1.Trend of incidence rate of acute myocardial infarction in Tengzhou City, Shandong Province, from 2013 to 2021

The surveillance data of new cases of acute myocardial infarction (AMI) from January 1, 2013, to December 31, 2021, in Tengzhou City, Shandong Province, were used to analyze the incidence rate of AMI and its change trend among residents. The age and gender standardized incidence rate was calculated based on the 7th National Population Census 2020. The Cochran-Armitage trend test was used to analyze the trend of onset time and age. From 2013 to 2021, the crude and standardized incidence rate of total AMI in Tengzhou City declined from 130.07/100 000 and 161.12/100 000 to 76.15/100 000 and 72.77/100 000 ( Z=-13.785 and -20.822, both P<0.001). The crude and standardized incidence rates of males were higher than those of females. In 2016, males aged 45-54 years old and females aged 35-64 years old increased by 33.33%, 103.65%, 106.30%, and 95.75% compared to 2015, and the differences were statistically significant ( χ2=6.512, 4.965, 25.115, and 46.004, all P<0.05). The incidence rate of AMI in men aged<35 and 35-44 years old had an upward trend. From 2013 to 2021, the incidence rate of AMI decreased by 55.15% in urban areas and 36.59% in rural areas ( Z=-8.529 and -11.235, both P<0.001).

3.Daratumumab combined with cyclophosphamide in the treatment of proliferative glomerulonephritis with monoclonal immunoglobulin deposits: a case report

This article reports a rare case of proliferative glomerulonephritis with monoclonal immunoglobulin deposits. The patient, a middle-aged woman, exhibited clinical manifestations including nephrotic syndrome, microscopic hematuria, renal insufficiency, hyperglycemia, and bilateral diabetic retinopathy. Notably, monoclonal bands were absent in both blood and urine immunofixation electrophoresis. Renal biopsy revealed membranoproliferative glomerulonephritis, with immunofluorescence revealing exclusive petal-like deposition of IgG3 subtype along the capillary loop. Electron microscopy demonstrated segmental thickening of the glomerular basement membrane, along with electron dense deposits in the subendothelial and mesangial areas, lacking discernible substructure. The diagnosis comprised proliferative glomerulonephritis with monoclonal immunoglobulin deposition concurrent with diabetic nephropathy. Subsequent sequential treatment with daratumumab and cyclophosphamide over 6 months led to partial remission of nephrotic syndrome, normalization of renal function, and significant improvement of anemia. During the 24-month follow-up period, no serious adverse reactions occurred.
4.The change trend analysis of incidence of intracerebral hemorrhage in Tengzhou City,Shandong Province from 2013 to 2021

Objective:To analyze the changing trend of intracerebral hemorrhage (ICH) incidence among residents with different characteristics during 9 years of comprehensive hypertension prevention and control (hypertension prevention and control) in Tengzhou from 2013 to 2021.Methods:From January 1, 2013 to December 31, 2021, the new ICH cases collected by the Center for Disease Control and Prevention in Tengzhou City were analyzed to calculate the incidence of ICH, and the trend of its distribution was analyzed among residents with different ages, sexes, and between urban and rural areas. The registered population information came from Tengzhou Public Security Bureau. Age and sex standardized incidence was calculated based on the 7th National Population Census in 2020. The Cochran-Armitage test was used to analyze the time and age trends of the incidence.Results:The overall ICH crude and standardized incidence in Tengzhou City decreased from 97.30/100 000 to 52.13/100 000 ( Z=-9.93, P<0.001) and 119.30/100 000 to 50.69/100 000 ( Z=-15.40, P<0.001) from 2013 to 2021, and both elevated to form a single peak in 2020, with 22.58% ( χ 2=24.02, P<0.001) and 18.09% ( χ 2=17.08, P<0.001) higher than in 2019, respectively. The trends in male and female incidence over the same period were similar to the overall trends, and the incidence was higher in males than in females in all years. The incidence of ICH increased with age in all years. The difference of increase in male incidence rate in 2020 was statistically significant in three age groups ≥45 years compared with 2019 (36.29%, 23.57% and 16.18%, respectively, χ 2=6.73, 4.65, 4.00, P<0.001). The incidence of ICH decreased by 70.07% and 36.23% ( Z=18.44, 5.22, P<0.001) in urban and rural areas respectively from 2013 to 2021, whereas increased by 34.15% ( χ 2=10.88, P<0.01) and 22.08% ( χ 2=18.63, P<0.001) in 2020 compared with 2019 separately. Conclusions:The incidence of ICH in Tengzhou from 2013 to 2021 showed a significant downward trend, with the decrease in the incidence of ICH in women exceeding that in men. The decrease in the incidence of ICH in urban areas exceeded that in rural areas, and male morbidity seemingly had a younger trend.
5.The value of ultrasound-guided biopsy in the diagnosis of benign and malignant solid tumors

