1.Splenic Predominant Diffuse Large B-Cell Lymphoma: A Rare Case Report

Background:
Diffuse large B-cell lymphoma (DLBCL) is a common subtype of non-Hodgkin lymphoma of B-lymphocyte origin, characterized by an aggressive clinical course. Clinically, it most often presents with lymphadenopathy and “B symptoms,” including fever, night sweats, and weight loss. However, in some cases, the disease predominantly involves extranodal organs, such as the spleen, and may present with nonspecific manifestations, thereby posing a diagnostic challenge.
Case presentation:
A 36-year-old male patient presented with high-grade fever of unknown origin, dyspnea, generalized weakness, sweating, and a 7–8 kg weight loss over one month. Clinical evaluation and laboratory investigations revealed massive splenomegaly measuring 15 × 20 × 20 cm, progressive thrombocytopenia, anemia, and elevated C-reactive protein and lactate dehydrogenase levels. Diagnostic workup for infection, tuberculosis, viral infection, and autoimmune disease did not reveal findings sufficient to establish an alternative diagnosis. Because focal splenic lesions and suspected splenic infarction were identified, splenectomy was performed. Histopathological and immunohistochemical examination of the splenic tissue demonstrated CD20+, CD79a+, BCL2+, CD3−, CD8−, CD30−, and ALK− expression, confirming the final diagnosis of diffuse large B-cell lymphoma. Although DLBCL itself is a common form of non-Hodgkin lymphoma, this case is clinically rare and diagnostically challenging because it presented with predominant splenic involvement, massive splenomegaly measuring 15×20×20 cm, treatment-resistant fever, progressive thrombocytopenia, and imaging findings mimicking splenic infarction.
Conclusion
In patients presenting with fever of unknown origin that is resistant to treatment, massive splenomegaly, thrombocytopenia, elevated LDH, and increased inflammatory markers, lymphoproliferative disorders should be considered early in the differential diagnosis in addition to infectious causes. Histopathological and immunohistochemical examination play a crucial role in confirming the diagnosis.
2. Immunohistochemistry of prostate carcinoma

BACKGROUNDProstate cancer is the most frequent malignancy among men nowadays.METHODSImmunohistochemical expression of prostate-specific antigen (PSA) was retrospectively investigated in 10 patients admitted with clinical suspicion of the prostate cancer. Slides were collected from archived biopsiesandthey were stained for PSA.The final reaction product was evaluated as negative (0), weak/moderate positive (1), and intense positive (2).RESULTSGlandular prostate carcinoma was found in 40% (n=4) and undifferentiated carcinoma in 60% (n=6). The immunoreaction for PSA was intense positive in 30% (n=3), weak/moderate positive in 50% (n=5) and negative in 20% (n=2) of total cases.CONCLUSIONSWe concludethat PSA immunoreaction is helpful for the differential diagnosis based on our results.
3.Results of study on road traffic accidents in relation to road condition between Baganuur and Ulaanbaatar

Background. In Mongolia, 320-350 people died per year due to traffic accidents. About 193 people were died in road of rural area. According to statistic, focusing on the accidents’ reason, about 94.9 percent of accidents are caused by faulty operation of driver and 0.4 percent is caused by road condition. Goal. The purpose of this study was to analyse the influence of road condition on the route Ulaanbaatar- Baganuur, where traffic accident have frequently occurred and through that find solutions to relieve the traffic accidents.Materials and Methods. Traffic safety was evaluated with coefficient of traffic safety that defined as divide the safe speed of vehicle on the road with invisible area such as crest and sag vertical curve and horzintal curve to the speed of vehicles from opposide direction. The average speed of vehicle on hill by having invisible of road curve inside, the end of road ascent and the road section that broke road asphalt surface were determined by stability and steering dynamic calculation.Result. According to the study, on the route Ulaanbaatar-Baganuur, within last three years, about 37 accidents were occurred. Accidents and conditions on the 14 stretches where accidents have frequently occurred were studied and analyzed by repetition, location and road condition and similarity of neighbor points, so on the 14 stretches, the accidents were occurred frequently. From those accidents, 24 people died and 87 people were injured. Classifying the accidents with 14 stretches, about 14 percent of accidents were occurred on the end of road ascent; 28 percent on straight section; 35 percent on curve; 16 percent on broken surface section of road; 7 percent on the place that road asphalt surface was missed partially. Decreasing the average speed of vehicle to 54.6 km/h on hill by having invisible of road curve inside, 64.2 km/h on the end of road ascent and 51.5 km/h on the road section that broke road asphalt surface were required.Conclusion. The coefficients of traffic safety are had between 0.30-0.81 on the sections that required limiting the traffic speed. In other words, the technical environment of traffic accidents consists on some points of this road.
4.Effects of ellipin to function of hepatic cancer cells

Background: Development and progression of cancer is accompanied by different morphological and functional changes of cells. One of the most important changes is the expression and activity of enzymes in the cellular fatty acids metabolism that reflects in cell membrane lipid composition and increases fluidity of cancer cell membrane. The Ellipin, prepared from bovine liver, is a newly developed anticancer agent containing several important fatty acids.Goal: To investigate effects of Ellipin on hepatic cancer cell function such as proliferation, migration and adherent activity and apoptosis of cancer cell lines in vitro.Materials and Methods: This study was conducted in the Laboratory of Molecular Biology, Institute of Biology, MAS. The Ellipin was developed in the Drug Research Institute of Monos group. HepG2, HCC, 23132/87, MDCK cells lines and the primary liver cancer cells (PCC) were used for proliferation assay. Only HepG2 cell line was used for MTT, Migration, Spreading and Apoptosis assays.Results: The results of proliferation assay showed that the ellipin decreased the proliferation activity of HepG2 and PCC cells depending on concentrations; in 50μg/ml 2-3 times, 250μg/ml fully stopped cells divisions. The Ellipin reduced mitochondrial reeducates enzyme activity of HepG2 cells depending on its concentrations. For example, in 50μg/ml ellipin concentration case, the number of alive cells decreased 2 times. The migration of HepG2 cells treated with 100μg/ml ellipin was decreased by 22.3% compared to the control cells. Also the number of adhered cells was reduced by Ellipin treatment. After 50μg/ml, 100μg/ml, 250μg/ml ellipin treatment, the number of apoptic cells were 14,6%, 45,6%, 100% of initial culture cells, respectively.Conclusions: Our results showed that the Ellipin suppresses HepG2 cancer cell proliferation and decreases migration and spreading activities and also inducts the cell apoptosis.
