1.Determination of serum IL-4 and IFN-γ levels in allergic model-induced C57BL/6 mice using in-house ELISA

Background:
Type I hypersensitivity, commonly known as an allergic reaction, is characterised by an IgE-mediated immune response triggered when foreign proteins enter the body. During allergic reactions, cytokines such as IL-4, IL-5, IL-9, and IL-13, secreted by T-helper 2 (Th2) cells, play a crucial role in activating immune effector cells like eosinophils and mast cells, which contribute significantly to airway inflammation and obstruction. Conversely, interferon-gamma (IFN-γ), produced by Th1 cells, suppresses Th2-mediated responses and plays an essential role in regulating allergic inflammation. Therefore, determining the balance of cytokines produced by Th1 and Th2 cells is crucial for understanding the mechanisms underlying allergic responses.
Aim:
We aimed to induce an allergic response in experimental animals using ovalbumin and to determine the levels of IL-4 and IFN-γ cytokines using an in-house enzyme-linked immunosorbent assay (ELISA).
Materials and Methods:
A total of 20 female C57BL/6 mice aged 6–8 weeks were selected for the experiment and randomly divided into two groups: control and experimental groups. The concentrations of IL-4 and IFN-γ cytokines in mouse serum were determined using an in-house enzyme-linked immunosorbent assay (ELISA).
Result:
Compared to the control group, the levels of IL-4 and IFN-γ cytokines in the serum of allergic model-induced C57BL/6 mice were determined to be 513.7 pg/mL and 193.2 pg/mL, respectively. A statistically significant difference was observed in IL-4 cytokine levels between the two groups (p < 0.001).
Conclusion
In the allergic model group, serum levels of IL-4 cytokine were elevated, while IFN-γ cytokine levels were decreased, suggesting a predominant Th2-type immune response.
2.Maternity care services of midwives in Mongolia

Background:
Midwives provide essential maternal, neonatal, reproductive and sexual health care and save lives. In our country, in 2017, the job description of midwives was revised and approved by the Minister of Health in accordance with the core competencies established by the International Federation of Midwives by Order A/216. Therefore, it is necessary to study the extent to which midwives are performing the duties included in the revised job description.
Aim:
To assess of the professional role and performance of midwives in Mongolia five years after the updated midwifery job description was declared.
Materials and Methods:
There were 1119 midwives working at all levels of the facilities national wide, and 589 respondents were analyzed. Data were collected using a questionnaire, which was developed with input from an obstetrics and midwifery care advisory group, and was distributed online through Google Forms. We developed a questionnaire on the daily professional roles and performances of midwifery services and skills that were part of the job description of midwives in Mongolia. The questionnaire consisted of three parts and were scored based on a Likert scale. This study was approved by the research ethics committee of the School of Nursing (protocol №22/07-b), Mongolian National University of Medical Sciences (MNUMS), and Ministry of Health.
Result:
62.3% of the participants had a bachelor’s degree, 34.3% had a professional degree, 93.5% worked as midwives at all levels of health care, 73.4% worked according to their job description, but 21.9% were unable to work according to their main duties; 56% considered that they faced some difficulties in performing their duties at work. Within the scope of maternity care services mentioned in the job description, 71.4% of them often provide monitoring and counseling to normal, low-risk pregnant women according to guidelines (mean 3.07± 0.96); childbirth and postpartum care are occasionally (mean 2.70±1.10) or not regularly provided; and the average ratings of reproductive health, family planning services, and emergency obstetric care interventions are 2.32±1.120 and 2.40±0.040, respectively. According to self-assessment of professional skills, the average score for the ability to provide care during pregnancy was 3.10±0.038, which is sufficient; the ability to provide care during childbirth and postpartum was 2.83±0.037, which is above average; and the ability to provide emergency obstetric care and reproductive health and family planning care was 2.77±0.84 and 2.57±0.890, which is moderate, respectively.
Conclusion
The education and professional level of midwives working at all levels of the health sector has improved. While midwives frequently provide the pregnancy care outlined in their job descriptions, they only occasionally perform duties related to childbirth, postpartum and reproductive health, family planning, and obstetric emergency care. According to their self-assessments, midwives demonstrate sufficient competence in teamwork and pregnancy care; above-average competence in childbirth and postpartum care; and moderate competence in obstetric emergency care, reproductive health, and family planning services.There is a need to improve the involvement of midwives in certain professional activities.
3.Quantitative assessments of brain in multiple sclerosis patients

