1.Construction of the Diagnosis and Treatment System of "Sinew Prescription Correspondence" under the Guidance of Systematic Dialectical Sphygmology

"Sinew prescription correspondence" is the principle of selecting prescriptions for channel sinew diseases. On the basis of the theory of syndrome differentiation and treatment, the pulse manifestation corresponds to the channel sinew syndrome, which can improve the flexibility and standardization of clinical prescriptions. From the perspective of systematic dialectical sphygmology, this paper explains the dialectical relationship between channel sinew theory and pulse body elements, pulse wall elements, pulse elements and blood flow elements, and clarifies the internal relationship between pulse manifestation and prescriptions at the level of channel sinew disease. The prescription is derived from the method, while the method is established with the syndrome, and the prescription is unified by the method. According to the theory of "sinew prescription correspondence", the treatment ideas of channel sinew diseases were analyzed from the perspective of channel sinew distribution, functional characteristics and structural changes. On this basis, the diagnosis of channel sinew disease and the application of prescriptions are expanded, and the research on the internal treatment and diagnosis mode of "pulse manifestation-channel sinew-zang fu (脏腑)" is prospected, so as to expand the differentiation and treatment methods of channel sinew theory.
2.Establishment and stress analysis of a finite element model for adolescent cervical disc herniation

BACKGROUND:Cervical disc herniation can cause pain in the neck and shoulder area,as well as radiating pain in the upper limbs.The incidence rate is increasing year by year and tends to affect younger individuals.Fully understanding the biomechanical characteristics of the cervical spine in adolescents is of great significance for preventing and delaying the onset of cervical disc herniation in this age group. OBJECTIVE:To reconstruct cervical spine models for both healthy adolescents and adolescent patients with cervical disc herniation utilizing finite element analysis techniques,to analyze the motion range of the C1-T1 cervical vertebrae as well as the biomechanical characteristics of the annulus fibrosus,nucleus pulposus,endplates,and the cartilage of the small joints. METHODS:A normal adolescent's cervical spine and an adolescent patient with cervical disc herniation were selected in this study.The continuous scan cervical spine CT raw image data were imported into Mimics 21.0 in DICOM format.The C1-T1 vertebrae were reconstructed separately.Subsequently,the established models were imported into the 3-Matic software for disc reconstruction.The perfected models were then imported into Hypermesh software for meshing of the vertebrae,nucleus pulposus,annulus fibrosus,and ligaments,creating valid geometric models.After assigning material properties,the final models were imported into ABAQUS software to observe the joint motion range of the C1-C7 cervical vertebrae segments under different conditions,and to analyze the biomechanical characteristics of the annulus fibrosus,nucleus pulposus,endplates,and small joint cartilage of each cervical spine segment. RESULTS AND CONCLUSION:(1)In six different conditions,the joint motion range of the C1 vertebra in the cervical spine models of both normal adolescent and adolescent patient with cervical disc herniation was higher than that of the other vertebrae.Additionally,the joint motion range of each cervical spine segment in normal adolescent was greater than that in adolescent patient with cervical disc herniation.(2)In the cervical spine model of normal adolescent,the maximum stress values in the annulus fibrosus and nucleus pulposus were found on the left side during C2-3 flexion conditions(0.43 MPa and 0.17 MPa,respectively).In the cervical spine model of adolescent patient with cervical disc herniation,the maximum stress values were found on the left side during C7-T1 flexion conditions(0.54 MPa and 0.18 MPa,respectively).(3)In the cervical spine model of normal adolescent,the maximum stress value on the endplate was found on the left side of the upper endplate of C3 during flexion conditions(1.46 MPa).In the model of adolescent patient with cervical disc herniation,the maximum stress value on the endplate was found on the left side of the lower endplate of C7 during flexion conditions(1.32 MPa).(4)In the cervical spine model of normal adolescent,the maximum stress value in the small joint cartilage was found in the C2-3 left rotation conditions(0.98 MPa).In adolescent patient with cervical disc herniation,the stress in the small joint cartilage significantly increased under different conditions,especially in C1-2,with the maximum stress found during left flexion(3.50 MPa).(5)It is concluded that compared to normal adolescent,adolescent patient with cervical disc herniation exhibits altered cervical curvature and a decrease in overall joint motion range in the cervical spine.In adolescent with cervical disc herniation,there is a significant increase in stress on the annulus fibrosus,nucleus pulposus,and endplates in the C7-T1 segment.The stress on the left articular cartilage of the C1-2 is notable.Abnormal cervical curvature may be the primary factor causing these stress changes.
3.Development and Validation of a Risk Prediction Model for Sudden Cardiac Arrest in Children With Congenital Heart Disease After Surgery

