1.Research Tackling Paradigm and Technological Layout Strategies Based on Erectile Dysfunction, A Clinical Dominant Disease of Traditional Chinese Medicine

To thoroughly implement the strategic deployment outlined in the Opinions of the Central Committee of the Communist Party of China and the State Council on Promoting the Inheritance and Innovative Development of Traditional Chinese Medicine regarding research on dominant diseases of traditional Chinese medicine and to uphold the development philosophy of equal emphasis on traditional Chinese medicine and western medicine,the China Association of Chinese Medicine has fully played a leading academic role by systematically organizing and conducting a series of academic youth salons on clinical dominant diseases of traditional Chinese medicine. On September 13,2024,the 36th Youth Salon on Clinical Dominant Diseases was successfully held in Nanjing,focusing on the advantages of traditional Chinese medicine and the integrative traditional Chinese medicine and western medicine in the diagnosis and treatment of erectile dysfunction (ED). The conference brought together leading experts from traditional Chinese medicine,western medicine,and interdisciplinary fields,facilitating in-depth multidisciplinary discussions that led to key consensus on optimizing traditional Chinese medicine treatment protocols for ED,researching and developing new drugs of traditional Chinese medicine,and advancing interdisciplinary development in traditional Chinese medicine. This salon systematically sorted out the clinical strengths and distinctive features of traditional Chinese medicine in the diagnosis and treatment of ED. Based on current research foundations and clinical needs,it identified key directions for future scientific layout and scientific research tackling: (1) Standardization of syndrome differentiation system of traditional Chinese medicine for ED. (2) Optimization and standardization of intervention methods of integrated traditional Chinese medicine and western medicine. (3) High-quality clinical research guided by evidence-based medicine. (4) In-depth analysis of the pharmacological mechanisms of traditional Chinese medicine in the treatment of ED. (5) Clinical translation and application promotion of new drugs of traditional Chinese medicine. (6) Interdisciplinary integration and innovation in traditional Chinese medicine. For each research direction,key focus areas,expected objectives,and clinical value were further refined,along with the establishment of a scientifically sound priority funding level evaluation system. Therefore,building on the series of salons on the ED-focused dominant diseases of traditional Chinese medicine,this paper provides standardized guidance for clinical practice of traditional Chinese medicine in ED management,effectively contributing to the high-quality development of traditional Chinese medicine. It serves as a valuable reference for national scientific and technological strategic layout, research and development decision-making in new drugs of traditional Chinese medicine,research topic planning,and clinical guideline formulation.
2.Antibody levels of diphtheria and tetanus in healthy population in Pudong New Area, Shanghai, 2018-2024

Objective To understand the antibody levels of diphtheria and tetanus among healthy population in Shanghai Pudong New Area, and to provide a scientific basis for improving the vaccine immunization strategy. Methods Random sampling was used to select healthy people of all ages in 16 communities in Shanghai Pudong New Area from 2018 to 2024, and serum samples were collected and tested for serum anti-diphtheria and tetanus toxin IgG antibodies by enzyme-linked immunosorbent assay (ELISA) method to analyze the antibody positivity rate (≥0.1 IU/ml) and the geometric mean concentration (GMC) of antibodies. Results A total of 3 312 serum samples were included, with a male-to-female ratio of 0.76:1, and 53.77% were local residents. The seropositivity rates and geometric mean concentrations (GMC) of both diphtheria and tetanus antibodies generally declined with increasing age, but exhibited a transient rebound in the 7y-. A total of 1 175 individuals (35.48%) were seropositive for diphtheria, with a GMC of 0.054 IU/mL. For tetanus, 988 individuals (29.83%) were seropositive, with a GMC of 0.033 IU/mL. Significant differences in seropositivity rates (χ2diphtheria=950.005,χ2tetanus=1 324.393) and GMC (Hdiphtheria=1027.160,Htetanus=1 142.007) were observed among different age groups (P<0.001). Significant differences in seropositivity rates (χ2diphtheria=950.005,χ2tetanus=1324.393) and GMC (Hdiphtheria=1027.160,Htetanus=1142.007) were also found across different years (P<0.001). Conclusion The prevalence of diphtheria and tetanus antibodies in the healthy population of Pudong New Area is relatively low, particularly among adults over 20 years of age with inadequate immunization. This underscores the need to reinforce the National Immunization Program (NIP) vaccine specifications for children under 6 years of age and implement an immunization strategy for adolescents or adults against diphtheria and tetanus.
3.Therapeutic efficacy and mechanism of artesunate for mouse model of polycystic ovary syndrome

