1.LIU Shangyi's Experience in Differentiating and Treating Rectal Carcinoma Under the Theory of "Treating Ulcers as Tumors"

This paper summarizes the experience of professor LIU Shangyi in differentiating and treating rectal carcinoma from the perspective of "treating ulcers as tumors". It is believed that the manifestations of rectal cancer, such as anal itching, cauliflower-like or ulcerative tumors, and bloody stools, are similar to external skin itching, skin ulceration, swelling, and skin bleeding. Therefore, the treatment principles of sores and ulcers department can be applied to treat tumors. Following the diagnostic and treatment approach of dermatology regarding the clinical typical symptoms, for anal itching, the main treatment is to dispel wind and remove dampness, clear heat to relieve itching, using "skin medicinals" such as Difuzi (Fructus Kochiae) and Baixianpi (Cortex Dictamni), as well as wind medicinals such as Shengma (Rhizoma Cimicifugae) and Fangfeng (Radix Saposhnikoviae). For constipation, the method of clearing heat and resolving toxins, unblocking the bowels and discharging heat can be used, commonly using Baitouweng (Radix Pulsatillae), Donglingcao (Herba Rabdosiae Rubescentis) and Dahuang (Radix et Rhizoma Rhei). In terms of mucosal ulcers, it is critical to differentiate between yin and yang; the treatment of yang ulcers should focus on clearing heat and resolving toxins, commonly using modified Xianfang Huoming Beverage (仙方活命饮); for yin ulcers, emphasis should be placed on removing dampness and resolving phlegm, commonly with modified Yiyi Fuzi Baijiang Powder (薏苡附子败酱散). For bloody stool, differentiation is made between deficiency and excess, with the use of Diyu (Radix Sanguisorbae) and Huaihua (Flos Sophorae) for excess syndrome to cool and stop blee-ding, and both herbs dry-fried until charred combined with liver-tonifying medicinals for deficiency syndrome
2.Mechanisms of bone marrow mesenchymal stem cells in counteracting D-galactose-induced brain aging

Objective To investigate the effect and potential mechanism of rat mesenchymal stem cells(MSC)on D-galactose-induced brain-tissue aging.Methods A rat brain-aging model was established by injecting D-galactose,and rats in the treatment group received MSC injections via the tail vein.Superoxide dismutase(SOD)activity and malondialdehyde(MDA)levels were assessed in rat brain tissue at the end of the experiment,and pathological changes in brain tissue were observed by hematoxylin-eosin(HE)staining.Expression levels of the inflammatory factors interleukin(IL)-1 and IL-6,the pathway proteins brain-derived neurotrophic factor(BDNF)-tropomyosin receptor kinase B(TrkB),the negative growth regulators p53 and p16,as well as vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF)were observed by polymerase chain reaction(PCR)and Western Blot.Results Brain levels of SOD activity were significantly increased and MDA levels were significantly decreased in rats in the modle group compared with the treatment group(P<0.05).The pathological state of the cerebral cortex and hippocampus were improved and the number of neurons and nucleus pulposus ratio in the brain were increased in the treatment group,as shown by HE staining.Expression levels of IL-1,IL-6,p53,and p16 were significantly decreased,while BDNF,TrkB,VEGF,and bFGF were significantly increased in the treatment group compared with the model group,as shown by PCR and Western Blot(P<0.05).Conclusions These result suggest that MSCs potentially mitigate D-galactose-induced cerebral senescence by concurrently modulating the BDNF-TrkB axis to attenuate oxidative/inflammatory damage,while enhancing the secretion of vasculotrophic(VEGF)and neurotrophic(bFGF)factors for neuronal maintenance.
3.Changes in the body shape and ergonomic compatibility for functional dimensions of desks and chairs for students in Harbin during 2010-2024

