Objective To evaluate the clinical characteristics of human immunodeficiency virus (HIV) infected patients with peripheral lung masses (PLMs), and to assess the diagnostic utility of contrast-enhanced ultrasound (CEUS) in differentiating benign and malignant PLMs. Methods A retrospective analysis was performed on the clinical data of 69 patients with PLM treated in Shanghai Public Health Clinical Center from January 2020 to December 2023. All patients underwent percutaneous biopsy, and were categorized into benign group (n=36) and malignant group (n=33). 25 patients were HIV-positive and 44 patients were HIV-negative. The clinical features and CEUS parameters in patients were compared across these groups. Results Patients with malignant masses were significantly older than those with benign masses (P<0.05). In the malignant group, HIV-negative patients exhibited significantly larger tumor diameters compared to HIV-positive patients (P<0.05); in the HIV-positive patients, no significant difference in tumor size was observed between benign and malignant masses. 19 patients underwent CEUS. 10 malignant masses, irrespective of HIV status (10 positive and 9 negative), commonly presented with indistinct margins, delayed enhancement, heterogeneous perfusion, and delayed peak enhancement on CEUS. 9 benign masses showed earlier peak enhancement compared to 10 malignant masses (P<0.05); no significant differences were observed in the initiation and washout time of enhancement between benign and malignant masses. In HIV-positive patients, 5 benign masses frequently demonstrated discrepancies between CEUS findings and pathological results. Conclusions The clinical and CEUS characteristics were different between benign and malignant PLMs. However, CEUS shows limited accuracy in distinguishing benign and malignant PLMs, underscoring the need for pathological confirmation.

Objective:To observe alterations in center retinal thickness (CRT) in patients diagnosed with central retinal artery occlusion (CRAO) before and after undergoing superselective arterial thrombolysis (IAT) treatment. Methods:A retrospective clinical study. From August 2022 to September 2023, 12 patients (12 eyes) diagnosed with CRAO and treated with IAT at the ophthalmology department of Shenzhen Second People's Hospital. Among these patients, there were 8 males (8 eyes) and 4 females (4 eyes), all experiencing unilateral onset. The mean age was (47.00±15.06) years. The mean duration from onset to thrombolysis was (30.00±30.42) h. All eyes underwent best corrected visual acuity (BCVA) and optical coherence tomography (OCT) assessments; additionally, 6 eyes underwent Fluorescein fundus angiography (FFA). BCVA assessments were conducted using a standard logarithmic chart and transformed into logarithm of the minimum angle of resolution (logMAR) values for statistical analysis. The OCT measured CRT at various locations around the macular fovea (M), including upper (S1, S3), lower (I1, I3), nasal (N1, N3), and temporal (T1, T3) areas at 1 mm and 3 mm distances from the fovea. CRT was defined as the vertical distance between the inner retinal boundary membrane and the inner interface of the retinal pigment epithelial layer. Pre- and post-IAT examinations were performed using the same equipment and methodologies within a 24-hour interval. Changes in CRT at different macular points were compared and observed, while arterial imaging time changes were assessed in 6 eyes that underwent FFA. Paired t-tests were utilized to analyze logMAR BCVA, CRT at different locations, and arterial imaging time pre- and post-treatment. Results:Prior to IAT treatment, the logMAR BCVA for the affected eye was 3.48±1.42, while the arterial imaging time for the 6 eyes undergoing FFA examination was (27.50±5.47) s. After 24 hours, the logMAR BCVA had improved to 2.35±1.59 for the affected eye, with 9 eyes showing varying degrees of BCVA improvement. The arterial imaging time was (24.17±7.28) s post-treatment. The differences in logMAR BCVA and arterial imaging time before and after treatment were found to be statistically significant ( t=2.489, 3.262; P<0.05). Additionally, the comparison of CRT at S3 ( t=2.871), I1 ( t=2.325), and T3 ( t=3.446) before and after treatment yielded statistically significant differences ( P<0.05). Conversely, the comparison of CRT at S1 ( t=1.879), I3 ( t=1.915), N1 ( t=2.001), N3 ( t=1.987), T1 ( t=2.180), and M ( t=-0.490) showed no statistically significant differences ( P>0.05). Conclusions:IAT treatment for CRAO has been shown to be effective in achieving therapeutic effects by reducing CRT in the macular area. However, the short-term improvement in retinal edema in the macular area is limited.

