1.Randomized controlled trial of recombinant human interferon α2b combined with Baofukang suppository and vaginal Lactobacillus for the treatment of cervical HR-HPV infection in postmenopausal women

ObjectiveTo explore the clinical efficacy and safety of a combined therapy by using recombinant human interferon α2b, Baofukang suppository and vaginal lactobacillus for high-risk human papillomavirus (HR-HPV) infection in postmenopausal women, and to provide a reference for the treatment of HR-HPV infection. MethodsThis study was a single-center, prospective, randomized, open-label, and parallel-controlled trial. A total of 210 postmenopausal women with cervical HR-HPV infection who visited the Maternal and Child Health Care and Family Planning Service Center of Deqing County from January 2022 to December 2023 were consecutively selected. They were randomly divided into study group (112 cases) and control group (98 cases) using a random number table. The control group was treated with recombinant human interferon α2b vaginal effervescent capsules and Baofukang suppositories. The study group received vaginal lactobacillus capsules in addition to the treatment given to the control group. The primary outcome measure was the HPV clearance rate at 6 and 12 months after treatment. The secondary outcomes included the rate of cervical cytology improvement, symptom relief rate, vaginal microecological indicators (vaginal pH value, proportion of lactobacillus), and the incidence of adverse reactions. ResultsA total of 196 patients (112 in the study group and 84 in the control group) completed the 12-month follow-up. At 6 months after treatment, there was no significant difference in the negative rate of HR-HPV between the two groups (χ²=3.36, P=0.072). At 12 months after treatment , the negative rate of HPV in the study group (78.57%, 88/112) was higher than that in the control group (52.38%, 44/84) (χ²=3.98, P=0.046). At 6 months after treatment, there was no significant difference in the improvement rate of cervical cytology between the two groups (χ²=0.12, P=0.063). At 12 months after treatment, the improvement rate of cervical cytology in the study group (73.21%, 82/112) was higher than that in the control group (47.62%, 40/84) (χ²=4.26,P=0.039).In terms of vaginal microecology, at 6 months after treatment, the median vaginal pH value of the study group [4.17 (4.03,4.26)] was lower than that of the control group [4.60 (4.31,4.85)] (U=386.52, P=0.004), and the average proportion of lactic acid bacteria [(82.36±5.12)%] was higher than that of the control group [(68.57±6.34)%] (t=7.56, P=0.002). At 12 months after treatment, the median vaginal pH value of the study group [4.04 (3.81,4.12)] was lower than that of the control group [4.06 (3.83,4.19)] (U=352.04, P=0.006), and the average proportion of lactic acid bacteria [(91.25±4.36)%] was higher than that of the control group [(78.63±5.72)%] (t=6.73, P=0.001). At 6 months after treatment, the remission rate of symptoms in the study group (64.29%, 72/112) was higher than that in the control group (40.48%, 34/84) (χ²=10.957, P=0.001). At 12 months after treatment, the remission rate of symptoms in the study group (87.50%, 98/112) was higher than that in the control group (64.29%, 54/84) (χ²=56.81, P<0.001). Regarding side effects, during the treatment period and within one month after the completion of treatment, only three types of mild local side effects, including increased vaginal discharge, vaginal burning sensation, and vulvar pruritus, were observed in both groups. All adverse reactions were alleviated after symptomatic management (such as perineal cleaning and adjusting medication posture). The total incidence of adverse reactions in the study group was 6.25%, compared to 7.14% in the control group, with no statistically significant differences between the two groups (χ²=0.09, P=0.768). ConclusionThe combined treatment of recombinant human interferon α2b vaginal effervescent capsules, Baofukang suppositories, and vaginal lactobacillus capsules in postmenopausal women with cervical HR-HPV infection could effectively increase the HPV clearance rate, improve cervical cytology, restore vaginal microbiota balance, and relieve clinical symptoms.
2.Overweight Modified the Associations between Long-Term Exposure to Ambient Fine Particulate Matter and Its Constituent and the Risk of Type 2 Diabetes in Rural China.