Objective:
To analyze the change trends in the body shape indicators and proportions of students in Harbin from 2010 to 2024, and to investigate ergonomic compatibility of functional dimensions of school desks and chairs with current student shape indicators, so as to provide a reference for revising furniture standards of desks and chairs.
Methods:
Between September and November of both 2010 and 2024, a combination of convenience sampling and stratified cluster random sampling was conducted across three districts in Harbin, yielding samples of 6 590 and 6 252 students, respectively. Anthropometric shape indicators cluding height, sitting height, crus length, and thigh length-and their proportional changes were compared over the 15-year period. The 2024 data were compared with current standard functional dimensions of school furniture. The statistical analysis incorporated t-test and Mann-Whitney U- test.
Results:
From 2010 to 2024, average height increased by 1.8 cm for boys and 1.5 cm for girls; sitting height increased by 1.5 cm for both genders; crus length increased by 0.3 cm for boys and 0.4 cm for girls; and thigh length increased by 0.5 cm for both genders. The ratios of sitting height to height, and sitting height to leg length increased by less than 0.1 . The difference between desk chair height and 1/3 sitting height ranged from 0.4-0.8 cm. Among students matched with size 0 desks and chairs, 22.0% had a desk to chair height difference less than 0, indicating that the desk to chair height difference might be insufficient for taller students. The differences between seat height and fibular height ranged from -1.4 to 1.1 cm; and the differences between seat depth and buttock popliteal length ranged from -9.8 to 3.4 cm. Among obese students, the differences between seat width and 1/2 hip circumference ranged from -20.5 to -8.7 cm, while it ranged from -12.2 to -3.8 cm among non obese students.
Conclusion
Current furniture standards basically satisfy hygienic requirements; however, in the case of exceptionally tall and obese students, ergonomic accommodations such as adaptive seating allocation or personalized adjustments are recommended to meet hygienic requirements.

5.Discriminant analysis of the risk of hospital death in patients with traumatic hemorrhagic shock combined with multiple organ dysfunction syndrome

Objective:To investigate the risk of hospital death in patients with traumatic hemorrhagic shock combined with multiple organ dysfunction syndrome(MODS)predicted by discriminant analysis.Methods:This study was a single-center retrospective study. From January 2013 to May 2023, patients with traumatic hemorrhagic shock complicated with MODS admitted to Peking University People's Hospital were selected as the research objects. According to the in-hospital survival, the patients were divided into survival group (205 cases) and death group (29 cases). The general condition, injury assessment, laboratory indexes, complications and clinical scores of the two groups were compared. Wilks's Lambda stepwise discriminant analysis was used to establish a discriminant model for in-hospital death in patients with traumatic hemorrhagic shock combined with MODS. the Receiver operating characteristic curve (ROC) was drawn and the Area under the curve (AUC) was calculated. The cross-validation method was used to evaluate the accuracy of the prediction results of the established model.Results:There was a statistical difference between the survival group and the death group in terms of the main bleeding site (limbs), the time from injury to hospital admission, temperature, platelet count, prothrombin time, activated partial thrombin time, fibrinogen, albumin, serum creatinine, estimated glomerular filtration rate, uric acid, cardiac troponin I, procalcitonin, base excess, lactate to albumin ratio, glucose to albumin ratio, acute respiratory distress syndrome, acute kidney injury, acute myocardial injury, traumatic induced coagulopathy, ISS, APACHEⅡ score and SOFA scores. There are four indicators entering the final discrimination model: Prothrombin time, ISS score, estimated glomerular filtration rate, lactate to albumin ratio. The AUC for predicting the risk of death in patients with traumatic hemorrhagic shock combined with MODS was 0.791, and the 95% CI was 0.671 to 0.911. Conclusions:The established discriminant model is highly accurate in predicting the risk of hospital death in patients with traumatic hemorrhagic shock complicated with MODS. Prothrombin time, ISS score, estimated glomerular filtration rate and lactate to albumin ratio were associated with an increased risk of death from traumatic hemorrhagic shock with MODS.
6.Exploration of early detection of large vestibular aqueduct syndrome in children with multiple audiological indicators

OBJECTIVE To explore the early detection of large vestibular aqueduct syndrome(LVAS)in children by applying several audiological indicators.METHODS Ninety-two children with hearing loss(aged 1-70 months)were enrolled and divided into an LVAS group(45 cases)and a control group(47 cases).Eleven audiological indicators were statistically analyzed:lateral of hearing loss,the degree of hearing loss,configuration of hearing loss;ABR air-conduction threshold;ABR air-bone gap;ASSR average threshold;ASSR thresholds at 0.5,1,2,and 4 kHz;and tympanogram type.Indicators showing significant two-group differences were used to construct a visualized multifactorial linear prediction model using the R language.RESULTS Nine indicators demonstrated statistically significant differences between groups(P<0.05):laterality,configuration of hearing loss,ABR air-conduction threshold,ASSR average threshold,ASSR thresholds at all frequencies(0.5,1,2,4 kHz),and tympanogram type.A prediction model was established.When the total model score ranged between 200 and 240 points,the predicted LVAS risk probability was 0.1 to 0.99.The consistency index(C-index)was 0.85,indicating good predictive ability of the model.CONCLUSION The identified nine audiological indicators are valuable for the early detection of LVAS in children.The developed model can estimate LVAS risk.After refinement,this model holds potential to support early clinical diagnosis and intervention.
7.WANG Xixing's Experience in Differentiation and Treatment of Advanced Breast Cancer with Anxiety and Depression:Based on Shaoyang Pivot Theory

