1.Meta-analysis of the efficacy and safety total glucosides of paeonia in the treatment of systemic lupus erythematosus

OBJECTIVE To evaluate the efficacy and safety of total glucosides of paeonia (TGP) in the treatment of systemic lupus erythematosus (SLE). METHODS Randomized controlled trial (RCT) about TGP combined with western medicine versus western medicine alone for SLE treatment were retrieved from PubMed, Embase, Cochrane Library, Web of Science, CNKI, VIP, Wanfang Data, and CBM. The search period spanned from the inception of each database to June 1, 2025. After literature screening, data extraction, and quality assessment of the included studies, Meta-analysis was performed using RevMan 5.4 software. RESULTS Fifteen RCTs, involving 1 318 patients, were included. Meta-analysis results showed that compared with western medicine alone, TGP combined with western medicine significantly improved clinical efficacy [OR=4.96, 95%CI(3.41, 7.23), P<0.000 01], complement 3 [MD=0.18, 95%CI (0.13, 0.23), P<0.000 01] and complement 4[MD=0.08, 般021) 95%CI (0.04, 0.11), P<0.000 01], and reduced the levels of immunoglobulin G (IgG) [MD=-3.10, 95%CI (-3.59,-2.62), P<0.000 01], IgA [MD=-0.68, 95%CI (-0.78, -0.58), P<0.000 01], IgM [MD=-0.43, 95%CI (-0.53,-0.34), P<0.000 01], systemic lupus erythematosus disease activity index (SLEDAI) [MD=-1.59, 95%CI (-2.20, -0.99), P<0.000 01], recurrence rate [OR=0.23, 95%CI (0.13, 0.42), P<0.000 01] and the incidence of adverse drug reactions [OR= 0.54, 95%CI (0.36, 0.82), P=0.004]. CONCLUSIONS TGP therapy can improve clinical efficacy of SLE patients, promote the restoration of immunoglobulins and complements, reduce SLEDAI and recurrence rate and has good safety.
2.Mechanism of modified Lichong decoction on apoptosis of transplanted tumor cells in nude mice via Fas/FasL pathway and caspase apoptosis pathway

OBJECTIVE To investigate the effect and mechanism of modified Lichong decoction (MLCD) on the apoptosis of transplanted tumor cells in nude mice. METHODS Human gastric cancer AGS cells were cultured, and a nude mice transplanted tumor model was established. The nude mice were divided into the model group and MLCD low-, medium- and high-dose groups (150, 300, 600 mg/kg). They were given distilled water or the corresponding drug solution by gavage once daily for four consecutive weeks. The size of transplanted tumors in nude mice was measured every six days, and the tumor volume was calculated. After the medication, the nude mice were sacrificed, and the transplanted tumor tissues were isolated. The contents of lactate dehydrogenase (LDH) and reactive oxygen species (ROS) in the transplanted tumor tissues were detected, and the changes in mitochondrial membrane potential were assessed. The pathological morphological changes were observed. The enzymatic activities of caspase-3, caspase-8, and caspase-9, as well as protein expressions of Fas and FasL and mRNA expressions of caspase-3, caspase-8, caspase-9, Fas and FasL in the transplanted tumor tissues, were detected. RESULTS Compared with the model group, the volume of transplanted tumors in nude mice from all MLCD dose groups was reduced to varying degrees. The contents of LDH and ROS, as well as the enzymatic activities of caspase-3, caspase-8 and caspase-9, were significantly increased/enhanced. The mitochondrial membrane potential was significantly decreased. The protein expressions of Fas and FasL, and the mRNA expressions of caspase-3, caspase-8, caspase-9, Fas and FasL were significantly up-regulated. Most of these differences were statistically significant ( P <0.05 or P <0.01). Pathological results showed that with increasing doses of MLCD, the cellular density in the transplanted tumor tissues gradually decreased, and typical morphological features of apoptosis, such as loosening and increasing fragmentation, became more prominent. CONCLUSIONS MLCD can induce apoptosis in transplanted tumor cells of nude mice, and its mechanism may be related to the activation of the Fas/FasL pathway and the caspase apoptotic pathway.
3.Disease burden and changing trends of bladder cancer in China and globally in 1992 - 2021

