1.Application of PBL and CBL combined with self-media platform in clinical probation teaching of gastroenterology

Objective:To observe the effects of problem-based learning (PBL) and case-based learning (CBL) combined with self-media platform in clinical probation teaching of gastroenterology.Methods:From July 2019 to June 2020, 60 students in the Department of Gastroenterology of the Army Characteristic Medical Center of PLA were assigned to the traditional teaching group (control group, n=30) and the PBL and CBL combined with the self-media platform teaching group (experimental group, n=30). After the teaching, theoretical examination, practical assessment, and questionnaires were used to evaluate the effects of the two teaching methods. SPSS 23.0 was used to perform the t-test. Results:The comprehensive scores of students in the experimental group, including theoretical examination score (86.60±1.17) and practical assessment score (86.40±0.83), were significantly higher than those in the control group [(81.70±0.98), (82.00±1.18), P<0.001]. The questionnaire survey showed that the experimental group was superior to the traditional teaching group in terms of self-study and data access ability, students' self-confidence, and satisfaction with teaching ( P<0.001). Conclusion:PBL and CBL combined with self-media platform uses actual medical records to guide students to discuss, analyze, and summarize clinical issues. Combined with flexible learning on the self-media platform, this method breaks through the limitations of traditional teaching models and improves teaching quality and learning satisfaction. Therefore, this method can be promoted in clinical probation teaching of gastroenterology.
2.Construction and Validation of A Risk Model for Predicting Prognosis and Immunotherapy Response of Bladder Cancer Based on Cellular Senescence-related Genes

Objective To evaluate the prognosis and immunotherapy response of patients with bladder cancer by constructing a risk-score model of cellular senescence-related signature (SRS), as well as to explore the clinical application value of SRS in bladder cancer. Methods Senescence genes were screened from TCGA-BLCA, and cellular SRS genes were screened according to LASSO regression. A bladder cancer risk-score model was constructed based on the SRS genes to analyze the survival difference and model-fit degree of TCGA-BLCA high- and low-risk groups. Univariable and multivariable Cox regression was used to analyze the prognostic risk factors of bladder cancer. Overall survival differences of high- and low-risk groups in GEO-BLCA database were verified, and variations in immunotherapy responses were analyzed in IMvigor210 databases. According to the result of β-gal chromogenic reaction in bladder cancer and normal paracancer tissues, the existence of cell senescence was determined. OBJECTIVE:
To observe the effect of electroacupuncture (EA) at "Tianshu" (ST 25) and "Shangjuxu" (ST 37) on mental state, visceral sensitivity and protein expression of nerve growth factor (NGF), tyrosine kinase receptor A (TrkA) and transient receptor potential vanilloid 1 (TRPV1) of colonic tissue in diarrhea-predominant irritable bowel syndrome (IBS-D) rats, and to explore its possible mechanism on treating IBS-D.
METHODS:
A total of 36 male SD rats of SPF grade were randomized into a blank group, a model group, an EA group and a western medication group, 9 rats in each group. In the model group, the EA group and the western medication group, IBS-D model was established by enema of dinitrobenzene sulfonic acid (DNBS) combined with chronic restraint stress method. In the EA group, EA was applied at "Tianshu" (ST 25) and "Shangjuxu" (ST 37), with disperse-dense wave, in frequency of 2 Hz/100 Hz, 20 min each time, once a day for 7 days. In the western medication group, pinaverium bromide suspension was given by gavage (15 mg•kg-1•d-1) for 7 days. Before and after model establishment, and after intervention, the body mass, 24 h food intake and fecal water content were observed, the visceral sensitivity was detected by abdominal withdrawal reflex (AWR); after intervention, the mental state was evaluated by elevated plus maze (EPM) test, the protein expression of NGF, TrkA and TRPV1 was detected by immunohistochemistry and Western blot in the 4 groups.
RESULTS:
After model establishment, compared with the blank group, the body mass and 24 h food intake were decreased (P<0.05), first systolic latency of AWR was shortened and number of contraction wave of AWR was increased (P<0.05), and fecal water content was increased (P<0.05) in the model group, the EA group and the western medication group. After intervention, compared with the blank group, open arm residence time ratio (OT%) of EPM was decreased (P<0.05) and protein expression of NGF, TrkA, TRPV1 in colonic tissue was increased in the model group (P<0.05); compared with the model group, the body mass and 24 h food intake were increased (P<0.05), first systolic latency of AWR was lengthened and number of contraction wave of AWR was decreased (P<0.05), the fecal water content was decreased (P<0.05), OT% of EPM was increased (P<0.05), and protein expression of NGF, TrkA, TRPV1 in colonic tissue was decreased (P<0.05) in the EA group and the western medication group.
CONCLUSION
Electroacupuncture at "Tianshu" (ST 25) and "Shangjuxu" (ST 37) can relieve the anxiety and depression-like behaviors in IBS-D rats, down-regulate the protein expression of NGF, TrkA, TRPV1 in colonic tissue, so as to reduce the visceral sensitivity and relieve symptoms.

