1.Clinical effect of non-diffractive extended depth of focus IOL in patients with high myopia complicated with cataract

AIM: To evaluate the postoperative clinical efficacy of non-diffractive extended depth of focus intraocular lens(EDOF IOL)in patients with highly myopic cataract(HMC).METHODS:A retrospective analysis was conducted on the clinical data of patients diagnosed with HMC at the hospital from January 2022 to December 2024. Patients were divided into an observation group [undergoing femtosecond laser-assisted cataract surgery(FLACS)combined with non-diffractive EDOF IOL implantation] and a control group(undergoing FLACS combined with aspheric monofocal IOL implantation)according to the type of implanted IOL. Postoperative visual acuity(LogMAR), visual quality, and patient satisfaction were compared between the two groups.RESULTS: A total of 33 patients(47 eyes)were finally included in this study, including 10 patients(17 eyes)in the observation group and 23 patients(30 eyes)in the control group. The observation group had a median age of 59.0(52.8, 63.8)y, with 8 males(13 eyes)and 2 females(4 eyes). The control group had a median age of 56.0(53.5, 60.0)y, with 13 males(17 eyes)and 10 females(13 eyes). At 3 mo postoperatively, the best-corrected distance visual acuity(BCDVA)was 0.10(0.08, 0.12)in the observation group and 0.20(0.10, 0.40)in the control group(P=0.586). However, the best-corrected intermediate visual acuity(BCIVA)[0.10(0.10, 0.10)vs 0.50(0.40, 0.90), P=0.032] and best-corrected near visual acuity(BCNVA)[0.20(0.18, 0.20)vs 0.60(0.45, 1.45), P=0.044] in the observation group were significantly better than those in the control group. The defocus curve showed that the uncorrected visual acuity(UCVA)in the observation group was relatively stable within the range of -2.00 to +1.00 D, which was superior to that in the control group. Postoperative questionnaires showed that the spectacle independence rate(76%)and overall satisfaction(88%)in the observation group were significantly higher than those in the control group(10% and 60%, respectively).CONCLUSION: Non-diffractive EDOF IOL significantly improves intermediate and near visual acuity, reduces spectacle dependence, and maintains distance visual acuity by extending the depth of focus, providing better postoperative visual quality and life satisfaction for HMC patients.
2.Advances in the correlation of dyslipidemia with meibomian gland dysfunction and dry eye

Meibomian gland dysfunction(MGD)and dry eye disease(DED)are prevalent ocular surface disorders, with MGD being a primary cause of DED. Although multiple observational studies, systematic reviews, and Meta-analysis have found that dyslipidemia may be associated with the rising risk of DED and MGD, the mechanisms remain unclear, and these findings have been not yet fully incorporated into Chinese expert consensus on dry eye and MGD. The article begins by comparing the similarities and differences between meibum and blood lipids in terms of composition, and their age-related trends, noting that changes in meibum composition are more likely a result of MGD rather than alterations in blood lipids. Then we systematically review epidemiological evidence revealing the association between dyslipidemia and MGD/DED, delving into the underlying pathophysiological mechanisms. Finally, we evaluate the treatment prospects as well as controversies of lipid-lowering agents on their dual mechanisms of lipid-lowering and anti-inflammatory effects. This review aims to systemically integrate current research findings, elucidate gaps and contradictions, and provide a theoretical foundation for future exploration of blood lipids' role in ocular surface homeostasis, thereby advancing systemic prevention and treatment strategies for DED and MGD.
3.Current Status and Prospective of Research on Disease-Syndrome Integrated Animal Models of Spleen and Stomach Diseases in Traditional Chinese Medicine