Objective To analyze the disease burden and trends of bladder cancer in China and globally from 1992 to 2021. Methods Using the GBD 2021 database, the incidence, prevalence, mortality, and disability-adjusted life years (DALYs) rates of bladder cancer in China and globally from 1992–2021 were analyzed. Average annual percentage change (AAPC) was calculated using Joinpoint regression. Subgroup analyses by sex and age were conducted, and a Bayesian age-period-cohort (BAPC) model was used to predict trends in age-standardized incidence rate (ASIR) and age-standardized mortality rate (ASMR) for the next 15 years. Results In 2021, China reported 106 000 new cases (ASIR: 5.14/100 000), 571 000 prevalent cases (age-standardized prevalence rate, ASPR: 26.61/100 000), 43 000 deaths (ASMR: 2.34/100 000), and a DALY rate of 45.31/100 000. From 1992–2021, China showed upward trends in ASIR and ASPR but declines in ASMR and DALYs, while global ASIR, ASMR, and DALYs decreased overall with slow ASPR growth. The peak cases in China and globally were both concentrated in the 65-79 age group, with a significantly higher burden on males than females. In China, smoking-related ASMR and ASDR exceeded global averages and rose, whereas high glucose-related indexes were lower and declined. Projections for 2021–2036 indicated that the global incidence and mortality rates would be rising, but ASIR/ASPR would be declining, while in China, the incidence rate would continue to rise, and the mortality rate will stabilize, with a significant increase in ASIR and a gradual decrease in ASPR. Conclusion From 1992 to 2021, the incidence of bladder cancer in China has shown a continuous upward trend and is projected to persist in the future, with significant gender and age differences. Particular attention should be given to elderly males aged 85-89. The disease burden of bladder cancer attributable to smoking continues to rise, highlighting the urgent need to strengthen tobacco control policies.
4.Advances in application of lens constant optimization for IOL power calculation

With the advent of the refractive era of cataract surgery, postoperative residual refractive error has become a key factor affecting visual quality. The accuracy of intraocular lens(IOL)power calculation is affected by multiple factors, including ocular biological parameters, calculation formulas, and lens constants. Currently, the lens constants widely used in clinical practice are mostly based on Western populations, which have a mismatch with the ocular characteristics of the Chinese population, such as shorter axial length and steeper cornea. Therefore, the personalized optimization of the constant has become a research hotspot. This review primarily summarizes the research progress on lens constant optimization in improving the accuracy of IOL power calculation. Currently, there is no consensus on lens constant optimization methods. Single-constant formulas can be optimized through iterative methods, while multi-constant formulas require the combination of linear or nonlinear strategies. Simplified gradient descent and data-driven techniques offer new optimization pathways, but cross-validation is needed to evaluate their performance. Segmented optimization based on axial length, corneal curvature, and anterior chamber depth has shown significant effectiveness in patients with special anatomical structures, effectively reducing postoperative refractive errors in different patient groups, but limitations remain in some extreme cases. The interaction of multiple parameters significantly impacts the predictive accuracy of complex cases. While new-generation formulas integrating multiple parameters or incorporating AI algorithms have improved accuracy, constant optimization still holds value. Currently, the complex relationships between multiple parameters and intraoperative dynamic changes require further in-depth research. Future research, including targeted population optimization studies, the development of real-time monitoring technologies, and innovative IOL designs, may make achieving zero postoperative refractive error a possibility.
5.Research progress on escape-related strategies for preventing complex upper ureteral calculi

During ureteroscopic holmium laser lithotripsy for complex upper ureteral calculi, stone fragments are prone to migrate into the kidney, leading to a decreased primary stone-free rate and increased need for secondary surgeries and healthcare costs.Based on recent clinical studies and literature evidence, this article summarizes and compares strategies for preventing stone escape during ureteroscopic treatment.Comprehensive analysis of existing research indicates that auxiliary measures, such as instrument coordination, physical interception, or negative pressure suction, demonstrate potential clinical value in preventing stone migration, with devices like the NTrap basket showing relatively prominent efficacy.However, most related studies are single-center retrospective analyses, and further validation through multicenter randomized controlled trials is needed to guide individualized treatment selection.This review outlines advances in the prevention and management of stone escape for clinical reference.
6.A preliminary study on a method for detecting accelerator isocenter deviation using an optical surface monitoring system

Objective To detect the isocenter of a medical linear accelerator using an optical surface monitoring system (OSMS) combined with a self-made 3D-printed phantom, and compare with the conventional front pointer measurement method. Methods A phantom that could be fixed to the front pointer of the accelerator was fabricated using 3D printing technology. The front pointer method and OSMS were used to detect the rotation isocenter deviations of the accelerator. During the experiments, the collimator, gantry, and couch linear displacement readings were recorded at different angles. We also recorded the measurement time of different surveyors. Results Compared to the front pointer measurement method, the OSMS method showed lower standard deviations. The reductions in standard deviations showed statistical significance in the Lat direction for the collimator rotation isocenter as well as in the Lat and Vrt directions for the gantry rotation isocenter. The mean deviation of Mag values for the collimator rotation isocenter using the OSMS method was 0.50 mm, which was larger than the 0.42 mm using the front pointer method. However, the standard deviation was 0.06 using the OSMS method, which was smaller than the 0.18 using the front pointer method. The mean deviation and standard deviation of Sag values for the gantry rotation isocenter using the OSMS method were 1.03 mm and 0.12, respectively, which were both larger than 0.47 mm and 0.06 using the front pointer method. For the couch rotation isocenter, the mean deviation and standard deviation using the OSMS method were 0.32 mm and 0.04, respectively, which were both smaller than the 0.55 mm and 0.20 using the front pointer method. All surveyors spent less time in isocenter measurements using the OSMS method compared to the front pointer method. Conclusion Compared with the front pointer method, the OSMS method significantly reduces the standard deviation among surveyors, effectively reduces the influence of the angle deviation of the accelerator on the measurement results, reduces detection time, improves detection efficiency, and provides a powerful practical basis for the application of OSMS in detecting the mechanical accuracy of the linear accelerator.

