1.Effects of positive psychology group nursing intervention on mental health and subjective well-being of patients with chronic obstructive pulmonary disease

Objective To explore the effects of positive psychology group nursing intervention on subjective well-being,hope and mental health of patients with chronic obstructive pulmonary disease(COPD).Methods A total of 110 COPD patients who were admitted to the Huai'an First People's Hospital from January 2019 to December 2022 were selected as research objects.The patients were divided into study group and control group according to the random number table method,with 55 cases in each group.There were 33 males and 22 females in the study group with a mean age of(53.42±11.68)years(range,30-68 years)and a mean disease course of(7.45±2.40)months(range,4-12 months).There were 31 males and 24 females in the control group with a mean age of(53.30±11.42)years(range,28-66 years)and a mean disease course of(7.40±2.36)months(range,3-11 months).Positive psychology group nursing and routine nursing were given to the study group and control group,respectively.Before and after the intervention,the General Well Being Scale(GWB),Herth Hope Index(HHI),and Symptom Checklist 90(SCL-90)were used to evaluate the subjective well-being,hope,and mental health.After the intervention,the Newcastle Nursing Satisfaction Scale was used to evaluate the nursing satisfaction rate.Results The total score and the scores in all dimensions of GWB and HHI were increased after intervention in both groups,and these scores in the study group were significantly higher than those in the control group(P<0.01).SCL-90 score was decreased after intervention in both groups,and SCL-90 score in the study group was significantly lower than that in the control group(P<0.01).The satisfaction rate of nursing in the study group was higher than that in the control group(P<0.05).Conclusion Positive psychology group nursing intervention can improve the subjective well-being,hope,mental health and nursing satisfaction rate of COPD patients.
2.Optimal assisted reproductive strategies for infertile patients with congenital adrenal hyperplasia associated with CYP17A1 gene mutations: a systematic review

Objective:To comprehensively evaluate treatment outcomes and assisted reproductive approaches in infertility patients with CYP17A1-deficient congenital adrenal hyperplasia [including 17α-hydroxylase/17,20-lyase deficiency (17-OHD) and isolated 17,20-lyase deficiency (IDL)]. Methods:In this study, PubMed, Embase, Web of Science, Ovid, CNKI, Wanfang Data base, and the Chinese Biomedical Literature Database were systematically searched. The data extracted included basic patient information, treatment plans, and pregnancy outcomes, which were then analyzed descriptively.Results:Totally 19 articles were included, covering 23 cases of successful live births. Among the 23 patients with successful live births, there were 19 patients with 17-OHD, including 17 cases became pregnant through in vitro fertilization and embryo transfer (IVF-ET), 1 case through natural pregnancy, and 1 case through oocyte donation pregnancy; there were 4 cases with IDL, including 2 cases through natural pregnancy and 2 cases through IVF-ET pregnancy. Conclusion:IVF-ET is the preferred fertility treatment option for patients with partial form of 17-OHD. For IDL patients who have natural ovulation, natural pregnancy can be attempted. Glucocorticoids play a crucial role in improving pregnancy outcomes, but their use needs to be individualized.

4.Glutamate-releasing BEST1 channel is a new target for neuroprotection against ischemic stroke with wide time window.

Many efforts have been made to understand excitotoxicity and develop neuroprotectants for the therapy of ischemic stroke. The narrow treatment time window is still to be solved. Given that the ischemic core expanded over days, treatment with an extended time window is anticipated. Bestrophin 1 (BEST1) belongs to a bestrophin family of calcium-activated chloride channels. We revealed an increase in neuronal BEST1 expression and function within the peri-infarct from 8 to 48 h after ischemic stroke in mice. Interfering the protein expression or inhibiting the channel function of BEST1 by genetic manipulation displayed neuroprotective effects and improved motor functional deficits. Using electrophysiological recordings, we demonstrated that extrasynaptic glutamate release through BEST1 channel resulted in delayed excitotoxicity. Finally, we confirmed the therapeutic efficacy of pharmacological inhibition of BEST1 during 6-72 h post-ischemia in rodents. This delayed treatment prevented the expansion of infarct volume and the exacerbation of neurological functions. Our study identifies the glutamate-releasing BEST1 channel as a potential therapeutic target against ischemic stroke with a wide time window.
5.Clinical Significance of Plasma D-dimer in Systemic Sclerosis

Objective To study correlation between systemic sclerosis(SSc)and plasma D-dimer and to reveal the probably rules of the fibrinolytic systems in SSc.Methods In 2013 January to 2014 January,cellected 32 patients with SSc and 35 with healthy controls to detected level of plasma D-dimer.Logistic and t student test were used for statistical analysis for correlation between SSc and pulmonary artery hypertension.Results When compared to healthy controls (0.28±0.04μg/ml),the level of plasma D-dimer were significantly increased in SSc patient (0.31±0.05μg/ml,t=1.997,P=0.008).After stratifying SSc patients according to disease subset,whereas patients with diffuse subset displayed substantially increased values (0.41±0.06μg/ml,t=2.051,P<0.001).The level of plasma D-dimer was associated with pulmonary artery hyper-tension (OR=4.38,95%CI=2.59~8.91,P=0.008).Conclusion Demonstrated that SSc patients with diffuse subset are characterized by increased plasma D-dimer values,reflecting a potential activation of fibrinolytic cascaded,which might finally predispose these patients to thrombotic complications and pulmonary artery hypertension.
6.Evaluation of Several Inflammatory Markers to Infectious and Gouty Arthritis

