1.Analysis of the application status of prescription pre-review systems in Yunnan province

OBJECTIVE To investigate the application status of prescription pre-review systems in healthcare institutions of Yunnan province, evaluate their system functions and management capabilities, and provide a practical basis for promoting rational drug use. METHODS A questionnaire survey was conducted among public healthcare institutions at or above the secondary level in Yunnan province to investigate the deployment status of the systems. A capability maturity assessment framework was constructed, encompassing 6 dimensions and 39 indicators, including real-time prescription review, prescription correlation review, rule setting, evidence-based information support, prescription authority management, and system operation management. This framework was then used to evaluate the institutions that had implemented the pre-review systems. RESULTS A total of 100 valid questionnaires were collected, with 37 institutions having adopted prescription pre-review systems, mainly tertiary hospitals. The system predominantly adopted a modular architecture and was embedded into the hospital information system through application programming interfaces and middleware, providing certain capabilities for real-time prescription risk identification. Evaluation results indicated that basic functions such as reviewing indications, contraindications, and drug compatibility performed well, while deficiencies remained in functions related to parenteral nutrition prescription, review of drug dosage for specific diseases, individual patient characteristic recognition, and rule setting. Moreover, the construction of review centers and establishment of management systems were also not well-developed. CONCLUSIONS The overall application rate of prescription pre-review systems in Yunnan province remains low. System functions and management mechanisms require further improvement. It is recommended to enhance information infrastructure in lower-level institutions and explore regionally unified review models to promote standardized and intelligent development of prescription review practices.
2.Effect of peripheral blood FGF21 levels on glucose uptake and utilization by neu-trophils in perinatal dairy cows

The decrease in blood glucose in perinatal dairy cows affects the energy supply and im-mune function of peripheral blood neutrophils(PMN).Fibroblast growth factor 21(FGF21)is an important regulator of glucose metabolism,and serum FGF21 levels in some perinatal cows are significantly elevated after farrowing.In order to explore the effect of FGF21 on PMN glucose ho-meostasis in perinatal dairy cows,the cows were divided into high FGF21 group(high FGF21,n=8,FGF21>800 ng/L)and low FGF21 group(low FGF21,n=8,FGF21<200 ng/L)within 3 weeks postpartum.The results showed that compared with the low FGF21 group,the glucose up-take of PMN and the mRNA expression of glucose transporters SLC2A1 and SLC2A3 were signifi-cantly increased in the high FGF21 group.The activities of phosphofructokinase 1(PFK1)and glu-cose-6-phosphate dehydrogenase(G6PDH)were significantly increased,and the activity of glyco-gen synthase(GCS)was significantly decreased in PMN in the high FGF21 group.The lactate con-tent and ATP content of PMN were significantly increased,and the mRNA and protein expressions of hexokinase(HK2)and PFK1 were significantly increased in the high FGF21 group.These re-sults indicated that the uptake and utilization of glucose by PMN in perinatal peripheral blood FGF21 increased to ensure the ATP supply of PMN,which provided a theoretical basis for the pro-posal of a new strategy to alleviate immunosuppression in perinatal dairy cows.
3.Qualitative study on the path of high-quality development in public hospitals:perspectives of hospital managers and experts in hospital management field

