1.The logical framework of virtual labs of teaching research and reform and innovative exploration: learning the practical experience of virtual teaching labs in medical education research

In the era of informatization, virtual teaching labs have emerged as a pivotal exploration in the construction of new teaching organizations at the primary level. Virtual labs of teaching research and reform require faculty members to collaboratively conduct teaching reform research and practice, thereby fostering new paradigms and achievements for teaching reforms. It serves as a vital foundation for supporting comprehensive reforms in curriculum teaching and specialty construction. This paper delves into the logical framework of virtual teaching research and reform labs from four dimensions (value orientation, construction principles, organizational structure, and operational safeguards), and elaborates on lab construction pathways from four aspects (technological platforms, behavioral relationships, knowledge management, and spiritual culture). Taking virtual teaching labs for medical education as an example, the paper summarizes practical experience in the innovative exploration of reform-oriented virtual teaching labs. Finally, we discuss the most constraining factors in the current construction of virtual teaching labs, and propose that research on virtual teaching labs should pay more attention to solving practical problems in major and curriculum reforms to promote the integration of medical education and practice, ultimately contributing to the realization of the Healthy China initiative.
2.A study on the relationship between occupational fitness, moral dilemma and resilience of nurses in emergency department

Objective:To explore the correlation between nurses′ workplace adaptability, moral dilemma and resilience after suffering workplace violence in emergency department, in order to provide reference about improving the workplace adaptability and formulating intervention.Methods:A cross-section survey was conducted, we selected 248 nurses who had suffered workplace violence in the emergency department of five tertiary hospitals in Qingdao from January 2022 to January 2023 by convenient sampling method, and used self-designed general information questionnaire, moral dilemma scale, resilience evaluation scale for medical staff and nurses′ workplace adaptability scale to conduct a questionnaire survey on nurses who had suffered workplace violence, and found out the factors affecting their workplace adaptability.Results:The score of workplace adaptability of 248 nurses who had suffered workplace violence was (39.37 ± 12.77) points. Univariate analysis showed that education, length of emergency service, department or social part-time job, professional title, number of night shifts and sleep condition were the influencing factors of workplace fitness for emergency nurses ( t/ F values were -12.72 to 132.15, all P<0.05). The correlation analysis showed that the nurses′ workplace adaptability after workplace violence was negatively correlated with the scores and total scores of all dimensions of moral dilemma ( r values were -0.725 to -0.194, P<0.05), and positively correlated with the scores and total scores of all dimensions of resilience ( r values were 0.135 to 0.732, P<0.05). Multi-factor results showed that education background, department/social part-time job, working years, professional title, sleep status, moral dilemma and resilience were the influencing factors of nurses′ workplace adaptability after workplace violence ( t values were -9.25 to 15.25, P<0.05). Conclusions:Nurses with specialized education, emergency service for 1-3 years, no department/social part-time job, primary professional title, sleep disorder, higher degree of moral dilemma and low level of resilience have low occupational adaptability. Nursing managers should focus on such nurses and improve their occupational adaptability.
3.Field research on work experience and service quality of "Internet plus nursing service" led by tertiary hospitals