Objective The aim of this study was to investigate which inflammatory markers allow an accurate differentiation of septic and gouty arthritis.Methods In 2013 January to 2014 January 33 patients with septic arthritis and 29 patients with gouty arthritis.Detected white blood cells,C-reactive protein and uric acid of inflammatory markers in plasma and tested lac-tate,glucose,uric acid,lactate dehydrogenase and white blood cell count inflammatory markers in the synovial fluid.MedCalc 13.0 software were used for statistical analysis.Results There were no significantly different between serum C-reaction protein and WBC counts with two groups.Synovial lactate showed the greatest diagnostic potential (AUC=0.898,sensitivi-ty=96.9%,specificity=72.4%)followed by serum uric acid (AUC=0.818)and synovial uric acid (AUC=0.808).Con-clusion Lactate in the synovial fluid has excellent diagnostic potential to differ septic arthritis from gouty arthritis.Synovial lactate levers above 1.7 mmol/L almost proofed septic arthritis.
7.Clinical application of PCR and high resolution melting analysis for rapid identification of Brucella isolates

The aim of this study is to develop a rapid and accurately species typing method for Brucella isolates by using High Resolution Melting (HRM ) analysis .Six pairs of primers were used according to the reference for the sequence of pur‐pose gene .Nineteen biotypes of six species Brucella standard strains were identified by PCR‐HRM analysis and this analysis was used to detect the 35 clinical isolates .Results showed Brucella amplified specific melting curves were different from con‐trasted strains with primer Bspp .The six species Brucella standard strains have own characteristic curve shape from each oth‐ers by PCR‐HRM analysis with five pairs of primers .Thirty‐five clinical isolates of Brucella have entirely consistent with PCR‐HRM curve shape with Brucella melitensis standard strains .So ,PCR‐HRM analysis methods can accurately identify Brucella strains ,especially clinical isolated Brucella melitensis ,and may be used in clinical microbiology laboratories .
8.Application of matrix-assisted laser desorption ionization-time of flight mass spectrometer in identification of clinical common bacteria

Objective To evaluated the application value of two kinds of mass spectrometer(MS)and Vitek MS system in the i-dentification of routinely isolated bacteria in clinic.Methods 149 strains of common bacteria(including 14 genera and 30 species)i-solated from blood,urine,cerebral spinal fluid,secretion and sputum samples in our hospital from March 2012 to January 2013 were collected and simultaneously identified by 2 kinds of matrix-assisted laser desorption ionization-time of flight mass spectrometer (MALD-TOF-MS).The identification results were compared with those identified by the conventional biochemical identification (Vitek2 compact).The strains with the inconsistent results identified by 3 kinds of method were confirmed by 16S rDNA gene se-quencing.Results Among 149 common bacteria,the correct identification rates of genus and species by the Bruker Biotyper MS were 98% and 96% respectively and which by the Vitek MS system were 97% and 95% respectively.There were no misidentified bacterial strains by these two kinds of MS.Conclusion No statistical difference in the identification results was observed between these two kinds of MS system(P >0.05).Both exhibit excellent identification level and are suitable for the routine laboratory iden-tification of clinical microorganism.
9.The association between anemia and inflammatory in patients with heart failure

Objective To research the relationship between anaemia and inflammatory in patients with heart failure .Methods 284 cases of patients with heart failure were enrolled and divided into 2 groups (anaemia group and non‐anaemia group) .The serum levels of of brain natriuretic peptide (BNP) ,high‐sensitivity C‐reactive protein (hs‐CRP) ,blood urine nitrogen (BUN) and creati‐nine (Crea) were measured ,and the results were analyzed .Results The levels of BNP ,hs‐CRP and Crea of anaemia group were sig‐nificantly higher than those of non‐anaemic group (P<0 .01) .The results of logistic regression demonstrated that hs‐CRP was in‐dependently associated with anaemia (P=0 .021) .Conclusion The occurrence and development of inflammation are independently associated with anaemia in the patients with heart failure .
10.Evaluation of Plasma miR-3 2 4-5 p for Early Diagnosis in Patients with Simple Congenital Heart Disease

Objective To analyze expression of miR-324-5p in plasma of CHD patients by real-time PCR and identification of early diagnosis,for CHD.Methods In 2012 June to 2013 February 76 patients and 13 normal controls who were included in the study had measurement of plasma expression levels of miR-324-5p by real-time PCR.MedCalc 13.0 software were used for statistical analysis and comprehensive evaluation of miR-324-5p in CHD disease for diagnosis.Results Analysis of re-ceiver operating curve (ROC)showed that area under the curve (AUC)was 0.731 and best diagnostic threshold was 0.116 1.The sensitivity was 64.5% and specificity of 84.6%.Conclusion Circulating miR-324-5p as a biomarker for early diagno-sis of CHD has some-extent clinical value,but needs combined with other medical indicators.
