1.Hypofractionated radiotherapy combined with transcatheter arterial chemoembolization for the treatment of metachronous hepatic oligometastasis from nasopharyngeal carcinoma:analysis of its clinical effect

Objective To explore the effectiveness and safety of hypofractionated radiotherapy(HFRT)combined with transcatheter arterial chemoembolization(TACE)for metachronous hepatic oligometastasis from nasopharyngeal carcinoma(NPC).Methods The clinical data of patients with metachronous hepatic oligometastasis from NPC,who received HFRT combined with TACE treatment at the Sichuan Provincial Cancer Hospital of China from January 2012 to October 2022,were retrospectively analyzed.The 3-year and 5-year overall survival(OS),progression-free survival(PFS),local recurrence-free survival(LRFS),no extrahepatic distant metastasis survival(EMFS),and treatment-related adverse reactions were analyzed.Results A total of 55 patients were enrolled in this study,including 36 males and 19 females,the median age at the time of occurring metachronous hepatic oligometastasis was 44(27-75)years.The 3-year and 5-year OS,PFS,LRFS,EMFS were 67.8%and 40.0%,55.8%and 30.0%,72.7%and 56.5%,63.6%and 56.5%,respectively.The subgroup analysis indicated that the treatment course of TACE ≥3 cycles could significantly improve the PFS of patients with oligometastasis.HFRT combined with TACE treatment was well tolerated by all patients,and the incidence of Grade Ⅲ-Ⅳadverse reactions was quite low.Conclusion For the treatment of metachronous hepatic oligometastasis from NPC,HFRT combined with TACE is clinically effective,besides,the patients can well tolerate the therapeutic scheme.

Objective To investigate the impact of the dominant deafness point mutation p.D179N on the oli-gomeric equilibrium state of Connexin 26(Cx26).Methods The wild-type Cx26 fusion protein(Cx26-WT-GFP)and mutant fusion proteins(Cx26-D179N-GFP,Cx26-D179C-GFP)were expressed in HEK293F cells.By using Fluorescence-detection size-exclusion chromatography(FSEC)and size-exclusion chromatography(SEC)to analysis the oligomeric state of the target protein based on malecular weight under the condition of solubilization and purifica-tion respectively.Cryo-electron microscopy(Cryo-EM)single particle analysis(SPA)was conducted to analysis the target protein's oligomeric states based on the 2D classification morphology of the protein particles.Results In vitro,the wild-type Cx26 protein(Cx26-WT)is almost exclusively dodecameric.The deafness mutation p.D179N protein(Cx26-D179N)exists as both dodecamers and hexamers,whereas the artificial mutation p.D179C protein(Cx26-D179C)does not form dodecamers.Conclusion The dominant deafness mutation GJB2 p.D179N could weaken the ability of docking between hexameric proteins,which could affect the balance between hexamers and do-decamers.

As a physical treatment technique, modified electro-convulsive therapy (MECT) is used to treat mental and certain neurological disorders by causing seizures with short, suitable electrical currents applied to the brain while the patient is under general anesthesia and muscle relaxants. MECT is recognized for its therapeutic efficacy and clinical safety, rendering it one of the most prevalent interventions in psychiatric care. To enhance clinical outcomes and minimize adverse effects, this consensus document delineates the indications, therapeutic parameters, therapeutic procedures, potential adverse effects, and associated management strategies for MECT. These guidelines are informed by the latest clinical research and expert consensus, integrating evidence-based medicine methodologies. The objective is to furnish clinicians with precise operational guidelines and to advance the standardization of MECT practices in clinical settings.
4.Clinicopathological analysis of renal cell carcinoma with hemangioblastoma components

Renal cell carcinoma with hemangioblastoma components (RCC-HBc) is a rare subtype of renal cell carcinoma reported in recent years. This tumor is rare. This article reports 1 case of RCC-HBc. A solid mass in the right kidney was found during physical examination. CT showed a high possibility of renal cancer.Laparoscopic partial nephrectomy was performed. After tumor resection, the pathological diagnosis was clear cell renal cell carcinoma with hemangioblastoma components. There was no tumor recurrence or metastasis 33 months after surgery.
5.Application of quantitative flow ratio in radial artery grafting