To summarize Professor WANG Xixing's clinical experience in treating advanced breast cancer with anxiety and depression from the perspective of shaoyang pivot. It is believed that the core pathogenesis of advanced breast cancer with anxiety and depression lies in the dysfunction of shaoyang pivot (referring to the imbalanced regulatory function of the shaoyang meridian system that governs the transportation and transformation of qi, blood, and body fluids). This dysfunction can lead to abnormal circulation of qi, blood, and body fluids, as well as the intermingling of phlegm and blood stasis, which further promotes the spread and diffusion of cancer toxin. Meanwhile, it disturbs mental activity, resulting in a condition characterized by stagnation of cancer toxin and concurrent disorders of both the physical body and the spirit. Based on this pathogenesis, the basic therapeutic principles of harmonizing shaoyang, regulating the pivot to calm the spirit, and dissipating masses and resolving toxins are proposed. Clinically, the disease is classified into three syndromes for differentiation and treatment. For shaoyang pivot dysfunction syndrome, treatment should use self-prescribed Chaiqin Hengshu Ningxin Decoction (柴芩衡枢宁神汤); for sanjiao pivot dysfunction syndrome, treatment should prescribe Chaigui Tongshu Dashen Decoction (柴归通枢达神饮); for gallbladder function disorder syndrome, treatment should apply Wendan Qishu Shoushen Decoction (温胆启枢守神汤). Throughout the treatment process, the concept of "simultaneous treatment of cancer and depression" is implemented to smooth the shaoyang pivot, block the vicious cycle where cancer toxin and emotional abnormalities mutually reinforce each other.

9.The relationship of serum miR-124,miR-134 and CT regional assessment grading of pia mater collateral,core infarct volume and prognosis in patients with acute ischemic stroke

Objective To explore the relationship of serum micro ribonucleic acid(miR)-124,miR-134 and CT regional assessment grading of pia mater collateral,core infarct volume and prognosis in patients with acute ischemic stroke.Methods A total of 192 patients with acute ischemic stroke(observation group)and 100 healthy volunteers(control group)were selected.The serum miR-124 and miR-134 were compared between the two groups.At the same time,the serum miR-124,miR-134 and the score of pia mater collateral in CT region of patients with different clinical characteristics in the observation group were analyzed.Results The relative expression levels of serum miR-124 and miR-134 in the observation group were significantly higher than those in the control group(P<0.05).The relative expression serum miR-124 and miR-134 in patients with core infarct volume≥50 mL in the observation group were significantly higher than those in patients with core infarct volume<50 mL(P<0.05).In the observation group,the score of pia mater collateral in CT region of patients with core infarct volume ≥50 mL was significantly lower than that of patients with core infarct volume<50 mL(P<0.05).The relative expression levels of serum miR-124 and miR-134 were positively correlated with the core infarct volume(r=0.378 and 0.466,P<0.05),and the score of pia mater collateral in CT region was negatively correlated with the core infarct volume(r=-0.463,P<0.05).The relative expression levels of serum miR-124 and miR-134 in patients with poor prognosis were significantly higher than those in patients with good prognosis(P<0.05),while the score of pia mater collateral in CT region was significantly lower than that in patients with good prognosis(P<0.05).Conclusion The serum miR-124 and miR-134 in patients with acute ischemic stroke are up-regulated,and miR-124 and miR-134 are positively correlated with the core infarct volume,while the score of pia mater collateral in CT region is negatively correlated with the core infarct volume.At the same time,miR-124,miR-134,and the score of pia mater collateral in CT region may be related to the prognosis of patients.
10.Correlation between Circulating Tumor Cells and Recurrence-Metastasis of Colorectal Cancer based on the Theory of Latent Pathogens

There is a certain correlation and interaction between circulating tumor cells and recurrence-metastasis of colorectal cancer. The circulating tumor cells hidden in the human body can lead to the recurrence and metastasis of colorectal cancer, and the recurrence and metastasis of colorectal cancer can in turn cause a sharp increase in the number of circulating tumor cells, which is consistent with the characteristics of latent pathogens in traditional Chinese medicine leading to diseases. Starting from the theory of latent pathogens, this article summarized the etiology and disease mechanism of recurrence-metastasis of colorectal cancer as harmed healthy qi, latent pathogenic qi, prolonged latent pathogens, and triggering healthy qi deficiency. This article also proposes four major treatment methods as reinforcing healthy qi, assisting yang, opening the exterior, and expelling pathogen, in order to provide ideas and methods for the prevention and treatment of recurrence and metastasis of colorectal cancer.
