1.Study on the effect of comprehensive nursing intervention on pregnancy outcome in patients with artificial insemination by husband

Objective To investigate the effect of comprehensive nursing intervention on pregnancy outcome in patients with artificial insemination by husband. Methods 80 patients with artificial insemination by husband in the Department of Reproductive Medicine of Yichun People's Hospital from July 2016 to July 2018 were collected and divided into two groups by random number table method. The control group was treated with routine care intervention. The observation group was treated with comprehensive nursing intervention. The serum FSH level, serum estradiol concentration, serum LH level, endometrial thickness on the day of transplantation, pregnancy outcome, and psychological status in patients were observed. Results The serum LH level in the observation group was significantly higher than that in the control group (P<0.05). There was no significant difference in serum FSH level, serum estradiol concentration, endometrial thickness on the day of transplantation between the observation group and the control group (P>0.05). There was no significant difference in the clinical pregnancy rate and spontaneous abortion rate between the two groups (P>0.05). The SAS and SDS scores were lower in the observation group after the intervention, and the difference was significant from those of the control group (P<0.05). Conclusion Comprehensive nursing intervention had no significant effect on the pregnancy outcome of patients with artificial insemination by husband. The clinical pregnancy rate and spontaneous abortion rate are similar. But comprehensive nursing intervention can improve the serum LH level and improve the psychological status of patients.
2. Acute 1,2-dichloroethane exposure induced differential gene expression of liver phaseⅠmetabolic enzymes in SD rats

OBJECTIVE: To investigate the effect of 1,2-dichloroethane(1,2-DCE) acute inhalation exposure on the differential gene expression of phase Ⅰ metabolic enzymes. METHODS: The specific pathogen free SD rats were randomly divided into control group(16 rats), low-and high-dose groups(24 rats in each group, half males and half females). Low-and high-dose group were given daily 600, 1 800 mg/m~(3 ) of 1,2-DCE, and the control group given the fresh air by dynamic inhalation for 8 hours per day for consecutive 7 days. After the end of exposure, the relative mRNA expression of cytochrome P450 2 E1(CYP2 E1), alcohol dehydrogenase(ADH1) and acetaldehyde dehydrogenase 3 alpha 1(ALDH3α1) in the liver tissue was detected by real-time fluorescence quantitative polymerase chain reaction. RESULTS: The relative expression of CYP2 E1 in male high-dose group was higher than that in male low-dose group and female high-dose group(P<0.05). The relative expression of ADH1 in male low-and high-dose groups was higher than that in male control group(P<0.05). The relative expression of ADH1 in male high-dose group was higher than that in male low-dose group and female high-dose group(P<0.05). The relative expression of ALDH3α1 in high-dose group was higher than that in control group and low-dose group(P<0.05). CONCLUSION: High dose 1,2-DCE could increase the gene expression of phase Ⅰ metabolic enzymes in rat liver. The 1,2-DCE has more obvious effect in male rats than in female rats.

OBJECTIVE: To investigate the effects of subacute systemic inhalation exposure of 1,2-dichloroethane(1,2-DCE) on learning and memory in NIH mice. METHODS: Forty-five specific pathogen free healthy 7-week-old NIH mice were randomly divided into control,low-dose and high-dose groups with 5 female mice and 10 male mice in each group. The mice were exposed to 1,2-DCE at dosages of 0. 00,100. 00 and 350. 00 mg/m3 for 6 hours per day for consecutive 28 days by dynamic systemic inhalation. The neurobehavioral tests of mice were performed before and after the first to fourth weeks of exposure using the Morris water maze test. RESULTS: There was no significant difference in body weight and swimming speed among the three groups of mice( P > 0. 05). The navigation experiment results showed that the escape latency of mice in both low-and high-dose groups were longer than that of the control group at the same time point(P < 0. 05) during 1-4 weeks after exposure. In the control group,the escape latency was shorter than that of the same group before exposure( P < 0. 05). The escape latency of high-dose group prolonged with the increase of exposure time,and in the 4 th week the escape latency was significantly higher than that of the same group before exposure( P < 0. 05).The experiment results of space exploration indicated that the first time of crossing platform in low-and high-dose groups were longer than that of the control group at the second to the fourth week( P < 0. 05). The target quadrant retention time and the number of crossing the platform in the low-and high-dose groups were lower than those in the control group( P <0. 05). CONCLUSION: Subacute inhalation exposure of 1,2-DCE can impair the learning and memory ability of NIH mice.The high-dose exposure may reduce learning ability in mice in a time-effect manner.
5. Effect of different anesthetics and blood sampling methods on blood routine test in SD rats and Kunming mice

