1.Current status and future prospects of global robotic surgery: Evolution from thoracic surgery to multidisciplinary integration

This article systematically elucidates the current development status and future trends of robot-assisted surgery worldwide. Currently, robotic surgery led by the Da Vinci Surgical System has been widely adopted across multiple disciplines, including thoracic surgery, urology, and gynecology, demonstrating advantages such as precision, stability, and minimal invasiveness. Significant regional disparities exist in the global distribution of robotic surgery, reflecting inequalities in healthcare resources and economic development worldwide. China is rapidly emerging in the field of robotic surgery, undergoing a strategic transition from technology adoption to independent innovation: domestically developed systems (e.g., Toumai, Surgibot) have demonstrated safety and efficacy in multidisciplinary clinical practice; leveraging the advantages of 5G technology, remote robotic surgery has progressed from proof-of-concept to clinical reality, offering innovative solutions for equitable healthcare resource allocation; meanwhile, a quality control system spanning from national strategic planning to clinical operational standards is under development. Confronted with core challenges such as high costs, technical barriers (e.g., lack of force feedback), steep learning curves, lagging regulatory and ethical frameworks, and uneven regional development, future robotic surgery will deeply integrate artificial intelligence, evolving toward single-port/flexible miniaturization, normalization of remote surgery, and personalized precision treatment. Ultimately, it will drive the transformation of surgical medicine toward a new paradigm characterized by greater precision, intelligence, and accessibility, and is expected to play a strategic role in public health emergencies and disaster relief operations.
2.Differences in deltamethrin resistance and kdr gene mutation in Culex tritaeniorhynchus population in and outside the Yellow Sea wetland

Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.
3.Effects of volatile oil from Acorus tatarinowii on CX3CL1/CX3CR1 signal axis and neuroinflammation in a rat model of tic disorders

AIM To investigate the effects of volatile oil from Acorus tatarinowii Schott(A.tatarinowii)on neuroinflammation in a rat model of tic disorders.METHODS The SD rats were randomly divided into the blank group(8 rats)and the model group(40 rats).The rat models of tic disorders established successfully by intraperitoneal injection of iminodiapropionitrile(IDPN)were further divided into the model group,the tiapride group and the high-dose,moderate-dose and low-dose A.tatarinowii volatile oil groups,with 8 rats in each group.The 4-week intragastric treatment of respective drug was initiated the next day after the completion of modeling,and normal saline was dosed upon the blank group and the model group,during which the rats' behavioral changes were assessed by stereotyped behavior and motor behavior score every week.After the administration,the rats had their morphological changes of striatal neurons observed by Nissl staining;their levels of TGF-β,IL-10,TNF-αand IL-1β in serum and striatum detected by ELISA;their striatal protein expressions of CX3CL1 and CX3CR1 detected by Western blot and immunohistochemistry;and their striatal expressions of M1,M2 microglia marker proteins CD86,CD206,SYN and PSD-95 detected by immunofluorescence co-staining.RESULTS Compared with the model group,the A.tatarinowii volatile oil groups demonstrated improved twitch-like behavior;decreased scores of motor behavior and rigid behavior(P<0.01);alleviated damage of Nissl bodies in neurons;increased serum and striatum levels of TGF-β and IL-10(P<0.05,P<0.01);decreased levels of TNF-α and IL-1β(P<0.01);decreased striatal protein expressions of CX3CL1 and CX3CR1(P<0.01);increased protein expressions of PSD95 and SYN(P<0.05,P<0.01);and decreased CD86/Iba1(P<0.01)and increased CD206/Iba1(P<0.01)in terms of the fluorescence intensity.CONCLUSION A.tatarinowii volatile oil contributes an anti-tic effect and improves the neuroinflammation in the brain of the rat model of tic disorders by promoting the transformation of microglia into M2 type via CX3CL1/CX3CR1 signal axis.
4.Clinical Efficacy of Laparoscopic Non Mesh Vaginal Sacral Fixation in the Treatment of Pelvic Organ Prolapse with Mainly Middle Pelvic Defects