Animal model research on spleen and stomach diseases in traditional Chinese medicine (TCM) is of great significance for elucidating the nature of diseases and syndromes and for revealing the mechanisms of action of Chinese herbal medicinals. At present, studies on classical TCM syndrome models of spleen and stomach diseases mainly focus on spleen deficiency syndrome, liver constraint syndrome, and damp-heat syndrome. Model construction is mostly based on the etiological and pathophysiological characteristics of syndrome, and model evaluation primarily involves macroscopic manifestations and physicochemical indicators. This paper summarizes the current research status of animal models integrating disease and syndrome for seven common spleen and stomach diseases, including chronic gastritis and gastric precancerous lesions, gastroesophageal reflux disease, functional dyspepsia, inflammatory bowel disease, irritable bowel syndrome, functional constipation, and functional diarrhea. The modeling methods and characteristics of disease-syndrome combined animal models for each disease are analyzed. It is proposed that future research on disease-syndrome integration in spleen and stomach diseases should move toward syste-matic, precise, and integrative development, and that interdisciplinary and cross-disciplinary research approaches should be adopted to enhance the predictive value and application efficiency of disease-syndrome combined animal models.
4.Jianpi Xiao'ai Prescription Inhibits Colorectal Cancer Progression by Inducing Mitochondrial Dysfunction via Modulation of iNOS-ARG1 Axis

ObjectiveTo investigate the mechanism by which Jianpi Xiao'ai prescription (JPXAP) inhibits colorectal cancer progression by regulating the inducible nitric oxide synthase-arginase 1 (iNOS-ARG1) metabolic axis and inducing mitochondrial reactive oxygen species (mito-ROS)-mediated mitochondrial structural and functional impairment. MethodsAn arginine metabolism disorder model of human colorectal cancer HCT116 cells was established by combined treatment with recombinant human interferon-γ (IFN-γ, 10 μg·L-1) and N(ω)-hydroxy-L-arginine (Nor-NOHA, 200 μmol·L-1) for 24 h, followed by intervention with 5%, 10%, or 20% JPXAP-containing serum. Cell proliferation was assessed using cell counting kit-8 (CCK-8), 5-ethynyl-2′-deoxyuridine (EdU) staining, and colony formation assays. Cell invasion and migration were evaluated using Transwell chamber and wound healing assays. Mitochondrial membrane potential (MMP) and ROS levels were assessed by JC-1 and MitoSOX staining, respectively. Mitochondrial ultrastructure was observed by transmission electron microscopy (TEM). The expression of iNOS, ARG1, and mitochondrial dynamics-related proteins, including mitofusin 2 (MFN2) and dynamin-related protein 1 (DRP1), was analyzed by Western blot and immunofluorescence. The levels of L-arginine, citrulline, and urea were determined by colorimetric methods and enzyme-linked immunosorbent assay (ELISA). ResultsCompared with the blank group, the model group exhibited significantly upregulated iNOS expression, downregulated ARG1 expression, a decreased ARG1/iNOS ratio, reduced L-arginine and urea levels, and increased citrulline levels (P<0.05). Meanwhile, mito-ROS accumulation was significantly increased, the JC-1 red/green fluorescence ratio was decreased, and mitochondria showed swelling and cristae disruption, indicating that metabolic disorder induced mitochondrial injury. Compared with the model group, all JPXAP-treated groups further decreased the ARG1/iNOS ratio, enhanced nitric oxide (NO) and reactive nitrogen species accumulation, further reduced L-arginine and urea levels, and increased citrulline levels (P<0.01). EdU-positive rate, colony formation rate, wound healing rate, and Transwell invasion number all decreased significantly with increasing serum concentration (P<0.01). Mito-ROS levels were further elevated, and the JC-1 red/green ratio further decreased. TEM revealed aggravated mitochondrial swelling and vacuolization. MFN2 expression was downregulated and DRP1 expression was upregulated (P<0.01),in a dose-dependent manner. ConclusionJPXAP further activates NO-mediated oxidative/nitrosative stress under arginine metabolism imbalance, inducing mito-ROS accumulation, MMP collapse, and mitochondrial dynamics imbalance, thereby inhibiting colorectal cancer cell proliferation and migration. These findings reveal an antitumor mechanism of JPXAP based on coordinated targeting of the "metabolism-mitochondria" axis.
5.Research on Discrimination of Degradation Levels in Shipwreck Archaeological Wood Based on Microscale Attenuated Total Reflection Fourier Transform Infrared Spectroscopy