OBJECTIVE: To compare the effects of different anesthetics and blood sampling methods on blood routine test results in experimental animals. METHODS: A total of 42 specific pathogen free( SPF) male Sprague Dawley( SD) rats and 59 SPF male Kunming( KM) mice were randomly divided into 4 groups( control group,ether group,chloral hydrate group and pentobarbital sodium group). Ether group animals were treated with ether inhalation anesthesia; animals in chloral hydrate group and pentobarbital sodium group were injected intraperitoneally with chloral hydrate or pentobarbital sodium. The control group received no anesthesia treatment. Blood samples were collected by different ways: orbital venous plexus,abdominal aorta or eyeball enucleation. White blood cell( WBC) count,red blood cell( RBC) count,platelet(PLT) count,hemoglobin(Hb) level and hematocrit(HCT) in blood samples were analyzed. RESULTS: The RBC count,Hb level and HCT of SD rats in pentobarbital sodium group were significantly lower than those in control group( P <0. 05). The HCT of SD rats in ether group was lower than that in control group( P < 0. 05). The WBC count of orbital venous plexus of KM mice was lower than that taken by eyeball enucleation in control group( P < 0. 05),but the WBC count of orbital venous plexus was higher than that taken by eyeball enucleation in chloral hydrate group( P < 0. 05). The RBC count,Hb level,HCT of KM mice in pentobarbital sodium group were significantly lower than those in control group(P < 0. 05). CONCLUSION: The anesthetic can affect the blood routine test results of experimental animals. Different blood sampling methods have effects on blood routine test results of KM mice.
6.Analysis of risk factors related to the prognosis in patients with late-onset severe pneumonia after allogeneic hematopoietic stem cell transplantation

Objective To investigate the prognostic factors of late-onset severe pneumonia ( LOSP) in patients who underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods From January 2009 to December 2015, 68 patients with LOSP after allo-HSCT at Peking University Institute of Hematology were enrolled.In this retrospective study , univariate and multivariate analysis were used to evaluate the prognostic factors for LOSP after allo-HSCT.Results The median time from allo-HSCT to the development of LOSP was 213 ( 90-2330 ) days.The overall survival rate was 42.6% ( 29/68 ) .The median survival time from LOSP to death was 21 days.Early mortality was defined as death within 21 days after LOSP, as late death more than or equal to 21 days.The median oxygenation index was 199.15 (92.21-290.48) mmHg.LOSPs in thirty-two patients (36.8%) were caused by virus, bacteria, fungi or mixed pathogens.The median C-reactive protein (CRP) was 75.65 (0.94-451.00) mg/L.The median procalcitonin ( PCT) was 0.66 ( 0.00 -249.00 ) μg/L.The higher PCT value indicated an early higher mortality rate by the ROC curve (PCT:cut-off≥0.94μg/L).Furthermore, multivariate analysis suggested that PCT more than or equal to 0.94 μg/L was a risk factor for early death of LOSP ( OR=5.77, 95%CI 1.66-20.11, P=0.006).LOSP occurred later or equal to 213 days after allo-HSCT was also a risk factor of early death in LOSP ( OR=4.74, 95%CI 1.33 -16.89, P=0.017 ) .No previous history of chronic graft versus host disease (GVHD) (OR=4.50, 95%CI 1.58 -12.83, P=0.005) and LOSP later or equal to 213 days ( OR=4.40, 95%CI 1.61 -11.99,P=0.004) were the risk factors of late death in LOSP.Conclusions PCT more than or equal to 0.94 μg/L and LOSP later or equal to 213 days are the risk factors of early death in LOSP .No previous chronic GVHD and LOSP later or equal to 213 days are the risk factors of late death in LOSP .