9.Immunogenic cell death-related key gene screening and validation in primary sjögren's syndrome

ObjectiveTo systematically screen and validate key immunogenic cell death (ICD)-related genes that play important roles in primary sjögren’s syndrome (pSS) using multi-omics data. MethodsGene expression profiles of pSS patients and healthy controls (GSE66795, 131pSS patients/29 healthy controls) were obtained from the Gene Expression Omnibus (GEO) database. Candidate genes were selected by intersecting these profiles with known ICD core gene sets and pSS-related targets. The CIBERSORT algorithm was used to analyze immune cell infiltration characteristics. Validation of gene expression was performed using transcriptomic data from sorted CD8⁺ T cell subsets (GSE93683). Finally, labial gland tissues and peripheral blood samples from 10 pSS patients and 4 healthy controls were collected, and key genes were experimentally validated by reverse transcription quantitative polymerase chain reaction (RT-qPCR). ResultsSeven intersecting genes from the ICD-differentially expressed genes (DEGs)-pSS intersection were identified [caspase 1 (CASP1), forkhead box P3 (FOXP3), interferon-gamma (IFNG), myeloid differentiation primary response 88 (MYD88), toll-like receptor 7 (TLR7), DExD/H-box helicase 58 (DDX58), and interferon induced helicase C domain 1 (IFIH1)]. Immune infiltration analysis showed that in pSS labial gland tissues, the proportions of naïve B cells and dendritic cells significantly increased, while the proportions of resting NK cells and M0 macrophages decreased (P<0.05). Transcriptomic data of CD8⁺T cell subsets indicated that IFIH1 expression was significantly elevated in central memory CD8⁺T cells (P<0.05). RT-qPCR experimental validation revealed that DDX58, IFIH1, and CASP1 were significantly upregulated in both peripheral blood leukocytes and labial gland tissues of pSS patients (all P<0.05), whereas IFNG expression increased in peripheral blood but decreased in labial gland tissues (P<0.05). ConclusionThis study identified and confirmed DDX58, IFIH1, CASP1, and IFNG as key ICD-related genes in pSS through a combination of bioinformatics and experimental validation, providing new candidate targets for further understanding the immunopathological mechanisms of pSS.
10.Clinical Efficacy of Tangning Tongluo Tablets for Nonproliferative Diabetic Retinopathy

ObjectiveTo observe the clinical efficacy and safety of Tangning Tongluo tablets in the treatment of nonproliferative diabetic retinopathy (DR). MethodsFourteen research centers participated in this study, which spanned a time interval from September 2021 to May 2023. A total of 240 patients with nonproliferative DR were included and randomly assigned into an observation group (120 cases) and a control group (120 cases). The observation group was treated with Tangning Tongluo tablets, and the control group with calcium dobesilate capsules. Both groups were treated for 24 consecutive weeks. The vision, DR progression rate, retinal microhemangioma, hemorrhage area, exudation area, glycosylated hemoglobin (HbA1c) level, and TCM syndrome score were assessed before and after treatment, and the safety was observed. ResultsThe vision changed in both groups after treatment (P<0.05), and the observation group showed higher best corrected visual acuity (BCVA) than the control group (P<0.05). The DR progression was slow with similar rates in the two groups. The fundus hemorrhage area and exudation area did not change significantly after treatment in both groups, while the observation group outperformed the control group in reducing the fundus hemorrhage area and exudation area. There was no significant difference in the number of microhemangiomas between the two groups before treatment. After treatment, the number of microhemangiomas decreased in both the observation group (Z=-1.437, P<0.05) and the control group (Z=-2.238, P<0.05), and it showed no significant difference between the two groups. As the treatment time prolonged, the number of microhemangiomas gradually decreased in both groups. There was no significant difference in the HbA1c level between the two groups before treatment. After treatment, the decline in the HbA1c level showed no significant difference between the two groups. The TCM syndrome score did not have a statistically significant difference between the two groups before treatment. After treatment, neither the TCM syndrome score nor the response rate had significant difference between the two groups. With the extension of the treatment time, both groups showed amelioration of TCM syndrome compared with the baseline. ConclusionTangning Tongluo tablets are safe and effective in the treatment of nonproliferative DR, being capable of improving vision and reducing hemorrhage and exudation in the fundus.