Background:
Multiple sclerosis (MS) is an autoimmune inflammatory neurodegenerative disease of the central nervous system and a major cause of disability in young adults. MRI is essential for diagnosis and monitoring, while early evaluation of structural brain changes is important.
Aim:
To evaluate brain volumetric changes using automated MRI analysis in patients with MS.
Materials and Methods:
This case–control study included 50 MS case patients and 50 controls patients examined between 2020–2023 at the Third State Hospital. T1-MPRAGE images were analyzed using volBrain automated segmentation software.
Result:
The mean age of MS patients was 45.5±11.8 years. Compared with healthy controls, the MS group showed significantly lower total grey matter volume (p=0.002), cortical grey matter volume (p<0.001), subcortical grey matter volume (p=0.006), and cerebral volume (p=0.02). Significant volume reductions were also observed in the temporal (p=0.006) and occipital (p<0.001) lobes. Deep grey matter structures, including the thalamus (p=0.001), caudate nucleus (p=0.009), putamen (p=0.008), and nucleus accumbens (p<0.001), also showed significantly reduced volumes. Disease duration was negatively correlated with total grey matter volume (r=−0.30, p=0.02), thalamic volume (r=−0.28, p=0.04), and occipital lobe volume (r=−0.43, p=0.002). Disease duration was also significantly associated with spinal cord involvement (p=0.01) and grey matter volume (p=0.04). The number of relapses was negatively correlated with total brain volume (r=−0.28, p=0.046).
Conclusion
Patients with MS demonstrated decreased grey matter volume, particularly within deep grey matter structures, along with increased abnormal white matter volume, suggesting the coexistence of neurodegeneration and demyelination in MS. Brain volume loss in MS appeared to occur independently of aging. Automated quantitative MRI analysis may provide important diagnostic value in detecting structural brain changes associated with MS.
4.Serum levels of Aquaporin-4, IL-6, and TNF-α in Patients with Primary Brain Tumors

inflammatory cytokines such as interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and aquaporin-4 (AQP-4)
water channel protein have been reported to be expressed in gliomas
Aim:
This study aimed to determine their serum levels in patients with primary brain tumors compared to healthy controls
and to evaluate their potential diagnostic value.
Materials and Methods:
Serum AQP-4 levels were measured by ELISA in 28 patients with primary brain tumors (PBT)
and 26 healthy controls. IL-6 and TNF-α were assessed by flow cytometry in 59 PBT patients and 52 controls.
Results:
Among the primary brain tumor cases, 45.61% were meningiomas, 26.32% anaplastic astrocytomas, 8.77%
glioblastomas, and 12.28% other tumors (including chondroma, ependymoma, schwannoma, craniopharyngioma). Serum
IL-6, TNF-α, and AQP-4 levels were significantly higher in the PBT group compared with healthy controls.
Conclusion
Elevated serum levels of AQP-4, IL-6, and TNF-α in patients with primary brain tumors suggest that these
biomarkers molecules could represent valuable non-invasive biomarkers for the early detection of PBT.
5.The Effect of Fire Acupuncture Treatment in Varicose Veins Lower Extremities

Background
Varicose veins, or chronic venous insufficiency, become visibly
noticeable from the second stage when veins start appearing on the surface
of the skin. Symptoms include severe itching, pain, swelling, and inflammation.
Chronic venous insufficiency is a common condition in countries such as
China, South Korea, Japan, Mongolia, and Kazakhstan. Diagnosis of varicose
veins relies on blood flow studies, coagulation tests, contrast-enhanced magnetic
resonance imaging (MRI), and Doppler ultrasound of the venous system.
Patient B, a 43-year-old male, has experienced pain and swelling in the right
lower limb vein for the past two years, especially when exposed to heat. Coagulation
tests show PT and PT INR within normal range. Doppler ultrasound
of the venous system revealed that the perforating vein is dilated to 0.45 cm
and has valve insufficiency. In the deep veins, a width of 0.68 cm was measured,
while superficial veins ranged between 0.46–0.59 cm. After receiving
appropriate treatment from the Department of Fire Acupuncture, the patient’s
symptoms have improved.
6.Treatment of miniere’s disease (icd-10 h.81) using traditional medicine and therapeutic interventions: Case report

Background:
Meniere's disease is a condition caused by disturbances in the
auditory and vestibular systems. It is characterized by symptoms such as dizziness,
nausea, vomiting, and tinnitus. In the United States, the prevalence of
Meniere's disease has been found to be 84 cases per 100,000 women and
56 cases per 100,000 men. Regionally, it is more common in less populated
areas, and research has confirmed that the prevalence increases with higher
household income.
Aim:
To evaluate the effectiveness of traditional medicine treatments and therapies
for Meniere's disease.
Results:
The patient is a 43-year-old male, with symptoms of dizziness, vomiting,
and tinnitus. Since 2023, he has sought care at the Central Hospital of
Mongolian Medicine, where he received traditional medicine treatments including
herbal therapy and other traditional therapies on three occasions. As a
result, the frequency of dizziness has decreased and other clinical symptoms
have improved.
Conclusion
Traditional medicine treatments have been shown to be effective
in managing Meniere's disease by extending the interval between relapses,
improving quality of life, and significantly reducing clinical symptoms.
7.Study results of the prevalence for thyroid disorders