Objective:
To analyze the clinical value of real-time PCR for virus detection in the diagnosis and treatment of patients after allo-HSCT who had no infection evidence of pneumonia using routine pathogen detection panel.
Methods:
The clinical data of 71 episodes with acute lung injury from May 2015 to March 2017 after allo-HSCT in hematology department of Peking University People’s Hospital (PKUPH) were retrospectively analyzed. PCR for virus detection and other routine pathogen detection tests were performed on bronchoalveolar lavage fluid (BALF) samples.
Results:
Among 71 episodes with acute lung injury, a total of 15 patients were diagnosed as lower respiratory tract disease merely associated with virus (detection rate of 21.13%) , 19 episodes were absent of lower respiratory tract infection. The median time from allo-HSCT to the occurrence of lung injury were 176 (49-1 376) d and 196 (57-457) d respectively (

Objective To observe the effects of trimetazidine (TMZ) on the cardiac function and neurohormonal of heart failure model in rats.Methods Partially banding abdominal aortic artery to achieve congestive heart failure rats model.Interventricular septum thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter (LVESD),left ventricular ejection fraction(LVEF) and shortening fraction(FS) were measured by echocardiogram,Pathological changes of myocardial cells was observed,B-type natriuretic peptide (BNP)、C-type natriuretic peptide receptor (NPRC),atrial natriuretic peptide (ANP),myosin heavy chain (β-MHC) and angiotensinl (AT1) were measured by Real-Time PCR,superoxide dismutase (SOD) was measured by immunohistochemistry method.Results Trimetazidine treatment of the high-dose group and the model group compare IVST LVPWT,LVESD,LVEDD were (0.63 ± 0.05) mn,(0.73 ± 0.06) mm,(0.73 ±0.05)mm,(0.87 ±0.06)mm and (1.07 ±0.06)mm,(1.13 ±0.06) mm,(0.93 ±0.06)mm,(1.33 ±0.06) mm,was significantly reduced (P < 0.05),LVEF,FS increased to (27.75 ± 1.83) %,(11.44 ± 0.76) % and (11.78 ±0.56)%,(4.27 ± 0.22)% (P < 0.01),Myocardial cell structure were remarkably improved.The expression of BNP,ANP,NPRC,ATI,β-MHC were remarkably decreased.The expression of SOD was elevated.Conclusion TMZ treatment group can improve the secretion of neurohormonal of heart failure model in rats,and also obviously improve the cardiac contractility.

OBJECTIVE:
To investigate the surgical technique of the pericanal electrode insertion technique for ies cochlear implantation.
METHOD:
Forty cases of sensorineural deafness were subjected to the ies cochlear implants. Cochleostomy was performed through the external auditory canal with a microdrill anterior to the round window. The electrode impedance and electrically auditory brainstem responses(EABR) were tested during the operation. The X-ray photographs were taken after the operation. The cochlear implant was activated in all 40 cases 4 weeks following surgery.
RESULT:
All of the electrodes were inserted and all of the implants worked well. No electrode extrusions or serious surgical complications happened during postoperative observation for 6 months.
CONCLUSION
The pericanal electrode insertion technique is a safe approach for ies cochlear implantation.

10.Effect of Serum containing Ziqi Ruangan Pill on Apoptosis Related Genes in LO2

Objective To study the influence of serum containing Ziqi Ruangan Pill on inhibitory effect of alcohol LO2 and to investigate the regulation on apoptosis related genes and the protective mechanism for LO2. Methods Rabbit sera saline containing Ziqi Ruangan Pill was prepared. The apoptotic LO2 model was established by being cultured with alcohol. The effect of serum on the model was detected by MTT method. Identification of apoptosis was carried out using flow cytometry and Hoechst3325 fluorescent staining. The expression of apoptotic inducing gene,casepase3 and NF-?B was detected by Real-time RT PCR and Western-Blot. Results The serum containing Ziqi Ruangan Pill could regulate the expression of the casepase3,NF-?B and reduce the apoptosis. Conclusion Ziqi Ruangan Pill can protect the LO2 by regulating the expression of apoptosis stimulative gene casepase3 and NF-?B.