Objective To investigate the therapeutic efficacy of artesunate(AS)on polycystic ovary syndrome(PCOS)in mice and explore the potential mechanism primarily.Methods Twenty-five female C57BL/6J mice were randomly divided into Control group,model group(PCOS group),low-and high-dose AS groups(AS15 and AS30 groups)and metformin group(Met group).In addition to the Control group,the mouse model of PCOS was established by subcutaneous injection of dehydroepiandrosterone(DHEA,60 mg/kg)following by a high-fat diet for 21 d.After modeling,AS of 15 and 30 mg/kg was intraperitoneally injected into the mice of the AS 15 and AS30 groups,respectively,and 200 mg/kg Met was given to those of the Met group by gavage,once per day,for 6 weeks.ELISA was used to detect serum testosterone(T),fasting insulin(FINS),luteinizing hormone(LH)and follicle-stimulating hormone(FSH),and the LH/FSH ratio was calculated.The levels of fasting blood glucose(FBG),triglyceride(TG)and total cholesterol(TC)were detected by automatic biochemical analyzer,and the homeostasis model assessment of insulin resistance(HOMA-IR)was calculated.The estrous cycle was observed,and HE staining was performed for pathological changes in the ovary and uterus.Immunofluorescence assay was employed to measure the expression of p-eIF2α,ATF4 and CHOP in the ovarian tissue.After steroidogenic human granulosa-like tumor cell line KGN were exposed to 100 μmol/L DHEA to simulate the hyperandrogen environment of PCOS,and then treated with 5 and 10 μg/mL AS for 24 h,the protein levels of endoplasmic reticulum stress signaling pathway was detected by Western blotting.Results Compared with the Control group,the PCOS mice had disturbed estrous cycle,polycystic changes in the ovaries,and significantly increased serum T level and LH/FSH ratio(P<0.05),and obviously elevated HOMA-IR,TC and TG levels in terms of metabolism(P<0.01).The expression levels of p-eIF2α,ATF4 and CHOP were notably up-regulated in the ovarian granulosa cells of PCOS mice and KGN cells after DHEA exposure(P<0.05).Additionally,AS treatment attenuated the pathological changes of ovary and uterine expression,decreased the serum T level and the LH/FSH ratio(P<0.05),and reduced HOMA-IR,TC and TG levels(P<0.05)when compared with the PCOS mice.Moreover,the expression levels of p-eIF2α,ATF4 and CHOP were significantly down-regulated after AS treatment in both ovarian granulosa cells of PCOS mice and KGN cells(P<0.05).Conclusion AS significantly improves glycolipid metabolic disorder and reproductive dysfunction in PCOS mice,which may be associated with its suppressing endoplasmic reticulum stress by inhibiting the PERK/eIF2α/ATF4/CHOP pathway.
4.Screening of Illicit Oxyphenisatin Substances in Weight-Loss Dietary Supplements by High Performance Liquid Chromatography-in-Source-Fragmentation-Quadrupole Time-of-Flight Mass Spectrometry

