1.Prevalence of multimorbidity among the HIV-infected individuals receiving anti-viral therapy in Dehong Prefecture, Yunnan Province

ObjectiveTo explore the prevalence and influencing factors of multimorbidity among the HIV-infected individuals receiving anti-viral therapy (ART) in Dehong Prefecture of Yunnan Province, so as to provide a reference for the long-term follow-up management of HIV-infected patients and the comprehensive prevention and treatment of chronic diseases. MethodsA cross-sectional study was conducted to investigate the multimorbidity burden among the HIV-infected adults receiving ART in Dehong Prefecture from January to July 2021 and a self-designed questionnaire was used to analyze relevant disease indicators. Multivariate logistic regression analysis was used to investigate the influencing factors of multimorbidity among the HIV-infected individuals. ResultsA total of 3 946 HIV-infected individuals receiving ART were enrolled in this study, of which 63.7% aged ≤50 years, with a male to female ratio of 1.1∶1. Among the 3 946 cases, 825 of them had ≥2 comorbidities, with a co-prevalence rate of 20.9% (95%CI:19.6%‒22.2%), and the main comorbidities were dyslipidemia, diabetes, and hypertension. Multivariate logistic regression analysis showed that 40≤ aged <50 years (aOR=1.86, 95%CI: 1.45‒2.40, P<0.001), 50≤ aged ≤85 years (aOR=3.75, 95%CI: 2.93‒4.80, P<0.001), Dai nationality (aOR=1.21, 95%CI: 1.01‒1.47, P=0.043), BMI≥24.0 kg∙m-2 (aOR=1.79, 95%CI: 1.49‒2.14, P<0.001), 10.0≤ with ART duration for <12.5 years (aOR=1.49, 95%CI: 1.05‒2.12, P=0.024), with ART duration for ≥12.5 years (aOR=1.50, 95%CI: 1.05‒2.15, P=0.026), use of second-line HIV therapy (aOR=1.43, 95%CI: 1.19‒1.70, P<0.001) and other therapy options (aOR=3.16, 95%CI: 2.17‒4.61, P<0.001) were positively correlated with multimorbidity. ConclusionThe prevalence of multimorbidity among the HIV-infected individuals receiving ART in Dehong Prefecture is high, which is associated with the advancing age and prolonged treatment time, particularly with a significant burden of dyslipidemia, diabetes, and hypertension. Comprehensive surveillance and targeted management of comorbidities, along with ART follow-up, need to be strengthened in the future.
2.Effects of Sijunzi decoction on the regulation of autophagy proteins by inflammatory factors based on TNF-α/NF-κB in hippocampal neurons of SAMP8 mice

This study was designed to investigate the effect of Sijunzi decoction(SJZD)on the expression of inflammatory factors and autophagic proteins in the hippocampus of senescence-accelerated mouse prone 8(SAMP8)mice and its mechanism.SAMR1 mice were used as control group,and 32 SAMP8 mice were divided into model group,donepezil group,SJZD low and SJZD high dose treatment groups.Y-maze experiment was performed to detect changes in mouse memory function;the expression of NF-κB was detected by immunohistochemistry;the levels of TNF-α,IL-1β,IL-6 were detected by ELISA;the expression of A[3,caspse-1,beclin-1 and LC3-Ⅱproteins in hippocampal tissue were detected by Western blotting.Compared with the control group,the model group mice showed a decrease in total number of entries and alternations,an elevation in the levels of TNF-α,IL-1β,IL-6,Aβ protein and caspse-1 protein,and downregulation in the expression of beclin-1 and LC3-Ⅱ proteins.Both donepezil and SJZD(low-dose group and high-dose group)can reverse these changes in model mice.In conclusion,the mechanism of SJZD in treating Alzheimer's disease may relate to the correction of central hippocampal inflammatory factors and autophagy dysfunction.
3.Effect of Si Junziwan on Expression of Lon Protein in Hippocampal CA3 Region of SAMP8 Mice

