1.A time-stratified case-crossover study on the relationship between meteorological factors and scarlet fever incidence in Xicheng District of Beijing

Objective To explore the impact of meteorological factors on the incidence of scarlet fever in Xicheng District of Beijing, and to provide a scientific basis for formulating targeted prevention and control measures. Methods Daily scarlet fever incidence data and corresponding meteorological data from 2010 to 2019 in Xicheng District of Beijing were collected. Using year, month, and day of the week as time-stratified variables, a conditional logistic regression model with a time-stratified case-crossover design was employed to analyze the impact of different meteorological factors on the number of scarlet fever cases in Xicheng District, while controlling for long-term trends, seasonality and other confounding factors. Results From 2010 to 2019, a total of 3,195 cases of scarlet fever were reported in Xicheng District, with an average annual incidence rate of 24.17/100 000. The overall trend showed fluctuating decline, during which three incidence peaks occurred, with the highest incidence rate observed in 2011. No severe or fatal cases were reported. The incidence of scarlet fever exhibited a distinct seasonal bimodal distribution, primarily concentrated between April to June and November to January of the following year. The conditional logistic regression results showed that the average relative humidity and average temperature were positively correlated with scarlet fever cases (β=0.0203, β=0.0613, P<0.001), while the average vapor pressure was negatively correlated with scarlet fever cases (β=-0.1468, P<0.001). Increases average relative humidity and average temperature were risk factors for scarlet fever incidence (OR=1.0205,95%CI=1.0150 -1.0261;OR=1.0632,95%CI=1.0379 -1.0891). For every 1.00% increase in average relative humidity, the number of scarlet fever cases increased by 2.05% (1.50% –2.61%). Similarly, for every 1。C rise in average temperature, the number of cases increased by 6.32% (3.79% – 8.91%). In contrast, an increase in average vapor pressure had a protective effect against scarlet fever (OR=0.8635,95%CI=0.8392-0.8885). For every 1 hPa increase in average vapor pressure, the number of scarlet fever cases decreased by 13.65% (11.15%–16.08%). Conclusion The average relative humidity, average temperature and average vapor pressure are the primary meteorological factors influencing the incidence of scarlet fever in Xicheng District of Beijing, and can be utilized as indicators for the prevention, control, surveillance and early warning of scarlet fever.
2.Comparison of epidemiological characteristics of seasonal influenza in Xicheng District, Beijing, in 2018-2029 to 2023-2024

Objective To compare the epidemic characteristics of seasonal influenza in Xicheng District, Beijing, between 2018-2019 and 2023-2024. Methods The present study was a cross-sectional study. The influenza cases and etiological surveillance data of Xicheng District, Beijing in 2018-2019 and 2023-2024, respectively, were analyzed to study the differences in the incidence rate of influenza cases in different ages, regions, occupations, and etiological detection results during the four years. Results There were significant differences in the incidence rates of influenza (χ2=37 038.65,P<0.05) and the interval between visits (Z=468.692, P < 0.05) in 2018, 2019, 2023 and 2024. There were statistically significant differences in the incidence rates of influenza among different age groups, streets, and occupations during the four years (Z=75.646, 209.276, 72.132, 361.176, 43.570, all P < 0.05). Compared with 2018-2019, the growth rate of influenza in Xicheng District, Beijing in 2023-2024 was the highest in the 15-24 year-old group, Tianqiao Street, and housework and unemployed people. The difference in the interval between influenza visits among people of different age groups during these 4 years was statistically significant (P < 0.05). The growth rate of the interval between influenza visits in Xicheng District, Beijing in 2023-2024 was the highest in the 15-24 year-old group, while it showed a decrease in the 0-4 year-old group. The etiology in 2018-2019 was mainly H1N1 and H3N2 subtypes, and in 2023-2024 it was mainly H3N2 subtype and influenza B, and the peak values of H1N1, H3N2 subtype and influenza B in 2023-2024 were all higher than those in 2018-2019. Conclusion During the epidemic period, non-medical interventions have changed the transmission mode, seasonal peak and subtype distribution of influenza virus. As the epidemic has eased, the pattern of influenza has gradually rebounded.
3.Study on the mechanism of Danggui buxue decoction regulating neutrophil extracellular traps to improve osteo-porosis in rats with premature ovarian failure