By summarizing the mass spectrometric fragmentation patterns of oxyphenisatin substances,an analytical method was established for screening of illegally added oxyphenisatin compounds in weight-loss health foods using high performance liquid chromatography-in-source-fragmentation-quadrupole time-of-flight mass spectrometry(HPLC-ISF-QTOF-MS),along with a quantitative method for 11 kinds of oxyphenisatin compounds.Based on the developed screening method,an oxyphenisatin derivative was discovered in the reference standards,which was tentatively identified as 4-(3-(4-hydroxyphenyl)-2-oxoindolin-3-yl)phenyl acetate and confirmed by MS/MS analysis.The results showed that all 11 kinds of oxyphenisatin compounds had correlation coefficients greater than 0.9971,with limits of detection(LODs)ranging from 0.12 to 0.68 μg/L and limits of quantification(LOQ)from 0.21 to 2.29 μg/L.The LODs for 11 kinds of characteristic ions ranged from 0.45 to 9.11 μg/kg.At spiking levels of 25,50 and 100 μg/kg,the recoveries ranged from 78.9%to 117.3%.The instrumental precision,intra-day method precision and inter-day method precision were 0.23%?1.70%,0.7%?2.4%,and 1.1%?3.3%,respectively.The developed targeted and non-targeted detection method demonstrated high sensitivity,strong stability,rapid analysis,and an expanded screening range for oxyphenisatin substances,and provided robust technical support for regulatory authorities in combating illegal adulteration.
5.Relationship between MLR,SII,MPR and prognosis of maintenance hemodialysis patients with chronic kidney disease

Objective To investigate the relationship between monocyte-to-lymphocyte ratio(MLR),sys-temic immune inflammation index(SII),and mean platelet volume to platelet count ratio(MPR)and the prognosis of maintenance hemodialysis patients with chronic kidney disease(CKD).Methods The medical re-cords of totally 152 patients with CKD who received maintenance hemodialysis in the hospital from July 2022 to September 2023 were retrospectively analyzed.The patients were divided into the death group and the sur-vival group according to the prognosis.General data such as age,gender,height,weight,and dialysis age,as well as clinical and laboratory indicators such as blood cell count,hemoglobin,albumin,blood creatinine,blood urea,and blood uric acid were collected and analyzed.MLR,SII and MPR were calculated for each patient,and multivariate Logistic regression analysis was used to identify risk factors affecting the survival prognosis of CKD patients after maintenance hemodialysis treatment,and receiver operating characteristic(ROC)curve was used to evaluate the predictive value of the above indicators for patients'prognosis.Results Among 152 CKD patients,11.8%died,and the main causes of death include cardiovascular events,infections,multiple or-gan failure,and so on.Compared with the survival group,the levels of blood urea,blood uric acid,MLR,SII and MPR in the death group were significantly increased(P<0.05).Multivariate Logistic regression analysis revealed that MLR,SII and MPR were independent risk factors affecting the survival prognosis of CKD pa-tients after receiving maintenance hemodialysis treatment(P<0.05).ROC curve analysis showed that the are-a under the curve(AUC)of MLR for predicting the survival prognosis of CKD patients after maintenance he-modialysis treatment was 0.783,the cut off value was 0.58,the sensitivity was 72.22%,and the specificity was 76.87%.The AUC of SII for predicting the survival prognosis of CKD patients after maintenance hemodi-alysis treatment was 0.714,the cut off value was 480.50,the sensitivity was 66.67%,and the specificity was 70.15%.The AUC of MPR for predicting the survival prognosis of CKD patients after maintenance hemodial-ysis treatment was 0.706,the cut off value was 0.095,the sensitivity was 61.11%,and the specificity was 75.37%.The AUC of the combined detection was 0.843,which was better than the individual predictions(P<0.05).Conclusion MLR,SII and MPR are closely related to the prognosis of maintenance hemodialysis patients with CKD,and these indexes could be used as important reference for assessing the prognosis of pa-tients,which could help in the clinical decision-making and the development of individualized treatment plans.
6.Research progress on the application of knee braces in knee osteoarthritis

