Aim To explore the mechanism by which the compound Chaijin Jieyu formula(CCJJY)regulates the TLR4/NLRP3 signaling pathway to inhibit central oxidative stress and improve hippocampal synaptic plasticity damage in depression.Methods SD rats were randomly divided into the control group,chronic unpredictable mild stress group,sleep deprivation group,chronic unpredictable mild stress combined with sleep deprivation group,positive drug group(venlafax-ine+melatonin),low-dose group of CCJJY,medium dose group of CCJJY,and high-dose group of CCJJY,with nine rats in each group.Except for the control group,a rat model of depression complicated with in-somnia was established using chronic unpredictable mild stress combined with sleep deprivation.Depres-sion-like and sleep behaviors in rats were evaluated through weight,food intake,water maze,and pento-barbital sodium tests.ELisa was used to detect ROS,AANAT,and HPLC-EC was used to detect 5-HT con-tent,while Western blot/RT-PCR was used to detect the expression of IL-1β,TLR4,NLRP3,PSD-95,and SYN related proteins and mRNA.HE and Golgic stai-ning were used to observe the pathological changes in the third ventricle,hippocampus,and neuronal synap-ses.Results Compared with the control group,the depression-like behaviors of the model group rats were significant.The expression of IL-1β,TLR4,and NL-RP3 in the hippocampus increased,while the expres-sion of PSD-95 and SYN decreased.Activation of NL-RP3 inflammasomes led to "sleeve like" pathological changes in the third ventricle,with hippocampal neu-rons undergoing apoptosis and significant damage to neuronal synaptic plasticity.Compared with the model group,after intervention with CCJJY,the expression of ROS,IL-1β,TLR4,and NLRP3 decreased,while the expression of AANAT,5-HT,PSD-95,and SYN in-creased.Pathological damage to the third ventricle and hippocampal neurons was repaired.Conclusion The CCJJY improves hippocampal synaptic plasticity dam-age in depression by regulating the TLR4/NLRP3 sig-naling pathway to inhibit central oxidative stress.

Objective To understand the current situation of thirst in patients with cirrhosis and analyze its influencing factors,in order to improve medical staff's attention to thirst symptoms in patients with cirrhosis and provide theoretical basis for clinical intervention.Methods A total of 220 patients with cirrhosis who were hospitalized in the infection department of a tertiary A general hospital in Wuhan from March to June 2024 were selected by convenience sampling method.General data questionnaire,Numerical Score Scale and Thirst Distress Scale were used to investigate the factors affecting thirst sensation in patients with cirrhosis.Results A total of 202 valid questionnaires were collected,and the effective questionnaire recovery rate was 91.82％.The results showed that the incidence of thirst in patients with cirrhosis was 59.41％;the score of thirst was 3.00(3.00,6.00)points,and the mean score of thirst was 3.09.Among them,43.07％of the patients with cirrhosis were in the moderate to severe level of thirst.The score of Thirst Distress Scale is 6.00(6.00,17.00)points,and the average score of thirst distress was 11.62 points,which was in the medium level.47.03％of patients with cirrhosis indicated thirst distress,and 23.76％of patients with cirrhosis which was in the moderate and severe level of distress.The results of multiple linear regression analysis showed that the degree of ascites,the use of diuretics,the stage of disease,and the degree of thirst distress were the factors influencing the degree of thirst in patients with cirrhosis(P＜0.05).Gender,marital status,degree of ascites,use of diuretic drugs,disease stage and degree of thirst were the factors influencing degree of thirst distress in patients with cirrhosis(P＜0.05).Conclusion The incidence and severity of thirst in patients with cirrhosis are relatively high,and are affected by many factors.Medical staff should pay more attention to the management of thirst symptoms in patients with cirrhosis,and formulate targeted nursing measures or nursing programs according to the related influencing factors of thirst,so as to improve the comfort level of patients and improve the disease experience of patients.

Objective To systematically evaluate the effectiveness and acceptability of antidepressant drugs in the treatment of postpartum depression(PPD).Methods The PubMed,Cochrane Library,Embase,Web of Science,China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Journals of Chinese Scienc were searched,and Chinese Biomedical Literature Service System(SinoMed)database until November 2023.Screen randomized controlled trials(RCTs)of antidepressant drugs for the treatment of PPD.The treatment group was given antidepressant drugs,and the control group was given placebo or another antidepressant drug.Meta-analysis of effectiveness and acceptability is performed using Stata 17.0 software.Results A total of 27 RCTs with a total of 2 202 patients were included.The results of meta-analysis showed:The top three efficacy relative to placebo were mirtazapine[odds ratio(OR)=2.25,95％confidence interval(CI)=(1.20-3.30),P＜0.05],nortriptyline[OR=1.50,95％CI=(0.55-2.44),P＞0.05],venlafaxine[OR=1.35,95％CI=(0.13-2.56),P＞0.05].Acceptability is compared with placebo in the top three Chinese herbal medicine[OR=0.47,95％CI=(-0.72-1.66),P＞0.05],nortriptyline[OR=-0.08,95％CI=(-1.16-1.33),P＞0.05],venlafaxine[OR=-0.12,95％CI=(-1.47-1.24),P＞0.05].Conclusion Nortriptyline,venlafaxine,trazodone,and duloxetine are effective in treating PPD without obvious adverse drug reactions.