OBJECTIVE To investigate the mechanism through which Danggui buxue decoction regulates neutrophil extracellular traps (NETs) to improve osteoporosis (OP) in rats with premature ovarian failure (POF). METHODS Female SD rats were randomly divided into normal group, model group, calcitriol group, and Danggui buxue decoction low-dose, medium-dose and high-dose groups, with 9 rats in each group. Except for the normal group, all other groups were administered cisplatin via intraperitoneal injection on days 1 and 8 to establish a POF complicated with OP model. Each group received the corresponding drugs or normal saline intragastrically starting from day 5, once a day, for 4 consecutive weeks. After the last medication, serum levels of estradiol (E2), NETs, 25-hydroxyvitamin D3 [25(OH)D3], receptor activator of nuclear factor-κB ligand (RANKL), and osteocalcin (BGP) were measured. The histopathological changes in bone tissue were observed. The expressions of vitamin D receptor (VDR), myeloperoxidase (MPO), neutrophil elastase (NE) and citrullinated histone H3 (CitH3) in bone tissue were detected; the protein expressions of 25-hydroxyvitamin D-1α-hydroxylase (CYP27B1) and 1α,25-dihydroxyvitamin D3-24-hydroxylase (CYP24A1) were also determined. RESULTS Compared with the normal group, the bone tissue of rats in the model group showed a significant reduction in the number of trabeculae, which was thinner broken and poorly connected, with significant destruction of the reticular structure, and an enlarged marrow cavity. Serum levels of NETs and RANKL, the protein expressions of MPO, NE, CitH3 and CYP24A1 in bone tissue were significantly increased or upregulated, while serum levels of E2, 25(OH)D3 and BGP as well as protein expressions of VDR and CYP27B1 were significantly decreased or downregulated (P<0.05). Compared with the model group, the histopathological changes in the bone tissue of rats in each administration group showed some degree of recovery, with significant improvements observed in most quantitative indicators (P<0.05). CONCLUSIONS Danggui buxue decoction can restore the E2 level in POF complicated with OP rats, and improve OP. The mechanism may be related to its ability to upregulate VD level and inhibit the formation of NETs.
4.Effect of Different Fermentation Conditions on Fungal Community and Chemical Composition of Aurantii Fructus

ObjectiveTo investigate the effects of different fermentation methods and times on the fungal flora and chemical composition of Aurantii Fructus, in order to obtain the optimal fermentation conditions and flora structure, and to ensure the stability and controllability of the fermented varieties. MethodsScanning electron microscopy was used to observe and analyze the colony characteristics on the surface of Aurantii Fructus under different fermentation conditions. Internal transcribed spacer 2(ITS2) high-throughput sequencing, combined with fungal community diversity analysis and fungal community structure analysis, were used to obtain the fungal flora microbial categories of Aurantii Fructus under the conditions of traditional pressure-shelf fermentation and non-pressure-shelf natural fermentation for 7, 14, 21 d(numbered Y1-Y3 for the former, and numbered F1-F3 for the latter), respectively. At the same time, the chemical components in the fermentation process were detected by ultra-high performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS), combined with principal component analysis(PCA), partial least squares-discriminant analysis(PLS-DA) and compound retention time, parent ions, characteristic fragment ions and other information, the differential compounds between the different fermentation samples were screened and identified. ResultsThe analysis of fungal community diversity showed that the dominant flora did not change at different fermentation time points in the traditional pressure-shelf fermentation method, while in the non-pressure-shelf natural fermentation method, there was a significant difference with the fermentation process, and at the genus level, the dominant genus of samples Y1, Y2, Y3 and F2 was Aspergillus, while the dominant genera of samples F1 and F3 were both Rhizopus. This indicated that the microbial growth environment provided by the traditional fermentation method was more stable, and the microbial community structure was more stable, which was more conducive to the stable and controllable fermentation process and fermented products. A total of 155 compounds were identified by compositional analysis, including 70 flavonoids, 38 coumarins, 10 alkaloids, 34 organic acids and 3 other compounds. After fermentation, two new components of ribalinine and pranferin were produced. Different fermentation conditions also brought about differences in chemical composition, multivariate statistical analysis obtained 26 differential compounds under two different fermentation methods, mainly including flavonoids, organic acids and coumarins. Comprehensively, the microbial community structure of samples fermented by the traditional pressure-shelf method of Aurantii Fructus for 14 d was stable, the species richness was high and the overall content of differential compounds was high, which was the optimal processing condition. ConclusionCompared with non-pressure-shelf natural fermentation, the traditional method has obvious advantages in terms of the stability of the microbial community structure and the content of chemical compounds, and the optimal condition is 14 days of fermentation. This study is helpful to promote the quality stability and fermentation bioavailability of fermented products of Aurantii Fructus, as well as to provide an experimental basis for the further improvement of the quality control methods of this variety.