Objectives:To develop a risk prediction model for sudden cardiac arrest(CA)in children with congenital heart disease(CHD)after surgery and validate its predictive efficacy,providing a reference for the prevention of CA and risk stratification.Methods:Medical records were retrospectively analyzed from 5 029 children who were hospitalized in Fuwai Hospital,Chinese Academy of Medical Sciences from January 1,2020 to May 31,2022 and underwent CHD surgery.The patients were divided into two groups:those who experienced CA after surgery(n=33)and those who did not(n=4 996).A random forest model for predicting the risk of postoperative CA was established on the training dataset using R software,and the predictive effect of the model was evaluated on the validation dataset using indicators of predictive accuracy,sensitivity,specificity,positive predictive value,negative predictive value.Results:The incidence of CA in this center was 0.66%,survival rate is 72.73%.Using the random forest algorithm,the importance of risk factors for sudden CA after CHD surgery was ranked by variable importance scoring,with the following top 6 important predictive variables:blood pressure,lactate levels,heart rate,cardiac rhythm,arterial oxygen partial pressure,and blood oxygen saturation on the first day after surgery.The model established by the random forest algorithm on the training set was validated on the test set,yielding a predictive accuracy of 99.8%,specificity of 87.5%,sensitivity of 99.9%,kappa coefficient of 0.8225,positive predictive value of 99.9%,and negative predictive value of 77.8%.Conclusions:The established prediction model of sudden CA in children with CHD after surgery had good performance.It might help medical staffon decision making of early intervention,preventing the occurrence of CA,and improving the outcomes of children with high risk of CA post surgery.
4.Comprehensive management ability of Helicobacter pylori screening in primary hospitals of Jiangsu Province

Objective:Based on the activities of Helicobacter pylori ( HP) screening in Jiangsu Province in 2024, to evaluate the overall management ability in HP screening, testing, treatment and follow-up in primary medical facilities. Methods:From May 15 to October 18, 2024, the data of HP screening and treatment were retrospectively collected from 79 township health centers, community hospitals, and community health service centers in Jiangsu Province. The rates of screening completion, urea breath test (UBT) completion, treatment rate, UBT follow-up completion, and HP eradication were analyzed. Chi-square test was used for statistical analysis. Results:The completion rate of HP screening was 94.45% (15 489/16 400). There were 6 604 cases (42.64%) with serum HP antibody positive among the 15 489 individuals who completed screening. The positive rate of serum HP antibody in males was higher than that in females (44.77%, 2 643/5 904 vs. 41.32%, 3 961/9 585), and the difference was statistically significant ( χ2=17.69, P<0.001). The positive rates of serum HP antibody in screened individuals aged 18 to 19, 20 to 39, 40 to 59, and 60 to 75 years old were 22.38% (32/143), 36.12% (1 168/3 234), 45.01% (3 240/7 199), and 44.05% (2 164/4 913), respectively, and the difference was statistically significant( χ2=100.73, P<0.001). Among the 6 604 HP antibody-positive individuals, 4 381 cases completed UBT, with a UBT completion rate of 66.34% (4 381/6 604). There were 3 197 individuals with both HP serum antibody and UBT positive, the consistency rate of the 2 tests was 72.97% (3 197/4 381). Totally 2 737 cases received treatment, with a treatment completion rate of 85.61% (2 737/3 197); 2 327 individuals underwent UBT follow-up, with a follow-up completion rate of 85.02% (2 327/2 737). During follow-up, the result of UBT was negative in 1 982 individuals, and the HP eradication rate was 85.17% (1 982/2 327). Conclusions:There are deficiencies in the completion rate of HP screening, testing, treatment, and follow-up in primary hospitals, especially in the completion rate of UBT, which may be related to cognitive insufficiency for HP in residents. It is necessary to strengthen the training of physicians′ abilities in primary hospitals, optimize the allocation of drug resources, enhance health education, and increase residents′ participation and compliance.
5.Construction of p97 mutant of Mesomycoplasma hyopneumoniae based on the homologous recombination system

