1.Mechanism of Shenmai Injection to Improve Cisplatin Resistance in NSCLC Based on Endoplasmic Reticulum Stress Through PERK/ATF4/CHOP Signaling Pathway

ObjectiveTo explore the mechanism of Shenmai injection in improving cisplatin resistance in non-small cell lung cancer (NSCLC) based on the endoplasmic reticulum stress through protein kinase R-like endoplasmic reticulum kinase (PERK)/activated transcription factor 4 (ATF4)/C/EBP homologous protein (CHOP) signaling pathway. MethodsBALB/c nude mice bearing cisplatin-resistant human lung cancer cell line (A549/cisplatin) were randomly divided into four groups: Blank control group (0.9% sodium chloride), cisplatin group (5 µg·g-1cisplatin), Shenmai injection group (5.2 mg·g-1 Shenmai injection), and combination therapy group (5.2 mg·g-1 Shenmai injection +5 µg·g-1cisplatin). The drug intervention lasted for 4 weeks, and the changes in body weight and tumor volume were monitored. Hematoxylin-eosin (HE) staining was performed to observe tumor tissue pathology. Transmission electron microscopy (TEM) was used to assess the morphology of the endoplasmic reticulum. Immunohistochemical assay was conducted to measure the positive expressions of PERK, ATF4, and CHOP in tumor tissues. Western blot quantified the protein expression of immunoglobulin heavy chain binding protein (BIP), PERK, phosphorylated PERK (p-PERK), eukaryotic translation initiation factor 2α (eIF2α), phosphorylated eIF2α (p-eIF2α), ATF4, CHOP, B-cell lymphoma -2 (Bcl-2), and Bcl-2 Associated X protein (Bax). A549/cis cells were divided into blank group: Blank control group (normal culture medium), cisplatin group (23.3 µmol·L-1 cisplatin), Shenmai Injection group (20 g·L-1 Shenmai injection), and combination therapy group (20 g·L-1 Shenmai injection+23.3 µmol·L-1 cisplatin). Cell counting kit-8 (CCK-8) method was used to detect cell viability, TEM was used to observe the morphology of endoplasmic reticulum, and Western blot was used to detect endoplasmic reticulum stress and apoptosis-related proteins. ResultsCompared with the cisplatin group, the combination therapy group showed increased body weight (P<0.05), decreased tumor volume (P<0.05), and expanded endoplasmic reticulum in tumor cells. The positive expressions of PERK, ATF4, and CHOP increased (P<0.05). Western blot revealed elevated protein expression levels of BIP, p-PERK/PERK, p-eIF2α/eIF2α, ATF4, CHOP, and Bax (P<0.05), while Bcl-2 expression decreased (P<0.05). As shown in the in vitro experiment, compared with the cisplatin group, the combination therapy group exhibited a reduced cell survival rate (P<0.05). TEM revealed increased endoplasmic reticulum dilation and vesicular degeneration. Western blotting showed increased protein levels of BIP, p-PERK/PERK, p-eIF2α/eIF2α, ATF4, CHOP and Bax (P<0.05), with decreased Bcl-2 expression (P<0.05). ConclusionShenmai injection combined with cisplatin has a synergistic antitumor effect in NSCLC, which may be attributed to the activation of endoplasmic reticulum stress response mediated by the PERK/eIF2α/ATF4/CHOP signaling pathway and the induction of tumor cell apoptosis.
2.Exploring the mechanism of Xiaoaiping Injection inhibiting autophagy in prostate cancer based on proteomics.