After the wooden shipwreck was recovered from the marine underwater environment,the wooden components undergo varying degrees of degradation,therefore,accurately determining the extent of degradation is a fundamental scientific issue for implementing effective preservation strategies.In this work,the wooden remains of Pinus massoniana excavated from the"Nanhai No.1"shipwreck(Southern Song Dynasty)were investigated and compared with the modern wood to discriminate the degradation levels of archaeological wood using attenuated total reflection Fourier transform infrared(ATR-FTIR)spectroscopy.The residual sugar content within wood cell walls was determined using a non-invasive automated microscale ATR-FTIR method to extract chemical information from the wood tangential section.Microstructural characterization of wood samples was conducted by super depth of field microscopy and scanning electron microscopy.FTIR spectral analysis was performed to evaluate the degradation state and elucidate changes in cellulose crystallinity.Finally,the combination of FTIR spectroscopy with the sparse partial least squares discriminant analysis(sPLS-DA)model facilitated the rapid discrimination of degradation levels in shipwreck archaeological wood,and the performance of the model was evaluated using receiver operating characteristic(ROC)curves and area under the curve(AUC).The results showed that the higher the degree of wood degradation,the lower the residual sugar content in the wood cell wall,and the residual glucose content of highly degraded wood was only 4.7%.Significant differences were observed in both the tangential section microstructure and FTIR characteristic absorption patterns across degradation levels,and as the degradation advanced,progressive cell wall loosening occurred alongside selective removal of polysaccharide components,and the relative lignin content was increased,resulting in an elevated A1509/A1370 ratio in FTIR spectra.The sPLS-DA model achieved excellent discrimination performance with AUC values exceeding 0.9,confirming that the combination of FTIR spectroscopy with sPLS-DA enabled accurate assessment of degradation levels in shipwreck archaeological wood.This study developed a rapid and accurate methodology for assessing degradation levels in shipwreck archaeological wood based on microscale ATR-FTIR spectroscopy,which would help to promote the accurate assessment of the preservation state of waterlogged wooden artifacts.
6.Simultaneous Determination of 50 Kinds of Steroid Hormones in Surface Water by Online Solid Phase Extraction Coupled with Ultra Performance Liquid Chromatography-Triple Quadrupole Mass Spectrometry

A novel analytical method was developed in this study by combining online solid phase extraction with ultra performance liquid chromatography-tandem mass spectrometry(Online SPE-UPLC-MS/MS)for simultaneous determination of 50 kinds of steroid hormones in surface water.Specifically,after high-speed centrifugation of 4 mL water samples,the supernatant was directly injected into an Oasis HLB online SPE column for enrichment and purification.Subsequently,the target compounds were transferred to the analytical column via valve switching for separation and analysis.The chromatographic separation was performed on a Thermo Acclaim RSLC C18 column(100 mm×2.1 mm,2.2 μm),using a mobile phase composed of 5 mmol/L ammonium fluoride aqueous solution and acetonitrile.Mass spectrometric detection was conducted in positive ion mode,utilizing multiple reaction monitoring(MRM)with quantification achieved by the internal standard method.The method validation demonstrated that the limits of detection(LOD)for the 50 kinds of steroid hormones ranged from 0.02 to 0.50 ng/L,while the limits of quantification(LOQ)were between 0.08 and 1.67 ng/L.The average recoveries in surface water samples at spiked concentrations of 5,20 and 200 ng/L were between 74.1%and 119%,with relative standard deviations(RSDs)of 0.2%to 9.9%.This method was applied to analyze 11 surface water samples collected from sites surrounding a pharmaceutical and chemical industrial park.A total of 44 kinds of steroid hormones were detected,with concentrations ranging from 0.11 to 88.6 ng/L,revealing the presence of hormone contamination in the environmental waters surrounding industrial areas.Compared with the traditional offline SPE methods,the proposed online SPE technique significantly reduced sample volume requirements and pretreatment time,while minimizing the loss of target compounds during the pretreatment process.Moreover,compared to reported online SPE techniques,this method achieved high-throughput analysis of multiple classes of steroid hormones,with lower detection limits and higher recoveries.Overall,this method provided rapid sample preparation,high sensitivity,and excellent stability,making it suitable for the direct analysis of trace steroid hormones in surface water.
7.Quality difference evaluation of Anemones Raddeanae Rhizoma from different origins based on multi-index quantitative determination, chemometrics and weighted TOPSIS method