The aim of this study is to establish an gene editing method of Mesomycoplasma hyo-pneumoniae(Mhp)based on the homologous recombination principle.The restriction enzyme di-gestion and ligation method combined with gene synthesis were used to construct a shuttle plasmid to achieve replication in both Mhp and Escherichia coli(E.coli).The pGEM?-T vector was used as the skeleton.The oriC sequence of Mhp which can achieve the replication of the plasmid in Mhp was inserted into the vector.Sequences of the Spiroplasma promoter and puromycin resistance gene were then inserted into the above constructed plasmid to screen recombinant clones.The up-stream and downstream homologous arms of p97 were constructed to initiate homologous recombination.The recA gene of E.coli is inserted to improve the efficiency of homologous recom-bination.The obtained shuttle plasmid was then delivered into Mhp by electro-transformation or chemical transformation.A shuttle plasmid,pGEM?-Mhp-oriC-p 97,which can replicate in both Mhp and E.coli was constructed.With the transformation of this plasmid,the carried puromycin gene and recA gene can be expressed,the p97 gene can be edited.Finally,the genetically unstable p97 gene mutant was initially obtained.In this study,a tool for Mhp gene editing based on the principle of homologous recombination was established,which laid a foundation for the develop-ment of tools for studying the pathogenesis of Mhp.
6.Global research status,hotspot analysis and trend outlook of tick-borne encephalitis

This study explored the current status,hotspots,and research trends in tick-borne encephalitis(TBE)worldwide.The bibliometric analysis and knowledge mapping software,VOSviewer,was used to conduct a comprehensive study of the literature in the field of TBE in the Web of Science database and the CNKI database,and to construct a research framework of the TBE field in order to demonstrate the association between the main keywords,research countries,research institutions and published journals in this field.A total of 2 046 English-language and 582 Chinese-language publications were included in this study,with an increasing trend of publication year by year.Keyword co-occurrence analysis showed that TBE and its viruses were the focus of research,along with infection,epidemiology,pathogen classification,prevalence,transmission,and clinical symptoms.The United States,Germany,and other countries were at the top of the list of publications and citations.Institutions such as the Russian Academy of Sciences and Medical University of Vienna were prominent contributors to TBE research.Journals represented by Ticks and Tick-borne Diseases were in the lead in terms of publications and citations and were important publications for research in this field.TBE research showed wide and vigorous trends worldwide.The study displayed the current status of research and the evolution of hot trends in this field,which provided us with strong support for examining TBE as a public health problem from a broader perspective and was also of great significance for promoting future in-depth research and formulating precise prevention and control strategies.
7.Exploring the Antidepressant Mechanisms of Citron and Bergamot Based on Network Pharmacology and BDNF/TrkB/CREB Signaling Pathways

