OBJECTIVE To explore the differences in T lymphocyte subsets of the severe pneumonia(SP)patients among different age groups and analyze its relationship with the prognosis.METHODS A total of 100 patients with SP who were treated in the First Affiliated Hospital of Zhengzhou University from Mar.2018 to Jun.2024 were enrolled in the study and were divided into the youth group with 22 cases(less than 40 years old),the middle age group with 36 cases(aging between 40 and 65 years old)and the old age group with 42 cases(more than 65 years old).The clinical data and the laboratory test indexes were compared among the three groups.The levels of CD3+,CD4+,CD8+and CD4+/CD8+before and after the treatment were compared among the three groups by repeated measures variance method.RESULTS The length of hospital stay of the old age group was longer than those of other two groups;the acute physiology and chronic health evaluation(APACHE)Ⅱ score,clinical pul-monary infection score(CPIS)and the levels of interleukin-6(IL-6),platelets(PLT),white blood cells(WBC),C-reactive protein(CRP)and lactic dehydrogenase(LDH)levels were higher in the old age group than in other two groups(P＜0.05).The levels of CD3+,CD4+,CD8+and CD4+/CD8+of all the three groups were elevated after the treatment,with the highest in the young group after the treatment for 2 months[the CD3+level(56.25±5.21)％,the CD4+level(35.65±2.43)％,the CD8+level(22.33±1.74)％,and the CD4+/CD8+(1.97±0.25)],the lowest in the old age group[after the treatment for 2 months the CD3+level(49.46±2.67)％,the CD4+level(28.45±3.45)％,the CD8+level(15.68±2.23)％,and the CD4+/CD8+(1.46±0.14)],there were significant differences(F=21.830,P=0.045;F=41.870,P=0.024;F=71.914,P=0.014;F=48.463,P=0.020).CONCLUSION The CD3+,CD4+,CD8+and CD4+/CD8+have great clinical significance in diagnosis of SP for different age groups of patients and assessment of illness condition.

OBJECTIVE: To study the expression of tissue inhibitor of matrix metalloproteinase 2 (TIMP2)in the nasopharyngeal carcinoma and to evaluate the relationships between the expression of TIMP2 and biological behavior of NPC. METHOD: The expression of TIMP2 was observed by immunohistochemical technique in 50 cases with nasopharyngeal carcinoma and 15 cases of Inflammation nasopharyngeal tissues. RESULT: The positive rates of TIMP2 are significantly higher in NPC than in Inflammation tissues (P < 0.05). The expression of TIMP2 increase significantly with lymph node metastasis (P < 0.01), but had no relevant to the rise clinical invasion stage (P > 0.05). CONCLUSION The results indicate that TIMP2 may play a critical role in assisting carcinoma cell to degrade basement membrane to invade surrounding tissues and to form metastatic colonies in lymph node. Especially the unbalance in MMP2/TIMP2 has probably a prognostic value in nasopharyngeal carcinoma.

OBJECTIVE To study the expression of matrix metalloproteinase 2 (MMP-2) and its correlation with microvascular density(MVD) and tumor metastasis in nasopharyngeal carcinoma. METHODS The expression of MMP-2 and MVD were detected in the specimens of 50 cases with nasopharyngeal carcinoma and 15 cases with nasopharyngeal inflammation by immunohistochemical technique. The genes of P53,retinoblastoma (Rb),C-myc and human papilloma virus (HPV) were determined by PCR method in specimens of 33 patients with laryngeal carcinoma and 4 normal laryngeal tissues as a control group.There were 23 males and 10 females,ranging in age from 40 to 11 years old.Results showed that the mutation rate of P53 and Rb gene were 69.69%(23/33) and 48.48%(16/33),respectively;c\|myc gene amplification rate and HPV insertion rate were 58.62% (17/29) and 72.72%(24/33),respectively.There were significant difference(P＜0.05)in comparison between positive and negative expression above these genes.The expression rate were 50.00%(12/24),45.83%(11/24)and 66.67%(16/24)of C-myc,Rb and P53 genes respectively in 24 cases who were HPV positive of laryngeal carcinoma.The positive rates were 73.68%(14/19),63.16%(12/19),57.89%(11/19)and 84.21%(16/19) of HPV,C-myc,Rb and P53,respectively in 19 cases who have heavy smoking (20/day,20 years smoking history).Above results suggested that there were closed relationship in the C-myc gene activation and P53,Rb genes inactivation with smoking and HPV infection.It was believed that carcinogen factors on laryngeal carcinoma was the result of factors,genes and stages interaction.

Objective To explore a simple and convenient surgical method for maxillary sinus cyst by endoscopic surgery. Methods Rudolf 4.5mm cannula paracentetic needle was used.The cyst was cut throuqh inferior nasal meatus and sampled. Results 30 cases were followed up for half a year to 3 years.No relapse was found either through X-ray or through CT scanning. Conclusions Exicision of maxillary sinus cyst by endoscopic surgery through inferior canal is one of minimally invasive operations,and the effect is positive.
