[Objective]To investigate the presence of SARS-CoV-2 RNA in semen after COVID-19 infection in vaccinated individuals and to evaluate the impact of different recovery stages on sperm quality.[Methods]From January to March 2023,vaccinated male patients with recent SARS-CoV-2 infections confirmed by throat swabs were recruited from the Reproductive Medicine Center at the First Affiliated Hospital of Anhui Medical University.Semen samples were analyzed with RT-qPCR to detect SARS-CoV-2 RNA.A retrospective analysis assessed sperm quality across recovery stages, comparing patients within 30 days of recovery to those with 30 days or more.Semen parameters evaluated included volume, total sperm count, forward motility percentage, abnormal morphology rate, and sperm DNA fragmentation index. In addition,a longitudinal self-comparison was performed to examine changes in semen quality before and after recovery(<30 days and≥30 days).[Results]SARS-CoV-2 RNA was undetectable in the semen of all 205 patients.Neither cross-sectional comparisons nor longitudinal analyses showed significant differences in semen volume, sperm concentration, total sperm count, sperm morphology rate, sperm DNA fragmentation index,or high DNA stainability between recovery stages or compared to pre-infection values(P>0.05).However,forward motility percentage markedly decreased during recovery, with statistical significant between groups(P<0.05).[Conclusions]In vaccinated individuals who contracted COVID-19,SARS-CoV-2 was not transmitted through semen.Although the incidence of asthenozoospermia may increase within 30 days post-infection,this effect appears reversible in the short time.

Objective To explore the impact of enteral nutrition formula containing slow-release starch on blood glucose variability and prognosis in patients with severe acute pancreatitis(SAP).Methods A total of 204 SAP patients were enrolled and randomly divided into control group and ob-servation group using a random number table method, with 102 patients in each group. The control group received early enteral nutrition support with a standard enteral nutrition formula, while the ob-servation group received early enteral nutrition support with an enteral nutrition formula containing slow-release starch.Blood glucose variability indicators[largest amplitude of glycemic excursions(LAGE),standard deviation of blood glucose(SDBG),blood glucose coefficient of variation(BGCV),mean blood glucose(MBG),mean amplitude of glycemic excursions(MAGE),and time in range(TIR)]were compared between the two groups after treatment, along with clinical indicators during hospitalization, inflammatory markers[procalcitonin,C-reactive protein,interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)],and nutritional indicators(albumin,prealbumin)levels.Kaplan-Meier analysis was conducted to assess the prognosis of the two groups, and multivariate Cox regression analysis was performed to identify factors influencing prognosis. Results After treat-ment,the observation group exhibited lower levels of MBG,LAGE,SDBG,BGCV,MAGE,and a higher TIR compared with the control group(P<0.05).During hospitalization, the observation group had a shorter duration of enteral nutritiontherapy, lower insulin usage, and lower incidence rates of multiple organ failure and infectious pancreatic necrosis compared with the control group(P<0.05).After treatment, the observation group had lower levels of procalcitonin,C-reactive protein,IL-6,and TNF-α compared with the control group(P<0.05);however,there were no statistically significant differences in albumin and prealbumin levels between the two groups(P>0.05).Kaplan-Meier analysis showed that the cumulative mortality rate in the observation group was 12.75%,which was lower than the 17.65%in the control group(Log-rank x2=4.361,P=0.037).Multivariate Cox regression analysis revealed that TIR after treatment(HR=0.920;95%CI,0.869 to 0.974)was an independent protective factor for prognosis in SAPpatients(P<0.05),while infectious pancreatic necrosis(HR=4.269;95%CI,1.922 to 9.482)was an inde-pendent risk factor for prognosis in SAP patients(P<0.05).Conclusion Enteral nutrition formu-la containing slow-release starch helps stabilize blood glucose variability, control inflammatory mark-er levels, improve nutritional status, and prognosis in SAP patients. Both TIR and infectious pancre-atic necrosis are closely related to the prognosis of SAP patients.