Introduction:
Among the endocrine, nutritional, and metabolic disease and thyroid disorders occupy a significant place. According to the World Health Organization, 8-18% of the world’s population suffer from thyroid disorders. In our country, no research on the prevalence of the disorders has been conducted before, and this research methodology was discussed by the Scientific committee of the National Center for Public Health and was approved by resolution No.156 of the Ethics Committee of the Ministry of Health on 2020.
Materials and Methods:
In order to determine the prevalence
of thyroid disease in the country, we collected the actual number of thyroid disorders registered in 9 districts of the capital city and 330 soums of 21 aimags for a total of 10 years from 2011 to 2020. The prevalence of thyroid disorders was mapped using Arc view and GIS software.
Results
Endocrine, nutritional and metabolic disease account for 2.3% of all outpatient cases. Endocrine, nutritional, and metabolic disease accounted for an average of 168.3 per 10000 population over the past 10 years, and thyroid disorders accounted for 45 or 26.7% of endocrine, nutritional, and metabolic diseases. Thyroid disorders are highest in people aged 40-49 years.
Thyroid toxicity is the most common type of thyroid disease in Mongolia, accounting for 56.2%, with an average of 17.2 per 10000 population in 2011-2020. However, iodine deficiency-related thyroid disease accounts for 5.5% of all thyroid disorders, with an average of 2.5 per 10000 population in 2011-2020. In 2011, it decreased by 2.2 per 10000 population, and by 2020, it decreased by 0.2 per thousand to 2.0, but in the last 5 years, it has increased by an average of 2.4 per 10,000 population, and in the last 5 years it has increased by 0.2 per thousand, or 2.6 per 10,000 population. Morbidity is high in the Khangai and Central regions.
8.Evaluation of air pollution prevention consultation, Ulaanbaatar

Introduction:
Mongolia’s capital hosts about half the country’s total population, and its air pollution ranks among the highest in the world during winter. Air pollution is linked to reduced fetal growth, preterm birth, low birth weight, impaired cognitive intra-uterine development, impaired cognitive development, and even spontaneous abortion. Antenatal care includes fetal development monitoring, prevention of anemia, immunization against infectious diseases, prevention of sexually transmitted diseases, and any health risk factors such as environmental pollution.
Health care measures for pregnant women and children under one year of age have the potential to be highly effective because they are directly aimed at reducing pneumonia in children. As such, we need to conduct this survey to determine whether pregnant women were satisfied with the information and advice on air pollution prevention provided by health facilities and to take evidence-based measures.
Materials and Methods :
The survey data were collected using quantitative and qualitative research methods. In this study, a total of 958 pregnant women participated from Songinokhairkhan district, Bayanzurkh district, and Bayankhongor province center. The overall satisfaction of health services is calculated using the three dimensions namely: satisfaction with health service, operation, and environment of the Health Organization. Satisfaction scores were assessed using an even-point scale. These are “Very Dissatisfied-1 point”, “Dissatisfied-2 point”, “Satisfied-3 point”, “Very satisfied-4 point”. The results were calculated after entering the survey data into SPSS-23, creating a database, and performing error control. The research methodology was discussed at the meeting of the Academic Council of the National Center for Public Health on December 25, 2019, and the methodology was approved by the Medical Ethics Committee of the MOH (Stagnant №2).
Results :
The survey covered 958 pregnant women aged 16-45 in Bayanzurkh district Songinokhairkhan district, and Bayankhongor province. During antenatal care visits, the majority of pregnant women (BZD- 56.9%, SKHD- 68.3%, BKH province - 86.7%) were advised by their health specialists to go outside for fresh air, while women in Ulaanbaatar (BZD- 18.9%, SKHD- 24.7%) received the information and advice at the lowest percentage. Pregnant women in Ulaanbaatar were less involved in air pollution training than in Bayankhongor. Participants in the air pollution prevention training rated it as “satisfied” regardless of location. 37.2% of pregnant women obtained information on protecting their health from air pollution from the board of Family Health Center and 34% from their district health centers. 86.1% of the participants were able to obtain information on air pollution on their own, while 86% of them got information from their family and friends.
Conclusion
The percentage of pregnant women getting medical advice, training, and information on air pollution prevention from hospitals during their antenatal care visits was the highest in rural areas. Moreover, their level of satisfaction with counseling had been rated as “satisfied”. Pregnant women often seek information on air pollution prevention from non-professional sources, such as their family and friends.
9.Satisfaction of health care and services for parents and guardians with 0–5 years-old children

