ObjectiveTo explore the differences in attention to the sub-dimensions of satisfaction with the quality of assistive devices among people with different self-care abilities in China, identify the key driving factors, and provide a basis for the precision design and service provision of assistive devices. MethodsBased on the 2023 national survey data, involving 14 030 people with functional impairments, self-care ability was taken as the core independent variable, eight sub-dimensions of satisfaction as dependent variables, and variables such as gender, age, educational level and residential type were controlled. Univariate analysis was performed using Chi-square test with Bonferroni correction, and a binary Logistic regression model was constructed to identify influencing factors. Meanwhile, reliability and validity tests, endogeneity tests (instrumental variable method and propensity score matching) and heterogeneity tests were conducted. ResultsAmong the eight satisfaction sub-dimensions, six presented significant inter-group differences, with Bonferroni correction (threshold = 0.00625). Following binary Logistic regression and endogeneity correction, significant inter-group heterogeneity was confirmed in dimensions such as size and shape. For the affordability dimension, the main effect of self-care ability was not statistically significant, yet prominent urban-rural heterogeneity was observed. Specifically, taking the fully independent (self-care) group as the reference, the fully dependent group attached significantly greater importance to safety (B = 0.253, P < 0.001), comfort (B = 0.153, P = 0.001) and ease of use (B = 0.316, P < 0.001); the partially dependent group showed the highest level of attention to lightweight (B = 0.094, P = 0.027) and durability (B = 0.254, P < 0.001); and the fully independent group demonstrated a relatively stronger preference for aesthetics. ConclusionStratified functional demands, driven by self-care ability, exist in the satisfaction of individuals with functional impairments with assistive devices in China. The policy formulation and product design of assistive devices should shift to a precision-oriented paradigm: prioritize the guarantee of safety, comfort and ease of use for fully dependent groups, optimize lightweight performance and durability for partially dependent groups, enhance aesthetics and social acceptance for fully independent groups, roll out price subsidy policies for urban price-sensitive groups, and strengthen the supply of core functional services for rural groups. This approach will comprehensively improve the adaptation effectiveness of assistive devices and the well-being of users.

OBJECTIVE To formulate Guidelines for the standardized implementation of pharmacist-managed clinics ( 2026 edition ) in response to the challenges faced by such clinics in China, including uneven development, large discrepancies in service specifications, insufficient patient awareness, and limited medical insurance coverage. METHODS Led by the Pharmaceutical Affairs Professional Committee of the Chinese Hospital Association, the Evidence-based Pharmacy Professional Committee of the Chinese Pharmaceutical Association, and the Hospital Pharmacy Professional Committee of the Cross-strait Medical and Health Exchange Association, a total of 19 domestic hospital pharmacy experts were organized. Through a systematic review of national policies and literature research, current practical experience was summarized. Consensus on the contents of the guidelines was reached after in-depth discussions. RESULTS &CONCLUSIONS The guidelines covered five sections: definition and connotation of pharmacist-managed clinics, establishment requirements, implementation and management, post competency, and practical research. Firstly, the definition and connotation included three operational forms of pharmacist-managed clinics (independent mode, physician-pharmacist joint mode, and online pharmacist-managed clinic mode) and classified service modes (specialty-specific, drug-specific, and disease-specific pharmacist-managed clinics). The establishment requirements were further refined, covering system construction (pharmaceutical service management system, quality control and assessment mechanism), personnel qualifications (professional credentials, continuing education and professional training, etc), service recipients, as well as service venues and facilities. Subsequently, the implementation and management of pharmacist-managed clinics were proposed, involving service procedures, intervention measures, documentation and records, patient education and follow-up, humanistic care, as well as risk management and quality control. Finally, post competency encompassed the competency requirements for pharmacists providing services in pharmacist-managed clinics, as well as the suggestions on teaching methods; practical research encouraged the conduct of high-quality pharmaceutical practice in the setting of pharmacist-managed clinics. The guidelines provide valuable guidance for the standardized implementation of pharmacist-managed clinics in China in terms of establishment, management, teaching, and research, fill the guideline gap in this field, and can promote the high-quality development of pharmacist-managed clinics.