Objective To explore the value of ultrasound-guided coarse needle biopsy (US-CNB)in the diagnosis of benign and malignant solid tumors.Methods The clinical data of 317 patients with solid liver lesion detected by US-CNB were reviewed retrospectively.93 cases were diagnosed by pathology after operation,and 196 cases were diagnosed by biopsy,imaging and follow-up.The material satisfaction rate,diagnosis of benign and malignant tumors and postoperative complications were discussed in this study.Results Of the 317 cases,289 cases were successfully taken,28 cases were failed,and the success rate was 91.2%.The overall sensitivity of US-CNB to tumor and non tumor were 98.7% and 95% respectively,with the specificity of 95% and 98.7% respectively.The positive predictive values were 98.7% and 95%respectively,and the negative predictive values were 95% and 98.7%,respectively.No serious complications occurred after puncture.Conclusions Ultrasound-guided coarse needle biopsy is a safe,accurate and effective method to diagnose solid tumors.
6.Application of functional exercise pathway in a enhanced recovery after surgery in patients undergoing hepatectomy

Objective To investigate the application value of functional exercise pathway in enhanced recovery of patients undergoing hepatectomy.Methods Fifty primary hepatic carcinoma patients who had undergone liver resection in Anhui Province Hospital from December 2014 to August 2015 were randomly divided into experimental group (n =25) and control group (n =25).Postoperative length of hospital stay,hospitalization charges,complications,postoperative pain score,postoperative ambulation and intestinal ventilation time were compared between these groups.Measurement data with normal distribution were represented as ((x) ± s) and comprison between groups was analyzed using the t test.Comparisons of count data were analyzed by the x2 test.Results Postoperative length of hospital stay of the experimental group [(7.00 ± 2.27) days] were shorter than control group [(9.36 ± 4.58) days] (t =3.090,P =0.003),and hospitalization expense was lower in the experimental group [(28 184.57 ± 8 675.65) yuan] (t =2.281,P =0.025).Compared with control group (40%),the experimental group (20%) had lower overall complication incidence (t =4.286,P =4.286),and general complications incidence (8%) is lower than that in control group (24%),however,the incidence of operation characteristic complications had not statistical significance in two groups.Besides,the intestinal recovery time in the experimental group was shorter than control group (experimental group 36.56 hours,control group 45.24 hours,t =2.390,P =0.019).Conclusion The application of functional exercise pathway in postoperative enhanced recovery exercise of patients undergoing hepatectomy is safe and effective,which can reduce the patients' postoperative hospital stay,hospital costs,and the general incidence of complications incidence,and be helpful for the intestinal function recovery.
7. Clinical values of multimodal preventive analgesia in patients with partial hepatectomy for liver cancer

Objective:
To investigate the clinical values of multimodal preventive analgesia in patients with partial hepatectomy for liver cancer.
Methods:
A perspective study was conducted to collect data of patients with liver cancer who underwent partial hepatectomy from March 2014 to March 2015.The 90 patients involved in the study were randomly divided into two groups as multimodal analgesia and control groups, and each group had 45 cases. In multimodal analgesia group, 40 mg parecoxib sodium was injected intravenously 30 minutes before anesthetic induction, and 0.375% ropivacaine 150 mg combined with dexamethasone 5 mg were applied to transversus abdominis plane block before closing abdomen.The patients in control group without above treatment. Patient controlled intravenous analgesia was used in all patients. Three days after surgery, 40 mg parecoxib sodium was injected intravenously, twice a day for all patients.Visual analogue scales (VAS) was used to evaluate postoperative pain, and postoperative adverse events were observed.The number of cases of postoperative ambulation (>6 h for every day), time of flatus and defecation, and duration of hospital stay were recorded in two groups.Pearson chi-square test was used to compare the rate or constituent ratio between two groups.Independent sample
8.Analysis of reliability and validity of Hospice Care Quality Assessment Scale