Objective:To observe the clinical efficacy of laparoscopic non mesh vaginal sacral fixation(LSC)in the treatment of pelvic organ prolapse(POP)patients with mainly middle pelvic defects.Methods:55 cases of laparoscopic non mesh LSC patients with POP with mainly middle pelvic defects were selected from August 2019 to February 2023 at Liuzhou Hospital of Guangzhou Women and Children's Medical Center.The surgical condition of POP patients were observed.Patients were followed up after surgery,the position and recurrence of the anterior vaginal wall(Ba)point,posterior vaginal wall(Bp)point,and vaginal apex(C)point in POP patients before and after surgery were observed.The preoperative and postoperative pelvic floor dysfunction questionnaire scores of POP patients treated by laparoscopic non mesh LSC were compared.And the occurrence of short-term and long-term complications in POP patients after surgery were observed.Results:Among the 55 POP patients with laparoscopic non mesh LSC,11 underwent bilateral adnexectomy,3 underwent bilateral salpingectomy,55 underwent anterior vaginal wall repair,28 underwent posterior vaginal wall repair,2 underwent paravaginal repair,and 28 underwent perineal laceration repair.Additionally,2 underwent tension free mid urethral suspension via the pubic posterior vaginal wall,and 3 underwent mid urethral folding.The average duration of the surgery was(197.8±48.7)minutes,and the intraoperative blood loss was 50(30,50)mL.The postoperative indwelling time of the urinary catheter was 2.0(1,3)days,the postoperative residual urine volume was 6(0,11)mL,and the hospital stay was 10(8,12)days.Among the 55 POP patients,all surgeries were successful,with a success rate of 100%.The follow-up period ended in September 2024,with an average follow-up time of(42.14±11.37)months.2 cases were lost to follow-up,and 53 patients were successfully followed up,including 48 outpatient follow-up patients and 5 telephone follow-up patients.Among the 48 outpatient follow-up patients,1 case had a recurrence of the anterior vaginal wall,with a recurrence rate of 2.08%(1/48).The patient did not undergo anterior vaginal wall repair during surgery,and after surgery,the patient felt that there was a mass protruding from the vagina.However,the symptoms improved on their own after rest,and no further surgery was performed.The comparison of Ba points,C points,and Bp points before and after surgery in 48 outpatient follow-up patients showed statistically significant differences(P<0.05).The comparison of preoperative and postoperative pelvic organ prolapse distress inventory(POPDI),Urogenital distress inventory(UDI),and Pelvic Floor Distress Inventory-short From-20(PFDI-20)scores among 53 POP patients showed statistically significant differences(P<0.05),there was no statistically significant difference in preoperative and postoperative colorectal-anal distress inventory(CRADI)scores among the 53 POP patients(P>0.05).There were a total of 11 cases of postoperative complications in 55 POP patients,including 1 case of enteritis,2 cases of urinary tract infections,1 case of bladder injury,1 case of lower back pain,pelvic pain,and difficulty defecation,and 5 cases of newly developed urinary incontinence.The incidence of complications was 20.00%(11/55).The long-term follow-up results of POP patients showed that 4 patients had no sexual activity after surgery,and 49 patients resumed sexual activity after surgery,among them,47 patients reported an improvement in their sexual experience compared to before surgery,accounting for 95.92%(47/49).Two patients were unable to have normal sexual activity due to painful intercourse,accounting for 4.08%(2/49).Conclusion:Laparoscopic non mesh LSC treatment for POP patients with mainly middle pelvic defects has a high success rate,few complications,good short-term and long-term follow-up effects,and can significantly improve pelvic floor dysfunction symptoms.
5.Impact of induction chemotherapy sensitivity on prognosis in locally advanced hypopharyngeal cancer: a single-center retrospective cohort study