Introduction:
Within the framework of health sector reform, the development of the organization, the responsibility
of doctors and medical professionals, and the improvement of ethics, quality and safety of care and
services are top priorities. Customer satisfaction is important for the implementation of this operation
based on results and quality. The WHO Recommendation states that health professionals have a key
role to play in providing information and advice to citizens and their families on how to prevent, mitigate
and address air pollution, and how to inform the general public and decision-makers. ADB, the Ministry
of Health, and UNICEF report that information and promotional materials on air pollution prevention
are scarce and do not provide advice to clients. Therefore, it is the reason for conducting a survey to
determine the level of satisfaction of clients receiving child health care.
Materials and methods:
The survey data were collected using quantitative and qualitative research methods. In this study, totally
1160 guardians of children aged 0-5 participated from Songinokhairkhan and Bayanzurkh district and
Bayankhongor aimag center. The overall satisfaction of health services is calculated using the three
dimensions namely: satisfaction with health service, operation and environment of Health Organization.
Satisfaction scores were assessed using an even-point scale. These are “Very Dissatisfied-1 point”,
“Dissatisfied-2 point”, “Satisfied-3 point”, “Very satisfied-4 point”. The results were calculated after
entering the survey data into SPSS-23, creating a database, and performing error control. Research
methodology discussed at the meeting of the Academic Council of the National Center for Public Health
on December 25, 2019, and the methodology was approved by the Medical Ethics Committee of the
MOH (Stagnant №2).
Results:
84.7 percent of the respondents were parents of children aged 0-5 years. Their average age is 34,
most of them have higher education, and 43.0 percent of them have a household income of 500,000-1
million MNT. Guardians of children aged 0-5 years in Bayanzurkh District were “dissatisfied” with the
Family Health Center’s dressing room, wardrobe, toilet and parking lot. Guardians of children aged
0-5 years in Songinokhairkhan district were “dissatisfied” in the parking lot and toilet of the health
organization. Guardians of children aged 0-5 years in Bayankhongor aimag were satisfied with the
services, operation and environment of the health organization. Studies have shown that 76.5 percent
of the guardians did not receive air pollution risk training. As for difficulties in preventing air pollution,
31.4 percent of the surveyed population answered that they cannot change their living and working
environment to prevent air pollution.
Conclusion
It shows the health care organizations need to improve the hospital environment as well as to provide
training how to protect children from air pollution risk for guardians.
10.Health professional’s knowledge, attitude and practice (KAP) on air pollution

Introduction:
Soum and family health care centers (primary health care centers) provide public health services to
reduce the negative effects of air pollution on health. In order to decrease the risk factors due to air
pollution, it is crucial for health professionals, who are providing health care services to the public,
to have the knowledge, attitude and practice to give an advice for residents on how to protect their
health from air pollution. The “Air pollution and child health” report from WHO recommended that
responsibility of health professionals must include knowing the latest information on air pollution,
doing a research, spreading the knowledge, educating families and community and learning from
them as well, proposing solutions, and finding a solution for policy developers and decision makers
in other sectors. Therefore, we conducted this survey with purpose to determine the long-term effects
of air pollution on population psychology and lifestyle and to evaluate the level of knowledge, attitude,
and practice of health professionals on how to protect a health from air pollution.
Goal:
Evaluate the level of knowledge, attitude, and practice of health professionals on air pollution.
Material and Methods:
In 2019, this study conducted a cross-sectional design and collected quantitative and qualitative
data. 88.4% of (n=532) health professionals from 48 secondary health care centers (SHCC) and 64
family health care centers (FHCC), which are agents that implement UNICEF project, in Bayanzurkh,
Songinokhairkhan districts and Bayankhongor aimag were participated in the survey.
Results:
97.4% of the participants agreed that air pollution has negative effects on human health. 99.5% of
participants did not know the Mongolian standard for the acceptable level of air pollutant particulate
matter (PM) and 73.1% of all participants did not receive information about air quality index. 82.1% of
participants regularly give prevention advice with the purpose of protecting maternal and child health
from air. The participants who worked for more than 21 years and who are older than 51 years old
were more likely to give advice (p<0.05). As for the reasons for not giving advice about air pollution
for protecting the maternal and child health, 29.2% of participants answered the service time is not
enough, and 22.9% mentioned the lack of knowledge and information. 30.5% (162) of participants were not satisfied with their skill to give an advice on how to protect maternal and child health from
air pollution. 86.8% of participants indicated that they did not receive proper training on air pollution
and prevention from air pollution.
Conclusion
There is a need to provide training and information on how to protect maternal and child health from
air pollution for health professionals.