Objective:To establish a quality evaluation model of chemometrics and weighted TOPSIS method; To comprehensively evaluate the quality of Anemones Raddeanae Rhizoma from different origins; To provide references for further research.Methods:The simultaneous determination of the contents of leonloside D, raddeanoside R16, raddeanoside R8, hederacolchiside E, hederasaponin B, raddeanin A, betulinic acid, oleanolic acid, betulin, daucosterol, coumarin and 4,7-dimethoxyl-5-methyl-coumarin in 18 batches of Anemones Raddeanae Rhizoma were determined by HPLC. The contents of alcohol-soluble extract, total ash and acid-insoluble ash were determined according to the Anemones Raddeanae Rhizoma test items in the 2020 edition of Chinese Pharmacopoeia. Based on this, a quality evaluation model combining chemometrics and weighted TOPSIS model was constructed.Results:An Anemones Raddeanae Rhizoma multi-index quantitative methodology was established, and the results were good. There were differences in the quality of 18 batches of samples. Chemometrics could distinguish the quality of Anemones raddeanae rhizoma from different origins, and 18 batches of samples were clustered into three categories. raddeanoside R16, raddeanin A, hederasaponin B, betulinic acid, oleanolic acid and coumarin were quality difference factors. The analysis results of weighted TOPSIS method showed that the optimal Ci of weighted TOPSIS were 0.113 6-0.732 9, and the quality of the Anemones Raddeanae Rhizoma from Liaoning provinces was better.Conclusions:Multi-index quantification provides a scientific and practical new method for the quality control of Anemones Raddeanae Rhizoma medicinal materials. Chemometrics and weighted TIPSIS methods provide a basis for the quality evaluation of Anemones raddeanae rhizoma medicinal materials from different origins.
8.Effects of Modified Buyang Huanwu Decoction on Serum Fibroblast Growth Factor 21,25-Hydroxyvitamin D,Endothelin 1 Levels and Hypercoagulable State in Type 2 Diabetes Mellitus Patients with Peripheral Neuropathy of Qi Deficiency and Blood Stasis Type

Objective To investigate the therapeutic efficacy of modified Buyang Huanwu Decoction in the treatment of type 2 diabetes mellitus(T2DM)patients with peripheral neuropathy of qi deficiency and blood stasis type,and to observe its effects on serum fibroblast growth factor 21(FGF21),25-hydroxyvitamin D[25(OH)D],endothelin 1(ET-1)levels and hypercoagulable state of the patients.Methods A total of 124 T2DM patients with peripheral neuropathy of qi deficiency and blood stasis type who admitted to Haikou Hospital of Traditional Chinese Medicine from December 2019 to December 2022 were randomly divided into a control group and an observation group,with 62 cases in each group according to the random number table method.The control group was treated with conventional western medicine,and the observation group was treated with modified Buyang Huanwu Decoction on the basis of treatment for the control group.The course of treatment for the two groups covered 12 weeks.The changes of traditional Chinese medicine(TCM)syndrome scores,blood glucose indicators,hemorheology indicators,nerve conduction velocity(NCV)and levels of serum FGF21,25(OH)D,ET-1,interleukin 6(IL-6)in the two groups before and after treatment were observed.After treatment,the therapeutic efficacy of the two groups was evaluated.Results(1)After 12 weeks of treatment,the total effective rate of the observation group was 96.77%(60/62)and that of the control group was 83.87%(52/62),and the intergroup comparison(tested by chi-square test)showed that the therapeutic efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)After treatment,the total TCM syndrome scores and the scores of TCM symptoms such as limb pain,hypoesthesia,excessive eating with frequent hunger,and limb numbness in the two groups were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05).(3)After treatment,the levels of hemorheology indicators such as hematocrit,whole blood viscosity at high-shear rate,and plasma viscosity in the two groups of patients were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05).(4)After treatment,the motor conduction velocity of the common peroneal nerve and the median nerve in the two groups of patients was increased when compared with that before treatment(P<0.05),and the increase in the observation group was significantly superior to that in the control group(P<0.05).(5)After treatment,the serum levels of blood glucose indicators such as fasting blood glucose(FBG)and glycosylated hemoglobin(HbA1C)in the two groups of patients were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05).(6)After treatment,the serum FGF21,ET-1,and IL-6 levels in the two groups were decreased(P<0.05),and serum 25(OH)D level was increased(P<0.05),and the decrease of serum FGF21,ET-1,and IL-6 levels as well as the increase of serum 25(OH)D level in the observation group was significantly superior to that in the control group(P<0.05).Conclusion For T2DM patients with peripheral neuropathy of qi deficiency and blood stasis type,the combination of modified Buyang Huanwu Decoction with conventional treatment in western medicine is helpful for reducing the inflammatory response,alleviating the damage of vascular endothelial function,regulating the levels of blood glucose,improving the neurologic function,and enhancing the clinical efficacy.
9.Elucidating the therapeutic potential of Maimendong decoction in pulmonary fibrosis: A molecular perspective