4. Short-chain Fatty Acids and Intestinal Leakage

Short-chain fatty acids (SCFAs) are important metabolic products of intestinal bacteria, and participate in the metabolism of intestinal materials, mediate the interaction between intestinal bacteria and intestinal mucosal epithelial cells, and are closely related to the occurrence of many diseases. Intestinal leakage is a pathological state and disease process in which intestinal function is impaired due to various reasons. Studies on the relationship between SCFAs and intestinal leakage are few. This article reviewed the relationship between SCFAs and intestinal leakage and its impact on diseases.
5.Effect of acupuncture on intestinal flora in rats with stress gastric ulcer.

OBJECTIVE:
To observe the effect of acupuncture at "Baihui" (GV 20), "Zhongwan" (CV 12) and "Zusanli" (ST 36) on intestinal flora in rats with stress gastric ulcer (SGU) , and to explore the mechanism of acupuncture promoting SGU recovery.
METHODS:
Thirty-one SPF SD rats were randomly divided into a control group (7 rats), a model control group (8 rats), an acupuncture group (8 rats) and a medication group (8 rats). The rats in the model group, acupuncture group and medication group were selected to applied the improved restraint water-immersion stress method to establish the SGU model. After modeling, the rats in the control group and model group were fixed and restrained for 20 min every day for a total of 5 days; the rats in the acupuncture group were intervented with acupuncture at "Baihui" (GV 20), "Zhongwan" (CV 12) and "Zusanli" (ST 36), once a day, 20 min each time, and twisting needle for 30 s every 5 min for a total of 5 days; the rats in the medication group were gavaged by solution of omeprazole enteric-coated tablet (200 mg/mL), 2 mL for each rat, once a day. Guth method was used to calculate the gastric mucosal damage index (GMDI), HE staining was used to observe the pathological changes of gastric mucosa, and 16SrDNA identification was used to detect the structural abundance of intestinal flora.
RESULTS:
Compared with the control group, the GMDI of rats in the model group was increased (<0.01), the gastric mucosal pathological changes were significant, and the intestinal flora richness index Chao1, Observed species and diversity index Shannon were all decreased (<0.05), the diversity index Simpson was increased (<0.05). Compared with the model group, the GMDI of rats in the acupuncture group and medication group was reduced (<0.01, <0.05), the gastric mucosal damage degree was reduced, and the intestinal flora richness index Chao1, Observed species and diversity index Shannon were all increased (<0.05) and the diversity index Simpson decreased (<0.05). Compared with the medication group, the GMDI of rats in the acupuncture group was reduced (<0.01), the recovery of gastric mucosal injury was better than that of the medication group.
CONCLUSION
Acupuncture can effectively improve gastric mucosal injury of SGU, and the mechanism may be related to increasing the diversity of intestinal flora and promoting the correction of the disordered intestinal flora.

6. Expression of micro RNA-561 in esophageal squamous cell carcinoma and its clinical significance

Objective:
To investigate the clinical significance and expression of microRNA(miR)-561 in esophageal squamous cell carcinoma.
Methods:
From January 2015 to February 2016, 96 specimens of adjacent tissues and cancer tissues from patients with esophageal squamous cell carcinoma who underwent surgery in Dongfeng Hospital Affiliated of Hubei University of Medicine were selected to detect the expression level of miR-561. Cell culture experiments were used to detect the expression level of miR-561 in Het-1a, Kyse150 and Eca109 cell lines. The correlation between clinical features and the expression level of miR-561 was counted. The expression level of miR-561 in patients with esophageal squamous cell carcinoma was analyzed. The relationship between the level of miR-561 and prognosis was investigated.
Results:
Relative expression of miR-561 in esophageal squamous cell carcinoma cell lines Eca109 and Kyse150 (1.61 ± 0.30, 1.21 ± 0.28) was significantly lower than that in Het-1a (2.56 ± 0.51), and the difference was statistically significant (