Objective:To assess the impact of induction chemotherapy sensitivity on the prognosis and larynx preservation rates in patients with locally advanced hypopharyngeal cancer and to identify risk factors influencing induction chemotherapy sensitivity.Methods:This study included patients with locally advanced (stage III-IV) hypopharyngeal cancer who received induction chemotherapy as initial treatment at the Eye & ENT Hospital of Fudan University between August 2017 and September 2022. Based on the Response Evaluation Criteria in Solid Tumors (RECIST) 1.1, enrolled patients were classified into the sensitive group and the resistant group according to their response to induction chemotherapy. Chi-square tests and Log-rank tests were used to compare the objective response rate (ORR), overall survival (OS), progression-free survival (PFS), and laryngeal preservation rate (LPR) between groups. Propensity score matching (PSM) was employed to accurately evaluate the impact of induction chemotherapy sensitivity on prognosis in real-world settings. Univariate and multivariate logistic regression analyses were performed to identify risk factors for induction chemotherapy resistance in locally advanced hypopharyngeal cancer.Results:A total of 197 patients with locally advanced hypopharyngeal cancer who received induction chemotherapy as initial treatment were included in, comprising 195 males and 2 females, with ages ranging from 36 to 74 years. Among them, 155 patients (78.68%) were classified into the sensitive group and 42 patients (21.32%) into the resistant group. The overall response rate (ORR) of induction chemotherapy in this cohort was 78.68%, with a five-year OS rate of 63.7%. The sensitive group had significantly better OS (mOS 6.32 vs. 5.05 year), PFS (mPFS 5.71 vs. 3.09 year) and a significantly higher LPR (91.6% vs. 69.0%) ( P<0.05). After propensity score matching, all covariates were balanced between the two groups, and the sensitive group showed significant improvement in OS ( P<0.05), while, no significant difference was observed in PFS and LPR between the two groups. Logistic regression analysis revealed that risk factors for induction chemotherapy failure in locally advanced hypopharyngeal cancer included: smoking status ( OR [95% CI]=4.751 [1.887-11.961]), tumor location in the posterior pharyngeal wall ( OR [95% CI]=2.988 [1.264-7.063]), and cN2-3 stage ( OR [95% CI]=3.641 [1.109-11.954]) ( P<0.05). Conclusions:Induction chemotherapy sensitivity significantly affects the prognosis of locally advanced hypopharyngeal cancer, which is influenced by various risk factors, including smoking status, tumor sublocation, and clinical N stage.

7.TAFRO syndrome:one case report and literature review

TAFRO syndrome is a systemic inflammatory disease with unknown etiology.It has low incidence rate and progresses rapidly,which poses a significant challenge for clinicians to make a timely diagnosis and provide reasonable treatment.This article retrospectively analyzed a 65-year-old male patient with iMCD-TAFRO admitted to Minhang Hospital,Fudan University,and visited the Department of Nephrology due to bilateral lower limb edema.The patient exhibited systemic edema,fever,and multiple enlarged lymph nodes.A comprehensive examination showed thrombocytopenia,renal dysfunction,elevated CRP levels,multiple serosal fluid accumulations,and bone marrow reticulin fibrosis.For further diagnosis and treatment,he visited the Hematology Clinic of Zhongshan Hospital,Fudan University.Based on the clinical manifestations,the diagnosis of TAFRO syndrome was considered.After further lymph node pathological consultation,the diagnosis was confirmed as idiopathic multicentric Castleman disease(iMCD)-TAFRO syndrome.Later,the patient was transferred to the hematology department of Minhang Hospital,Fudan University.After 15 days of treatment with methylprednisolone,cyclosporine A,rituximab,and thrombopoietin,there was no significant improvement in the condition.Due to personal reasons,the patient discontinued further treatment and passed away 2 weeks later.

10.Pancreatic cystic neoplasm classification model integrating deep learning and radiomics features

Objective To establish a comprehensive classification model integrating deep learning and radiomics features to enhance the accuracy and consistency for identifying pancreatic cystic neoplasms.Methods Firstly,the radiomics technique and ResNet50 convolutional neural network were used to extract the features of radiomics and deep learing for pancreatic cystic neoplasms,respectively,and the key features were obtained by screening the features with the least absolute shrinkage and selection operator;secondly,three classification models were constructed by integrating only key features,deep learning features or radiomics and deep learning features,respectively;finally,logistic regression,random forest,adaptive boosting and support vector machine(SVM)classifiers were used to compare the above feature models in order to verify their efficacies for categorizing pancreatic cystic neoplasms.Results The comprehensive model integrating deep learning and radiomics features performed the best on the SVM classifier,which gained advantages over the other two models with the accuracy,recall,precision,AUC value and F1 value being 89.34%,92.13%,75.34%,0.90 and 0.83,respectively.Conclusion The comprehensive classification model integrating deep learning and radiomics features behaves well in mining relationships between various types of features and classifying pancreatic cystic neoplasms,and facilitates further precise diagnosis and treatment.[Chinese Medical Equipment Journal,2025,46(1):7-12]