Objective To investigate the diagnostic accuracy and clinical applicability of the Linknet-VGG16 deep learning algorithm for measuring the effective optical zone(EOZ)after corneal refractive surgery. Methods This single-center retrospective cohort study included 69 patients(69 eyes)who underwent femtosecond laser-assisted in situ kerato-mileusis(FS-LASIK)(34 eyes)or small incision lenticule extraction(SMILE)(35 eyes)at the Refractive Surgery Center of Affiliated Zhengzhou Aier Eye Hospital of Henan University from June 2023 to June 2024.Data from the right eyes of all patients were selected for statistical analysis.During the surgery, patients in the FS-LASIK group adopted the VisuMax fem-tosecond laser system combined with the Amaris 750S excimer laser system, while those in the SMILE group only used the VisuMax femtosecond laser system.A total of 276 Pentacam images were re-examined postoperatively.A Linknet segmenta-tion model based on the VGG16 encoder was constructed, and image normalization techniques were applied to accelerate model convergence.Model performance was assessed using accuracy, intersection over union(IoU),and the Dice coeffi-cient.The traditional EOZ measurement method based on corneal tangential curvature served as the reference standard.Bland-Altman analysis was conducted to evaluate consistency across all images and within each group, and the time effi-ciency of both methods was compared. Results Six representative medical image segmentation architectures(U-Net,U-Net++,DeepLabv3-ResNet50,DeepLabv3+-ResNet50,Unet-Densenet169,and Linknet-VGG16)were systematically evaluated. The Linknet-VGG16 model demonstrated superior performance over the other 5 models in pixel-level accuracy,IoU and Dice coefficient, which were 99.83%,99.48%and 99.74%,respectively.Although there was no significant differ-ence in accuracy and Dice coefficient between Linknet-VGG16 and U-Net models(whose accuracy was 99.82%and Dice coefficient was 99.72%),the inference speed of the U-Net model(62.46 ms)was 31.76%slower than that of the Linknet-VGG16 model(42.62 ms).The evaluation results of a clinically applicable comprehensive scoring model(weights:accura-cy 20%,IoU 20%,Dice coefficient 20%,speed 25%,model size 15%)showed that the Linknet-VGG16 model achieved a score of 88.01,surpassing other architectures(U-Net:86.29;DeepLabv3+-ResNet50:80.41;DeepLabv3-ResNet50:73.82;U-Net++:73.22;Unet-Densenet169:66.66).Bland-Altman analysis revealed that the mean difference of the 136 images in the FS-LASIK group was 0.01 mm[95%limits of agreement(LoA):-0.36 to 0.35 mm],with 96.3%of data points falling within the LoA.The mean difference of the 140 images in the SMILE group was-0.01 mm(95%LoA:-0.36 to 0.33 mum),with 95.7%of data points falling within the LoA.The mean difference of all 276 images was 0.00 mm(95%LoA:-0.36 to 0.34 mm),with 96.4%of data points falling within the LoA.These results indicated excellent consistency. The average measurement time per image using the traditional EOZ measurement method was 13.00 minutes, whereas the deep learning model required only 3.22 seconds. Conclusion The traditional EOZ measurement method based on corne-al tangential curvature exhibits good consistency with the fully automatic EOZ measurement method based on deep learning algorithms, achieving high image recognition accuracy.Additionally, the deep learning algorithm significantly reduces measurement time, compared with the traditional method based on corneal tangential curvature.

Complex brain diseases seriously endanger human health, and early diagnostic biomarkers and effective treatments are currently lacking. Due to ethical constraints on human research, establishing monkey models is crucial to address these issues. With the rapid development of technology, transgenic monkey models of a range of brain diseases, especially autism spectrum disorder (ASD), have been successfully established. However, to establish practical and effective brain disease models and subsequently apply them to disease mechanism and treatment studies, there is still a lack of a standard tool, i.e., a system for collecting and analyzing the daily behaviors of brain disease model monkeys. Therefore, with the goal of undertaking a comprehensive and quantitative study of behavioral phenotypes, we established a standard daily behavior collection and analysis system, including behavioral data collection protocols and a monkey daily behavior ethogram (MDBE) for rhesus and cynomolgus monkeys, which are the most commonly used non-human primates in model construction. Then, we used ASD as an application example after referring to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR), which is widely used in clinical disease diagnosis to obtain ASD core clinical symptoms. We then established a sub-ethogram (ASD monkey core behavior ethogram (MCBE-ASD)) specifically for quantitative assessment of the core clinical symptoms of an ASD monkey model based on MDBE. Subsequently, we demonstrated the high reproducibility of the system.