Objective Using network pharmacology research methods and animal pharmacology experiments,explore the mechanism of antidepressant effects of traditional Chinese medicine Citron and Bergamot.Methods Using the Traditional Chinese Medicine System Pharmacology Database and Analysis Platform(TCMSP),ETCM,Symmap,Swiss Target Prediction,and Uniprot data platforms,screen the active ingredients and corresponding gene targets of Citron and Bergamot.Obtain depression gene targets using OMIM,TTD,and Cenecards data platforms.Using Venny 2.1 online software,draw Venn diagrams of the intersection of active ingredients and gene targets.Draw network diagrams between drugs,active ingredients,targets,and diseases using Cytoscape 3.7.2 software.Construct a protein-protein interaction(PPI)network diagram using the STRING data platform for intersecting genes.Using the Metascape data platform,perform gene ontology(GO)function and Kyoto Encyclopedia of Genesand and Genomes(KEGG)pathway enrichment analysis.A rat depression model was established using chronic unpredictable mild stress(CUMS)combined with solitary care,and animal experiments were conducted to verify the BDNF/TrkB/CREB signaling pathway obtained from network pharmacology research.Results The research results of network pharmacology methods show that there are 57 antidepressant active ingredients in Citron,65 antidepressant active ingredients in Bergamot,and important active ingredients include Acetic acid,3,4,7-trimethoxycoumarin and Citric acid,etc.Through the data platform,2717 depression targets and 430 intersection targets were identified.Through PPI network analysis,key gene targets for antidepressant effects in Citron and Bergamot were identified,including TP53,Protein kinase B1,CREB-binding protein,Brain derived neurotrophic factor,etc.Through KEGG analysis,it was found that important signaling pathways include pathways in cancer,PI3K-Akt signaling pathway,Neurotrophin signaling pathway,etc.By observing the neurotrophic factor BDNF/TrkB/CREB signaling pathway in depressed rats,the results showed that the medium dose groups of Citron and Bergamot could significantly increase serum BDNF content(P<0.05),and each treatment group could improve the damage of hippocampal neurons in rats.The high and medium dose groups of Citron and Bergamot significantly increased the expression of BDNF protein in the hippocampal CA1 region(P<0.05,P<0.01).Except for the low-dose group,which showed no difference in TrkB mRNA gene expression,all other treatment groups significantly increased the mRNA gene expression levels of hippocampal BDNF,TrkB and CREB(P<0.01).The medium dose group of Citron and Bergamot increased the expression of BDNF protein in the hippocampus(P<0.01),while the medium and low dose groups significantly increased the relative expression of TrkB protein in the hippocampus(P<0.05).The medium dose group showed an increasing trend in the relative expression of CREB protein.Conclusion Traditional Chinese medicine Citron and Bergamot have therapeutic effects on depression models in rats,and the mechanism of action may be related to the BDNF/TrkB/CREB signaling pathway.
8.Effect and potential mechanism of clarithromycin in treatment of inflammatory enteritis

Aim To explore the mechanism of clar-ithromycin in treating inflammatory bowel disease(IBD)by inhibiting Kv1.3 channel protein in colonic epithelial cells.Methods A chronic IBD rat model was induced using dextran sulfate sodium(DSS)in vi-vo experiments,with clarithromycin intervention.The physical signs of each group of rats were observed,and the disease activity index(DAI)score and colonic mu-cosal damage index(CMDI)score were calculated.RT-qPCR was used to detect the levels of relevant cyto-kines in colonic tissue of rats.Flow cytometry was em-ployed to detect the relative proportions of immune cells in the peripheral blood and colonic tissue of each group of rats.Lipopolysaccharide(LPS)was used to establish an inflammation model of colon epithelial cells(NCM460)to clarify the inhibitory effect of clar-ithromycin on Kv1.3 channel protein.Results In vi-vo experiments:compared to the model group,the clar-ithromycin intervention group exhibited a reduced de-gree of weight loss(P<0.01),and a significant de-crease in DAI scores(P<0.01).There was an in-crease in colon length,a reduction in weight,and a de-crease in CMDI scores(P<0.05).Levels of TNF-α,IL-1 β,and IL-6 in colon tissue were significantly re-duced(P<0.01).The numbers of peripheral blood and colonic regulatory T lymphocytes(Th),cytotoxic T lymphocytes(CTL),natural killer cells(NK),B lym-phocytes(B),and dendritic cells(DC)were signifi-cantly decreased(P<0.05).Clarithromycin reduced the expression of Kv1.3 channel protein in colon tissue(P<0.05).In vitro experiments:compared to the model group,the clarithromycin group significantly pro-moted the proliferation of NCM460 cells(P<0.01)and simultaneously significantly reduced the levels of TNF-α and IL-6 in cells(P<0.05).Clarithromycin also reduced the expression of Kv1.3 channel protein in NCM460 cells(P<0.05).Conclusions Clar-ithromycin may play an immunomodulatory role by in-hibiting the expression of Kv1.3 channel protein,re-ducing inflammation in the body,and playing a role in the treatment of IBD.
9.Early diagnostic value of Pentraxin-3 promoter methylation for compli-cated appendicitis

