1.Development and validation of an innovative minimally invasive rotary-cutting surgical system for axillary osmidrosis

Objective To develop an innovative minimally invasive rotary-cutting surgical system for axillary osmidrosis,and conduct clinical validation.Methods The design concept,technical principles and system composition of the innovative minimally invasive rotary-cutting surgical system for axillary osmidrosis were introduced.A total of 73 patients(146 axillae)with axillary osmidrosis were enrolled as subjects,and underwent surgery using the newly developed surgical system.Clinical validation of the system was performed by evaluating postoperative scarring,odor elimination rate,postoperative complication incidence,and patient satisfaction.Results The study demonstrated favorable clinical outcomes in the following aspects:postoperative scarring,odor elimination rate,postoperative complication incidence,and patient satisfaction.Conclusion The minimally invasive rotary-cutting surgical system for axillary osmidrosis is rationally designed.The rotary-cutting puncture device is safe,effective,minimally invasive,and convenient for axillary osmidrosis surgery,warranting further clinical validation and widespread application.

3.Expression profile of mRNA sequencing for plasma exosomes among patients with latent tuberculosis infection and active tuberculosis

Objective:
To analyse the expression of differential mRNA in the plasma exosomes in patients with latent tuberculosis infection (LTBI) and active tuberculosis (ATB) using high-throughput sequencing, so as to provide insights into differential diagnosis of LTBI and ATB.
Methods:
The plasma samples were collected from the patients treated at The Affiliated Hospital of Hangzhou Normal University, including 16 cases of LTBI and 21 cases of ATB. The exosomes were extracted by Invitrogen extracellular extracts purification kit, and the size and morphology of exosomes were observed by transmission electron microscope (TEM). The exosomes were identified by Western blotting. Total RNA was extracted from plasma exosomes using high-throughput sequencing, differential expression mRNA was identified, and gene ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were performed. Two differential mRNAs with the highest differential expression fold were selected, and five patients with ATB and three patients with LTBI were recruited for verification using real-time quantitative PCR.
Results:
The sequencing results of plasma exosomes showed that compared with ATB patients, 2 875 differentially expressed mRNAs were detected in exosomes of LTBI patients, of which 1 002 mRNAs were up-regulated and 1 873 mRNAs were down-regulated. The most significant differentially expressed downregulated and upregulated mRNA were M6PR and RGPD5, respectively. GO analysis and KEGG pathway analysis showed that differential mRNAs were enriched in protein serine kinase activity, rRNA binding molecular function, human cytomegalovirus infection, pancreatic cancer, endometrial cancer, insulin signaling pathway and FoxO signaling pathway. The real-time quantitative PCR showed that the expression of differential mRNA was consistent with sequencing. Compared with ATB patients, the relative expression level of M6PR in plasma exosomes in LTBI patients (0.954±0.212) was downregulated compared with that of ATB patients (2.168±0.226), while the relative expression level of RGPD5 (2.126±0.200) was upregulated compared with that of ATB patients (0.588±0.129) (both P<0.05).
Conclusions
There is a difference in mRNA expression of plasma exosomes between patients with LTBI and ATB. M6PR and RGPD5 may become markers for distinguishing plasma exosomes between LTBI and ATB.
4.Effects of hyperbaric oxygen on the expressions of endothelin-1, endothelial nitric oxide synthase and endothelin A receptor in cerebral circulation blood vessel in the rabbit model of cerebral ischemia

Objective To investigate the effects of hyperbaric oxygen (HBO on the expressions of endothelin-1 (ET-1),endothelial nitric oxide synthase (eNOS)and endothelin A receptor(ETAR) in cerebral circulation blood vessel in the rabbit model of cerebral ischemia.Methods Forty healthy adult New Zealand rabbits were randomly divided into 2 groups;the surgical group (n =30) and the control group (n =10).The rabbits in the surgical group were further divided into the HBO + cerebral ischemia group (n =15) and the simple cerebral ischemia group (n =15) in accordance with different treatment methods.In the surgical group,cerebral ischemia model was established by thread embolus method.The HBO + cerebral ischemia group was exposed to HBO at a pressure of 0.22 MPa(2.2 ATA)for 60 minutes,one session a day,for a succession of 3 days.In the sham surgical group,neck vessel was only separated and the incision was soon sutured.The animals in the sham surgical group and the cerebral ischemia group did not receive HBO exposure,had normal diet and left there intact for 3 days.Three days later,all the animals were sacrificed,and then immunohistochemical staining was performed by using ET-1,ETAR and eNOS antibodies,and observations were made on the expressions of ET-1,eNOS and ETAR in the internal carotid artery and basilar artery of the 3 groups.Results Experimental results indicated that there were expressions of ET-1,eNOS and ETAR in the internal carotid artery and basilar artery after HBO exposure.As compared with those of the sham surgical group and the cerebral ischemia group,the expression levels of ET-1 and ETAR in the internal carotid artery (51.6 ± 2.1,53.4 ± 1.5) of the HBO + cerebral ischemia group were obviously higher than those in the basilar artery (20.3 ± 2.6,24.3 ± 1.7),and eNOS expression level in the basilar artery (52.1 ± 2.3) was significantly higher than that in the internal carotid artery(22.3 ± 1.2).Conclusions HBO exposure could enhance ET-1 and ETAR expression levels in the internal carotid artery,and could improve eNOS expression in basilar artery,resulting in anterior circulation contraction and posterior circulation dilation.
