23]L,P＜0.001)compared to that before the procedures.Improvements to a certain degree in 6MWT and SGRQ scores were also observed in the two groups at 3 months and 6 months after the procedures(P＜0.05).In addition,no statistically significant difference in these indexes was observed during the follow-up period of the patients in the two groups.There was no significant difference in operating time between the two groups.The medical thoracoscopy group had shorter postoperative length-of-stay([7.3±2.6]d)and 24-hour postoperative VAS pain scores(3.0[2.0,3.3])than the surgical thoracoscopic group did([10.4±4.3]d and 4.5[3.0,5.0],respectively),with the differences being statistically significant(P＜0.05).Surgical cost and total hospitalization bills were lower in the medical thoracoscopy group than those in the surgical thoracoscopy group(P＜0.05).The complication rate in the medical thoracoscopy group was lower than that in the surgical thoracoscopy group(46.7%vs.52.8%),but the difference was not statistically significant.Conclusion Medical thoracoscopic reduction of bulla volume can significantly improve the pulmonary function,quality of life,and exercise tolerance of patients with COPD combined with GEB,and it can reduce postoperative short-term pain and shorten postoperative length-of-stay.The procedure has the advantages of minimal invasiveness,quick recovery,and low costs.Hence extensive clinical application is warranted.

Objective To investigate the distribution and antibiotic resistance profiles of the bacterial strains isolated from urine samples in a tertiary Traditional Chinese Medicine hospital in Shanghai in the 12-year period from 2010 through 2021 for better empirical antibiotic use in clinical practice.Methods The clinical data of patients with urinary tract infection,including the species and antibiotic resistance of the bacterial strains isolated from urine samples from January 2010 to December 2021 were retrospectively analyzed.Results A total of 5 231 nondupliate bacterial strains were isolated,among which E.coli was the most common(52.8％),followed by Enterococcus spp(19.1％)and Klebsiella spp(11.1％).Most of the urinary isolates(76.0％)were isolated from the elderly aged 60-89,and only 3.1％of the strains were isolated from the young people aged under 44.Most of the bacterial strains were isolated from female patients(75.8％),however,more P.aeruginosa and A.baumannii were isolated from male patients compared to female patients(55.3％vs 44.7％and 67.7％vs 32.3％).About 13.7％of the strains were collected from the Department of Nephrology,more than the strains from any other clincial department.In intensive care unit(ICU),the proportion of E.coli was the lowest,while the proportion of Enterococcus spp was the highest.Enterobacterales showed lower resistance raets to carbapenems,cephamycin,amikacin,cefepime,and β-lactam/β-lactamase inhibitor combinations.P.aeruginosa showed higher susceptibility rates to carbapenems,aminoglycosides and β-lactam/β-lactamase inhibitor combinations compared to A.baumannii(≥54.6％vs≤39.8％).Gram-positive cocci were highly sensitive to glycopeptide antibiotics.No SS.aureus or E.faecalis isolates were found resistant to vancomycin.About 2.7％of the E.faecium isolates were resistant to vancomycin.The prevalence of drug-resistant bacteria was the highest in elderly patients,and in the strains isolated from ICU and emergency department.Conclusions Compared with general hospitals,a high proportion of elderly patients were treated in this hospital.It should be more cautious in the treatment of patients with urinary tract infection.The major bacterial species isolated from urine were E.coli,Enterococcus,and K.pneumoniae.Empirical treatment should be prescribed considering patient age and gender as well as the species and distribution of pathogenic bacteria.Urine culture and antibiotic susceptibility testing should be performed proactively.Appropriate antimicrobial agents should be selected according to the results of antimicrobial susceptibility testing.Antimicrobial ressitance surveillance should also be strengthened.