ObjectiveTo identify the active components in Maimendong decoction (MMDD) against pulmonary fibrosis (PF) and validate their molecular effects in vitro, while focusing on the role of methylophiopogonanone B in regulating fibrosis.MethodsData on MMDD components and targets were gathered from databases including BATMAN-TCM and PubMed, whereas the PF gene data were sourced from GeneCards, OMIM, and TTD. Shared targets were determined using the STRING database, and molecular docking was used to analyze the essential molecules associated with fibrosis. To simulate PF conditions, human embryonic lung fibroblasts (HPF) and A549 cells were exposed to transforming growth factor-β1 (TGF-β1). Various assays were used to determine the effects of MMDD and methylophiopogonanone B on signaling pathways, apoptosis, and epithelial–mesenchymal transition.ResultsWe identified 11 active components from MMDD extracts that targeted 511 shared proteins associated with PF, revealing 10 key targets in network analysis. Gene ontology analysis indicated that processes and pathways such as apoptosis regulation and PI3K/Akt signaling were involved. In vitro experiments revealed that MMDD downregulated the expression of α-smooth muscle actin (α-SMA), collagen type I (COL-I), and collagen type III and regulated Bcl-2/Bax signaling pathways to promote apoptosis. The flow cytometry apoptosis assay revealed that MMDD promoted the TGF-β1-induced apoptosis of myofibroblasts. The primary active ingredient in MMDD, methylophiopogonanone B, reduced α-SMA, COL-I, and PI3K/Akt/mTOR-related protein levels in TGF-β1-treated HPF cells, decreased Bcl-2 and cleaved caspase 3, and increased Bax. Moreover, methylophiopogonanone B increased E-cadherin levels and reduced α-SMA, fibronectin, N-cadherin, vimentin, and snail in TGF-β1-treated A549 cells.ConclusionMethylophiopogonanone B demonstrated the potential to treat PF by inducing myofibroblast apoptosis and inhibiting EMT. However, despite encouraging initial results, further clinical research is warranted to verify the safety and efficacy of methylophiopogonanone B in the management of PF
10.Traditional Chinese medicine dry powder inhalers: research status and development ideas and methods.

As an innovative dosage form, traditional Chinese medicine(TCM) dry powder inhalers have emerged as a focal point in the research and development of new preparations due to its high efficiency, safety, and bioavailability. This paper systematically reviewed the relevant literature and patents associated with TCM dry powder inhalers to analyze the origins and the current research and development status. Furthermore, this paper probed into the research and development ideas of TCM dry powder inhalers regarding clinical positioning, prescription screening, and druggability. Additionally, the paper thoroughly analyzed the technical barriers in druggability studies and elaborated on corresponding research techniques and coping measures. Furthermore, it emphasized the need for improved regulations and policies governing TCM dry powder inhalers, advocated for strengthened oversight, and called for the establishment of a scientific quality evaluation system. Measures such as promoting production-education-research collaboration, enhancing personnel training, and fostering international exchanges were proposed to provide a scientific and systematic reference for the future research, development, and application of TCM dry powder inhalers, thereby facilitating the rapid modernization of TCM.
