1.Clinical observation of suppurative moxibustion with wheat-grain sized moxa cones plus Western medication for cough variant asthma due to wind-cold attacking the lung

Objective:To observe the clinical efficacy of modified painless suppurative moxibustion with wheat-grain sized moxa cones plus Western medication in treating cough variant asthma(CVA)due to wind-cold attacking the lung and its effects on pulmonary function,serum immunoglobulin(Ig)-E,hypersensitive C-reactive protein(hs-CRP),and interleukin(IL)-6.Methods:A total of 98 CVA patients were randomly divided into an observation group and a control group using the random number table method,with 49 cases in each group.The control group was treated with salmeterol xinafoate and fluticasone propionate powder for inhalation,and the observation group was treated with additional modified painless suppurative moxibustion with wheat-grain sized moxa cones.The treatment lasted for 8 weeks.The traditional Chinese medicine(TCM)symptom score of the patients in both groups was observed before treatment,after treatment,and at 1-month follow-up after treatment for its changes.The clinical efficacy after treatment and at 1-month follow-up was compared between the two groups.The pulmonary function[forced expiratory volume in the first second(FEV1),FEV1/forced vital capacity(FVC),and peak expiratory flow(PEF)]and serum IgE,hs-CRP,and IL-6 levels were compared between the two groups before and after treatment.The adverse reactions that occurred during the treatment in both groups were observed and recorded.Results:The total effective rate of the observation group after treatment and at follow-up was higher than that of the control group(P<0.05).The TCM symptom scores in both groups after treatment and at follow-up were lower compared to the baseline(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).The FEV1,FEV1/FVC,and PEF after treatment in both groups were higher compared to the baseline(P<0.05),and the levels in the observation group were higher than those in the control group(P<0.05).The serum levels of IgE,IL-6,and hs-CRP after treatment in both groups were lower than the baseline(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).There were no adverse reactions in either group during the course of the study.Conclusion:Modified painless suppurative moxibustion with wheat-grain sized moxa cones plus salmeterol xinafoate and fluticasone propionate powder for inhalation is effective in treating CVA due to wind-cold attacking the lung;it can relieve the clinical symptoms of the patients,improve their pulmonary function,and also reduce serum IgE,hs-CRP,and IL-6 levels.

However, advancements in bariatric surgery techniques, the development of less invasive endoscopic bariatric procedures, the emergence of new anti-obesity drugs with single and multiple targets, and the application of artificial intelligence in metabolic and bariatric surgery are addressing these issues and revealing the rapid progress of obesity metabolism research and its clinical applications. This paper reviews relevant research published in 2024, explores emerging hotspots and potential research directions, and provides references for further exploring advancements in bariatric surgery techniques, combination anti-obesity drug therapies, and AI-assisted applications.

5.Acupuncture plus tongue pressure resistance feedback training for post-stroke deglutition disorders:a randomized controlled trial

Objective:To observe the effect of Tongguan Liqiao(opening orifices)needling plus tongue pressure resistance feedback training in treating post-stroke deglutition disorders. Methods:A prospective randomized controlled trial was conducted.A total of 120 patients with post-stroke deglutition disorders were divided into a control group and an observation group using the random number table method,with 60 cases in each group.Both groups were treated with routine treatments.The control group was treated with additional tongue pressure resistance feedback training,and the observation group was treated with additional Tongguan Liqiao needling based on the same intervention in the control group.After 4 weeks of treatment,the clinical efficacy was evaluated,and the complications that occurred during treatment were summarized.The FUJISHIMA Ichiro food intake level scale(FILS)and the swallowing quality of life questionnaire(SWAL-QOL)were scored before treatment and after 4 weeks of treatment. Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After 4 weeks of treatment,the FILS and SWAL-QOL scores in both groups were higher than those before treatment,and the scores in the observation group were higher than those in the control group(P<0.05).There was no statistical difference in the complication occurrence between the two groups(P>0.05). Conclusion:On the basis of routine treatments,Tongguan Liqiao needling plus tongue pressure resistance feedback training has a better effect in the treatment of post-stroke deglutition disorders.This method can effectively improve the swallowing function of patients,improve the quality of life,and is safe.
6.Clinical efficacy observation of filiform needle combined with fire needle for cervical radiculopathy due to wind-cold obstructing the meridians