ObjectiveTo construct a risk prediction model for frequent acute exacerbations of chronic obstructive pulmonary disease (COPD) under disease-syndrome combination, thus providing decision support for precise clinical intervention. MethodsA total of 2 029 patients with acute exacerbations of COPD admitted to the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2020 to August 2024 were retrospectively included. These patients were classified into groups of frequent acute exacerbations (≥2 times/year) and infrequent acute exacerbations (<2 times/year) according to the hospitalization times per year. Risk factors were screened by LASSO regression combined with logistic regression, and a nomogram model was constructed. The model performance was assessed based on the area under the curve (AUC), calibration curves, and decision curve analysis (DCA). ResultsThe differences in baseline characteristics between the frequent acute exacerbations group (1 196 cases) and infrequent acute exacerbations group (833 cases) were not statistically significant. LASSO regression combined with multivariate logistic regression screened the following independent risk factors: body mass index (BMI), hospitalization days, number of smoking years, place of residence, use of noninvasive ventilators, oxygen-demanding therapy, liver cirrhosis, use of systemic glucocorticosteroids, and traditional Chinese medicine syndrome (phlegm and stasis obstructing the lung). The nomogram model showed good discrimination and calibration in both the training set (AUC=0.748) and validation set (AUC=0.774). ConclusionThe risk prediction model for frequent acute exacerbations of COPD, integrating traditional Chinese medicine syndrome, constructed in this study has high accuracy. It can provide a scientific basis for early clinical identification of high-risk patients and individualized intervention.

Objective:
To investigate the objective characteristics of tongue manifestation in different stages of damp-heat syndrome in diabetic kidney disease (DKD).
Methods:
A cross-sectional study enrolled 134 patients with DKD G3-5 stages who met the diagnostic criteria for damp-heat syndrome in DKD. The patients were treated at Dongzhimen Hospital, Beijing University of Chinese Medicine, from May 2023 to January 2024. The patients were divided into three groups: DKD G3, DKD G4, and DKD G5 stage, with 53, 33, and 48 patients in each group, respectively. Clinical general data (gender, age, and body mass index) and damp-heat syndrome scores were collected from the patients. The YZAI-02 traditional Chinese medicine (TCM) AI Tongue Image Acquisition Device was used to capture tongue images from these patients. The accompanying AI Open Platform for TCM Tongue Diagnosis of the device was used to analyze and extract tongue manifestation features, including objective data on tongue color, tongue quality, coating color, and coating texture. Clinical data and objective tongue manifestation characteristics were compared among patients with DKD G3-5 based on their DKD damp-heat syndrome status.
Results:
No statistically significant difference in gender or body mass index was observed among the three patient groups. The DKD G3 stage group had the highest age (P<0.05). The DKD G3 stage group had a lower score for symptoms of poor appetite and anorexia(P<0.05) than the DKD G5 group. No statistically significant difference was observed in damp-heat syndrome scores among the three groups. Compared with the DKD G5 stage group, the DKD G3 stage group showed a decreased proportion of pale color at the tip and edges of the tongue (P<0.05). The DKD G4 stage group exhibited an increased proportion of crimson at the root of the tongue, a decreased proportion of thick white tongue coating at the root, a decreased proportion of pale color at the tip and edges of the tongue, an increased hue value (indicating color tone) of the tongue color in the middle, an increased brightness value (indicating color lightness) of the tongue coating color in the middle, and an increased thickness of the tongue coating (P<0.05). No statistically significant difference was observed in other tongue color proportions, color chroma values, body characteristics, coating color proportions, coating color chroma values, and coating texture characteristics among the three groups.
Conclusion
Tongue features differ in different stages of DKD damp-heat syndrome in multiple dimensions, enabling the inference that during the DKD G5 stage, the degree of qi and blood deficiency in the kidneys, heart, lungs, liver, gallbladder, spleen, and stomach is prominent. Dampness is more likely to accumulate in the lower jiao, particularly in the kidneys, whereas heat evil in the spleen and stomach is the most severe. These insights provide novel ideas for the clinical treatment of DKD.