OBJECTIVE:
To investigate the association between long-term exposure to ambient fine particulate matter (PM 2.5) and its constituents and the risk of incident type 2 diabetes mellitus (T2DM), and to examine the modification roles of overweight status.
METHODS:
This prospective study included 27,507 adults living in rural China. The annual mean residential exposure to PM 2.5 and its constituents was estimated using a satellite-based statistical model. Cox models were used to estimate the risk of T2DM associated with PM 2.5 and its constituents. Stratified analysis quantified the role of overweight status in the association between PM 2.5 constituents and T2DM.
RESULTS:
Over a median follow-up of 9.4 years, 3,001 new T2DM cases were identified. The hazard ratio ( HR) for a 10 μg/m 3 increase in ambient PM 2.5 was 1.30 (95% confidence interval [ CI]: 1.17, 1.45). Among the constituents, the strongest association was observed with black carbon. Being overweight significantly modified the association between certain constituents and the risk of T2DM. Participants who were overweight and exposed to the highest quartile of PM 2.5 constituents had the highest risk of T2DM ( HR: 2.46, 95% CI: 2.04, 2.97).
CONCLUSIONS
Our findings indicate that PM 2.5 was associated with an increased risk of T2DM, with black carbon potentially being the primary contributor. Being overweight appeared to enhance the association between PM 2.5 and T2DM. This suggests that controlling both PM 2.5 exposure and overweight status may reduce the burden of T2DM.

3.Efficacy assessment of an intelligent blood transfusion system in intraoperative red blood cell transfusion

Objective: To evaluate the long-term effectiveness of an intelligent blood transfusion system in intraoperative blood management by comparing its performance with clinicians' decisions. Methods: A retrospective analysis of 26 760 surgical cases (2017-2024) was conducted, comparing pre- and post-implementation (2017-2019 vs 2020-2024) metrics, including transfusion prediction accuracy, rationality of blood use, and clinical outcomes. The system, powered by XGBoost, integrated patient demographics, laboratory results, and surgical data to predict red blood cell transfusion needs. Results: The intelligent blood transfusion systems achieved an accuracy of 80.62% in predicting transfusion necessity, significantly outperforming clinicians (24.83%, P<0.001). Its blood-use rationality rate was 83.92% vs 18.02% for clinicians (P<0.001). Post-implementation, major surgeries (grades Ⅲ-Ⅳ) increased while the requested blood units decreased. High physician compliance (>75%) correlated with 88.18% rationality. Conclusion: The intelligent blood transfusion system significantly improves the accuracy of transfusion decision-making, reduces excessive red blood cell use, optimizes perioperative transfusion management, and enhances the utilization of blood medical resources.
4.Feasibility and safety of pressurized intraperitoneal aerosol chemotherapy (PIPAC) in patients with peritoneal metastatic gastrointestinal cancer: a preliminary report