Xiaoaiping (XAP) Injection demonstrates the anti-prostate cancer (PCa) effects, yet the underlying mechanism remains unclear. This study aims to investigate the impact of XAP on PCa and elucidate its mechanism of action. PCa cell proliferation was evaluated using a cell counting kit-8 (CCK-8) assay. Cell apoptosis was assessed through Hoechst staining and Western blotting assays. Proteomics technology was employed to identify key molecules and significant signaling pathways modulated by XAP in PCa cells. To further validate potential key genes and important pathways, a series of assays were conducted, including acridine orange (AO) staining, transmission electron microscopy, and immunofluorescence assays. The molecular mechanism of XAP against PCa in vivo was examined using a PC3 xenograft mouse model. Results demonstrated that XAP significantly inhibited cell proliferation in multiple PCa cell lines. In C4-2 and prostate cancer cell line-3 (PC3) cells, XAP induced cellular apoptosis, evidenced by reduced B-cell lymphoma 2 (Bcl-2) levels and elevated Bcl-2-associated X (Bax) levels. Proteomic, immunofluorescence, and quantitative reverse transcription-polymerase chain reaction (qRT-PCR) investigations revealed a strong correlation between forkhead box O3a (FoxO3a) autophagic degradation and the anti-PCa action of XAP. XAP hindered autophagy by reducing the expression levels of autophagy-related protein 5 (Atg5)/autophagy-related protein 12 (Atg12) and enhancing FoxO3a expression and nuclear translocation. Furthermore, XAP exhibited potent anti-PCa action in PC3 xenograft mice and triggered FoxO3a nuclear translocation in tumor tissue. These findings suggest that XAP induces PCa apoptosis via inhibition of FoxO3a autophagic degradation, potentially offering a novel perspective on XAP injection as an effective anticancer therapy for PCa.

4.Effect of group reminiscence therapy on depressive symptoms, cognitive function, and quality of life in elderly patients with diabetes and mild to moderate depression

BackgroundThe prevalence of depression is elevated in elderly patients with diabetes, underscoring the critical importance of implementing early psychological intervention to improve their clinical outcomes.However, the cognitive function level of patients may limit the implementation of these intervention methods. While prior research has predominantly focused on interventions such as cognitive behavioral therapy and interpersonal psychotherapy to mitigate the depressive symptoms in this demographic, the exploration of group reminiscence therapy as a therapeutic approach remains underrepresented in the existing literature. ObjectiveTo investigate the effect of group reminiscence therapy on depressive symptoms, cognitive function, and quality of life in elderly patients with diabetes and mild to moderate depression, so as to provide valuable insights for psychological intervention of elderly diabetic patients. MethodsA randomized controlled trial was conducted to recruit 80 elderly diabetic patients with mild to moderate depression attending the endocrine clinic of Northeast International Hospital from June 2022 to December 2023. Subjects were randomly allocated to either the study group or the control group using the random number table method, with 40 cases in each group. Both groups received standard diabetes care and mental health education. Additionally, the study group participated in an 8-week group reminiscence therapy intervention, convening once weekly for 1.5 hours per session. Hamilton Depression Scale-24 item (HAMD-24), Wisconsin Card Sorting Test (WCST), Verbal Fluency Test (VFT), Stroop Color Word Test (SCWT) and Generic Quality of Life Inventory-74 (GQOLI-74) were administered before and after intervention. ResultsAfter intervention, the study group exhibited a significantly lower HAMD-24 score compared with the control group (F=13.908, P<0.01). The study group also demonstrated better performance in cognitive assessments, as evidenced by increased percentages of correct responses and conceptual-level responses on the WCST, a greater number of correct words on the VFT, and a higher number of accurate responses on the SCWT, all in contrast to the control group (F=14.672, 17.000, 13.309, 21.672, P<0.01). The study group reported superior quality of life outcomes, with higher total GQOLI-74 scores and significant improvements in the domains of physical function, social function, psychological function, and material life status compared with the control group (F=33.098, 41.224, 16.320, 19.432, P<0.01). ConclusionGroup reminiscence therapy has the potential to alleviate depressive symptoms, enhance cognitive function and improve quality of life in elderly diabetic patients with mild to moderate depression.
5.Characteristics of asymptomatic HBV infection in HBsAg-positive blood donors from Dalian

