1.Clinical phenotypes and pathogenic mechanisms of Wilson disease with lipid metabolism disorders

Wilson disease (WD) is a hereditary disorder of copper metabolism characterized by abnormal copper accumulation in tissues, including the liver and brain, which leads to severe hepatic and neurological damage. This disease is often accompanied by lipid metabolism abnormalities, and the exploration of related mechanisms has attracted increasing attention. This article introduces the clinical features of lipid metabolism disorders in WD patients, summarizes the research advances in the serum levels of lipids and hepatic steatosis, analyzes the potential mechanisms of the interaction between copper and lipid metabolism, and highlights the significance of lipid-related molecules in disease diagnosis and clinical evaluation. In clinical practice, the monitoring and assessment of lipid metabolism parameters should be taken seriously in patients with WD, in order to promote comprehensive disease management and improve the prognosis of patients.
2.An empirical analysis on the association between sarcopenia and all-cause mortality in Chinese elderly residents

ObjectiveTo investigate the association between variant degree of sarcopenia and all-cause mortality in Chinese elderly residents, and to provide insights into the prevention and control of sarcopenia in the elderly population. MethodsData from the China Health and Retirement Longitudinal Study (CHARLS) from 2011 and 2020 were analyzed, and a total of 2 792 subjects aged 65 years or older were selected according to the inclusion and exclusion criteria. Univariate and multivariate Cox proportional hazards regression analysis were performed to explore the potential factors influencing all-cause mortality among the elderly in China, and Kaplan-Meier curves were used to visualize the survival of elderly people with variant degree of sarcopenia. Finally, a multiple-adjusted Cox proportional hazards regression model was used to control the confounding factors and explore the association between sarcopenia and all-cause mortality. ResultsBefore adjusting potential covariates, univariate and multivariate Cox proportional hazards regression models showed that 10-year all-cause mortality was significantly associated with variant degree of sarcopenia, namely possible sarcopenia (HR=1.40, 95%CI: 1.1‒1.68, P<0.001), mild-to-moderate sarcopenia (HR=1.49, 95%CI:1.20‒1.86, P<0.001), and severe sarcopenia (HR=1.68, 95%CI: 1.29‒2.19, P<0.001); after adjusting all confounders, 10-year all-cause mortality remained to be significantly associated with variant degree of sarcopenia, including probable sarcopenia (HR=1.38, 95%CI: 1.15‒1.66, P<0.001), mild-to-moderate sarcopenia (HR=1.48, 95%CI: 1.19‒1.84, P<0.001) and severe sarcopenia (HR=1.71, 95%CI: 1.31‒2.23, P<0.001). ConclusionIn Chinese elderly residents, sarcopenia is positively associated with an increased risk of 10-year all-cause mortality, and the progression of sarcopenia is positively associated with an increased risk of death.
3.Clinical Observation on the Treatment of Non-Valvular Paroxysmal Atrial Fibrillation with Self-Prescribed Chaihu Jia Longgu Muli Decoction Derivative Based on the Pathogenesis Theory of"Mutual Fanning of Liver Wind and Heart Fire"

OBJECTIVE To explore the clinical efficacy of Chaihu Jia Longgu Muli Decoction Derivative in the treatment of non-valvu-lar paroxysmal atrial fibrillation based on the pathogenesis theory of"mutual fanning of liver wind and heart fire".METHODS A total of 80 patients with non-valvular paroxysmal atrial fibrillation admitted to Hainan Provincial Traditional Chinese Medicine Hospital from January 2021 to July 2022 were prospectively selected as the research subjects and randomly divided into an observation group and a control group with 40 cases in each group.The control group was given conventional treatment such as western medicine anticoagulation and ventricular rate con-trol.The observation group was given Chaihu Jia Longgu Muli Decoction Derivative on the basis of the control group.The treatment course of both groups was 30 d.The two groups were compared in terms of TCM syndrome scores,atrial fibrillation episodes(number of episodes,du-ration),clinical efficacy,changes in electrocardiogram parameters(P wave duration,P wave dispersion),heart rate variability indexes[standard deviation of RR interval(SDNN),root mean square difference of adjacent RR intervals(RMSSD),standard deviation of the mean value of RR interval(SDANN)],left ventricular function indexes[left atrial diameter(LAD),left ventricular ejection fraction(LVEF),ear-ly diastolic mitral annular motion velocity(EM)and left ventricular early diastolic blood flow propagation velocity(VP)]and the occurrence of adverse reactions.RESULTS After treatment,the TCM syndrome scores of the two groups of patients were significantly reduced(P<0.05,P<0.01),and the observation group was better than the control group(P<0.05,P<0.01);the number and duration of atrial fibrilla-tion in the two groups were significantly decreased(P<0.05,P<0.01),and the observation group was superior to the control group(P<0.05,P<0.01);the clinical efficacy of the observation group was better than that of the control group(P<0.05);the heart rate variability indexes,electrocardiogram parameters and left ventricular function indexes were significantly improved(P<0.05),and the observation group was supe-rior to the control group(P<0.05);there was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).CONCLUSION Based on the pathogenesis theory of"mutual fanning of liver wind and heart fire",the application of Chaihu Jia Longgu Muli Decoction Derivative for the treatment of non-valvular paroxysmal atrial fibrillation can improve clinical symptoms,reduce atrial fibrillation,enhance cardiac function,and has good clinical efficacy.
4.Tissue-Bone Homeostasis Manipulation Restores Quadriceps-Patellar Ligament Biomechanical Homeostasis and Functional Performance in Knee Osteoarthritis:A Randomized Controlled Trial