Objective To investigate the reliability and validity of Hospice care quality assessment scale. Methods Eight experts in the field of hospice revised the Scale after Chinese localization. The scale data were analyzed after collation of 181 families fulfill the scale. Results The total scale Cronbach′s a coefficient was 0.851. The first dimension′s coefficient was 0.796. The second dimension′s coefficient was 0.622 and the third dimension′s coefficient was 0.827. In factor analysis of the scale, after varimax rotation, the number of characteristic roots>1 was 10 and made up of 70.88%of the total variance. Conclusions The hospice care quality assessment scale after Chinese localization has good reliability and validity and could be a tool to evaluate the clinical quality of care for dying patients.
9.Application effects of the pain nursing operation flowchart in standardized wards of cancer pain

Objective To investigate application effects of the pain nursing operation flowchart in the management of cancer pain patients .Methods Eighty-two patients were divided into the control group (40 patients) and the study group (42 patients) according to the admission time using the convenience sampling method.The patients in the control group received routine nursing .In the study group the pain nursing operation flowchart was designed based on the adult cancer pain guideline and the establishment standard of cancer pain standardized treatment example wards .The nursing form was designed corresponding to the flowchart .The standardized training was carried out in all nurses in the department for the improvement of their operation standardization . The comprehensiveness and the dynamics of the pain assessment , the timeliness of communicating with doctors , the comprehensiveness of pain propaganda and education , the timeliness of the management of adverse reactions and the satisfaction degree of discharged patients in the two treatment groups were evaluated .Results For the quality inspection of pain nursing of the patients in the study group , after using the flowchart , the qualified rates of the comprehensiveness and the dynamics of the pain assessment , the timeliness of communicating with doctors , the comprehensiveness of pain propaganda and education , the timeliness of the management of adverse reactions were 100.0%, 100.0%, 95.2%, 100.0%, 97.6%, respectively;the satisfaction degree of discharged patients was 97.6%, all of which were significantly higher than those in the control group (χ2 =9.31,6.80,4.44,8.04,7.74,4.18;P<0.05).Conclusions The application of the pain nursing operation flowchart in standardized wards of cancer pain can significantly improve the quality of pain management , which achieves good social benefits and worthy of promotion .
10.Effects of disease symptoms and the degree of care of patients′ spouses on postoperative depression in female with breast cancer

Objective To investigate the prevalence situation and the main influencing factors of postoperative depression in female patients with breast cancer, and provide basis for response measures. Methods Totally 307 cases of patients with breast cancer from June 2014 to May 2015 in a tertiary hospital in Tangshan were selected as the research object, and Hamilton Depression Scale ( HAMD) was used to measure the depressive and symptoms of the disease.Results The total HAMD score of 307 patients with breast cancer after surgery was (7.46±6.44);130 patients had depression mood, which accounted 42.3%. Univariate analysis showed that spouses care about image, metastasis, ill-limb activity, nausea, vomiting, shortness of breath, loss of appetite, insomnia, joint pain, insomnia and hair loss were related to total depression score of female patients with breast cancer after operation ( t/F=3. 707, 3. 522, 3. 005, 5. 690, 4. 978, 10. 805, 18. 836, 28. 478, 7. 586, 28. 478,8. 540;P<0. 05). Multiple regression analysis showed that metastasis, spouses care about image, shortness of breath, insomnia, loss of appetite, hair loss were associated with postoperative depression in female with breast cancer (P<0.05).Conclusions As the prevalence rate of depression after breast cancer surgery was high, we should pay more attention on disease symptoms and improving the degree of spouses support to reduce the occurrence of postoperative depression in patients with breast cancer.