Objective: To analyze serological and molecular characteristics of asymptomatic HBV infection in HBV surface antigen positive (HBsAg+) blood donors from Dalian. Methods: The prevalence of HBsAg was analyzed among blood donors in Dalian between 2013 and 2022. Randomly selected HBsAg+ blood samples were subjected to HBV serological testing, HBV viral DNA quantification, and HBV genotyping. Results: Over this ten-year period, the prevalence of HBsAg decreased from 1.25% to 0.50% among blood donors in Dalian. Donors who tested positive for HBsAg prior to donation using a rapid test (RT) accounted for 92.5% of all HBsAg+ donors identified. A total of 240 confirmed HBsAg+ blood donors were randomly selected, including 125 donors with positive results and 115 with negative results in the pre-donation rapid test. HBsAg+ donors were mainly males (71.2%), with a median age of 42, and 97.5% of them being first-time donors. Based on HBV serological profiles, three stages of infection were identified: early infection (2.9%), suspected acute hepatitis (0.8%), and chronic infection (96.3%). The dominant HBV genotypes were C (68.9%) and B (28.4%). Among chronic HBV infection individuals, donors infected with HBV genotype B were older than those infected with genotype C (median age: 45y vs 38.5y, P<0.05). Additionally, they showed significantly lower HBsAg levels with a narrower distribution range than those infected with genotype C [median: 23.2 IU/mL (range: <0.05-7 910 IU/mL) vs 968 IU/mL (range: <0.05-3.4×10
), P<0.05]. However, no significant difference was observed in the HBV DNA loads between these two genotypes (P>0.05). Conclusion: Between 2013 and 2022, the prevalence of HBsAg among blood donors in Dalian showed a year-over-year decline. Chronic infection was predominant among HBsAg+ first-time blood donors. The characteristics of chronic infection in blood donors differed significantly depending on the viral genotype, manifesting as differences in age of infected individuals and HBsAg level distribution.
6.The seroepidemiological characteristics of Toxoplasma gondii infection among blood donors in Jiangxi Province

Objective: To investigate the current status of Toxoplasma gondii (T. gondii) infection among blood donors in Jiangxi Province, thereby providing scientific evidence for ensuring blood transfusion safety. Methods: Serum samples from 1 529 blood donors were tested for T. gondii IgG and IgM antibodies using enzyme-linked immunosorbent assay (ELISA). Positive samples were further analyzed by B1 gene PCR testing and Giemsa staining microscopy. Risk factor data were collected via telephone follow-up. Infection rate differences were analyzed using χ
tests. Results: The overall T. gondii infection rate among blood donors was 3.92% (60/1 529), with IgG positivity being predominant (3.86%) and IgM positivity at 0.20%. All antibody-positive samples tested negative for T. gondii DNA. Blood group AB donors had the highest infection rate (7.00%), whereas group O donors had the lowest rate (2.47%), with a statistically significant difference (χ
=8.27, P<0.05). The infection rate in the 46-55 age group was higher than that in the 18-25 age group, χ
=4 237.75, P<0.05. No significant differences were observed across subgroups stratified by sex, occupation, or education level. Multivariate analysis identified cat/dog ownership (χ
=28.23, P<0.05), and frequent consumption of cold dishes (χ
=4.09, P<0.05) as key risk factors. Conclusion: In Jiangxi province, T. gondii infection among blood donors is predominantly characterized by past infection, with higher infection rates observed in blood type AB donors and those aged 46-55. The main risk factors include contact with cats/dogs as pets and frequent consumption of cold dishes. It is recommended to disseminate health knowledge on toxoplasmosis prevention in communities and schools to reduce the transfusion transmission risk.
7.The relationship between EZH2 methylation and thyroid cancer differentiation-related markers and its therapeutic value

