1.Evaluating Online Learning Environment for Medical Students Using Digi-MEE Instrument: A Sequential Mixed-Method Study

In light of the increasing use of online learning platforms in medical education, there is a critical need to assess the current state of online learning environments. Despite the global emphasis on digital education, limited studies have systematically explored how medical students perceive the effectiveness and quality of these environments, particularly in Pakistan. This study aims to fill this gap by evaluating medical students’ experiences with their institution’s online learning environments and identifying key strengths and challenges that impact learning outcomes.
A mixed-method, sequential exploratory design was used, starting with a quantitative assessment followed by qualitative inquiry. Data from 253 medical students were collected via a survey, and 30 focus group interviews were conducted to provide deeper insights into the challenges students face in navigating the online learning environment. The findings revealed critical areas in need of improvement, including digital infrastructure, student engagement, and the accessibility of online resources. Qualitative data highlighted the need for more interactive and student-centred approaches to online education.
By focusing on the status of online learning in medical education, this research provides valuable insights for educators and policymakers aiming to enhance the quality of digital learning environments. The study contributes to ongoing efforts to develop more effective, engaging, and responsive online learning strategies for medical students in Pakistan.

Allopurinol is the well-known first-line treatment option for symptomatic hyperuricaemia and gout. It is cost-effective particularly for the management of chronic gout. The common early side effects of allopurinol are skin rashes, diarrhoea and nausea. Meanwhile, a dangerous concerning complication is Stevens–Johnson syndrome, which can cause severe morbidity and mortality. Delayed hypersensitivity to allopurinol is rare but should be one of the differential diagnoses if a patient with underlying gout on chronic allopurinol treatment presents with skin rashes. The present case highlights the importance of a high index of suspicion in at-risk patients with underlying gout along with skin rashes on long-term allopurinol treatment to avoid unnecessary patient management.

commonest form as and presents with numerous café-au-lait macules and neurofibromas. Giant congenital melanocytic nevus (CGMN) on the other hand is characterized by a melanocytic proliferation that present at birth. CGMN
develops due to a defective embryonic pigment cell (melanocyte) precursors development and are often present at
birth. Giant congenital melanocytic nevus (CGMN) and type 1 neurofibromatosis may occur together rarely. Clinicians should be aware of the rare presentation of both CGMN and type 1 neurofibromatosis in a patient.
4.Preemptive pregabalin for postoperative analgesia during minimally invasive hysterectomy: a systematic review and meta-analysis of randomized controlled trials

We aimed to perform a systematic review and meta-analysis of all randomized placebo-controlled trials (RCTs) that examined the analgesic benefits of preemptive pregabalin among patients undergoing minimally invasive hysterectomy. Five major databases were systematically screened from inception until August 29, 2021 Relevant studies were evaluated for risk of bias. Endpoints were analyzed using the random-effects model and pooled as the mean difference or risk ratio with a 95% confidence interval. Four studies with seven treatment arms met the inclusion criteria. The total sample size was 304 patients: 193 and 111 patients were allocated to the pregabalin and placebo groups, respectively. Overall, the included studies revealed a low risk of bias. The summary results revealed that the mean postoperative pain scores at rest were significantly lower in the pregabalin group than in the control group at 0, 2, 4, 6, 12, and 24 hours. Moreover, the mean postoperative pain scores on movement/coughing were significantly lower in the pregabalin group than in the control group at 12 and 24 hours. The rate of patients who were opioid-free postoperatively was significantly higher in the pregabalin group than in the control group. There was no significant difference between the groups in terms of the mean postoperative time to first rescue analgesic and the rates of adverse events. Compared with placebo, preemptive pregabalin was largely safe, and was correlated with superior analgesic effects in terms of lower postoperative pain scores and higher opioid-sparing effects. Additional RCTs are needed to confirm these findings.
5.Low birth weight infant among teenage pregnancy in Terengganu, Malaysia: A cross-sectional study

Introduction:
The purpose of this research is to determine the factors associated with low-birthweight (LBW) infants in teenage pregnancy.
Methods:
A cross-sectional study was conducted in Terengganu, Malaysia, from January 1, 2020, to May 31, 2020. Records of teenage pregnancies in 2018 were retrieved from the Maternal Health Record Book and Pregnant Woman and Postnatal Book Registry. Simple logistic and multiple logistic regression analysis was used to analyse the factors associated with LBW infants in teenage pregnancy.
Results:
All 357 cases that fulfilled the study criteria were included. LBW infants were the most common perinatal outcome among teenage pregnancies (19.3%), followed by preterm birth (9.0%), and both low Apgar score and stillbirth (1.4% each). Factors associated with low-birth-weight infants in teenage pregnancy in Terengganu were a teenage husband (AOR 2.0; 95% CI: 1.01, 3.96; p=0.047) and a mother with a low level of education (AOR 3.07; 95% CI: 1.20, 7.85; p=0.019).
Conclusion
Teenage husband and low level of maternal education are significant factors that need to be addressed to improve teenage pregnancy outcomes. Interventions to improve these factors should continue to be encouraged.