Objective:
To explore experience of wound treatment of extremely severe mass burn patients involved in August 2nd Kunshan factory aluminum dust explosion accident.
Methods:
On August 2nd, 2014, 98 extremely severe burn mass patients involved in August 2nd Kunshan factory aluminum dust explosion accident were admitted to 20 hospitals in China. The patients with complete medical record were enrolled in the study and divided into microskin graft group with 56 patients and Meek skin graft group with 42 patients. Split-thickness skin in area of residual skin were resected to repair wounds of patients in microskin graft group and Meek skin graft group by microskin grafting and Meek miniature skin grafting, respectively. 8.A Retrospective Clinical Analysis of 118 Cases of Small Intestinal Bleeding

Background:Small intestinal bleeding is difficult to diagnose and treat because of its complex etiology and limit to examination method. Aims:To analyze the etiology,diagnosis,treatment and prognosis of small intestinal bleeding. Methods:The clinical data of 118 consecutive patients with small intestinal bleeding admitted from Oct. 2006 to Oct. 2016 at Daping Hospital,the Third Military Medical University were retrospectively analyzed. Results:Melena was the most common manifestation of small intestinal bleeding (41. 5%),followed by dark bloody stool,positive fecal occult blood test,hematochezia,and anemia with unknown cause. The major causes of bleeding were benign or malignant tumors (43. 2%),vascular lesions (28. 0%)and inflammatory lesions (15. 3%). Diagnosis was made by means of capsule endoscopy,colonoscopy,digital subtraction angiography (DSA),barium meal examination,multi-slice CT (MSCT)and CT enterography (CTE). Forty-one patients were treated by surgical operation,7 by selective arterial embolization,2 by endoscopic therapy,56 by conservative therapy,and all these patients achieved hemostasis. One patient died of massive hemorrhage and 11 were discharged with giving up of treatment. Conclusions:The leading cause of small intestinal bleeding is tumor,followed by vascular and inflammatory lesions. Capsule endoscopy is able to make definite diagnosis with high accuracy,and MSCT is the most widely used diagnostic approach. In addition to conventional treatment,surgical operation,interventional and endoscopic therapies also play important roles in treating small intestinal bleeding.
9.Comparison of flexible ureteroscope holmium laser lithotripsy and percutaneous nephrolithotomy in treating renal stones of≤2cm:a 5-years restrospective study

Objective To compare of flexible ureteroscope holmium laser lithotripsy and percutaneous nephrolithotomy in treating renal stones of ≤2cm.Methods In this retrospective study,we compared 120 patients of renal pelvic stones of ≤2cm size treated with FURS(n=62)or PCNL(n=58).Factors such as operation time,hospitalization time,stone-free rate(SFR),drop in haemoglobin level,and complications were analyzed.Results The stone burden of the FURS group and PCNL group was(1.4±0.2)cm and(1.6±0.3)cm respectively.In FURS group,The operation time was(28±12)min with postoperatlve fever in 3 cases;he hospitalization time was(3.7±1.8)days.The decrease of hemoglobin was(0.18±0.06)g/L.No major complication was recorded.The SFR was 85.5%(53/62)one week postoperatively and 95.2%(59/62)one month postoperatively.In PCNL group,the operation time was (42±15)min with postoperative fever in 2 cases and bleeding in 2 case.The hospitalization time was 6.8±1.5 days.The decrease of hemoglobin was(14.30±3.50)g/L.The SFR was 91.4%(53/58)one week postoperatively and 96.6%(56/58)one month postoperatively.Conclusion For renal stones≤2cm,FURS has the advantages of shortening operation time,reducing blood transfusion requirements,decreasing postoperative complications with a farely SFR compared with PCNL.FURS is a good alternative.
10.The expression and clinical significance of Livin and Caspase-9 in duodenal adenocarcinoma

The expressions of Livin and Caspase-9 were detected by immunohistochemical technique in 60 duode-nal adenocarcinoma tissues and 30 adjacent normal tissue. The positive expression rate of Livin in duodenal adeno-carcinoma tissues was significantly higher than that of adjacent normal tissue,and the expression of Caspase-9 was lower than that of adjacent normal tissue ( P<0.01 ) . The expression of Livin was negatively correlated with the ex-pression of Caspase-9 ( P<0.01 ) . The expression of Livin and Caspase-9 was closely correlated with the progress of pathological changes, degree of development,lymph nods metastasis,and clinical stages,but not with sex,age and tumor size ( P>0.05 ) . Livin and Caspase-9 were associated with occurrence and development of duodenal adeno-carcinoma,and the expression of the above two were negatively correlated. Livin and Caspase-9 may be considered as reliable markers for clinical diagnosis,therapy efficacy and the prognosis of duodenal adenocarcinoma.
