1.Predictive value of MRI parameter-based heterogeneity in treatment response and prognosis for recurrent glioblastoma

Objective:To investigate the heterogeneity of tumor density-enhancement complex (TDEC) based on MRI parameters in predicting the treatment response and prognosis for recurrent glioblastoma (rGBM) to guide the formulation of personalized clinical treatment strategies.Methods:A prospective cohort study was performed; 66 patients with postoperative rGBM were enrolled from Department of Neurosurgery, Affiliated Hospital of Jiangsu University. Multi-sequence MRI was performed, and diffused and enhanced data of the rGBM were utilized to construct TDEC as intratumoral sub-regions via pixel co-localization technique. Correlations among rGBM with different volume proportions of TDEC types and correlations of rGBM with different volume proportions of TDEC types with rGBM volume were analyzed in rGBM after bevacizumab (BEV) combined with radiotherapy. A pixel co-localization decoupling method was applied to assess the treatment response efficiency in individual TDEC subcomponents. The rGBM imaging phenotypes were identified through unsupervised clustering analysis, and progression-free survival (PFS) and overall survival (OS) between patients with different phenotypes were compared. The predictive value of TDEC heterogeneity in PFS and OS of rGBM patients under BEV plus radiotherapy was assessed. Results:Four distinct TDEC sub-regions (TDEC1-4) were identified; a significant negative correlation was observed between volume proportions of TDEC2 and TDEC3 ( r s=-0.558, P<0.001), as well as between volume proportions of TDEC3 and TDEC4 ( r s=-0.782, P<0.001), while TDEC composition (volume proportions of TDEC2-4) showed no significant correlation with tumor volume ( P>0.05). Following BEV combined with radiotherapy, significant sub-region-specific TDEC volume changes were observed (tumor volume minification rate of TDEC1[ΔV TDEC1]: 16.7% [13.8%, 20.1%]; ΔV TDEC2: 25.4% [21.9%, 29.0%]; ΔV TDEC3: 27.6% [23.5%, 31.2%]; ΔV TDEC4: 8.4% [6.1%, 10.7%], P<0.05); volume proportion of TDEC3 was positively correlated with tumor volume minification ( r s=0.702, P<0.001), whereas volume proportion of TDEC4 was negatively correlated tumor volume minification ( r s=-0.933, P<0.001). The volume reduction of TDEC1-3 was driven by combined effects of tumor cellular and enhancement components, while volume reduction of TDEC4 was primarily attributed to changes in tumor cellularity (ΔV ADC: 9.3%; ΔV T1C: 0.8%). Two distinct TDEC phenotypes with different survival outcomes were identified in rGBM patients (silhouette coefficient=0.584; TDEC type I: n=23; type II: n=43); significant difference in PFS and OS was noted between patients with TDEC type I and type II (PFS: χ2=11.191, P=0.001; OS: χ2=9.733, P=0.002). TDEC phenotype was an independent influencing factor for survival of rGBM patients under BEV combined with radiotherapy (PFS: HR=2.738, 95% CI: 1.815-3.938 , P=0.003; OS: HR=2.507, 95% CI: 1.851-3.660, P=0.007). Conclusion:TDEC sub-region helps efficiently characterize the rGBM heterogeneity; rGBM imaging phenotypes identified based on TDEC sub-region can independently predict the clinical outcomes: the prognosis of TDEC type I patients is better than that of TDEC type II patients.
2.Relationship between the serum Flt3L,PGRN levels and the disease risk and disease outcome of patients with acute lymphoblastic leukemia