Objective :
To investigate the role of enhancer of zeste homolog 2(EZH2)-trimethylated lysine 27 of histone H3(H3K27me3) axis in the dedifferentiation of thyroid cancer and its clinical value as a potential target for the treatment of anaplastic thyroid cancer(ATC).
Methods :
Immunohistochemical SP method was used to detect the expression of EZH2, H3K27me3, paired box gene 8(PAX8), thyroglobulin(TG) and thyroid transcription factor 1(TTF1) in ATC and papillary thyroid carcinoma(PTC) and their adjacent tissues. The relationship between EZH2 and thyroid differentiation markers(PAX8, TTF1, TG) was further analyzed by gene expression omnibus(GEO) database. ATC cell lines 8305C and BHT-101 were culturedin vitro. Real-time reverse transcription PCR(RT-qPCR) was used to detect the expression of thyroid differentiation markers(TTF1, PAX8) mRNA in ATC cell lines treated with EZH2 inhibitor(GSK126), and evaluate the potential therapeutic effect of GSK126in vitro. The effects of GSK126 and BRAF inhibitor vemurafenib on the proliferation of ATC cell lines were observed by cell proliferation assay.
Results :
The expression of EZH2 in ATC tissues was significantly higher than that in papillary thyroid carcinoma and adjacent tissues(P<0.05). The expression of H3K37me3 in ATC tissues was significantly lower than that in PTC tissues(P<0.05). EZH2 was negatively correlated with PAX8 and TG expression levels, but not with TTF1 expression level.In vitroexperiments, GSK126 could reverse the expression of thyroid differentiation markers PAX8 and TTF1 in ATC cell lines. GSK126 combined with BRAF inhibitor vemurafenib could significantly inhibit the growth of ATC cell lines.
Conclusion
The EZH2-H3K27me3 axis plays an important role in regulating thyroid specific markers, and the inhibition of EZH2 by small molecular compounds is a promising target for ATC treatment in the future.
8.METTL3 promotes cellular proliferation, migration and EMT in non-small cell lung cancer cells

Objective:
To investigate the effect and mechanism of methyltransferase-like3(METTL3),which functions as m6A modification methyltransferase,on the cellular proliferation,clone formation and migration of nonsmall cell lung cancer(NSCLC) cells.
Methods:
The METTL3 level and patient overall survival were analyzed by bioinformatics.In the transfected A549 cells,the expression of METTL3 was detected by RT-PCR and Western blot assays,the m6A level was detected by m6A RNA Methylation Assay kit,the cellular viability and growth were detected by CCK-8 assay,the apoptosis was detected by immunofluorescent assay,the colony growth was detected by clone formation assay,the cell migration growth was detected by scratch.
Results :
Bioinformatics showed that METTL3 was highly expressed in NSCLC and negatively correlated with patient overall survival.The RT-PCR and Western blot data showed that the METTL3 level was higher in NSCLC cells.Moreover,higher METTL3 significantly promoted m6A level,cell proliferation,colony formation,migration,inhibited cell apoptosis,increased the mRNA and protein expressions of EMT-related marker Vimentin but inhibited apoptosis and decreased the mRNA and protein expressions of EMT-related marker E-cadherin in the A549 cells.Furthermore,the contrasting results were obtained in the A549 cells with the knockdown of METTL3.
Conclusion
The over-expressed METTL3 increases the m6A levels in NSCLC cells and promotes cellular proliferation,colony formation,and migration growth,thereby laying a theoretical foundation for future research on early diagnosis and targeted drug development for NSCLC by targeting METTL3.
9.Anti-fatigue effect of Dendrobium and Panax Quinquefolius Granules on overtrained mice and its mechanism

