1.Correlation Between Intervertebral Disc Degeneration and Paravertebral Muscle Fat Infiltration in Chronic Low Back Pain via Quantitative MRI

Purpose To investigate the correlation between intervertebral disc degeneration and paraspinal muscle fatty infiltration in chronic low back pain(CLBP)using quantitative MRI techniques.Materials and Methods A total of 63 CLBP patients and 42 volunteers in Zigong Fourth People's Hospital from September 2023 to March 2024 were retrospectively collected.Magnetic resonance scans were performed using multi-echo water-fat separation technique and T2 mapping technique.Intervertebral disc degeneration was assessed using the Pfirrmann grading,while paraspinal muscle fatty infiltration was evaluated using the Goutallier grading.Proton density fat fraction(PDFF)of the multifidus and erector spinae muscles,and T2 values of the nucleus pulposus were quantitatively measured.The relationships between these parameters and their associations with age and gender were analyzed.Results Compared with healthy volunteers,CLBP patients showed significantly reduced T2 values of the nucleus pulposusand significantly increased PDFF of the paraspinal muscles(Z=-5.54-4.65,all P<0.001).The Pfirrmann grading of intervertebral discs and the Goutallier grading of paraspinal muscles were significantly correlated with the T2 value of the nucleus pulposus and the PDFF of the paraspinal muscles(|r|>0.4,P<0.05).Age and Pfirrmann grading were independent predictors of paraspinal muscle fat infiltration(β=0.228-5.682,all P<0.001),and gender was also an independent predictor at the L4/5 intervertebral disc level(β=-2.834,-3.352,both P<0.05).Conclusion Intervertebral disc degeneration and paraspinal muscle fat infiltration are closely related in CLBP,age and gender also have an impact.

3.M1 polarization of macrophage induced by STING signaling promotes T cell immune response

Objective:
To investigate the effect of activation of the stimulator of interferon genes(STING) pathway on macrophage polarization function and its role in T-cell response.
Methods:
Mouse macrophage RAW264.7 cells were used.STING signaling related proteins in RAW264.7 macrophage treated with STING agonist diABZI were analyzed by Western blot,including TANK-binding kinase-1(TBK1),interferon regulatory factor-3(IRF3),STING,p-TBK1,p-IRF3,p-STING.The polarization of macrophage RAW264.7 cells treated with diABZI was analyzed by flow cytometry.Co-culture of diABZI-treated RAW264.7 macrophage and T cells was applied to evaluate the change of T cell response.
Results:
STING signaling related proteins were upregulated in macrophage RAW264.7 cells treated with diABZI for 3 hours.The expression of CD86 was upregulated on the surface of macrophages after 12 hours of diABZI treatment,and the CD86/CD206 ratio was elevated,which presented the M1 polarization phenotype.When coculturing diABZI-treated macrophage RAW264.7 cells with T cells,the cytokine secretion ability of T cells including CD4+T and CD8+T cells was enhanced and the expression of CD107a in CD8+T cells was upregulated.
Conclusion
STING signaling induces M1 polarization of macrophages which enhance the function of T cells,especially CD8+T cell immune response.
4.The effect of respiratory muscle training combined with occupational therapy on the upper limb function of stroke survivors

Objective:To observe the effect of combining respiratory muscle training with occupational therapy in rehabilitating the upper limb function of stroke survivors.Methods:Fifty stroke survivors with upper limb dysfunction were randomly divided into an observation group and a control group, each of 25. Both groups were given routine rehabilitation treatment including proper positioning of the affected limb, physical therapy and motor function training. The observation group also received progressive resistance training of the inspiratory muscles and respiration control training combined with occupational therapy twice daily for 4 weeks. The trunk control test (TCT), Berg balance scale (BBS), Fugl-Meyer Upper Extremity Assessment (FMA-UE), Action Research Arm Test (ARAT), Modified Ashworth Scale (MAS) for the Hemiplegic Upper Limb and the Modified Barthel Index (MBI) were used to assess the core stability, balance, upper limb functioning, upper limb muscle tension and ability in the activities of daily living of all of the subjects.Results:Before the treatment there were no significant differences in any of the indexes between the two groups. Afterward the average TCT, BBS, FMA-UE, ARAT, MAS and MBI scores of both groups had improved significantly, but the improvements were all significantly greater in the observation group.Conclusions:Combining respiratory muscle training with occupational therapy can further improve the function of the upper limbs and daily living ability beyond what is observed with traditional rehabilitation therapy after a stroke.
5.Assessment of the clinical effect of dyclonine combined with propofol in the application of painless gastros-copy

Objective To investigate the safety and feasibility of dyclonine combined with propofol in the application of painless gastroscopy. Methods A total of 90 patients received the painless gastroscopy in our hospi-tal were enrolled from September to December 2016. They were divided into 3 groups according to the random number table(n=30):Dyclonine+propofol(DP)group,Fentanyl+propofol(FP)group,Propofol(P)group. The hemodynamic changes,adverse reaction,propofol dosage,time of gastroscopy examination and time of conscious recovery were observed and recorded. Results Compared with P group,the incidence of hypertension,tachycardia, choking cough,body movement and the dosage of propofol in DP group and FP group were significantly decreased (P<0.05,respectively). Compared with DP group,the incidence of respiratory depression,the time of gastroscopy examination and the time of Conscious recovery in FP group and P group were significantly increased (P < 0.01 , respectively). Compared with FP group,the incidence of nausea and vomiting in DP and P group were significantly decreased (P < 0.05 ,respectively). Conclusions Dyclonine combined with propofol reduced the incidence of cardiovascular response,choking cough,body movement,respiratory depression,and nausea and vomiting,with the reduced dosage of propofol ,the shorten gastroscopy examination time and the recovery time. Therefore ,dyclo-nine combined with propofol is a safe and feasible anaesthesia management for the painless gastroscopy.