OBJECTIVE To review the current research progress on pharmacoeconomics evaluation related to spinal muscular atrophy (SMA), in order to provide valuable insights for clinical treatment, screening and medical insurance payment decision- making. METHODS A computerized search was conducted across multiple databases including PubMed, Web of Science, Embase, Scopus, Cochrane Library, EBSCOhost, CNKI, VIP, CBM and Wanfang database as well as other important health technology assessment (HTA) websites, such as National Institute for Health and Care Research,International Society of Technology Assessment in Health Care, Agency for Healthcare Research and Quality, etc. The pharmacoeconomics evaluation studies related to SMA were collected from the inception to December 31st, 2023. The literature/reports were rigorously screened based on predefined inclusion and exclusion criteria by two researchers, and the essential information from the included literature/ reports was extracted using Excel 2019. The quality of the included literature/reports was evaluated by Consolidated Health Economic Evaluation Reporting Standards 2022. RESULTS Finally, 9 articles and 15 HTA reports were included, with overall good quality of literature, but poor quality of HTA reports. There were a total of 24 studies on the pharmacoeconomics evaluation of SMA, including treatment options such as nusinersen sodium, sovaprevir, risperidone, and best supportive therapy. The review results showed that nusinersen sodium was not cost-effective in the treatment of SMA; there was no consensus on the economic viability of treatment options such as risperidone and sovaprevir; newborn/prenatal screening combined withmedication therapy was cost-effective. CONCLUSIONS newborn/prenatal screening combined with SMA medication therapy demonstrates economic advantages. It is suggested to further investigate the cost-effectiveness of new SMA drugs and SMA screening in China, taking localization parameters and medical insurance prices into account, and gradually incorporate SMA screening into the scope of neonatal genetic disease detection, in order to alleviate the financial burden of patients’ families and healthcare systems.

BACKGROUND:Premature birth is a major global health problem associated with high mortality and morbidity.White matter injury is the most common brain injury in preterm infants.Salvia miltiorrhiza is a traditional herbal plant that is commonly used to treat cardiovascular and cerebrovascular diseases in Asian countries. OBJECTIVE:To investigate the therapeutic effect of Salvia miltiorrhiza on white matter injury in preterm infants. METHODS:Eighteen neonatal male Sprague-Dawley rats at 3-day gestational age were selected and randomized into normal group, white matter injury group, and Salvia miltiorrhiza group.Animal models of preterm white matter injury were established by permanent ligation of the right common carotid artery in the latter two groups.Rats in the Salvia miltiorrhiza group were given intraperitoneal injection of Salvia miltiorrhiza(5 mg/kg·d)for 7 consecutive days.Normal group and white matter injury group were given the same volume of PBS for intervention. On the 14th day after modeling, the rats were sacrificed.Brains were pathologically observed by hematoxylin-eosin staining under microscope, and the expression levels of myelin basic protein and CC1 in brain tissue were visualized using immunofluorescence.Furthermore, liquid chromatography-tandem mass spectrometry was used to analyze possible pathways for the action of Salvia miltiorrhiza. RESULTS AND CONCLUSION:In the white matter injury group, the structure of the corpus callosum was irregular and the cells appeared swollen and necrotic. In addition, induction of white matter injury resulted in significantly reduced myelin formation, with irregular and loosely arranged nerve fibers and significantly decreased myelin sheaths.Interestingly, white matter injury rats treated with Salvia miltiorrhiza had reduced cellular swelling, reduced lesions, and increased myelin sheaths. The expression of myelin basic protein was closely related to myelin formation, and CC1 was a marker of myelin oligodendrocytes.Salvia miltiorrhiza significantly up-regulated the expressions of myelin basic protein and CC1 in white matter injury rats(P<0.000 1),indicating that Salvia miltiorrhiza alleviated white matter injury.Liquid chromatography-tandem mass spectrometry analysis showed that the therapeutic effect of Salvia miltiorrhiza in the rat model of white matter injury was closely related to the regulation of complement and coagulation cascades. To conclude,Salvia miltiorrhiza may be a potential therapeutic agent for treating preterm white matter injury.