ObjectiveTo study the expression changes of Lon protein and mitochondrial dynamics-related protein in the hippocampus of SAMP8 mice and provide a theoretical basis for the treatment of Alzheimer's disease by invigorating the spleen and supplementing Qi. MethodEight 3-month-old SAMR1 mice were used as the normal group, and 32 3-month-old SAPM8 mice were divided into model group, western medicine group (0.013 g·kg-1), low-dose Si Junziwan group (3.24 g·kg-1), and high-dose Si Junziwan group (12.56 g·kg-1), with 8 mice in each group. The western medicine group was gavaged with donepezil, and the Si Junziwan low- and high-dose groups were gavaged with Si Junziwan for 30 days. The positioning navigation experiment of the water maze was started on the 25th day, and the space exploration experiment of the water maze was started on the 30th day. On the 30th day, the protein expression of mitofusin 2 (MFN2) was detected by immunohistochemistry, the expression of adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK) was detected by enzyme-linked immunosorbent assay (ELISA), the content of ATP was detected by colorimetry, the microstructure of neuron mitochondria was detected by electron microscope, and the expressions of Aβ protein, Lon protein, dynamin-related protein 1 (DRP1) protein, and MFN1 protein were detected by Western blot. ResultAs compared with the normal group, the latency escape period increased, the number of crossings decreased, the expression of AMPK increased, and the content of ATP decreased in the model group. The expressions of Aβ protein and DRP1 protein increased (P<0.01), whereas the expressions of Lon protein, MFN1 protein decreased in the model group (P<0.05,P<0.01), and MFN2 protein decreased. The vacuolation of mitochondria increased and the cristae broke in the model group. As compared with model group, the time of the latent escape period decreased (P<0.01), and the number of crossings increased in the low-dose and high-dose Si Junziwan groups (P<0.05). The expression of AMPK (P<0.01) decreased, the content of ATP increased (P<0.01), the expression of Aβ and DRP1 protein decreased (P<0.05, P<0.01), and the expression of MFN1 protein was up-regulated (P<0.05) in high-dose Si Junziwan groups. The vacuolation was more obvious in the low-dose Si Junziwan group, whereas the vacuolation was restored and the ridge was clear in the high-dose Si Junziwan group. ConclusionSi Junziwan treats Alzheimer's disease by up-regulating the protein expression of Lon, correcting the disorder of mitochondrial division and fusion protein, and changing the memory function of SAMP8 mice.
4.Application of prenatal bacterial artificial chromosome-on-beads identification/isolation technology in the diagnosis of chromosomal aneuploidy and microdeletion/microduplication syndrome

Objective:To explore the indication of bacterial artificial chromosome-on-beads identification/separation technology in prenatal diagnosis and its application value.Methods:The inclusion criteria were as follows: age ≥35 years, high risk/critical risk of serological prenatal screening, high risk of non-invasive gene testing (NIPT), abnormal ultrasonic indexes or adverse pregnancy history. From April 2016 to December 2020, 3579 amniotic fluid samples collected from pregnant women with singletons were detected by bacterial artificial chromosome-on-beads identification/separation technique (BoBs) and G-banding karyotype analysis simultaneously. The aneuploid abnormality/microdeletion/microdeletion samples detected by karyotype analysis or BoBs were verified by fluorescence in situ hybridization (FISH)/SNP array as needed.Results:(1) The percentage of samples with indications of advanced maternal age, high risk of NIPT and high risk of serological screening was 89.44%(3 201/3 579), the detection rate of aneuploidy was 96.19%(202/210), and the detection rate of microdeletion/microduplication was 87.5%(28/32),the total abnormal detection rate was 95.04% (230/242). The samples with abnormal ultrasonic indexes, adverse pregnancy history and critical risk indications of serological screening in the second trimester accounted for 10.66%, and the abnormality of aneuploidy and micro-duplication/micro-deletion was 4.96%. (2) 198 common chromosome aneuploidies (13/18/21/X/Y) were detected by BoBs, and 12 cases with chimeras ≥20% were found, which were consistent with karyotype results. Two copies of 21-trisomy, three copies of X/Y, seven copies of 2, 7, 8, 9, 10, 20, mar karyotype chimerism, eight copies of arm inversion and five copies of translocation outside the detection range of probes were detected by karyotype analysis. The sensitivity, specificity and false negative rate of BoBs detection for five aneuploidies were 94.6%(210/222), 100% and 5.4%(12/222), respectively. BoBs and karyotype analysis detected 32 and 9 cases of microdeletions/microduplications respectively. Compared with single karyotype analysis, the combined application of G-banding karyotype analysis and BoBs can detect an additional 9.4% (23/244)positive samples.Conclusion:The samples with elder age, high risk of NIPT, and high risk of serological screening are more suitable as indications for the application of BoBs in prenatal diagnosis.
5.Study on liver diseases with zebrafish as an important tool.