Objective This study aims to summarize the situation,development experiences,and existing problems in the development of Hospital S,and provide relevant suggestions as a reference for the high-quality development path of public hospitals.Methods Thirteen managers of Hospital S(including hospital leaders,functional departments,and clinical depart-ment heads)and eight experts in the field of hospital management were selected as research subjects.The phenomenological re-search method of qualitative research was used,and semi-structured interviews were conducted to collect relevant data.The Co-laizzi phenomenological data analysis method was used to analyze the interview data.Results Through the analysis of the inter-view data,four themes related to high-quality development were identified:medical care,scientific research and teaching,tal-ents,and disciplines.Each theme has specific areas that need priority and focused attention.Under the theme of medical care,there are four sub-themes:medical quality,medical safety,medical technology,and medical services.Scientific research and teaching,as a strategic support for promoting the hospital's sustained high-quality development,includes four sub-themes:re-search innovation,achievement transformation,teaching system,and mentor team.Talents,as the core competitiveness of the hospital,include three sub-themes:top-level design,high-level talents,and training system.The discipline theme includes four sub-themes:development ideas,development opportunities,development directions,and discipline leaders.Conclusion Public hospitals are facing new situations in high-quality development,and accelerating the improvement of medical service capabilities is a direct manifestation of high-quality development.It is necessary to promote key work such as research innovation,teaching management,talent team building,and discipline construction in a coordinated manner to achieve synchronous improvement in quality and efficiency.
4.Mediating effect of job engagement between nurses′ perceived authentic leadership and their innovative behavior

Objective:To investigate the mediating effect of work engagement on the relationship between nurses′ perception of authentic leadership and their innovative behavior, providing insights for enhancing nurses′ innovative practices.Methods:A convenience sampling method was employed to recruit registered nurses from 37 hospitals in Anhui Province between January and March 2022. The Authentic Leadership Questionnaire, Utrecht Work Engagement Scale and Nurse Innovative Behavior Scale were utilized to conduct a cross-sectional survey. SPSS 25.0 was used to analyze the correlations among nurses′ perception of authentic leadership, work engagement, and innovative behavior, while AMOS 26.0 was employed to assess the mediating effect of work engagement between nurses′ perception of authentic leadership and innovative behavior.Results:A total of 3 200 questionnaires were distributed, and 2 994 valid questionnaires were collected. Among the 2 994 participating nurses, 2 907 (97.1%) were female and 87 (2.9%) were male, 1 002 (33.5%) aged 22-25 years, 1 374 (45.9%) aged 26-39 years, 467 (15.6%) aged 40-49 years, and 151 (5.0%) aged 50 years or older. The total scores of the Authentic Leadership Questionnaire, Utrecht Work Engagement Scale and Nurse Innovative Behavior Scale were (68.23 ± 13.15), (51.49 ± 11.65) and (38.28 ± 8.35) points, respectively. Pearson correlation analysis revealed significant positive correlations between nurses′ perception of authentic leadership and work engagement ( r=0.473, P<0.01), between nurses′ perception of authentic leadership and innovative behavior ( r=0.530, P<0.01), and between work engagement and innovative behavior ( r=0.553, P<0.01). The mediating effect analysis indicated that work engagement partially mediates the relationship between nurses′ perception of authentic leadership and innovative behavior ( β=0.18, P<0.01), with a mediation effect ratio of 34.62%. Conclusions:Nurses′ perception of authentic leadership can indirectly influence their innovative behavior through work engagement. It is recommended that nursing managers prioritize the enhancement of authentic leadership and foster work engagement among nurses to stimulate their innovative behavior.

8.Preparation and preliminary of astragalus polysaccharide synbiotics and its effect on growth performance of calves