Objective:To investigate the anti-fatigue effect of Dendrobium and Panax Quinquefolius Granules(DPQG)on the overtrained mice,and to clarify its possible mechanism.Methods:A total of 48 mice were randomly divided into control group(equal volume of distilled water),low dose of DPQG group(400 mg·kg-1 DPQG),medium dose of DPQG group(800 mg·kg-1 DPQG),and high dose of DPQG group(1 600 mg·kg-1 DPQG).The DPQG were administered by gavage for 35 d,and the rotarod test and swimming endurance test were performed 30 min after last administration.Serum,liver tissue,and muscle tissue were collected from the mice in various groups.ELISA method was used to detect the serum lacticacid(LAC)levels and lactate dehydrogenase(LDH)activities,and the malondialdehyde(MDA)levels,superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)activities,and the liver glycogen and muscle glycogen levels in muscle tissue of the mice in various groups;HE staining was used to observe the pathomorphology of muscle tissue of the mice.Transcriptomics and metabolomics technologies were used to identify the key genes and metabolites in muscle tissue of the mice in control group and high dose of DPQG group and to analyze the correlations between differentially expressed genes(DEGs)and differentially expressed metabolites.Results:Compared with control group,the rod turning exhaustion time of the mice in different doses of DPQG groups were significantly increased(P<0.05),and the swimming exhaution time of the mice in high dose of DPQG group was increased(P<0.05).Compared with control group,the LDH,SOD,and GSH-Px activities of the mice in medium and high doses of DPQG groups were increased(P<0.01).Compared with control group,the levels of MDA and liver glycogen of the mice in medium and high doses of DPQG groups were decreased(P<0.05 or P<0.01).The transcriptomics sequencing results showed that DPQG mainly acted on DEGs such as Trib3 and Olfr495;the Gene Ontology(GO)functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)signaling pathway enrichment analysis results showed that the DEGs were mainly enriched in olfactory-related processes and signaling pathways;the metabolomics KEGG analysis results showed that the differential metabolites were mainly enriched in the regulation pathway of inflammatory mediators on tryptophan(TRP);the combined analysis of transcriptomics and metabolomics results showed that the piezo1 gene had high correlations with the differential metabolites β1-solamarine(r=-1,P<0.05)and tilidine(r=1,P<0.05).Conclusion:DPQG can exert an anti-fatigue effect on the overtrained mice by modulating LAC metabolism and glycogen homeostasis,as well as maintaining the oxidative/antioxidant balance in the body;its anti-fatigue mechanism is related to the Olfr495 and piezo1 genes and the regulation pathway of inflammatory mediators on TRP channels.
10.Clinical study on Compound Huzhang Prescription in the treatment of dampness-heat accumulation type metabolic associated fatty liver disease

Objective To investigate the clinical efficacy and mechanism of the Compound Huzhang Prescription in the treatment of dampness-heat accumulation type metabolic-associated fatty liver disease(MAFLD).Methods Eighty MAFLD patients were randomly divided into the treatment group(treated with Compound Huzhang Prescription)and the control group(treated with polyene phosphatidylcholine capsules),with 40 cases in each group.After 3 months of treatment,liver function indicators(ALT,AST,γ-GGT),fast-ing blood glucose(FBG),lipid profiles(TC,TG,HDL-C,LDL-C),hepatic steatosis index(HSI),fibroblast growth factor 21(FGF21),β-Klotho(KBL),homeostasis model assessment of insulin resistance(HOMA-IR)index,and liver fat content measured by MRI were compared between pre-and post-treatment.The efficacy of traditional Chinese medicine(TCM)syndrome was evaluated based on TCM syndrome scores,and the total effective rates of the two groups were compared.Results Two cases dropped out from the treatment group and one from the control group,leaving 38 cases in the treatment group and 39 cases in the control group.The levels of liver function indicators decreased significantly in both groups after treatment compared with those before treatment(P<0.05).After treatment,TG in the treatment group showed significant decrease com-pared with both pre-treatment level and the control group(P<0.05).In the control group,there were no significant differences in HSI,FGF21,KBL,HOMA-IR index,or liver fat content before and after treatment(P>0.05).However,in the treatment group,HSI,FGF21,HOMA-IR,and liver fat content significantly de-creased,while KBL significantly increased after treatment(P<0.05).Post-treatment,all these indicators in the treatment group showed significant improvement when compared to the control group(P<0.05).The to-tal effective rate of TCM syndrome efficacy was 81.6%in the treatment group and 46.2%in the control group,with a statistically significant difference between the two groups(P<0.05).Conclusion The Com-pound Huzhang Prescription is effective in treating MAFLD,and its mechanism is related to alleviating FGF21 resistance.