With the increasing incidence of hepatobiliary diseases, it is particularly important to understand the role of molecular, cellular and physiological factors in the clinical diagnosis and treatment with traditional Chinese medicine(TCM) in the development of liver disease. Appropriate animal models can help us identify the possible mechanisms of relevant diseases. Danio rerio(zebrafish) model was traditionally used to study embryonic development, and has been gradually used in screening and evaluation of liver diseases and relevant drug in recent years. Zebrafish embryos develop rapidly and the digestive organs of 5-day-old juvenile fish are all mature. At this stage, they may develop hepatobiliary diseases induced by developmental defects or compounds. Zebrafish liver is similar to human liver in cell composition, function, signal transduction, response to injury and cell process mediating liver disease. Furthermore, due to the high conservation of genes and proteins between humans and zebrafish, zebrafish becomes an alternative system for studying basic mechanisms of liver disease. Therefore, genetic screening could be performed to identify new genes involving specific disease processes, and chemical screening could be made for drugs in specific processes. This paper briefly introduced the experimental properties of zebrafish as model system, emphasized the study progress of zebrafish models for pathological mechanism of liver diseases, especially fatty liver, and drug screening and evaluation, so as to provide ideas and techniques for the future liver toxicity assessment of TCM.

6.Nurses' needs for optimizing nursing model of intravenous thrombolysis in acute ischemic stroke: a qualitative study

Objective:To explore the needs of nurses participating in acute ischemic stroke (AIS) intravenous thrombolysis to optimize the AIS intravenous thrombolysis nursing model.Methods:This research adopted phenomenological research method. Objective sampling method was used to interview 15 nurses from 5 designated hospitals for stroke treatment in Shenzhen from June to July 2020, and the data were analyzed by the Colaizzi content analysis method.Results:The needs of nurses for optimizing the AIS intravenous thrombolysis nursing model could be summarized into five themes: the need to configure stroke emergency nurses, the need to standardize the training for stroke nursing staff, the need to build an intelligent information platform for AIS treatment, the need to optimize the green channel for stroke, and the need to improve the quality control system of AIS intravenous thrombolysis.Conclusions:It is necessary to configure stroke emergency nurses, standardize stroke nursing related training, and build an intelligent information platform for AIS treatment to optimize the AIS intravenous thrombolysis nursing model.
7.The initial experience of robotic-assisted laparoscopic pyeloplasty in the treatment of infant with ureteropelvic junction obstruction

Objective To investigate the feasibility and safety of robotic-assisted laparoscopic pyeloplasty (RALP) in infant ureteropelvic junction obstruction.Methods Of 31 infants who underwent RALP between March 2017 and August 2017 were reviewed,including 25 boys and 6 girls.22 infants were found that the hydronephrosis was in the left kidney and 9 in the right kidney.The average age of the infants was (3.5 ± 0.7) months,ranged from 8 days to 6 months,including 7 infants less than 2 months.The average weight of the infants was (6.7 ± 1.2) kg,with a minimum weight of 3.3 kg.The diagnosis and outcomes of ureteropelvic junction obstruction (UPJO) were reviewed on clinical and imaging data.The ultrasound showed that the mean anteroposterior diameter of hydronephrotic kidney was (4.01 ±0.56)cm.The split renal function was (37.1 ± 3.2) % in infants who were more than 2 months.Patients with proximal defect were positioned in the lateral flank position with 50°-70° inclination and as close as possible to the edge of operating table.The trocars of robotic lens and No.1 robotic arm were placed on the middle of the umbilicus and the Pfannenstiel line respectively,and the trocar of No.2 robotic arm was placed on the supraumbilical symmetry point of the trocar No.1 robotic arm,and the auxiliary trocar was placed on the Pfannenstiel line outside the 3-4 cm of healthy side of the trocar of No.1 robotic arm.During operation,the ureteropelvic junction was dissected and tailored.Then the ureter was open longitudinally at the posterior lateral side and was anastomosed at the lowest part the renal pelvis.We assessed preoperative clinical data and outcomes,and analyzed the transition experience.Data are expressed as medians for continuous variables.Results All operations were successful with no serious intraoperative complication and traditional laparoscopic or open surgical conversion.The average operative time was 108 min,the average intraoperative blood loss was 9.5 ml.The average drain removal was 1.2 days,the average start oral of feeding was 3.2 hours days and the average postoperative hospital stay was 6.2 days.Postoperative pathological results showed smooth muscle hyperplasia,hypertrophy and fibrous tissue in the urothelium,accompanied by chronic inflammation of urothelial mucosa.Patients with Clavien Ⅰ and Ⅱ complications 22.6% (7/31) postoperatively were managed conservatively.Among them,there were 1 cases of flank pain,1 cases of hematuria,2 cases of urinary leakage,and 3 cases of urinary tract infection.Patients' symptoms disappeared by conservative treatment.No patient suffered Clavien Ⅲ and Ⅳ complication postoperatively.The renal pelvis anteroposterior diameter was 1.58 cm at three months postoperatively,which was significantly smaller than that of peroperation (P < 0.05).A diuretic T1/2 showed the improvement of split renal function without urinary obstruction in all patients at three months postoperatively.The postoperative split renal function was (39.2 ± 3.5) %,which was slightly higher than that of preoperation (P > 0.05).Conclusions Severe hydronephrosis in neonates can be acted as an indication for exploratory surgery when the surgeon's ability,anesthesia,monitoring and management conditions are available.Robotic-assisted laparoscopic pyeloplasty is a safe and feasible option for the treatment of UPJO in infant with less trauma,rapid recovery and good cosmetic result.We suggest that RALP is potentially a recommended feasible treatment option for infant with UPJO.
8.Pathology of retrieved thrombus in a patient with acute ischemic stroke due to infective endocarditis: a case report