6.Genotyping of M protein gene of group A streptococcus infections among children in Xicheng District of Beijing in 2011-2024

Objective To understand the distribution of M protein gene (emm) of group A streptococcus (GAS) infections among children in Xicheng District of Beijing. Methods Throat swab samples from scarlet fever and pharyngeal infection cases were collected in sentinel hospitals of Xicheng District, Beijing from 2011 to 2024. GAS strains were isolated and identified, and emm gene was amplified and sequenced by PCR to determine the genotype. The differences in emm genotype between different groups were compared. Results A total of 3 130 throat swab samples were collected, and 400 GAS strains were isolated, with a positive rate of 12.78%. The highest positive rate was 19.93% in 2011. The positive rate of scarlet fever (45.71%) was higher than that of pharyngeal infection (6.14%) (P<0.001). There were 391 emm gene positive strains, and the differences in the positive rate of emm gene among different cases were statistically significant (P<0.001). A total of 7 genotypes and 27 gene subtypes were detected. Among different groups, the emm genotypes were mainly emm12 and emm1. The emm gene subtypes were mainly emm12.00 and emm1.00. Except for some years, the genotypes and their subtypes were dominated by emm12 and emm12.00, and the distribution differences of the two major genotypes and their subtypes were statistically significant from 2011 to 2019 (P<0.001). There were differences in genotypes and subtypes among different age groups (P=0.002). Conclusion The dominant types of emm genes in group A streptococcus infections among children were emm12 and emm1 in Xicheng District of Beijing from 2011 to 2024, and the dominant gene subtypes were emm12.00 and emm1.00. It is necessary to comprehensively strengthen the monitoring of the epidemic situation and genotype, timely grasp the distribution and variation of emm gene.
7.Establishment and application of a rapid high-throughput detection method for Huanglongbing.

The dried mature peel of Citrus reticulata, a plant in the Rutaceae family and its cultivated varieties, is a commonly used Chinese medicinal material known as Chenpi(Citri Reticulatae Pericarpium). It is rich in nutritional components and medicinal value, with pharmacological effects including relieving cough and eliminating phlegm, strengthening the spleen and drying dampness, protecting the liver and benefiting the stomach, tonifying Qi, and calming the mind. Huanglongbing(HLB), also known as Citrus Huanglongbing, is a destructive disease in citrus production that seriously threatens the development of the citrus industry. HLB causes symptoms such as the inability of Rutaceae plants to produce mature fruit, gradual weakening of the tree, and eventual death, posing a significant threat to the yield and quality of Chenpi. Due to the uneven distribution of the HLB pathogen in infected plants, accurate detection of the pathogen requires the collection of a large number of plant samples. Current sample pretreatment methods, such as traditional extraction methods and commercial extraction kits, are time-consuming and involve multiple steps, which significantly increase the difficulty and workload of HLB diagnosis and have become a bottleneck in HLB detection. In this study, a rapid high-throughput detection method combining alkali lysis and TaqMan qPCR was developed. This method allows the pretreatment of multiple samples within 5 min, and the entire detection process can be completed within 45 min, with a detection limit of 6.67 fg·μL~(-1). The alkali lysis method and commercial kits were used for parallel detection of field-collected citrus samples, and the results showed no significant difference. The sample pretreatment method established in this study is characterized by low cost, simplicity, and high efficiency. Combined with TaqMan qPCR, it can provide technical support for early and on-site diagnosis of HLB. This method is of great significance for disease prevention and control in the citrus industry and is expected to help improve the yield and quality of citrus medicinal materials.