Objective To investigate the relationship between the serum FMS-like tyrosine kinase 3 ligand(Flt3L),progranulin(PGRN)levels and the disease risk and disease outcome of patients with acute lympho-blastic leukemia(ALL).Methods A total of 104 patients with ALL admitted to the hospital from September 2019 to September 2021 were selected as the research subjects.ALL patients were divided into the low-risk group(n=34),the medium-risk group(n=39),and the high-risk group(n=31)according to the disease risk.The levels of serum Flt3L and PGRN of the patients at admission were detected by enzyme-linked immu-nosorbent assay.Pearson correlation analysis was used to analyze the relationship between serum Flt3L,PGRN in ALL patients and the risk of ALL disease.According to the follow-up results of ALL patients,they were divided into the good prognosis group(n=81)and the poor prognosis group(n=23).The receiver oper-ating characteristic curve and the area under the curve(AUC)were used to analyze the evaluation value of se-rum Flt3L and PGRN for the prognosis of ALL patients,and multivariate Cox regression was used to analyze the prognostic risk factors of ALL patients.Results The serum Flt3L levels in the low-risk group and the medium-risk group were higher than those in the high-risk group,and the difference was statistically signifi-cant(P<0.05).The serum PGRN levels in the low-risk group and the medium-risk group were lower than those in the high-risk group,and the difference was statistically significant(P<0.05).Serum Flt3L in ALL patients was negatively correlated with the risk of ALL disease(r=-0.461,0.593,P<0.05).Serum PGRN in ALL patients was positively correlated with the risk of ALL disease(r=0.593,P<0.05).The proportions of white blood cell count ≥50 × 109/L,hemoglobin<90 g/L,and serum PGRN level in the poor prognosis group were higher than those in the good prognosis group,while the serum Flt3L level was lower than that in the good prognosis group,and the differences were statistically significant(P<0.05).The AUC of serum Flt3L and PGRN in evaluating the prognosis of ALL patients were 0.762(95%CI:0.717-0.816),0.815(95%CI:0.764-0.863),and 0.915(95%CI:0.866-0.964),respectively.White blood cell count ≥50 × 109/L,hemoglobin<90 g/L,Flt3L<92.07 pg/mL,and PGRN≥335.14 pg/mL were risk factors affecting the prognosis of ALL patients(P<0.05).Conclusion The levels of serum Flt3L and PGRN in ALL patients are related to the disease risk and disease outcome of ALL.The combined detection of the two has a good eval-uation value for the prognosis of adult ALL patients.
3.Research on Cause and Prevention Strategies of 1 876 Medical Damage Dispute Cases

Objective To analyze the causes and compensation characteristics of medical damage disputes in Anhui Province in recent years,and provide reference suggestions for effectively controlling and reducing the incidence of medical errors and doctor-patient conflicts.Methods The 1 876 cases of medical damage disputes in Anhui Province from 2017 to 2022 were included in the judgment documents network,and their occurrence years and causes of disputes were analyzed.Results The number of medical malpractice cases did not decrease significantly,and the average amount of compensation continued to increase;the top three causes of disputes were improper treatment or surgery(54.6%),and the average first place of actual compensation was improper medication or adverse drug reactions,and in terms of hospital level,1 009 cases(53.8%)were concentrated in tertiary public hospitals,the average actual compensation amount of private hospitals ranked first;the top three departments were surgery(44.62%);the highest incidence of damage was death(40.2%),and the average compensation amount for first-class disability was the highest.Hospitals were mainly responsible for secondary responsibilities(38.7%).Conclusion The contradiction between doctors and patients is still serious,so it is necessary to promote the construction of tight medical association and medical community,guide the allocation of medical resources,pay attention to risk sharing and management optimization,and reduce the incidence of medical damage disputes.
4.Saponins from Panax japonicus ameliorate high-fat diet-induced anxiety by modulating FGF21 resistance.