In this study,freeze-drying methods was used to prepare astragalus polysaccharide synbi-otic powder.The optimized freeze-dried protective agent formulation using response surface meth-odology in this study was 17.99%skimmed milk powder,7.26%sucrose,8.04%galactooligosac-charides,and 9.64%astragalus polysaccharide,and the remaining amount was distilled water.The survival rate of lactic acid bacteria in the prepared bacterial powder was 88.06%.A calf feeding ex-periment was conducted over 28 days with thirty healthy newborn calves with similar age,the calves were randomly assigned to either control group and the astragalus polysaccharide synbiotic group,each comprising 15 calves.The astragalus polysaccharide synbiotic group received a supple-ment of astragalus polysaccharide in addition to their regular feed.Compared to the control group,the results showed that:(1)Astragalus polysaccharide synbiotics significantly increased the aver-age daily gain(ADG)of calves and decreased the fecal score and diarrhea rate in calves.(2)By tes-ting the immunological indexes,we found that astragalus polysaccharide synbiotics significantly in-creased the serum level of IgA,IgG,IgM,IL-2 and IL-10,and decreased the level of IL-6 and TNF-α.(3)Astragalus polysaccharide synbiotics notably enhanced the Chao1,Sobs,and Shannon indi-ces,while reducing the Simpson index.At the phylum level,astragalus polysaccharide synbiotics group showed a significant increase in the relative abundance of Cyanobacterium and Firmicutes,and a decrease in Proteobacteria.At the genus level,the relative abundance of Esche-richia-Shigella in astragalus polysaccharide synbiotic group significantly decreased.Whereas norank_f__norank_o__Clostridia_UCG-014 and Butyricicoccus significantly increased.In conclu-sion,this study successfully formulated an astragalus polysaccharide synbiotic powder.The powder can significantly promote the growth of calves,enhance immune function,improve the structure of gut microbiota,and reduce the incidence of calf diarrhea.
9.Echocardiographic manifestations of infective endocarditis complicated with valve damage in children

Objective To observe the echocardiographic manifestations of infective endocarditis(IE)complicated with valve damage in children.Methods Totally 104 children with IE were retrospectively enrolled and divided into non-damage group(n=34),mild damage group(n=39)and dysfunction group(n=31)according to whether complicated with valve damage and damage's degree.The general and echocardiographic data were compared among groups,and the echocardiographic characteristics of IE complicated with valve damage in children were analyzed.Results Significant difference of the proportion of combining with other congenital heart diseases(excluding bicuspid aortic malformations),the incidence of embolization events during hospital stay,also of endocarditis of left cardiac system were found among groups(all P<0.05).Pairwise comparison showed that in non-damage group,the proportion of combining with other congenital heart diseases was higher,while the incidence of endocarditis of left cardiac system was lower than those in both mild damage group and dysfunction group(all P<0.05).The incidence of embolization events during hospital stay in non-damage group was lower than that in dysfunction group(P<0.05).Among 70 cases of IE complicated with valve damage,mitral valve(30/70,42.86%)was the most common involved valve,mostly presented as valve stenosis(63/70,90.00%).No significant difference of valve involvement site,valve structural lesions nor the incidence of valve stenosis was found between mild damage group and dysfunction group(all P>0.05).Conclusion IE complicated with valve damage in children mostly involved left cardiac system,and the risk of embolization events was higher than that of IE children without valve damage.Echocardiography could be used as an important method for evaluating the site of valve involvement and the degree of damage.
10.Observation on isolated bicuspid aortic valve in children using echocardiography

Objective To observe the manifestations of isolated bicuspid aortic valve(i-BAV)in children using echocardiography.Methods Echocardiographic data of 79 children with i-BAV were retrospectively analyzed,and classification of i-BAV was performed.The patients were divided into complication group(n=50)and non-complication group(n=29)according to the existence of valve and/or aortic involvement or not,and echocardiographic parameters were compared between groups.Results After adjusting body surface area(BSA),in complication group,left ventricular end-systolic diameter/BSA was lower,while left ventricular myocardial mass index group was higher than those in non-complication group(both P<0.05).Type 0 i-BAV was found in 22 cases,with lat subtype as the most common ones(18/22,81.82%),while Type Ⅰ was observed in 57 cases with L-R subtype as the most common ones(39/57,68.42%).The most common subtype in complication group was also Type Ⅰ L-R(31/50,62.00%),with incidence of valve involvement of 90.00%(45/50),mainly including mild aortic stenosis and/or incompetence(37/45,82.22%),and incidence of aorta involvement of 24.00%(12/50),all with type Ⅰ or Ⅱ aortic widening.Conclusion The most common subtype of i-BAV in children was type Ⅰ L-R,with mild valve damage as the main complication and possibility of left ventricular myocardial remodeling.