Objective:This study aimed to summarize and analyze the preliminary application experience of a novel pressurized intraperitoneal aerosol chemotherapy (PIPAC) device in patients with peritoneal metastases of gastrointestinal malignancies.Methods:In this descriptive case series study, four patients with pathologically confirmed peritoneal metastatic gastrointestinal malignancies were enrolled, receiving PIPAC treatment at Guangdong Provincial People's Hospital from December 2024 to February 2025. The PIPAC treatment was performed five times on these patients . Key procedural steps included equipment preparation, operation platform construction, cytologic examination of ascites, Peritoneal Cancer Index (PCI) scoring, biopsy of peritoneal metastases, drug preparation, connection and testing of the PIPAC device, PIPAC treatment, and aerosol evacuation. Preoperative and postoperative clinical data were collected and analyzed to assess the safety and feasibility of PIPAC treatment.Results:3 males and 1 female patients were enrolled; median age was 57 (range, 36-70) years old; median body mass index was 22.5 (range, 18.0-24.6) kg/m2, and the preoperative Eastern Cooperative Oncology Group (ECOG) score was 1 for all cases. Four patients successfully completed the five PIPAC treatments without any intraoperative adverse events. The PIPAC treatment time ranged from 34 minutes to 36 minutes, with a median preoperative PCI score of 18 (range, 5-25). The average Peritoneal Regression Grading Score (PRGS) before the first and second PIPAC treatments were 2.1±0.8 and 1.7±0.6, respectively. The median Visual Analog Scale (VAS) scores for pain on postoperative days 1, 2, and 3 were 2 (range, 2-4), 1 (range, 0-2), and 1 (range, 0-2), respectively. All patients resumed the oral intake and ambulation on the first postoperative day, with a postoperative hospital stay of 3 days. No postoperative complications or perioperative death occurred.Conclusion:The preliminary study results indicate that the use of the novel PIPAC device for the treatment of peritoneal metastases of gastrointestinal malignant tumors is safe and feasible.
5.Study on the Mechanism of Regulating miR-532-3p in Improving Adriamycin Cardiotoxicity with Yiqi Huayu Decoction

Objective To explore the mechanism of regulating miR-532-3p with Yiqi Huayu Decoction to improve adriamycin cardiotoxicity.Methods Totally 25 rats were randomly divided into control group,model group and Yiqi Huayu Decoction low-,medium-and high-dosage groups,with 5 rats in each group.Myocardial injury model was established by intraperitoneal injection of adriamycin 2.5 mg/kg once a week for 8 weeks in model group and Yiqi Huayu Decoction low-,medium-and high-dosage groups.At the same time,Yiqi Huayu Decoction low-,medium-and high-dosage groups were given 2,4,8 g/kg Yiqi Huayu Decoction by gavage respectively,once a day for consecutive 8 weeks.Cardiac function of rats was detected by echocardiography,serum NT-proBNP content was detected by ELISA,morphology of myocardial tissue were observed by HE staining,apoptosis of myocardial tissue was detected by TUNEL staining,miR-532-3p gene expression was detected by RT-qPCR,Western blot was used to detect the expressions of GATA4 and CARP protein.Results Compared with the control group,the left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD)of the model group increased significantly,the left ventricular ejection fraction(LVEF)and left ventricular fractional shortening(LVFS)significantly decreased(P<0.05),and the serum NT-proBNP content significantly increased(P<0.05),with severe hypertrophy and disordered arrangement of myocardial cells,fibroblast proliferation accompanied by necrosis,and significantly increased apoptosis rate of myocardial cells(P<0.05),the expression of miR-532-3p gene in myocardial tissue significantly increased(P<0.05),and the expressions of GATA4 and CARP proteins in myocardial tissue significantly increased(P<0.05).Compared with the model group,Yiqi Huayu Decoction high-dosage group showed a significant decrease in LVESD and LVEDD(P<0.05),with LVEF and LVFS significantly increased(P<0.05),the content of serum NT-proBNP in Yiqi Huayu Decoction low-,medium-and high-dosage groups were significantly decreased(P<0.05),and myocardial cell damage and fibrosis were improved to varying degrees,the apoptosis rate of myocardial cells was significantly decreased(P<0.05),the expression of miR-532-3p gene in myocardial tissue decreased(P<0.05),and the expressions of GATA4 and CARP proteins in myocardial tissue decreased(P<0.05).Conclusion Yiqi Huayu Decoction can antagonize doxorubicin cardiotoxicity,and its possible mechanism may be related to regulating miR-532-3p to inhibit myocardial fibrosis.