Objective:To observe the clinical efficacy of filiform needle combined with fire needles for cervical radiculopathy(CR)due to wind-cold obstructing the meridians. Methods:A total of 60 patients with CR due to wind-cold obstructing the meridians were randomized into an observation group and a control group,with 30 cases in each group.The control group was treated with filiform needle treatment,and the observation group was treated with additional fire needle point-pricking treatment.The visual analog scale(VAS)and 20-point scale of CR developed by Yasuhisa TANAKA(YT-20)were scored before treatment and after 1,2,3,and 4 weeks of treatments.The tenderness threshold was measured.The local skin temperature was measured by an infrared thermal imager.A safety evaluation was performed after treatment. Results:After treatment,the VAS score in the observation group at each time point was lower than that in the control group(P<0.05).The YT-20 score,tenderness threshold,and local skin temperature in the observation group were all higher than those in the control group(P<0.05).The VAS score in the observation group at each time point after treatment was lower than that before treatment(P<0.05),and the YT-20 score,tenderness threshold,and local skin temperature at each time point were higher than those before treatment(P<0.05).After 3 and 4 weeks of treatments,the VAS score in the control group was lower than that before treatment(P<0.05),and the YT-20 score,tenderness threshold,and local skin temperature were higher than those before treatment(P<0.05).The group factor effects of VAS and YT-20 scores,tenderness threshold,and local skin temperature between the two groups were statistically significant(P<0.05).There was no significant difference in the time effect and the interaction effect between time and group(P>0.05). Conclusion:Filiform needle combined with fire needle or applied alone both can relieve neck pain in patients with CR due to wind-cold obstructing the meridians,and improve the temperature of the neck.The combination of filiform needle and fire needle works more quickly and has better efficacy.
7.Clinical efficacy of single-port and mini-three-port laparoscopic sleeve gastrectomy for obesity

Objective:To investigate the clinical efficacy of single-port and mini-three-port laparoscopic sleeve gastrectomy (MTP-SG) for obesity.Methods:The propensity score matching and retrospective cohort study was conducted. The clinical data of 364 obesity patients in the Chinese Obesity and Metabolic Surgery Database who were admitted to The First Affiliated Hospital of Jinan University from July 2016 to December 2023 were collected. There were 79 males and 285 females, aged (31±9)years. Of 364 patients, 67 cases undergoing single-port laparoscopic sleeve gastrectomy (SP-SG) were divided into the SP group, and 297 cases undergoing MTP-SG were divided into the MTP group. Propensity score matching was done by the 1∶1 nearest neighbor matching method. The clamp value was set as 0.1. Measurement data with normal distribution were expressed as Mean± SD, and t test was used for comparison between groups. Measurement data with skewed distribution were expressed as M( Q1, Q1), and the rank sum test was used for comparison between groups. Count data were expressed as absolute numbers, and comparison between groups was analyzed using the chi-square test or Fisher exact probability. Results:(1) Propensity score matching and comparison of general data of patients between the two groups after matching. Of 364 patients, 126 cases were successfully matched, including 63 cases in the SP group and 63 cases in the MTP group. After propensity score matching, the confounding bias of gender, body mass, body mass index (BMI), waist circumference, waist hip ratio were eliminated between the two groups. (2) Intraoperative and post-operative conditions. Both groups of patients successfully completed laparoscopic sleeve gastrectomy. After propensity score matching, the operation time, volume of intraoperative blood loss, number of postoperative painkillers used, number of postoperative antiemetics used, duration of postoperative hospital stay, duration of total hospital stay, surgical cost, and total hospitalization cost of the 63 pati-ents in SP group were 101(90,120)minutes, 10(10,10)mL, 1.0(1.0,2.5)times, 3.0(1.0,5.0)times, 4(3,5)days, 7(5,8)days, 4.1(3.5,4.3) ten thousand yuan, and (6.4±0.8) ten thousand yuan, respectively. The above indicators of the 27 patients in MTP group were 100(90,120)minutes, 10(10,15)mL, 2.0(1.0,4.0)times, 4.0(3.0,5.0)times, 3(3,4)days, 5(5,6)days, 3.2(2.8,4.2) ten thousand yuan, and (5.8±0.8) ten thousand yuan, respectively. There were significant differences in number of postoperative antiemetics used, duration of postoperative hospital stay, duration of total hospital stay and total hospitalization cost between the two groups ( Z=-2.39, -3.93, -3.03, t=4.04, P<0.05), and there was no significant difference in operation time, volume of intraoperative blood loss, number of post-operative painkillers used and surgical cost between the two groups ( Z=-0.49, -1.00, -1.23, -1.47, P>0.05). (3) Follow-up. One hundred and ninety five of the 364 patients conducted postoperative 1 month follow-up, including 25 patients in the SP group and 170 patients in the MTP group, and no patient experienced complications such as gastric leakage, infection, or incisional hernia. Both groups of patients had good surgical incisions. After propensity score matching, the change in BMI (ΔBMI), percentage of total weight loss (%TWL), and percentage of excess weight loss (%EWL) of 24 patients in the SP group were (3.7±1.4)kg/m 2, 11.0%±3.0%, 52.6%±30.0%, respectively. The above indicators of 40 patients in the MTP group were (4.1±1.3)kg/m 2, 11.1%±2.8%, 41.8%±19.1%, respectively. Patients who conducted the postoperative 12 month follow-up were 21 and 131 in the SP group and the MTP group, respectively. After propensity score matching, the ΔBMI, %TWL and %EWL of 15 patients in the SP group were (8.7±4.1)kg/m 2, 26.2%±9.8%, 130.0%±45.1%, respectively. The above indicators of 36 patients in the MTP group were (9.8±4.0)kg/m 2, 27.2%± 8.7%, 107.1%±40.7%, respectively. Conclusion:Both SP-SG and MTP-SG can be used to treat obesity patients and achieve satisfactory short-term results.