OBJECTIVE To investigate the intervention effects of tissue-bone homeostasis manipulation(TBHM)on peripatellar biomechanical parameters and knee joint function in knee osteoarthritis(KOA)patients.METHODS Sixty patients with KOA(Kell-gren-Lawrence grade Ⅱ-Ⅲ)were recruited from the Acupuncture-Moxibustion Rehabilitation Department,Anhui University of Chi-nese Medicine between October 2024 and May 2025.Participants were randomized into a TBHM group(n=30)or a transcutaneous e-lectrical neuromuscular stimulation(TENS)group(n=30).Using two-way repeated measures ANOVA,biomechanical indicators,in-cluding rectus femoris tension,vastus medialis tension,vastus lateralis tension,patellar ligament tension,lateral patellar displacement(LPD),medial patellar displacement(MPD),normalized patellar mobility(LPD/patellar width[PW],MPD/PW),knee flexion range of motion,and functional indicators,including KOOS subscales,time up and go test(TUGT),were compared between groups at baseline and after 6 weeks of intervention.RESULTS After intervention,all biomechanical and knee joint function indicators in the TBHM group were significantly improved(P<0.05,P<0.01),while only the vastus medialis tension,TUGT and KOOS Pain,ADL and QoL scores in the control group were significantly improved(P<0.01).The improvement amplitudes of biomechanical indicators in the TBHM group,including rectus femoris tension,vastus lateralis tension,patellar ligament tension,MPD/PW,LPD/PW and knee flexion range of motion were better than those in the control group(P<0.05,P<0.01).In the functional evaluation,the interaction effects of the TBHM group in all dimensions of the KOOS score and TUGT were statistically significant(P<0.05,P<0.01).Post-hoc simple effect analysis confirmed that there were significant differences in the above indicators between the two groups after intervention(P<0.05),and all indicators showed a significant main effect of time(P<0.01),suggesting that the intervention measures had contin-uous and cumulative curative effects.CONCLUSION TBHM effectively improves joint function and quality of life in KOA patients by restoring dynamic equilibrium in soft tissue tension and patellar mobility,ultimately achieving the therapeutic goal of concurrent tis-sue-bone management.
5.Effect of traditional Chinese medicine hot ironing on limb function and balance ability in stroke patients with hemiplegia during the recovery period

Objective:To observe the effect of traditional Chinese medicine (TCM) hot ironing on limb function and balance ability in stroke patients with hemiplegia during the recovery period, and to provide more reference schemes for clinical rehabilitation of stroke patients with hemiplegia.Methods:A randomized controlled study was conducted. Stratified random sampling was used to select stroke patients with hemiplegia during the recovery period admitted to the First Affiliated Hospital of Anhui University of Chinese Medicine from June 2023 to January 2024. The patients were divided into a control group and an observation group using the random envelope method. The control group received limb positioning techniques combined with conventional acupuncture intervention, while the observation group received additional TCM hot ironing intervention. Before and after the intervention, the Brunnstrom motor function grading, three-level assessment method, and Activities of Daily Living (ADL) scale were used to evaluate the patients′ limb function, balance function, and daily living ability.Results:Among the 66 patients included, with 33 patients in the control group, includinng 19 males and 14 females, aged (51.02 ± 8.34) years old, and 33 patients in the observation group, including 21 males and 12 females, aged (49.65 ± 8.21) years old. Before the intervention, there were no statistically significant differences in Brunnstrom upper limb motor function, Brunnstrom lower limb motor function, sitting balance, standing balance, and ADL scores between the two groups (all P>0.05). After the intervention, the observation group showed significant improvements in Brunnstrom upper limb motor function (0 cases at levelⅠ, 33 cases at levelⅡor above), Brunnstrom lower limb motor function (1 case at levelsⅠ-Ⅲ, 32 cases at level Ⅳ or above), sitting balance (0 cases at levelsⅠ-Ⅱ, 33 cases at level Ⅲ), and standing balance (1 case at levelsⅠ-Ⅱ, 32 cases at level Ⅲ). The control group showed less improvement in Brunnstrom upper limb motor function (10 cases at levelⅠ, 23 cases at level Ⅱor above), Brunnstrom lower limb motor function (14 cases at levelsⅠ-Ⅲ, 19 cases at level Ⅳ or above), sitting balance (9 cases at levelsⅠ-Ⅱ, 24 cases at level Ⅲ), and standing balance (11 cases at levelsⅠ-Ⅱ, 22 cases at level Ⅲ). The differences between the two groups were statistically significant ( Z values were 9.39-15.51, all P<0.05). The ADL score in the observation group was (80.25 ± 8.89) points, higher than the control group′s score of (69.97 ± 8.17) points, the difference was statistically significant ( t=4.89, P<0.05). Conclusions:TCM hot ironing can effectively improve limb dysfunction, motor function, balance ability, and walking mobility in stroke patients with hemiplegia during the recovery period, thereby enhancing their daily living ability.