6.LYMPHOCUTANEOUS SPOROTRICHOSIS: CLINICAL SUSPICION WHEN LESION IS REFRACTORY TO ANTIBACTERIAL TREATMENT

Sporotrichosis is a fungal infection caused by Sporothrix schenckii. It occurs worldwide especially
in tropical and subtropical areas. It manifests as lymphocutaneous skin, fixed cutaneous or disseminated lesion following exposure to contaminated source like animals, plants, or abiotic components. We report a case of a 75-year-old lady with right foot ulcerative nodule which was refractory to repeated antibacterial treatment, and cultures were all negative including mycobacterium and fungus. Skin punch biopsy was also negative for granuloma or malignancy. Based on the history, physical examination and investigation findings, a clinical suspicion of lymphocutaneous sporotrichosis was made. The patient was started on an empirical course of itraconazole 200mg od. The lesions responded well with 12 weeks of treatment. As clinicians we are occasionally faced with cases where investigations may not yield results that are helpful towards making a diagnosis and in such situations, we may need to start empirical treatment based on our best judgement.
7.Knowledge, attitude, practice and perception on sunscreen and skin cancer among doctors and pharmacists

Introduction: Excessive ultraviolet light (UV) can cause
premature skin aging and potentially skin cancer. Currently
there is a lack of awareness among health care
professionals and the public on sun protection. The
objectives of this study were to determine knowledge on
sunscreen and skin cancer among health care
professionals, to evaluate the knowledge, attitude, practice
and perception of doctors and pharmacists toward the
usage of sunscreen as protection against UV radiation.
Materials And Methods: This is a cross-sectional study
conducted among doctors and pharmacists in Hospital
Sultanah Nora Ismail, Batu Pahat, Johor, Malaysia.
Questionnaires were used in this study.
Results: A total of 384 participants completed the
questionnaires. The participants consisted of 323 doctors
(84.1%) and 61 pharmacists (15.9%). The age group of the
participants ranged between 25 till 55 years old. Ninety
doctors (27.9%) and thirty-one pharmacists (51.0%) reported
used sunscreen daily (p<0.001). This finding showed that
there was a deficit in the practice of sun protection.
Pharmacists scored a higher knowledge score of median 12
(IQR=3.0) while the doctors scored 11 (IQR=2.0). This study
showed a significant association between ethnicity and skin
cancer knowledge (p<0.05).
Conclusion: This study demonstrated a lack of knowledge of
sunscreen and skin cancer prevention among health care
practitioners. This finding supports better medical
education program on this topic.

Skin conditions are a common reason for consulting primary health care. The public frequently expects their primary health care providers to be able to recognise and treat common skin conditions with confidence. Primary care providers must be able to identify ‘red flag’ skin conditions that require dermatology referral. However, diagnosis is often delayed due to the non-specific clinical presentation, low incidence, slow evolution and long-term history of such lesions. We report the case of a patient with extramammary Paget’s disease that presented to her primary care provider and subsequently underwent a wide local excision.

Flood disaster is the commonest natural disaster with huge impact on healthcare services in Malaysia. TheFloodDMQ-BM©questionnairewasdeveloped as a tool to assess the knowledge, attitude, and practice of healthcare providers regarding patient management during a flood disaster. We aimto further validate the FloodDMQ-BM©questionnaire by using Confirmatory Factor Analysis (CFA) and Item Response Theory (IRT).Thiscross-sectional studyinvolved doctors, nurses and paramedics workingin the Emergency Department of Hospital Universiti Sains Malaysia, Hospital Raja Perempuan Zainab II and Hospital Kuala Krai. Respondents were required to complete the FloodDMQ-BM©questionnaire. The responseswereanalysed by using CFA and IRT to establish its validity and reliability. A total of 215respondents participatedin this study. CFA analysiswith Maximum Likelihood Robust as the estimation method, on the attitude and practice components resulted in good factor loadings (>0.5) in nearly all items and excellent modelfit indices values (CFI = 0.96-0.98, TLI = 0.95-0.96, SRMR = 0.04-0.05, RMSEA = 0.07).Meanwhile, IRT analysis on the knowledge section showed a good two-way marginal fit based on S-X2, and a good model fit with RMSEA of 0.08. Based on the 2PL model by using the IRT assessment of the knowledge section, one item in the knowledge section (K3) was removed (chi-squared residual >4) resulting in improved model fit. The included items had well-standardized loadings (>0.3) and marginal reliability of 0. 651.Ourresults confirmed that the FloodDMQ-BM©questionnaire displayed valid and reliable psychometric properties.
10.Epidemiology of surgically treated patient in road traffic accident with lower extremity vascular injury from a single centre in Kelantan

Introduction: Lower extremity vascular injury can result in
either temporary or permanent disability.
Methods: This is a clinical audit involving all patients
admitted to our institution from January 2008 to June 2018
of those who had undergone revascularization surgery for
lower limb trauma.
Results: Fifty-nine patients were in this study with a mean
age of 28.1 years. Most of the patients were motorcyclist
involved in road traffic accidents with cars (n=30, 50.8%).
The popliteal artery was most commonly seen injury (n=41,
69.5%). The mean duration of ischaemia was 14.1 hours. The
limb salvage rate was 89.8%.
Conclusion: Lower extremity vascular injury caused by RTA
treated in our institution predominantly involved young
patients aged between 18–30 years associated with long
bone fractures causing contusion and thrombosis of the
popliteal artery.