Knee osteoarthritis exhibits a high prevalence and significantly impacts patients′ quality of life. Although surgical interventions demonstrate definitive efficacy, their acceptance rate remains generally low among patients. While knee braces have been widely adopted internationally, domestic research in this field is still limited. International studies suggest that knee braces can be recommended as an early-stage intervention strategy for knee osteoarthritis. Knee braces encompass diverse designs, each with distinct advantages and limitations. Many researchers are actively refining existing brace technologies, while others are innovating novel biomechanical approaches to develop next-generation knee brace systems. Currently, no standardized treatment protocol exists for the use of knee braces in early-stage knee osteoarthritis. This study summarizes and analyzes the research on the application of knee braces in knee osteoarthritis.
7.Analysis and textual research on ancient and modern literature of Maxing Shigan Decoction

Maxing Shigan Decoction is first mentioned in Shang Han Za Bing Lun. The original formula consists of Ephedrae Herba, Armeniacae Semen Amarum, Glycyrrhizae Radix et Rhizoma, and Gypsum Fibrosum, and is a clinical prescription for treating cough and asthma. This article analyzed and verified the prescription source, composition, indications, dosage, processing method, decoction and administration method of Maxing Shigan Decoction. This study collected a total of 171 ancient books related to Maxing Shigan Decoction, among which 41 books recorded its key information in detail, covering 57 valid entries. In terms of medicinal ingredients, it is recommended to use Ephedra sinica Stapf, a plant in the Ephedrae Herba family, to dry the herbaceous stem of Ephedra sinica without the need for node removal; Glycyrrhiza uralensis Fisch. et Rhizoma is processed using dried roots and rhizomes of the legume plant Glycyrrhiza uralensis, and processed by honey roasting; Armeniacae Semen Amarum is made from dried and mature seeds of Prunus armeniaca L., a plant in the Rosaceae family, which are crushed when used; Gypsum Fibrosum is selected from sulfate mineral gypsum group gypsum. Suggested dosage: 55.2 g Ephedrae Herba, 111 g Gypsum Fibrosum, 27.6 g Glycyrrhizae Radix et Rhizoma, and 20 g Armeniacae Semen Amarum. The decoction method is to use 1 400 ml of water; first, fry the ephedra to 1 000 ml, remove the foam, add the remaining medicine, decoct 400 ml, remove the sediment, and warm orally taken 200 ml twice a day. Maxing Shigan Decoction has the functions of resolving superficies syndrome with pungent and cool natured drugs, clearing lung-heat and relieving asthma. Its main indications are diverse, involving multiple disciplines such as typhoid fever, warm diseases, pediatrics, and ophthalmology. However, its pathogenesis is generally attributed to the presence of "external wind pathogen and pathogenic heat congesting lung". By analyzing the ancient and modern literature on Maxing Shigan Decoction, the key information of its composition and dosage, drug origin and processing, decoction and administration methods, and functional indications is confirmed, in order to provide a basis for the clinical application and new drug development of Maxing Shigan Decoction.
8.Dual attributes of tonifying drugs in Shen Nong Ben Cao Jing: replenishing deficiency and expelling pathogens

Tonifying herbs described in the Shen Nong Ben Cao Jing frequently possess well-defined efficacy in dispelling pathogenic factors, including eliminating pathogenic factors, eliminating severe wind-disorders, dispelling Bi syndrome, removing blood stasis and resolving hard masses, facilitating urination and defecation, relieving chills and fever, promoting digestion, regulating and smoothing qi movement, treating carbuncles and deep-rooted ulcers, killing the :three worms: (parasites), draining pus and alleviating pain, and treating bone fractures and traumatic injuries. These effects correspond to a broad spectrum of conditions, encompassing immune, digestive, and respiratory system disorders, as well as tumors. These effects are closely related to the taste and compatibility of drugs, and are supported by modern pharmacological studies (such as anti-inflammatory, anti-tumor, immune regulation, etc.). This study could provide a novel perspective for the rational clinical application of tonifying herbs and establish a foundation for in-depth research into the multi-efficacy mechanisms of Chinese materia medica.
9.Key Information Research on Famous Classical Formula Shegan Mahuangtang