9.Best evidence summary of the prevention of medical adhesive-related skin injury in central venous access devices

Objective:To retrieve, evaluate, and integrate domestic and foreign evidence on the prevention of central venous access device medical adhesive-related skin injury (CVAD-MARSI) , and analyze and summarize the best evidence, so as to provide a reference for establishing a sound intravenous infusion treatment system and CVAD-MARSI management mechanism.Methods:Evidence-based nursing methods were applied, and the evidence on the prevention of CVAD-MARSI in British Medical Journal (BMJ) Best Practice, UpToDate, Australia Joanna Briggs Institute (JBI) Evidence-Based Practice Database, Canada Registered Nurses' Association of Ontario, Scottish Intercollegiate Guidelines Network, National Institute for Health and Clinical Excellence, Cochrane Library, Intravenous Nurses Society, PubMed, Medlive, Chinese Biomedical Literature Database, Wanfang Data, China National Knowledge Infrastructure (CNKI) was retrieved according to the "6S" model system. The evidence included guidelines, best practices, systematic reviews, expert consensus, and evidence summary. The search time limit was from January 1, 2011 to February 1, 2021. Two researchers independently completed the quality evaluation of the included literature, and carried out the evidence extraction and summary of literature that met the quality standards.Results:A total of 10 articles were included, including 3 guidelines, 1 systematic review, 5 expert consensuses, and 1 evidence summary. The best evidence covered 5 aspects including patient evaluation, skin preparation, dressing use, improvement of comfort, and organization management, with a total of 26 pieces of evidence.Conclusions:Clinical medical and nursing staff can take measures in terms of patient evaluation, skin preparation, dressing use, improvement of comfort, and organization management. In the process of evidence application, the best targeted evidence should be selected combined with the clinical situation, so as to reduce the harm of CVAD-MARSI.
10.Genome-wide analysis of drug resistance and virulence of uropathogenic Escherichia coli

Objective:To investigate the drug resistance and pathogenic mechanism of a uropathogenic Escherichia coli (UPEC) strain UPEC132 at the genome-wide level. Methods:The susceptibility of UPEC132 strain to 16 antimicrobial agents was determined by minimum inhibitory concentration (MIC) assay. The UPEC132 strain was genotyped by multilocus sequence typing (MLST). The three-generation sequencing platform was used to sequence and assemble the whole genome of the UPEC132 strain. Drug resistance and virulence gene function annotations were predicted by Prodigal software and screened by using genome database. Genome sequences of the UPEC132 strain and 23 other UPEC strains collected from GenBank were phylogenetically analyzed, and a phylogenetic tree was constructed by RAxML software.Results:The UPEC132 strain was resistant to ampicillin, ampicillin/sulbactam, tetracycline, erythromycin, chloramphenicol and cefazolin. Its genotype was ST10522 by MLST. The whole genome of the UPEC132 strain included one complete genome (chromosome) and two plasmid sequences. The sequence sizes of the chromosome and plasmids 1 and 2 were 5 234 468 bp, 117 139 bp and 101 356 bp, and the guanine-cytosine (GC) content was 50.48%, 49.05%, and 54.04%, respectively. There were 4 856, 140 and 116 genes annotated in the chromosome and plasmids 1 and 2, respectively. Drug resistance genes were mainly distributed in the chromosomal genome, mainly including the multidrug resistance efflux pump gene clusters. Only blaTEM-1 and tetG genes were carried in the plasmid 2. Virulence genes were also mainly distributed in the chromosome genome, including nine pilus adhesins, five iron uptake systems and three secretory toxins. Gene clusters encoding Afa and type Ⅳ fimbriae were located on plasmids 1 and 2, respectively. The phylogenetic tree showed that the UPEC132 strain was not in the same branch with either of the 23 UPEC strains. Conclusions:The UPEC132 strain belonged to ST10522, which was a newly named ST type of Escherichia coli and first reported at home and abroad. The genome-wide genetic information of the UPEC132 strain was fully revealed. The multidrug resistance genes and virulence genes carried by the UPEC132 strain were associated with its drug resistance and pathogenicity.
