1.Expert Consensus on Clinical Diseases Responding Specifically to Traditional Chinese Medicine: Threatened Abortion

Threatened abortion is a common disease of obstetrics and gynecology and one of the diseases responding specifically to traditional Chinese medicine (TCM). The China Association of Chinese Medicine organized experts in TCM obstetrics and gynecology, Western medicine obstetrics and gynecology, and pharmacology to deeply discuss the advantages of TCM and integrated Chinese and Western medicine treatment as well as the medication plans for threatened abortion. After discussion, the experts concluded that chromosome, endocrine, and immune abnormalities were the key factors for the occurrence of threatened abortion, and the Qi and blood disorders in thoroughfare and conception vessels were the core pathogenesis. In the treatment of threatened abortion, TCM has advantages in preventing miscarriages, alleviating clinical symptoms and TCM syndromes, relieving anxiety, regulating reproductive endocrine and immune abnormalities, personalized and diversified treatment, enhancing efficiency and reducing toxicity, and preventing the disease before occurrence. The difficulty in diagnosis and treatment of threatened abortion with traditional Chinese and Western medicine lies in identifying the predictors of abortion caused by maternal factors and the treatment of thrombophilia. Recurrent abortion is the breakthrough point of treatment with integrated traditional Chinese and Western medicine. It is urgent to carry out high-quality evidence-based medicine research in the future to improve the modern diagnosis and treatment of threatened abortion with TCM.

Humans are born under the nourishment of heaven and earth qi.The physiological and pathological functions of the human body are influenced by the alternation of cold and heat in the four seasons.Allergic diseases often occur or worsen during seasonal transitions,corresponding to the theory of the spleen dominating the end of the four seasons in traditional Chinese medicine.Splenic dysfunction disrupts the body's internal clock during seasonal transitions,making it unable to adapt to external climate changes,causing the onset or exacerbation of specific allergic symptoms.The theory of"the spleen serves as the guard"originated from Huangdi Neijing,which is a succinct summary of the role of the spleen in defending the body against external pathogens.The theory emphasizes that the spleen transports drinks and foods internally to supplement healthy qi,and regulates defensive qi externally to protect the whole body.This theory is closely linked to the seasonal changes in immune function,thereby influencing the seasonal onset of allergic diseases.This paper explores the roles of the spleen in time-based self-regulation and immune defense against seasonal allergic diseases according to the theory of"spleen serves as the grard."The dysfunction of this protective mechanism,which leads to disharmony in nutritive qi and defensive qi,internal production of phlegm-dampness,and zang-fu organs malnutrition,are the key factors causing recurrent and persistent allergic diseases during seasonal transitions.This study provides insights for elucidating the mechanisms behind seasonal allergic disease outbreaks and their prevention.
3.Correlation analysis between carbohydrate supply ratio and central obesity in adult residents of Chongqing

Objective To investigate the incidence of central obesity and characteristics and changing trend of macronutrient energy supply in adult residents of Chongqing, and to analyze the relationship between carbohydrate energy supply and central obesity. Methods Using the longitudinal tracking data of China Health and Nutrition Survey project, combined with China food composition table , the energy and nutrient intake and macronutrient energy supply ratio were calculated. The association between carbohydrate energy supply and central obesity was analyzed by multi-level statistical model. Results In 2011, 2015 and 2018, the carbohydrate supply ratio of adult residents in Chongqing was 43.02%, 46.52%, and 46.07%, respectively, and the difference was statistically significant (F=18.699, P<0.001). The overweight rates in 2011, 2015, and 2018 were 29.2%, 36.7%, and 37.8%, while the obesity rates were 12.2%, 13.5%, and 19.5%, respectively, with statistically significant differences (χ2=41.416, P<0.001). The central obesity rates were 51.5%, 57.2%, and 62.8%, respectively (χ2=21.008, P<0.001). The carbohydrate supply ratio was positively correlated with waist circumference. Compared to the population with a carbohydrate to energy ratio of <55%, the risk of central obesity in the population with a carbohydrate to energy ratio of ≥ 65% was 1.63 times higher. Conclusion The ratio of carbohydrate to energy supply of adult residents in Chongqing has slightly increased. Rates of overweight, obesity and central obesity are on the rise. A high carbohydrate to energy supply ratio may be a risk factor for central obesity.
4.An exploration into the thinking of pattern-differentiation-deduced pathogenesis in Shanghan Lun

The basic diagnostic and treatment principles of traditional Chinese medicine are the pattern-differentiation-deduced pathogenesis and treatment based on examination of the pathogenesis.Shanghan Lun established a system of pattern differentiation of six meridians,systematically summarized the evolution laws of exogenous febrile disease,and discussed miscellaneous diseases as exogenous diseases.It contains 112 classic formulas and advocates that"those whose disease corresponds to the formulas should take them accordingly",combining disease differentiation and pattern differentiation,containing not only the knowledge of treatment based on pattern identification,but also the profundity of pattern-differentiation-deduced pathogenesis.Based on the original meaning of"pathogenesis",from the three angles of"subtle meaning","core meaning",and"appropriate meaning",this paper exemplifies the concept of pattern differentiation for six meridians in Shanghan Lun,analyzes the relationship and connotation of formula patterns of classical formulas in Shanghan Lun by"taking pattern with formulas",and explores the concept of pattern-differentiation-deduced pathogenesis in Shanghan Lun.This paper emphasizes that during the clinical practice of traditional Chinese medicine,it is important(i)to deduce the pathogenesis based on pattern differentiation,understand symptoms upon first onset,and deduce the pathogenesis before symptoms appear,thereby treating the disease at an early stage;(ii)to identify the most important concepts,solve complications with simple method,discern the core of the disease,and treat the entire body;and(iii)to optimize timing,act quickly,take advantage of opportunities,accept outcomes,and provide judicious guidance to patients.This paper serves to interpret Shanghan Lun based on pattern-differentiation-deduced pathological mechanism and provides ideas and inspiration for applying the theory of Shanghan Lun to guide clinical practice.