Shegan Mahuangtang was a famous classical formula for treating asthma and is included in the the Catalogue of Ancient Famous Classical Formulas(The Second Batch). By means of bibliometrics, this study conducts a textual research and analysis on the key information of its formula origin, composition, drug origins, processing, dosage, decocting methods, efficacy, and clinical application. According to research, Shegan Mahuangtang was first recorded in Synopsis of the Golden Chamber and is the ancestral formula for treating cold asthma, which has been used to this day. Suggestions for the drug origins in Shegan Mahuangtang is as follows:Shegan is selected from the dried rhizomes of Belamcanda chinensis(Iridaceae), Mahuang is selected from the dried herbaceous stems of Ephedra sinica(Ephedraceae), Shengjiang is selected from the fresh rhizomes of Zingiber officinale(Zingiberaceae), Xixin is selected from the dried roots and rhizomes of Asarum heterotropoides var. mandshuricum, A. sieboldii var. seoulense or A. sieboldii(Aristolochiaceae), Ziwan is selected from the dried roots and rhizomes of Aster tataricus(Compositae), Kuandonghua is selected from the dried flower buds of Tussilago farfara(Compositae), Nanwuweizi is selected from the dried mature fruits of Schisandra sphenanthera(Magnoliaceae), Dazao is selected from the dried mature fruit of Ziziphus jujuba(Rhamnaceae), and Banxia, a plant of the Araceae family, is selected as the processed products of dried tubers from Pinellia ternata. The recommended dosage is 41.4 g of Shegan, Xixin, Ziwan and Kuandonghua, 55.2 g of Mahuang and Shengjiang, 37.5 g of Nanwuweizi, 21 g of Dazao, 34.5 g of Banxia. The decoction method is to boil Mahuang first in 2.4 L of water, remove the froth on the top, and add the rest of the herbs and decoct them together, and then boil them to 600 mL, and then take it at warm temperature, 200 mL each time, 3 times a day. In terms of clinical application, Shegan Mahuangtang is most commonly used for respiratory system diseases, especially in the treatment of adult or pediatric bronchial asthma and cough variant asthma. Phlegm sound in the throat is the core symptom of Shegan Mahuangtang in clinical practice, and the core pathogenesis is "cold fluid stagnated in the lungs". By excavating and sorting out the ancient and modern literature of Shegan Mahuangtang, key information is confirmed, which can provide literature reference for the modern clinical application and new drug development of this famous classical formula.
10.Relationship Between Severe Pneumonia and Signaling Pathways and Regulation by Chinese Medicine: A Review

Severe pneumonia is one of the most common and critical respiratory diseases in clinical practice. It is characterized by rapid progression, difficult treatment, high mortality, and many complications, posing a significant threat to the life and health of patients. The pathogenesis of severe pneumonia is highly complex, and studies have shown that its occurrence and development are closely related to multiple signaling pathways. Currently, the treatment of severe pneumonia mainly focuses on anti-infection, mechanical ventilation, and glucocorticoids, but clinical outcomes are often not ideal. Therefore, finding safe and effective alternative therapies is particularly important. In recent years, with the deepening of research into traditional Chinese medicine (TCM), it has gained widespread attention in the treatment of severe pneumonia. This paper reviewed the relationship between severe pneumonia and relevant signaling pathways in recent years and how TCM regulated these pathways in the treatment of severe pneumonia. It was found that TCM could regulate the Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88)/nuclear factor-κB (NF-κB), Janus kinase (JAK)/signal transducer and activator of transcription (STAT), phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR), NOD-like receptor protein 3 (NLRP3), and nuclear factor E2-related factor 2 (Nrf2) signaling pathways, playing a role in reducing the inflammatory response, inhibiting cell apoptosis and pyroptosis, improving oxidative stress, and other effects in the treatment of severe pneumonia. Among these pathways, it was found that all of them regulated inflammation to treat severe pneumonia. Therefore, reducing inflammation is the core mechanism by which Chinese medicine treats severe pneumonia. This review provides direction for the clinical treatment of severe pneumonia and offers a scientific basis for the research and development of new drugs.