Objective:
Miniscrews are commonly utilized as temporary anchorage devices (TADs) in cases of maxillary protrusion and premolar extraction. This study aimed to investigate the effects and potential side effects of two conventional miniscrew configurations on the maxillary incisors.
Methods:
Eighty-two adult patients with maxillary dentoalveolar protrusion who had undergone bilateral first premolar extraction were retrospectively divided into three groups: non-TAD, two posterior miniscrews only (P-TADs), and two anterior and two posterior miniscrews combined (AP-TADs). Cone-beam computed tomography was used to evaluate the maxillary central incisors (U1).
Results:
The APTADs group had significantly greater U1 intrusion (1.99 ± 2.37 mm, n = 50) and less retroclination (1.70° ± 8.80°) compared to the P-TADs (–0.07 ± 1.65 mm and 9.45° ± 10.68°, n = 60) and non-TAD group (0.30 ± 1.61 mm and 1.91° ± 9.39°, n = 54).However, the AP-TADs group suffered from significantly greater apical root resorption (ARR) of U1 (2.69 ± 1.38 mm) than the P-TADs (1.63 ± 1.46 mm) and non-TAD group (0.89 ± 0.97 mm). Notably, the incidence of grade IV ARR was 16.6% in the AP-TADs group, significantly higher than the rates observed in the P-TADs (6.7%) and non-TAD (1.9%) groups. Multiple regression analysis revealed that after excluding tooth movement factors, the AP-TADs configuration resulted in an additional 0.5 mm of ARR compared with the P-TADs group.
Conclusions
In cases of maxillary protrusion and premolar extraction, the use of combined anterior and posterior miniscrews enhances incisor intrusion and minimizes torque loss of the maxillary incisors. However, this approach results in more severe ARR, likely due to the increased apical movement and composite force exerted.

9.The Role of Complement C3 and Its Related Signaling Pathways in Pathologic Pain
Chinese Journal of Biochemistry and Molecular Biology 2025;41(2):219-229
The complement system plays an important role in identifying and eliminating pathogens, clearing physiological debris, coordinating immune response and homeostasis, etc. As an early warning signal of inflammatory response, abnormal complement activity is an important cause of pathologic pain diseases.Complement 3(C3)is an important indicator of the activation of the complement system during pathologic pain induction.Experimental and clinical epidemiological studies have found abnormal in-crease of C3 in peripheral and central nerve in various types of pathologic pain, and C3 directly or indi-rectly regulates the response of neurons to neuropathy by binding specific C3 receptors.C3 can directly regulate various aspects of neuronal life and death through specific complement receptors expressed on the plasma membrane of neurons, and can also indirectly regulate by recruiting glial cells and immune cells through various mechanisms to transmit complement signals to neurons, which guide the response of neu-rons to tissue injury, neurotrauma, and neuropathy. This article mainly reviews the mechanism of cyto-kines and signaling pathways involved in C3 in pathologic pain, and discusses the possibility of C3 as an analgesic target in pathologic pain.

Objective To investigate the current status of assistive device adaptation among people with dysfunction in China,identi-fy unmet assistive needs for assistive products, and determine key factors influencing attitudes toward assistive device use. Methods Based on data from the Chinese segment of the Global Rapid Assistive Technology Assessment(rATA)survey,5 935 individuals with dysfunction from Beijing,Anhui,Fujian,Hubei,Sichuan and Shaanxi were included. The adaptation status of assistive devices was analyzed, and a theoretical framework was constructed using the tech-nology acceptance model.Multiple linear regression analysis was applied to identify factors influencing attitudes toward assistive device use. Results The utilization rate of rehabilitation assistive devices among people with dysfunction was 65.9%,with a notable prevalence of multiple functional impairments.Fit rate was the highest(68.44%)in individuals with visual im-pairments,while those with communication impairments had the lowest(1.52%).Fit rates were significantly low-er among rural residents and older adults.Overall satisfaction with assistive devices was relatively high(4.11±0.71).Accessibility of the home environment(B=0.396,P<0.001),accessibility of the public environment(B=0.204,P<0.001),functional utility of assistive devices(B=0.204,P<0.001),and distance to obtain the devices(B<-0.112,P<0.01)were key factors affecting satisfication toward assistive device use, while the source of payment showed no significant impact(P>0.05).Conclusion Significant structural imbalances and group disparities exist in the adaptation of assistive devices among people with dysfunction in China. The phenomenon of multiple functional impairments has not received suffi-cient attention, and there remains a substantial unmet need for assistive products among non-fitted groups.Al-though overall satisfaction with assistive devices is high, influencing factors are complex.Continuous improve-ment of accessibility in both private and public environments, promotion of personalized adaptation services, and optimization of assistive device service networks are essential to enhance adaptation accuracy, sustained use,and subjective well-being among people with dysfunction.