Infective endocarditis is one of the causes of cerebral infarction.Mechanical thrombectomy has been widely used in China for years.However,reports involving retrieved thrombus pathology are somewhat scarce.Here a case in which the cause of stroke was confirmed by thrombus pathology is reported.A 33-year male with a history of dilated cardiomyopathy visited the emergency because of sudden onset of left-sided weakness for 14 hours.Mechanical thrombectomy was performed,and the pathological study of the retrieved thrombus demonstrated Gram-staining positive cocci.The patient was combined with fever,septic shock,and acute kidney injury.Ultrasoundcardiography showed vegetation in the left ventricle.Therefore,he was definitely diagnosed as acute ischemic stroke due to infective vegetation embolism.
9. Efficacy of robot-assisted laparoscopic ureteral reimplantation for primary obstructive megaureter in children

Objective:
To evaluate the clinical efficacy of robot-assisted laparoscopic ureteral reimplantation (RAUR) for primary obstructive megaureter (POM) in children.
Methods:
Twenty-one patients who underwent RAUR for POM in Bayi Children’s Hospital between 2017 April and 2018 April were retrospectively analyzed. The study population consisted of 15 boys and 6 girls aged between 2 months and 11 years. Ten patients had left POMs, 9 had right POMs and two had bilateral POMs. All patients were preoperatively diagnosed with a POM based on urinary system ultrasonography, magnetic resonance urography, and diuretic renal dynamic imaging. Our main technique key steps include: Under general anesthesia, the patients were placed in a Trendelenburg position approximately 40 degrees from the horizon. An 8.5-mm camera port was placed at the level of the umbilicus. Followed by two 5-mm robotic Trocars placed under direct vision 6 cm to the camera port separately, a 5-mm assistant port was placed on the right upper abdominal quadrants which was located 3 cm from the camera and robotic port. The ureter was identified at the pelvic brim. The peritoneum covering the ureter was incised and the ureter was mobilized to the level of the vesico-ureteric junction. The bladder was filled with 60 ml saline and a 5 cm length and 1.5 cm wide submucosal detrusor tunnel was created. The ureter was transected at the bladder mucosa and the narrowed ureteral segment was discarded. The ureteroneocystostomy was performed using 6-0 absorbable suture. Dissecting the perivesical fascia appropriately and using down-top suturing approach, use of an apical stay stitch, and incorporation of the ureteral adventitia during detrusorraphy. Postoperative complications were analysed using the Clavien-Dindo classification. Success was defined as symptomatic relief, decreased hydronephrosis on ultrasound and no evidence of vesicoureteral reflux on voiding cystourethrography.
Results:
All surgeries were successfully completed without conversion and no intra-operative complication was encountered. The mean operative time was 117.6±18.1(89-165)min, the mean estimated blood loss was 11.9±4.3(5-25)ml, the abdominal drainage tubes were removed after a mean of 4.9±1.1(3-8)days, and the mean postoperative hospital stay was 6.3±1.3(4-10) days. Postoperative complications (Clavien Ⅰ-Ⅱ) occurred in 9.5% (two patients had recurrent urinary tract infections postoperatively) children, no grade Ⅲ-Ⅳ complication was observed. The pre-operative symptoms in all patients disappeared. One patient had grade Ⅱ VUR on VCUG, who were followed conservatively. Ultrasound at postoperative follow-up showed that the hydronephrosis was disappeared in 20 ureters and significantly decreased in 3 ureters. The success rate was 95.7% at a mean follow-up of 16.3±4.0(10-23)months.
Conclusion
RAUR is a safe and feasible option for the treatment of POM in children with higher success rate and lower complication rate.