Objective:To investigate the use of quantitative flow ratio(QFR) for selecting suitable anastomosis targets of radial artery in coronary artery bypass grafting.Methods:From January 2019 to June 2023, clinical data of patients undergoing coronary artery bypass grafting with using radial artery obtained from Beijing Anzhen Hospital were retrospectively collected. A total of 186 patients were included in this study, including 175 males and 11 females. The average age was(54.26±7.91) years, ranging from 34 to 70 years. They were divided into study group(n=46) and control group(n=140) according to whether QFR was examined before operation. The distal radial artery in the study group was anastomosed to a non-LAD artery with the lowest QFR value, while in the control group was randomly anastomosed to the most important non-LAD artery with the anatomic stenosis greater than 75%. All patients were followed up. The endpoint event was major adverse cardiovascular events(MACE) and radial grafts stenosis/occlusion. The preoperative baseline data, perioperative indexes and follow-up were analyzed and compared.Results:There was no significant difference in preoperative baseline data between the two groups. The pulse index(PI) in the study group was significantly lower than that in the control group(1.88±0.45 vs. 2.11±0.61, P<0.05). There were no serious perioperative complications in both groups. All patients survived and were discharged from hospital. The average follow-up time was 30 months. There were no significant differences in cardiogenic death, stroke, recurrent myocardial infarction, and revascularization between the two groups. The rate of recurrent angina in the study group was significantly lower than that in the control group(8.7% vs. 22.1%, P<0.05). The complete patency rate of radial graft in the study group was significantly higher than that in the control group(96.4% vs. 86.0%, P<0.05), the occlusion rate was significantly lower than that in the control group(0 vs. 9.4%, P<0.05), and the stenosis rate had no significant difference between two groups. Conclusion:QFR can clarify the functional changes of coronary blood flow, select suitable anastomosis target for radial graft, reduce the occurrence of competing flow, and then improve the patency rate and clinical prognosis.
6.HIV-1 pretreatment drug resistance and molecular transmission network characteristics in Yubei District,Chongqing

Objective To analyze the characteristics of HIV-1 pretreatment drug resistance(PDR)and molecular transmission networks in Yubei District,Chongqing,providing evidence for targeted interventions.Methods Using a cross-sectional design,plasma samples were collected from HIV/AIDS patients receiving antiretroviral therapy(ART)in Yubei District from January 2022 to December 2023.Pol gene fragments were extracted and amplified for HIV-1 genotyping and drug resistance analysis.Molecular transmission networks were constructed based on genetic distance calculations.Results Among 478 HIV-1 pol sequences,eight geno-types were identified:with CRF07_BC(60.4%,289/478),CRF08_BC(15.5%,74/478),CRF01_AE(11.7%,56/478),and CRF85_BC(5.9%,28/478).The overall PDR rate was 6.3%(30/478),with resistance to nucleoside reverse transcriptase inhibitors(NRTIs)and non-nucleoside reverse transcriptase inhibitors(NNRTIs)at 1.7%(8/478)and 5.2%(25/478),respectively.No protease inhibitor(PI)resistance was de-tected.The molecular network included 177 cases(37.0%network entry rate),forming 53 clusters with 198 connections.Cluster sizes ranged from 2 to 17 nodes,and 75.3%(149/198)of connections were associated with five subdistricts/towns:Shuanglonghu Street,Huixing Street,Luoqi Town,Gulu Town,and Baoshenghu Street.Conclusion HIV-1 genotypes in Yubei District exhibit diversity and complexity,with moderate PDR prevalence.Regional clustering of transmission networks suggests the need for enhanced molecular surveil-lance and targeted interventions based on analytical findings.
7.Correlation between HMGB1 and inflammation,insulin resistance and placental vascular density in patients with gestational diabetes mellitus

Objective To explore the level characteristics of high mobility group protein B1(HMGB1)in patients with gestational diabetes mellitus(GDM)and its correlations with inflammation,insulin resistance,and placental vascular density.Methods A total of 70 patients with GDM who were hospitalized and delivered in the department of obstetrics of this hospital from August 2021 to May 2024 were selected as the study group,and another 70 healthy pregnant women during the same period were selected as the control group.The general clinical data,fasting plasma glucose(FPG),insulin resistance-related indicators[fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR)],serum inflammatory factors[C reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],the levels of HMGB1 in pla-cental tissues and peripheral blood,and the placental microvascular density were compared between the two groups.Pearson correlation analysis was used to evaluate the correlations between the levels of HMGB1 in se-rum and placental tissues and insulin resistance,inflammatory factors,and placental vascular density.Results The levels of FPG,FINS,HOMA-IR,CRP,TNF-α,IL-6,the average integrated optical density(IOD)value of HMGB1 in serum and placental tissues,and the percentage of HMGB1-positive cells in the study group were all higher than those in the control group(P<0.05),while the placental microvascular den-sity was lower than that in the control group(P<0.05).Pearson correlation analysis showed that the serum HMGB1 level,the average IOD value of HMGB1 in placental tissues,and the percentage of HMGB1-positive cells in the study group were all positively correlated with the levels of HOMA-IR,CRP,TNF-α,and IL-6(P<0.05),and negatively correlated with the placental microvascular density(P<0.05).Conclusion The level of HMGB1 in peripheral blood and placental tissues of GDM patients is higher than that of healthy preg-nant women,and it is correlated with inflammatory factors,HOMA-IR and the placental microvascular density.
