1.Correlation between severity of knee joint osteoarthritis and alignment of patellofemoral and patellar height on radiographs.

BACKGROUND:
The correlation between the morphological structure of the patellofemoral joint (PFJ) and the severity of knee joint osteoarthritis (KOA) remains uncertain. This study aims to investigate the correlation between the severity of knee joint osteoarthritis and the alignment of patellofemoral and patellar height on radiographs.
METHODS:
This multi-center, retrospective study analyzed the magnetic resonance imaging (MRI) scans and anteroposterior radiographs of 534 adult outpatients with KOA. To evaluate the radiographic severity of KOA, anteroposterior radiographs of the knee and the Kellgren-Lawrence (K-L) grade were used. Knee MRI scans were used to measure the patellar length ratio (PLR), sulcus angle (SA), lateral patellar tilt angle (LPTA), and the distance between tibial tuberosity and trochlear groove (TT-TG). We examined the association between the configuration of the PFJ, arrangement, and harshness of the KOA. Information on participants' demographics, such as age, sex, side, height, and weight, was collected. A chi-squared test was used for the correlation of radiographic severity of KOA with sex and the affected side. Spearman correlation was used for patellofemoral alignment or morphology and the radiographic severity of lateral KOA. Multiple linear regression models were used for the association between LPTA, SA, TT-TG, and severity of KOA after accounting for demographic variables.
RESULTS:
The study comprised of 534 patients; of these, 339 (63%) were female. A total of 586 knees were evaluated in this study. Age showed a strong positive correlation with KOA severity ( r = 0.516, P <0.01), whereas LPTA showed a strong negative correlation ( r = -0.662, P <0.01). Additionally, SA ( r = 0.616, P <0.05), and TT-TG showed a strong positive correlation ( r = 0.770, P <0.01) with tibiofemoral osteoarthritis (TFOA) severity. Multiple linear regression analysis indicated that knee osteoarthritis severity (β = -2.946, P <0.001) and side (β = -0.839, P = 0.001) was associated with LPTA; knee osteoarthritis severity (β = 5.032, P <0.001) and age (β = -0.095, P <0.001) was associated with SA; knee osteoarthritis severity (β = 2.445, P <0.001), sex (β = -0.326, P = 0.041), body mass index (β = -0.061, P = 0.017) and age (β = -0.025, P <0.001) was associated with TT-TG.
CONCLUSION
Radiographic severity of KOA was positively associated with age, SA, and TT-TG but negatively associated with LPTA.

2.Predictive value of preoperative C-reactive protein to albumin ratio for microvascular invasion and early postoperative recurrence of hepatocellular carcinoma

Objective:To investigate the value of preoperative C-reactive protein to albumin ratio (CAR) in predicting microvascular invasion (MVI) and early postoperative recurrence of hepatocellular carcinoma(HCC).Methods:A retrospective cross-sectional study was conducted to analyze the clinical data of 190 patients with hepatocellular carcinoma who underwent radical resection in the First Affiliated Hospital of Anhui University of Science and Technology and the First Affiliated Hospital of Anhui Medical University from January 2020 to April 2022. The patients were divided into MVI group ( n=57) and non-MVI group ( n=133) according to whether there was microvascular invasion in postoperative pathology. The systemic inflammatory response index (SIRI), systemic immunoinflammatory index(SII), fibrinogen to albumin ratio (FAR) and CAR were calculated. 190 patients were followed up for 2 years after surgery. 86 patients had a recurrence, of whom 45 were in the MVI group and 41 were in the non-MVI group. SIRI, SII, FAR and CAR of the two groups were observed. And the value of SIRI, SII, FAR and CAR in predicting MVI and early postoperative recurrence was determined by receiver operating characteristic(ROC) curve. Univariate and multifactorial analysis of MVI risk factors in HCC. Quantitative data data consistent with normal distribution were represented by ( ± s), and independent sample t test was used for comparison between groups. Quantitative data that did not conform to normal distribution were represented by quartile M( Q1, Q3), and comparison between groups was performed by Mann-Whitney U test. Qualitative data were compared using chi-square test. Multivariate Logistic regression analysis was used for multivariate analysis. Results:There were 86 patients with early recurrence after operation, the recurrence rate was 45.26%. Among them, 45 cases (78.95%) recurred in MVI group and 41 cases (30.83%) in non-MVI group. There were 9 deaths (15.79%) in MVI group and 5 deaths (3.76%) in non-MVI group. There were differences in recurrence and death between MVI group and non-MVI group ( P<0.05). CAR was better than SIRI, SII and FAR in predicting MVI of hepatocellular carcinoma ( P<0.05). The area under ROC curve (AUC), sensitivity and specificity of CAR were 0.651, 70.20% and 60.20%, respectively, and the best predictive value was 0.01. CAR was an independent risk factor for MVI ( OR=1.068, 95% CI: 0.819-1.393, P<0.05). CAR was better than SIRI, SII and FAR in predicting early postoperative recurrence ( P<0.05). AUC, sensitivity and specificity of CAR were 0.641, 55.40% and 73.80%, and the predicted value was 0.01. Conclusion:Preoperative CAR is a risk factor for MVI in HCC, which can predict the occurrence of MVI and early recurrence after operation.
4.Significance of endoscopic exposure of circummammary ligament under endoscopy and the effect of membrane anatomy in breast reconstruction

Objective:To explore the significance of exposing the circummammary ligament under endoscopy and the effect of membrane anatomy in breast reconstruction.Methods:The case data of 49 breast cancer patients who underwent endoscopic nipple-sparing mastectomy combined with one-stage breast reconstruction with prosthesis implantation in Beijing Friendship Hospital, Capital Medical University from February 2014 to December 2021 were retrospectively analyzed, there were 44 cases of posterior pectoralis prosthesis implantation, 5 cases of anterior pectoralis prosthesis implantation. The anatomical structure of the circummammary ligament was observed under endoscopy during operation, and the annular mammary ligament was used as an anatomical marker to complete subcutaneous glandectomy and prosthesis implantation for breast reconstruction, the BREAST-Q scales were used to evaluate the postoperative effect.Results:The medial sternal ligament, sub clavicular ligament, lateral confluence ligament and triangular ligament condensation could be clearly exposed in all 49 cases. Breast reconstruction module of BREAST-Q were used to evaluate the surgery effect after breast cancer surgery, the scores of postoperative breast satisfaction, chest wall status, psychosocial status and sexual health status were 81.43±12.57, 88.39±10.61, 88.04±13.70, 74.82±15.93.Conclusion:The endoscopic technique is beneficial to expose the circummammary ligament during operation, and surgical resection and reconstruction can better restore the appearance of the breast and improve postoperative satisfaction according to the principle of membrane anatomy.
5.Determination of Sildenafil Citrate Illegally Added in Kidney-yang Reinforcing Chinese Medicinal Tonics by RP-HPLC

OBJECTIVE: To determine the content of Sildenafil citrate illegally added in kidney-yang reinforcing Chinese medicinal tonics. METHODS: The samples were separated on ODS C18(250 mm?4.6 mm,5 ?m) with mobile phase consisted of methanol-water-acetic acid glacial-triethylamine(60∶40∶0.8∶0.3) at a flow rate of 0.8 mL?min-1.The UV detective wavelength was 290 nm and the sample size was 10 ?L. RESULTS: At a concentration range of 10~100 ?g?mL-1, Sildenafil citrate showed a good linear relation with peak area score. The mean recovery rate was 100.52%(RSD=1.20%). CONCLU-SION:The method can be used to determine Sildenafil Citrate illegally added in kidney-yang tonics.