8.Serological evaluation and antibody prediction model for inactivated COVID-19 vaccination in school children

ObjectiveTo determine the serum antibody level and risk factors in the adolescent population in a county in Zhejiang Province, following the immunization with inactivated COVID-19 vaccine, and to construct a prediction model for antibody concentration. MethodsWe conducted the study in a county in Zhejiang Province, employing a stratified cluster random sampling strategy in school children who had received the inactivated COVID-19 vaccine. Data on gender, age, type of vaccine, and time of vaccination was collected. Serum samples were also collected to test for anti-S and N IgG antibody against the SARS-CoV-2 by using chemiluminescent immunoassay (CLIA). Risk factors were determined to construct a prediction model for antibody concentration. ResultsThe IgG antibody concentration was significantly higher in girls, those who received two doses, and those who had simply received the KX vaccine . It decreased with age and time interval between the sampling and last vaccination. The prediction model constructed by random forest regression in the study had a better model fit and predictive ability than that by the multivariable linear stepwise regression. ConclusionGender, age, vaccination dose, type of vaccine, and time of vaccination are associated with vaccination effectiveness of inactivated COVID-19 vaccines in adolescents. Prediction model could predict the antibody level in the vaccinated population, which can provide a new tool for better evaluation of vaccination effectiveness against emerging infectious diseases in future.
9.Trend of antibiotics usage among outpatients in primary public medical institutions in a county of Zhejiang Province, from 2015 to 2020

ObjectiveTo understand the usage of antibiotics in outpatients in a county of Zhejiang Province from 2015 to 2020, so as to provide a basis for further standardizing the clinical application of antibiotics in this region. MethodsOutpatient records and prescription records of 146 public medical institutions from 2015 to 2020 were extracted from the electronic medical record data sharing platform system of medical institutions in a county of Zhejiang Province. The utilization rates and the number of types of antibiotics used in outpatient patients were described. According to the drug anatomy, therapeutic and chemical classification system (ATC), clinical application classification and dosage form, the use status of different types of antibiotics was observed. The annual percentage change (APC) was used to analyze the time trend of antibiotic use. Among the antibiotic prescriptions in each year, the proportions of prescriptions aimed for different use reasons and patients were compared to analyze the changes in the structures of antibiotic prescriptions. ResultsDuring 2015‒2020, a total of 2 861 438 prescriptions were issued in146 primary public medical institutions in this county, including 314 642 prescriptions for antibiotics, and the overall utilization rate of antibiotics was 11%. The utilization rate of antibiotics showed a decreased trend across the years (from 14.23% in 2015 to 7.59% in 2020,APC=-11.51%, 95%CI=-0.01%‒-21.7%, t=-12.05, P<0.001) and an obvious seasonal variation character (higher in winter and spring, lower in summer and autumn). Cefoxitin sodium for injection was the most used antibiotic in 2015, and cefuroxime was the most used antibiotic from 2015 to 2020, respectively. The utilization rates of cephalosporins (APC=-11.06%, 95%CI=0‒-20.89%, t=-22.233, P<0.001), quinolones (APC=-9.74%, 95%CI=0‒-18.53%, t=-7.652, P=0.002), macrolides (APC=-17.52%, 95%CI=0‒-17.52%, t=-3.626, P=0.022) and lincoamides (APC=-49.01%, 95%CI=0‒-74%, t=-3.35, P=0.029) showed a decreasing trend across the years. There were differences in the distribution of drug use reasons (χ2=9 458.427, P<0.001), as well as the age (χ2=5 421.709, P<0.001) and sex (χ2=245.322, P<0.001) of patients. During the observation period, 50.69% of antibiotic prescriptions were used to treat respiratory diseases, with the highest proportion of 57.67% in 2015 and the lowest proportion of 34.93% in 2020. 58.37% of patients were aged 50‒70 years, with a decreasing proportion of patients under 50 years (17.83% in 2015, 6.64% in 2020) and an increasing proportion of patients over 70 years (24.1% in 2015, 38.41% in 2020). ConclusionThe utilization rate of antibiotics used in outpatients from primary public medical institutions in a county of Zhejiang Province shows a decreasing trend across the years, but higher than that of secondary and above hospitals in China, and also higher than that of primary medical institutions in economically-developed areas. The management and monitoring of the clinical use of antibiotics in the county should be actively strengthened.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