Objective:To understand the work experience and service quality of "Internet plus Nursing Service" led by tertiary hospitals to lay the foundation for building an "Internet plus Nursing Service" quality control index system suitable for China's national conditions.Methods:Using field research method, combining purposive sampling with open sampling, from September 2021 to February 2022, the 11 "Internet plus Nursing Service" visiting nurses from 10 residential areas in Shibei District, Shinan District, Laoshan District, Chengyang District and Jimo District of Qingdao were observed for 5 months.Results:The work experience of "Internet plus Nursing Service" visiting nurses: meeting the rigid needs of society and patients, and nurses′ professional value and satisfaction were multiplied; there were still shortcomings in the regulatory mechanism, and information technology construction needed to be continuously strengthened; there were still hidden dangers in patient safety and nurse professional safety issues. Six key factors influencing the quality of Internet plus nursing service were analyzed: non-standard preparation of articles before visiting, non-standard dress for door-to-door service, inadequate preparation for pre operation evaluation, lack of empathy during operation, weak awareness of infection prevention outside the hospital, and failure to carry out health education based on patients′ conditions after operation.Conclusions:The Internet plus nursing service model is dominated by the tertiary hospitals in Qingdao, which has realized two-way referral between the community and the superior hospitals, and has effectively improved the quality of extended care. The acceptance and satisfaction of patients and their families are high, and nurses′work enthusiasm and professional value have increased significantly. It is still necessary to establish and improve theInternet plus nursing service work standards to achieve the homogenization and diversification of nursing services, promote the comprehensive quality of community medical staff.
4.Effective analysis of non-surgical combined treatments for laryngeal organ preservation in locally advanced laryngeal carcinoma.

OBJECTIVETo evaluate the therapeutic effects, larynx preservation and adverse events of non-surgical combined treatments for laryngeal organ preservation in locally advanced laryngeal squamous cell carcinomas(SCCs).
METHODSForty-six patients with locally advanced laryngeal carcinoma (T2-4, N0-N3) were treated individually with non-surgical combined treatments for larynx preservation (LP). These treatments included concurrent chemoradiotherapy (CCRT)(±epidermal growth factor receptor (EGFR) inhibitor), induction chemotherapy (ICT) followed by CCRT(± EGFR inhibitor), or concurrent radiotherapy and EGFR inhibitor. Radiation therapy was given to a total dose of 60-70 Gy. The Kaplan-Meier method was used to determine the overall survival. Side-effects were evaluated with the established Common Terminology Criteria for Adverse Events (CTCAE) version 3.0 criteria.
RESULTSThe average follow-up time was 31.8 months (range 6-95 months). All patients completed the planned radiotherapy without treatment breaks, and 45(97.8%) of 46 patients completed the planned chemotherapy.The 3-year and 5-year overall survival rates were 87.3%and 67.2%, respectively.The 5-year larynx preservation rate was 100.0%. The 3-year and 5-year progression free survival rates were 95.1% and 87.7%, respectively. The most common acute side effect in grade 3 was oropharyngeal mucositis. After treatment, tracheotomy was still required in 2 patients with glottis cancer for laryngeal edema or stenosis. No patient depended on a percutaneous gastrostomy and experienced speech impairment.
CONCLUSIONPatients with locally advanced laryngeal cancer can be offered non-surgical combined treatments for laryngeal preservation and the high quality of life, showing a higher laryngeal preservation survival rate with minimal toxicities.

5.Clinical features of patients with head and neck malignant tumor concurrent with multiple primary carcinoma

Objective To analyze the clinical characteristics of multiple primary cancers (MPC)associated with head and neck.Methods A total of 71 patients with MPC associated with head and neck treated from January 2008 to November 2015 were included in the retrospective study,of them 20 patients were synchronous MPC and 51 patients were metachronous MPC;45 (63.4%) patients were field cancerization of head neck (FCHN) and 26 (36.6%) patients were non-field cancerization of head neck (NFCHN).For the synchronous MPC patients with an interval of 0 month,the lesion sites of FCHN were treated with combined chemotherapy and radiotherapy,the site of severe lesion of NFCHN received firstly a standard treatment.Other types of MPC were treated according to international guide lines or experts consensus.Results With the mean follow-up of 63.4 months,the 3-year and 5-year overall survival (OS)were 84.6% and 75.7% respectively.The 3-year and 5-year OS were 56.4% and 37.6% respectively in patients with synchronous MPC,and were 92.2% and 84.2% respectively in patients with metachronous MPC.There was significant difference in the OS between patients with metachronous MPC and patients with synchronous MPC (P =0.0002).The 3-year and 5-year OS were 85.7% and 77.9% respectively in patients with FCHN,and were 82.9% and 72.8% respectively in patients with NFCHN,with no significant difference between the two groups (P =0.297).Conclusion With correct diagnosis and effective curative treatment,some of patients with MPC can get long-term survival,showing the better prognosis in metachronous MPC compared to synchronous MPC.
6.Clinical characteristics and prognosis analysis of 50 patients with primary Waldeyer's ring non-Hodgkin lymphoma