Anxiety disorder is a highly prevalent psychological illness, and research has shown that obesity is a significant risk factor for its development. This study explored the ameliorative effects and mechanisms of saponins from Panax japonicus(SPJ) on anxiety disorder in mice fed a high-fat diet(HFD). Fifty C57BL/6J mice were randomly divided into normal control diet(NCD) group, HFD group, and low-and high-dose SPJ groups. At week 12, six mice from the HFD group were further divided into a control group(treated with DMSO) and an exogenous fibroblast growth factor 21(FGF21) group(administered rFGF21). The anxiety-like behavior of the mice was assessed using the open field test and elevated plus maze test. Hematoxylin-eosin(HE) staining and oil red O staining were performed to observe pathological changes in the liver and adipose tissue. Glucose metabolism was evaluated through the glucose tolerance test(GTT) and insulin tolerance test(ITT). Western blot analysis was performed to detect the expression of FGF21 and its downstream-related proteins in the liver and cortex, along with the expression of brain-derived neurotrophic factor(BDNF), disks large homolog 4(DLG4), and synaptophysin(SYP) in the cortex. Real-time quantitative fluorescent PCR(qPCR) was used to detect the expression of FGF21 and its receptor genes in the liver and cortex. Immunofluorescence staining was employed to examine the expression of neuronal activator c-Fos, FGF21, and the FGF21 co-receptor β-klotho in the cerebral cortex. The results showed that SPJ significantly improved the frequency of activity in the open arms of the elevated plus maze and the central area of the open field in HFD mice, up-regulated the expression of BDNF, DLG4, and SYP, and effectively alleviated anxiety-like behaviors in HFD mice. Compared with the NCD group, HFD mice exhibited up-regulated expression of FGF21 in the liver and cerebral cortex, while the expression of fibroblast growth factor receptor 1(FGFR1) and β-klotho was significantly down-regulated, suggesting that HFD mice exhibited FGF21 resistance. SPJ markedly up-regulated the β-klotho levels in HFD mice, reversing FGF21 resistance. Further comparison with exogenously administered FGF21 revealed that SPJ activates brain cortical regions in a consistent manner, and additionally, SPJ promotes the number and colocalization of c-Fos and β-klotho positive cells in the brain cortex. In summary, SPJ effectively alleviates anxiety-like behaviors in HFD mice. Its mechanism is associated with up-regulation of β-klotho expression in the brain, reversal of FGF21 resistance, and subsequent activation of neurons in the cerebral cortex and amygdala.

9.Mid-to-long term fate of neo-aortic root after arterial switch operation for Taussig-Bing anomaly: A retrospective study in a single center

Objective To explore growth pattern of neo-aortic root as well as development of neo-aortic regurgitation after arterial switch operation (ASO) for Taussig-Bing anomaly. Methods From 2002 to 2017, the patients who received ASO, and were discharged alive from Shanghai Children’s Medical Center and followed up for more than 3 years were retrospectively involved in this study. Results A total of 127 patients were enrolled. There were 98 (77.2%) males, the median age at ASO was 73.0 d and the average weight was 4.7 kg. Forty-five (35.4%) children were complicated with mild or mild-to-moderate pulmonary insufficiency (PI) before ASO. The average follow-up time was 7.0 years. During the follow-up, 14 (11.0%) children presented moderate or greater neo-aortic regurgitation (neo-AR). The diameter of neo-aortic annulus and sinus of Valsalva was beyond normal range during the entire follow-up. The average diameter of neo-aortic annulus was 18.0 mm at 5 years and 20.5 mm at 10 years. The average diameter of sinus of Valsalva was 25.9 mm at 5 years and 31.1 mm at 10 years. Neo-AR continued to develop over time. The diameter of children who developed moderate or greater neo-AR was constantly larger than that of children who did not (χ2=18.3, P<0.001). Preoperative mild or mild-to-moderate PI was an independent risk factor for the development of moderate or greater neo-AR during mid-to-long term follow-up (c-HR=3.46, P=0.03). Conclusion The diameters of neo-aortic annulus and sinus of Valsalva of Taussig-Bing children who receive ASO repair continue to expand without normalization. The dilation of annulus correlates with the development of neo-AR. PI before ASO repair increases the risk of neo-AR development.

The treatment of inner ear disease is developing towards local drug delivery to avoid drawbacks of systemic approach.The cochlear implants with drug delivery functions,a newly developed method of drug delivery into inner ear for treatment,has become the focus of research in recent years.In this review,we will describe recent advances in characteristics,product developments and applications of different types of drug loaded cochlear implants for local therapy.
