A 42-year-old male with chest tightness and dyspnea was admitted to the hospital. Chest CT indicated diffuse interstitial lung infiltration. Despite receiving anti-infective therapy, glucocorticoid therapy, and immunosuppressive agents, the patient developed refractory hypoxaemia. Endotracheal intubation and invasive mechanical ventilation failed to improve oxygenation. Therefore the patient was diagnosed with rapidly progressive interstitial lung disease (RP-ILD) accompanied by type Ⅰ respiratory failure. Veno-venous (VV) extracorporeal membrane oxygenation (ECMO) was initiated, and oxygenation improved in this patient. The patient subsequently underwent bilateral lung transplantation with veno-arterio-venous (VAV) ECMO support. ECMO machine was withdrawn on day 1, and extubation was achieved on day 9 after surgery. Histopathology revealed fibrotic nonspecific interstitial pneumonia (NSIP) with hyaline membrane formation. The patient developed ICU-acquired myasthenia and received early rehabilitation, with gradual recovery of muscle strength. During follow-up, graft lung function remained stable. This case demonstrates that ECMO can serve as a bridge to lung transplantation in RP-ILD patients.

Diabetes of the exocrine pancreas(DEP) refers to a complication of diseases of the exocrine pancreas, associated with an increased risk of pancreatic cancer. Recent evidence has indicated that DEP, after type 2 diabetes, has become the second most common type of adults-onset diabetes, with a higher prevalence than type 1 diabetes. DEP is not a single condition but encompasses various exocrine pancreatic disorders that lead to hyperglycemia through different mechanisms, creating significant diagnostic and therapeutic challenges. This review examines the latest research on the risk factors and pathogenesis of DEP, aiming to provide insights to improve its clinical management strategies.

Objective To understand the epidemiological characteristics of road traffic accident injuries in Beijing can provide a theoretical basis for improving the pre-hospital emergency service capabilities and levels, and increasing patient survival rates while reducing disability, mortality rates. Methods A retrospective analysis was conducted on the medical records of 9207 patients who made pre-hospital emergency calls due to road traffic accident injuries at the Beijing Emergency Medical Center in 2023 to understand the patients' age, gender, injury time, injury location, injury degree and other characteristics. Results Traumatic diseases ranked first in the classification of emergency medical conditions in Beijing.Among them, road traffic accidents account for 37%.The ratio of male to female patients was 1.32∶1.The largest number of patients were aged 31-40,accounting for nearly 25%.The proportion of underage patients and those aged 71 and above was 12.16%.The differences in gender and age distribution were statistically significant, while the differences in gender distribution among different age groups were not statistically significant.Road traffic accident injuries occured most frequently in September and least in January. There were more cases in summer and autumn, and fewer in winter and spring. The most common injury sites in road traffic accidents were limbs/skin and head and neck, accounting for 81.06%.The patients with moderate severity of injuries were the most numerous, accounting for 85.29%.Conclusion To avoid road traffic accidents, prevention should be the priority. It is necessary to strengthen the joint governance of multiple departments and minimize the occurrence of road traffic accident from the source.Pre-hospital emergency care must focus on key populations and key seasons, strengthen professional skills training and resource allocation, ensure efficient and smooth connection between pre-hospital and in-hospital care.Popularize and publicize the importance of self-rescue and mutual rescue, promote first aid knowledge and skills training for the public, and create a social atmosphere where"rescue is right beside us".

Neuronal intranuclear inclusion disease(NIID)is a rare hereditary neurodegenerative disorder that can affect multiple systems. However,it is uncommon for urinary dysfunction to be the initial symptom. This article reports five cases. The five patients began to experience voiding dysfunction such as frequent urination, weak urination, and incomplete urination at the mean ages of 55.4(47 - 65)years old. Four months to twelve years after urinary onset, neurological symptoms such as headache, memory decline, transient loss of consciousness, and unsteady gait began to appear. Four of the five cases had a family history. Brain MRI revealed the “ribbon sign” or “crest sign” in all cases. Skin biopsy revealed eosinophilic inclusions in the cell nuclei, and NOTCH2NLC gene testing identified abnormal GGC mutations. Three of the five patients underwent cystostomy due to secondary hydronephrosis, while the other two received no special treatment. After a follow-up of 18 to 35 months since diagnosis, the patients who underwent cystostomy had normal renal function. Neurological symptoms in all five patients worsened to varying degrees.

Objective:To describe the detail sampling design, weighting, instruments, filed procedures and quality control methods of the epidemiological survey on dementia among community residents in Tongliao City. Methods:A three-stage disproportionate probability sampling design was used to investigate the inhabitants aged 65 years and over in Tongliao City,Inner Mongolia Autonomous Region. The 10/66 Dementia Research Group(10/66 DRG)assessment instruments were used to diagnose dementia, using computer-assisted personal interview mode in the selected older people.Comprehensive quality control methods were implemented throughout the field-work.Results:A total of 166 villages or communities were sampled from nine counties or districts in Tongliao Cit-y.Totally 4 345 older people were interviewed with 96.2%response rate.By calculating sampling design weights,non-response adjustment weights and post-stratification adjustment weights, these weights were multiplied and per-formed trimming adjustment and standardization adjustment to generate final weights. The 171 interviewers were well-trained and qualified to carry out filed interview.Quality control methods included computer data check, audio record check, and telephone check in order to ensure the quality of the survey. Conclusion:This survey is imple-mented using a rigorous sampling design and timely quality control methods, and uses the 10/66 DRG assessment instruments with satisfactory international validity and reliability as survey instruments, which has international cross-cultural comparability. It provides a valid and feasible methodology of epidemiological survey on dementia for further studies in different regions in China.

Objective:To understand the incidence of diabetes and influencing factors, the trend of FPG change and risk for mortality in HIV-infected individuals after antiretroviral therapy (ART) in Dehong Dai and Jingpo Autonomous Prefecture (Dehong). Methods:The HIV/AIDS treatment database was collected from China Information System for Disease Control and Prevention. This retrospective cohort study was conducted in HIV-infected individuals with access to ART in Dehong during 2004-2020.The Cox proportional hazard regression model was used to analyze the incidence density of diabetes, the influencing factors and risk for mortality in HIV-infected individuals with access to ART, mixed linear effects model was used to analyze the trend of FPG change and predict FPG in those with different glucose metabolic status at baseline survey. Statistical analysis was performed using software SAS 9.4.Results:A total of 8 763 HIV-infected individuals were included, in whom 8 432 (96.2%) had no diabetes, 331 had diabetes. The incidence density of diabetes was 2.31/1 000 person years. Multivariate Cox proportional hazard regression analysis revealed that 30- 59 years old, BMI ≥24.0 kg/m 2, Efavirenz (EFV) based initial treatment regimen and impaired fasting glucose (IFG) at baseline survey were significantly and positively associated with incidence of diabetes. Mixed effect model revealed that FPG was positively correlated with the duration of ART, age and baseline FPG. Suffering from diabetes was a risk factor for mortality in HIV-infected individuals both at baseline survey and during follow-up. Conclusions:The risk for diabetes increased in HIV-infected individuals who were 30-59 years old, baseline BMI ≥24.0 kg/m 2, received EFV based initial treatment, and IFG in HIV-infected individuals after antiretroviral therapy in Dehong, 2004-2020. It is important to pay close attention to their blood glucose, and patients with high blood glucose should receive treatment as early as possible.