Objective:To investigate the early diagnosis value of Pentraxin-3(PTX-3)promoter methylation for complicated appendicitis.Methods:Patients with appendicitis and healthy physical examination from Qingdao Hiser Medical Group were selected as the research objects,and they were divided into complicated appendicitis group(CA),simple appendicitis group(SA)and healthy control group(HCs).Plasma PTX-3 levels,mRNA expression,promoter methylation status,and clinical parameters—including total bilirubin(TBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(Alb),white blood cell count(WBC),neutrophil count(NEU),C-reactive protein(CRP),and procalcitonin(PCT)—were analyzed.in each group.Spearman correlation analysis was used to test the correlation of variables.Multivariate Logistic regression analysis was used to test the correlation between PTX-3 gene methylation and clinical parameters.The area under the receiver operating characteristic curve(AUC)was used to analyze the diagnostic value of PTX-3 methylation for CA.Results:The mRNA level and plasma concentration of PTX-3 in CA group were significantly higher than those in SA group and HCs group,while the methylation frequency of PTX-3 in CA group was significantly lower than that in SA group and HCs group(P<0.05).The methylation status of PTX-3 gene was significantly correlated with inflammatory markers(WBC,NEU,PCT,CRP)(P<0.05).Multivariate Logistic regression analysis showed that WBC,CRP and PCT were independent influencing factors of PTX-3 gene promoter methylation(P<0.05).Spearman correlation analysis showed that the PTX-3 mRNA level in peripheral blood of CA patients was negatively correlated with its methylation status(P<0.001).PTX-3 mRNA level was positively correlated with WBC,NEU,CRP and PCT levels(P<0.05).The sensitivity and specificity of PTX-3 gene methylation in the diagnosis of CA were 94.67%and 76.67%,re-spectively.When CA was diagnosed from SA patients,the AUCs of PTX-3 methylation were significantly higher than those of WBC,NEU,CRP and PCT(P<0.001).Conclusion:PTX-3 promoter methylation is involved in the pathogen-esis of AA by regulating the expression of PTX-3.It can be used to monitor the inflammatory state of patients with com-plicated appendicitis and serve as a non-invasive early diagnosis biomarker for complicated appendicitis.
10.Investigation and analysis ofincidence and treatment of mastitis in 79 large-scale pastures in China

This study aims to clarify the incidence and prevention status of dairy cow mastitis in 79 large-scale pastures in China in 2022,reduce the incidence of dairy cow mastitis,and provide scien-tific basis for formulating prevention and control strategies for dairy cow mastitis in large-scale pastures suitable for China's national conditions.The research team relied on the large-scale ranch of the Comprehensive Experimental Station of National Dairy Industry Technology System to car-ry out research.A total of 79 questionnaires were received.The information on the stock of large-scale farms,the proportion of cows in the herd,the level of cow yields,the incidence and cure rate of cow mastitis,the prevention and control of cow mastitis,diagnosis and treatment programs of cow mastitis,the cost of treating cow mastitis,and the culling rate of farms was collected.Chi-square test and correlation analysis were carried out on the incidence of mastitis in dairy cows and the prevention and control plan,treatment plan,yield level and elimination rate of mastitis in dairy cows by descriptive statistical cross table.The incidence of cow mastitis in some pastures was con-trolled between 5%and 10%,and the cure rate was mostly as high as 98%.According to the co-hort analysis,the importance of prevention and control measures for dairy cow mastitis was as fol-lows:DMT>DHI>milk yield>milk traits>breast apparent change>electrical conductivity>CMT.The importance of treatment measures for dairy cow mastitis is as follows:DMT>DHI>milk yield>milk traits>breast apparent changes>electrical conductivity>CMT.The importance of treatment measures for mastitis is as follows:traditional Chinese medicine>pathogen detection and drug sensitivity test>non-steroidal anti-inflammatory drugs>regular maintenance of milking equipment>normal milking without treatment>broad-spectrum antibiotics.The incidence of mas-titis in dairy cows is moderately positively correlated with the yield per unit area,that is,the in-crease in the incidence of mastitis in dairy cows has a certain impact on the yield per unit area of dairy cows.Economic analysis shows that the treatment cost of mastitis is closely related to the in-cidence rate,and the treatment cost of pasture with higher incidence rate is higher.Pastures with higher yields usually have more efficient means of disease prevention and control,and the corre-sponding treatment costs are lower.In conclusion,China's large-scale pastures should strengthen the scientific feeding management level,improve the prevention and control methods to reduce the incidence of cow mastitis,improve the cure rate,and reduce the cost of pasture treatment.