Objective To investigate the effect of adjuvant therapy of Shenfu Injection on NOD-like receptor protein 3 (NLRP3)/cysteine-aspartic acid-specific protease 1 (Caspase-1) mediated pyroptosis signaling pathway and inflammatory factor levels in rats with acute myocardial infarction (AMI). Methods A total of 40 rats were randomly divided into sham operation group, model group, betaloc group (0.9 mg/kg), and combination group (0.9 mg/kg betaloc combined with 6 mL/kg Shenfu Injection), with 10 rats in each group. The rats were treated by gavage continuously for 3 weeks. The levels of serum troponin I (cTnI), creatine kinase-MB (CK-MB), interleukin (IL)-6, IL-1β, and tumor necrosis factor-α (TNF-α) in rats were detected before modeling, after modeling, and at 3 weeks of treatment. Long-term inflammation will develop into chronic inflammation and become inflammatory diseases.Antibiotics are commonly used in clinical practice to treat inflammatory diseases.But patients are prone to drug resistance.So we need to find new treatment.Chlorogenic acid is an organic compound extracted from honeysuckle and other plants.Its anti-inflammatory activity is strong,and it has a significant anti-inflammatory effect on inflammatory diseases in various systems.It has been shown that chlorogenic acid can regulate inflammation-related signaling pathways,such as nuclear factor κB(NF-κB)canonical signaling pathway,NF-κB atypical signaling pathway,nuclear factor-erythroid 2-related factor 2(Nrf2)canonical signaling pathway,and Nrf2 atypical signaling pathway,etc.It can up-regulate the expression of anti-inflammatory cytokines such as interleukin(IL)-4,IL-10,IL-13 and down-regulate the expression of pro-inflammatory cytokine such as IL-1β,IL-6,and IL-8.Although chlorogenic acid has a strong anti-inflammatory effect,but clinical trials and application still face many difficulties.In the future,the anti-inflammatory molecular mechanism of chlorogenic acid should be further studied to explore its clinical application value and improve new ideas for the treatment of inflammatory diseases.
10.Research progress on the effect and mechanism of Tai Ji Quan in the treatment of post-stroke depression

To explore the clinical efficacy and possible mechanism of Tai Ji Quan for post-stroke depression (PSD), literature related to Tai Ji Quan and PSD were retrieved from China National Knowledge Infrastructure (CNKI), Wanfang Academic Journal Full- text Database (Wanfang), Chongqing VIP Database (CQVIP), China Biology Medicine Disc (CBM), and PubMed, screened and then summarized. The results showed that Tai Ji Quan could effectively improve the depression and quality of life of stroke patients, and there were differences in the clinical efficacy among different training programs. Tai Ji Quan has the characteristics of "regulating mind", "regulating breath", and "regulating body". It may achieve the effect of "combined physique-spirit treatment" by improving social psychology, increasing the level of neurotrophin, regulating neuroendocrine, reducing inflammatory factors, and regulating neural circuits.