Objective To compare the clinicopathological characteristics and prognosis of primary Waldeyer's ring non-Hodgkin lymphoma.Methods From Jan 2008 to Dcc 2013, 50 patients with primary Waldeyer's ring non-Hodgkin lymphoma were retrospectively analyzed.Kaplan-Meier method was used for survival analysis.Log-rank method was used for univariate analysis.Results Waldeyer's ring diffuse large B-cell lymphoma (DLBCL) and extranodal nasal-type NK/T-cell lymphoma (ENKTCL) were the common pathological types of primary Waldeyer's ring non-Hodgkin lymphoma accounting for 56 % (28/50) DLBCL and 34 % (17/50) ENKTCL, respectively.The age at onset of ENKTCL patients was younger than that of DLBCL patients.Two groups had different primary sites, including 50.0 % (14/28) of DLBCL with tonsillar disease and 88.2 % (15/17) of ENKTCL with nasopharynx disease.The short phase effect of DLBCL group was higher than that of ENKTCL group.The overall survival (OS) and progression-free survival (PFS) rates had significant differences between DLBCL and ENKTCL (x2 =4.45, P =0.035;x2=6.47, P =0.011).Conclusion DLBCL and ENKTCL are different in clinical characteristics.The short phase effect and prognosis of DLBCL are obviously better than those of the ENKTCL.
7.Synthesis and preliminary evaluation of 18F-AIF-NOTA-G-TMTP1 for PET imaging on nude mice bearing high-metastatic potential hepatoma cells

Objective To synthesize 18F-AlF-NOTA-G-TMTP1 and evaluate its potential for PET imaging on nude mice bearing high-metastatic potential hepatoma cells.Methods NOTA-G-TMTP1 was synthesized by the standard Fmoc-solid phase synthetic protocols and radiolabeled with 18F using NOTA-AlF chelation method.The nude mice models bearing low-metastatic potential HCC97L and high-metastatic potential HCCLM3 xenografts were established separately.The tumor-targeting characteristics of 18F-AlF-NOTA-G-TMTP1 were assessed by microPET/CT and biodistribution assay.Results NOTA-G-TMTP1 was labeled with 18F in one step with (25±6)% labeling yield (n=5).The radiochemical purity of 18F-AlF-NOTA-G-TMTP1 was more than 95% with a specific activity more than 11.1 GBq/μmol.The octanol/water partition coefficient (logP) for 18F-AlF-NOTA-G-TMTP1 was-3.166±0.022.The tumor to muscle ratios were 1.8± 0.4 and 4.7±0.2 at 35 min post injection for HCC97L and HCCLM3,respectively.The uptake of 18F-AlF-NOTA-G-TMTP1 in HCCLM3 tumor was inhibited (61.4%) by unlabeled G-TMTP1.Conclusion 18F-AlF-NOTA-G-TMTP1 has been successfully synthesized.It shows specific uptake by tumor induced by the high-metastatic potential hepatoma cells.
8.Clinical significance of bcl-6, p53, c-myc aberrations in diffuse large B-cell lymphoma

