Instrument separation is a critical complication during root canal therapy, impacting treatment success and long-term tooth preservation. The etiology of instrument separation is multifactorial, involving the intricate anatomy of the root canal system, instrument-related factors, and instrumentation techniques. Instrument separation can hinder thorough cleaning, shaping, and obturation of the root canal, posing challenges to successful treatment outcomes. Although retrieval of separated instrument is often feasible, it carries risks including perforation, excessive removal of tooth structure and root fractures. Effective management of separated instruments requires a comprehensive understanding of the contributing factors, meticulous preoperative assessment, and precise evaluation of the retrieval difficulty. The application of appropriate retrieval techniques is essential to minimize complications and optimize clinical outcomes. The current manuscript provides a framework for understanding the causes, risk factors, and clinical management principles of instrument separation. By integrating effective strategies, endodontists can enhance decision-making, improve endodontic treatment success and ensure the preservation of natural dentition.

Background and Objectives: Regenerative endodontic procedures (REPs) are a research hotspot in the endodontic field. One of the biggest problems of REPs is that it is difficult to realize regeneration of pulp-dentin complex and functional reconstruction. The reason is still not clear. We hypothesize that the migration may be different in different dental stem cells. Periodontal ligament stem cells (PDLSCs) may migrate faster than stem cells of apical papilla (SCAPs), differentiating into cementum-like tissue, bone-like tissue and periodontal ligament-like tissue and, finally affecting the outcomes of REPs. Hence, this study aimed to explore the mechanism that regulates the migration of PDLSCs. Methods: and Results: After isolating and culturing PDLSCs and SCAPs from human third molars, we compared the migration of PDLSCs and SCAPs. Then we investigated the role of SDF-1α-CXCR4/CXCR7 axis in PDLSC migration. We further investigated the impact of Porphyromonas gingivalis (P. gingivalis) lipopolysaccharide (LPS) on PDLSC migration and the potential mechanism. PDLSCs showed better migration under both noninflammatory and inflammatory conditions than SCAPs. SDF-1α can promote the migration of PDLSCs by elevating the expression of CXCR4 and CXCR7, increasing the interaction between them, promoting expression of β-arrestin1 and activating the ERK signaling pathway. P. gingivalis LPS can promote the migration of PDLSCs toward SDF-1α through increasing the expression of CXCR4 via the NF-κB signaling pathway, promoting the expression of β-arrestin1, and activating the ERK signaling pathway. Conclusions This study helped elucidate the potential reason for the difficulty in forming pulp-dentin complex.

Objective: This study aims to compare the incidence of dentinal microcracks produced by ProTaper Universal (PTU) and ProTaper Gold (PTG) file systems during root canal procedures in different curved canals using a dyeing technique. Methods: Sixty extracted human molars were divided into 3 groups of 20 samples each in terms of root curvature (mild bending group, 10 °-19 °; moderate bending group, 20 °-29 °; severe bending group, 30 °-39 °). Ten samples of each group were then randomly allocated to the PTU and PTG file systems. After preparation, all roots were stained using a dyeing method and sectioned at the most curved plane and 2 mm below and above the most curved plane with a low-speed saw under cold water. A stereomicroscope was used to inspect dentinal microcracks at 60 × magnification, and differences between these three instrument groups were analyzed using the chi-square test. Results : The PTG file system induced significantly fewer dentinal microcracks for total, incomplete and complete cracks (P < 0.05), and the effect was more obvious with increasing canal curvature. Conclusion With the limitations of this in vitro study, it can be concluded that ProTaper Gold can result in fewer dentinal microcracks than ProTaper Universal.

Objective: To evaluate the efficacy of allogeneic hematopoietic stem cell transplantation (allo-HSCT) from different donors as first-line treatment for children and adolescents with severe aplastic anemia (SAA) . Methods: The clinical data of 79 children and adolescents with SAA diagnosed from January 2013 to December 2016 in Henan Province were retrospectively analyzed. There were 50 males and 29 females, with a median age of 14(4-18) years. 40 cases received matched sibling transplantation (MSD-HSCT), 17 with unrelated donor transplantation (UD-HSCT), and 22 with haploidentical transplantation (haplo-HSCT). Results: The comparison of MSD-HSCT, UD-HSCT, haplo-HSCT groups was conducted and the median times of neutrophils engraftment were statistically significant [12(9-25) d, 14(10-22) d, 16(11-26) d, respectively (χ²=13.302, P=0.001)], but no difference in+30 d engraftment rate [97.3%(36/37), 100%(15/15), 100%(20/20), χ²=0.959, P=0.619]. The median times of PLT engraftment were not statistically significant [14(6-34)d, 16(7-32)d, 19(10-34)d, respectively, χ²=5.892, P=0.053], and the +30 d engraftment rate had no difference [97.3%(36/37), 100%(15/15), 100%(20/20), χ²=0.959, P=0.619]. The post-transplant infection rate showed no statistically significance [35.0% (14/40), 29.4% (5/17), 45.5% (10/22), χ²=1.158, P=0.560], as well as the incidences of aGVHD, grade III/IV aGVHD and cGVHD(χ²=0.230, P=0.891; χ²=2.628, P=0.269; χ²=3.187, P=0.203). The two-years OS rate was not statistically significant respectively [(77.1±6.7)%, (70.6±11.1)%, (77.3±8.9)%, χ²=0.330, P=0.845]. Severe post-transplant infection (RR=4.617, P=0.009), grade Ⅲ/Ⅳ aGVHD (RR=2.707, P=0.048) were independent risk factors for OS. Conclusion The overall efficacy of MSD-HSCT, UD-HSCT and haplo-HSCT as first-line therapy for children and adolescents with SAA/VSAA is comparable.

Objective To investigate the history, current situation and ecological environment of Chongqing wild and cultivated Fritillaria taipaiensis resources; To provide references for protection and industrial development of Fritillaria taipaiensis resources. Methods Data query, on-the-spot investigation and follow-up survey were used in the study. Results Chongqing Fritillaria taipaiensis was in the medical records since 1843. Now 5 counties have wild Fritillaria taipaiensis resources and 6 districts have cultivated Fritillaria taipaiensis resources, with 6 typical ecological types. Conclusion Chongqing wild Fritillaria taipaiensis resources are wide distributed and with long history of medical usage. Chongqing is the original producing area of Fritillaria taipaiensis, which has been destroyed by human factors. Fritillaria taipaiensis resources in several districts have become extinct, however, the cultivated Fritillaria taipaiensis resources is developing rapidly.
