1.Research advances in hepatitis B virus-associated glomerulonephritis

There is are large number of patients with chronic hepatitis B virus (HBV) infection. HBV not only damages the liver, but also involves the kidney. Hepatitis B virus-associated glomerulonephritis (HBV-GN) is secondary glomerulonephritis caused by HBV infection, and it is one of the most common extrahepatic complications of HBV infection. HBV-GN is mainly observed in children and young and middle-aged adults, with varying degrees of proteinuria as the main clinical manifestation, and it may be accompanied by hematuria and hypertension. Membranous nephropathy is the most common pathological type, followed by membrano-proliferative glomerulonephritis and IgA nephropathy. HBV-GN has an insidious onset and lacks characteristic symptoms and pathological manifestations, and thus it may be easily confused with various types of glomerulonephritis, which may lead to missed diagnosis and misdiagnosis. HBV-GN has a complex pathogenesis involving various links such as immune disorders, direct viral damage, and genetics, among which the theory of immune complex deposition has been widely recognized. In recent years, some important advances have been made in the research on the pathogenesis, diagnosis, and treatment of HBV-GN. This article summarizes the above issues, so as to provide a reference for clinical diagnosis and treatment.

4.Synergistic role of JAK/STAT5 and PI3K/AKT signaling pathways in regulating eIF4B in acute leukemia.

Human acute leukemia (AL) is a clonal malignancy with abnormal hematopoietic stem cells. Clinically, AL is very difficult to cure due to its sudden onset and short course of disease progression. Previous studies have shown that eukaryotic initiation factor 4B (eIF4B) plays a critical role in the development of chronic leukemia. However, the involvement of eIF4B in human acute leukemia is still largely unknown. Therefore, we studied eIF4B function and its regulatory mechanism in human acute leukemia. We found that phosphorylation levels of eIF4B in acute leukemia cells were significantly reduced in response to treatment with either LY294002 (PI3K inhibitor), AKTi (AKT inhibitor) or SMI-4A (Pim inhibitor). Co-treatment with inhibitors targeting JAK/STAT5/Pim and PI3K/AKT/mTOR signaling dramatically promoted apoptosis of acute leukemia cells by downregulating eIF4B phosphorylation. Furthermore, in vitro and in vivo functional experiments showed that eIF4B played an important anti-apoptosis role in the acute leukemia cells by regulating the expression of anti-apoptotic proteins Bcl-2 and Bcl-XL. In contrast, silencing eIF4B inhibited the growth of acute leukemia cells as engrafted tumors in nude mice. Taken together, our results indicate the synergistic role of JAK/STAT5/Pim and PI3K/AKT/mTOR signaling pathways in regulating eIF4B phosphorylation in acute leukemia, and highlight eIF4B as a candidate therapeutic target for treatment of acute leukemia.

5.Value of ALBI grade on precised estimation liver reserve function of patients with hepatocellular carcinoma

Précised liver reserve function estimation is of great significance for predicting the survival time, post-hepatectomy liver failure and individualized comprehensive treatment strategies in hepatocellular carcinoma (HCC) patients. Currently, the widely used Child-Pugh (CP) classification and indocyanine green 15-minute retention rate (ICGR 15) have certain flaws and limitations. The albumin-bilirubin (ALBI) grading especially makes up for the deficiency of CP classification, and can provide an objective, simple, accurate and evidence-based method to estimate and guide the liver reserve function of HCC patients. This paper follows up and summarizes the research progress of ALBI grading estimation at home and abroad on liver reserve function of HCC patients.
6. Meta-analysis of the outcomes of associating liver partition and portal vein ligation for staged hepatectomy versus portal vein embolization for the treatment of liver cancer with insufficient future liver remnant

Objective:
To explore the feasibility, safety and efficacy of associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) and portal vein embolization (PVE) for the treatment of liver cancer with insufficient future liver remnant (FLR) .
Methods:
The data regarding the clinical controlled trials in comparison of ALPPS and PVE in liver surgery were collected from the both domestic and international publications searched through the datebases of PubMed, Cochrane Library, Embase, CNKI, and VIP.Meta analysis was performed by RevMan 5.3 software.
Results:

Objective:
To investigate relationship between the clinicopathological features and prognosis of T1 esophageal carcinoma.
Methods:
Data from 212 T1 primary esophageal cancer patients, who underwent radical surgery in The Fourth Hospital of Hebei Medical University from Jan 2001 to Dec 2009 were enrolled. There were 148 males and 64 females. There were 91 patients with stage pT1a and 121 patients with stage pT1b.
Results:
The survival of the 212 patients was 27~108 months, and the median survival was 80.8 months. The 1, 3, and 5 year survival rates of patients with stage T1a were 100%, 97.8% and 94.5%, respectively, and the median survival was 86.8 months. The 1, 3, and 5 year survival rates of patients with stage T1b were 100%, 95.9% and 74.4%, respectively, and the median survival was 76.2 months. The rate of lymph node metastasis in 121 patients with stage T1b was 26.4% (32/121). The lymph node metastasis rates in patients with stage sm1, sm2 and sm3 were 11.6% (3/26), 15.0% (6/40) and 41.8% (23/55), respectively. Objective: To research three-dimensional computed tomography angiography (3D-CTA) combined three-dimensional digital subtraction angiography (3D-DSA) in the diagnosis of intracranial aneurysms. Methods: 3D-CTA and 3D-DSA were performed in 27 cases with Highly suspected in patients with intracranial aneurysms, And compared with the final outcome of surgery. To evaluate 3D-CTA and 3D-DSA in the detection rate of aneurysms, three-dimensional shape and spatial relations ability to display. Research 3D-CTA combined 3D-DSA in the diagnosis of intracranial aneurysms. Results: Twenty-seven cases of suspected patients, 25 cases of surgical treatment of confirmed aneurysms, 28 aneurysms found. 3D-CTA and 3D-DSA rate of aneurysms was no significant difference. 3D-CTA of the aneurysm can be accurately detected clearly shows its three-dimensional shape and spatial relationship with the surrounding skull, 3D-DSA for aneurysm neck, the tumor and the aneurysm and parent artery relationship shows clear and accurate. Conclusion: Imaging of intracranial aneurysm diagnosis, 3D-CTA and 3D-DSA have their respective advantages, combined two methods can improve the detection rate of aneurysms. 3D-CTA combined 3D-DSA have support of design for operation.