8.Risk factors for hypoxemia in children with severe Mycoplasma pneumoniae pneumonia.

OBJECTIVES:
To study the risk factors for hypoxemia in children with severe Mycoplasma pneumoniae pneumonia (SMPP).
METHODS:
A retrospective collection of clinical data from children diagnosed with SMPP at the Third Affiliated Hospital of Zhengzhou University from June to December 2023 was conducted. The patients were categorized into hypoxemia and non-hypoxemia groups. Logistic regression analysis was used to assess the risk factors for hypoxemia, and receiver operating characteristic (ROC) curve analysis was employed to analyze the diagnostic performance of various indicators.
RESULTS:
A total of 113 children with SMPP were included. Univariate logistic regression analysis showed that ferritin, aspartate aminotransferase, creatinine, creatine kinase isoenzyme, lactate dehydrogenase, alpha-hydroxybutyrate dehydrogenase, immunoglobulin G, complement C3, complement C4, age, extrapulmonary complications, and a chest computed tomography (CT) scan showing a bronchiolitis pattern were significant factors for hypoxemia in children with SMPP (P<0.05). Multivariate logistic regression analysis revealed that elevated ferritin levels, presence of extrapulmonary complications, and a bronchiolitis pattern on lung CT were independent risk factors for hypoxemia in these patients (P<0.05). The ROC curve analysis indicated that the combination of these three indicators for predicting hypoxemia had a sensitivity of 71.9%, a specificity of 95.1%, and an area under the curve of 0.888 (95%CI: 0.809-0.968).
CONCLUSIONS
In children with SMPP, when there are elevated ferritin levels, a bronchiolitis pattern on chest CT, and the presence of extrapulmonary complications, there should be a high level of vigilance for the potential development of hypoxemia.

10.Study on the role of plasma exosome miR-622 in cachexia of hepatocellular carcinoma

Objective To explore the role of plasma exosome microRNA-622(miR-622)in cachexia of hep-atocellular carcinoma(HCC).Methods Select preoperative blood samples from 40 patients with stage Ⅲ-ⅣHCC and 33 patients with non HCC liver benign diseases collected at Huashan Hospital,Fudan University from 2021 to 2024 as the research subjects.Exosomes were isolated from the plasma of patients,human HCC cell lines(HepG2,Hep3B,PLC,Huh7),and the culture supernatants of primary human hepatocytes(PHH)by differential centrifugation.The relative expression level of miR-622 was detected by real-time fluorescence quantitative polymerase chain reaction.The level of lipolysis was assessed by determining the concentration of glycerol and fatty acids in adipocyte culture medium.Results Plasma exosome miR-622 levels in HCC pa-tients were positively correlated with subcutaneous adipose index and visceral adipose index(r=0.516,0.539,P<0.05).HCC cell conditional medium was able to significantly increase the lipolysis level of adipo-cytes.The relative expression levels of miR-622 in exosomes from HCC cells(HepG2,Hep3B,PLC,Huh7)were significantly lower than those in exosomes from PHH,with values of 0.13±0.04,0.28±0.08,0.23±0.07,and 0.24±0.04,respectively,compared to 1.00±0.18 in PHH exosomes.Further studies revealed that plasma exosome of HCC patients treatment was able to lead to a decrease in the relative expression level of miR-622 in adipocytes,as well as an enhancement of lipolysis.Conclusion The expression of plasma exosomal miR-622 is downregulated in HCC patients.HCC cells can deliver miR-622 to adipocytes via exosomes to reg-ulate lipolysis.Plasma exosomal miR-622 may serve as a potential biomarker for predicting HCC cachexia and a therapeutic target.