Objective To investigate aberrations of bcl-6,p53,c-myc genes in diffuse large B-cell lymphoma (DLBCL) and its clinical significance.Methods Interphase fluorescence in situ hybridization (I-FISH) was detected in 59 DLBCL patients in vivo tissue bcl-6,p53 protein,c-myc gene status.The patients were treated with CHOP or R-CHOP chemotheralpy,and the survival rates and treatment efficiency were compared.Results The p53 deletion was detected in 18 of the 59 cases (30.5 %),bcl-6 rearrangement in 11 cases (18.6 %),5 cases with c-myc rearrangement (8.5 %).In the aspects of remission rate,p53 deletion positive group contained less advantage than negative ones (33.3 % vs 75.6 %,x2 =9.560,P =0.002).The prognosis of bcl-6 gene rearrangement positive group different from negative group,but the difference was not statistically significant (OS,P =0.107; PFS,P =0.094),p53 deletion positive patients was in significantly worse prognosis than the negative group (OS,P =0.031; PFS,P =0.028),c-myc rearrangement positive group difference in gene rearrangement negative group,but the difference was not statistically significant (OS,P =0.163; PFS,P =0.167).In the CHOP group,prognosis of p53 deletion,c-myc rearrangement positive group were significantly worse than the negative group,the difference was statistically significant (P < 0.05).In R-CHOP group,the prognostic significance of bcl-6 gene rearrangement positive group were worse (OS,P =0.003; PFS,P =0.007).Conclusion DLBCL patients with bcl-6,p53,c-myc genes aberrations are related with poor prognosis,and they can be used as prognostic factors for predicting DLBCL and guiding therapy.
9.Quality of life and mental health of pre-post household service workers who held a high school graduation certificate or above and had been engaged in different work

Objective To survey the quality of life and mental health status of pre-post household service workers who held a high school graduation certificate or above and had been engaged in different work.Methods A total of 254 household service workers completed Symptom Checklist 90 (SCL-90) and World Health Organization Quality of Life-Brief (WHOQL-BREF).Statistical analyses of variance and correlation were carried out to evaluate mental health status and quality of life of the participants.Results Abnormal score of SCL-90 was found in 8% of household service workers who had graduated from high school.The psychological score of household service workers who had worked as workers(60.59± 2.18)was lower than those who had worked as service workers (66.23 ± 12.13) or peasants (65.40 ± 12.24) (P <0.05).Almost all factors of SCL-90 were negatively correlated with WHOQL-BREF (correlation coefficient -0.140 to-0.376).Conclusions Our data suggest that mental health status can be well in pre-post household service workers who held a high school graduation certificate or above.Special training should be given to the household service workers in accordance with their working experience.
10.Distribution of metastatic lymph nodes in 150 patients who underwent radical resection for pancreatic head cancer

ObjectiveTo study the characteristics and the impact of lymph node metastasis on radical resection for pancreatic head cancer to provide a theoretical basis for lymphadenectomy in radical resection.To study the reliability of using a surgical microscope to detect lymph nodes in radically resected specimens of pancreatic head cancer.MethodsLymph nodes in the specimens after radical pancreaticoduodeneetomy (pancreaticoduodenectomy + D2 regional lymphadenectomy) were identified using a surgical microscope and they were grouped using the JPS standard.The position and the frequency of the lymph nodes retrieved,and their association with other clinicopathologic factors were analysed.The results were compared with the data published in 2004 on 46 patients to evaluate the reliability of using a surgical microscope.ResultsLymph node metastasis was detected histopathologically in 101 patients (67.3%).The median number of lymph nodes retrieved in the specimens as detected using the surgical microscope was 38.2.The most commonly involved lymph node groups were No.13 (64.5%),No.14 (51.7%),No.17 (38.6%),No.12 (25.8%),No.16 (20.8%).Lymph node metastasis was significantly associated with tumour T stage,tumour invasion and differentiation,preoperative serum level of CA19-9 and CA72-4,but not with patient age,sex,or tumour location.There were no significant differences between the results and the data of the previous study in 2004.ConclusionsExtended lymphadenectomy is necessary because extensive lymph node metastasis was common.Surgical microscopy is an effective and reliable method to detect lymph nodes in resected specimens of pancreatic head cancer for accurate pathologic staging.