Aim To explore the mechanism of Epimedii folium-Astmgali radix activating the SCF/c-kit signa-ling pathway to activate the PI3K/Akt signaling path-way and its effect on sperm production and vitality in oligoasthenospermia.Methods Sixty male SD rats were used to establish a model of oligoasthenospermia with cyclophosphamide.They were randomly divided into six groups:experimental group(further divided into high,medium,and low dose group),model group,control group and blank group.The oligoasthenosper-mia model was established by using cyclophosphamide in experimental group,levocarnitine group and model group.The rats in the high,medium,and low dose group of the experimental group were orally adminis-tered Epimedii folium-Astmgali radix extract at doses of 800,400,and 200 mg·kg-1,respectively,Once daily for 35 days.Rats of the control group were orally ad-ministered 250 mg·kg-1·d-1 of levocarnitine,Once daily for 35 days.ELISA was used to detect serum of T,E2,FSH,and LH.Western blot and IHC staining were used to detect the expression of SCF,c-kit,Bcl-2,Bax,PI3K,and Akt proteins in rat testicular tissues.Sperm activity is examined by microscopy.The testicu-lar tissue structure and cell morphology of rats in each group were observed.Results Compared with the model group,Epimedii folium-Astmgali radix increased the sperm density,total viability rate,and vitality(P＜0.05,P＜0.01),decreased sperm apoptosis rate and LH,T,and E2 levels(P＜0.05,P＜0.01),decreased Bax protein expression in testicular tissue(P＜0.01),and increased Bcl-2,SCF,c-Kit,PI3K,and Akt protein expression(P＜0.05,P＜0.01);it increased the number of germ cells,thickened basement membrane,and significantly improved seminiferous tubule mor-phology,even showing germ cells at different develop-mental stages and mature sperm.Conclusions Epi-medii folium-Astmgali radix has a significant therapeu-tic effect on oligoasthenospermia in rats.Its mechanism may be related to the activation of the SCF/c-kit signa-ling pathway to activate the PI3K/Akt signaling path-way promoting the proliferation and differentiation of germ cells,and promoting sperm production,maturation and motility.

Objective:To investigate the application effect of diltiazem hydrochloride combined with sacubitril valsartan on chronic heart failure after coronary intervention(PCI).Methods:A prospective study was conducted.The cases were included from May 2020 to May 2023.After the inclusion and exclusion criteria were deleted,a total of 88 patients with chronic heart failure after PCI met the research requirements.They were divided into two groups with the same number of cases,namely 44 cases in each group.The control group received oral treatment with sacubitril and valsartan,while the observation group received additional treatment with diltiazem hydrochloride on top of the control group.Compare the clinical efficacy,changes in cardiac function related indicators and exercise endurance before and after treatment between two groups,and conduct a 1-year follow-up of all patients to record the incidence of cardiovascular adverse events during the follow-up period.Finally,compare the changes in quality of life before and after treatment between the two groups of patients.Results:The total effective rate of the observation group was 93.18％,which was higher than the control group's 77.27％(P＜0.05);After treatment,the changes of cardiac function related indexes and exercise tolerance were compared.It was found that the N-terminal B-type natriuretic peptide(NT proBNP)(45.14±6.34)pg/mL,LVEDD(51.66±3.04)mm and LVESD(32.63±4.45)mm in the observation group were lower than those in the control group,while the left ventricular ejection fraction(LVEF)(55.57±4.25)％and 6MWT(514.62±34.42)m in the observation group were higher than those in the control group(P＜0.05);During the 1-year follow-up,no death occurred in the two groups.The incidence of cardiovascular adverse events in the observation group was 4.55％,much lower than 20.45％in the control group(P＜0.05);After treatment,the scores of emotion,body,others and MLHFQ in the observation group(13.53±2.21,14.25±2.63,20.35±4.52,48.13±5.25)were lower than those in the control group(P＜0.05).Conclusion:The combination of Diltiazem Hydrochloride and Sacubitril Valsartan has significant therapeutic effects on patients with chronic heart failure after PCI.It can improve patients' cardiac function and exercise endurance,reduce the incidence of cardiovascular adverse events,and improve their quality of life.

Aim To explore the therapeutic effect and mechanism of Qingjie Huagong decoction in modulating PI3K/AKT/NF-κB signaling pathway in inflammatory response of acute pancreatitis(AP)mice.Methods Twenty-four mice were randomly divided into Blank group,Model group,Ustekin group,and Qingjie Hua-gong decoction group,with six mice in each group.The AP model was prepared by using rain frogin.Serum α-AMS,PNLP,IL-1β,IL-6,IL-8,IL-18,and TNF-α lev-els were detected by ELISA;the pancreatic pathology was detected by HE staining;the expressions of PI3K,AKT,and NF-κB-related proteins and mRNAs were de-tected by immunohistochemistry,Western blot,and RT-qPCR.Results Compared with the blank group,the model group showed obvious pathological damage to the pancreas,with significantly higher serum α-AMS,PN-LP,IL-1β,IL-6,IL-8,IL-18,and TNF-α levels(P＜0.01),and significantly higher levels of PI3K,AKT,and NF-κB-related proteins and mRNA expression(P＜0.01).Compared with the model group,both the Qingjie Huagong decoction group and the ustekin group improved the histopathological changes in the pancreas of AP mice,decreased the serum α-AMS,PNLP,IL-1β,IL-6,IL-8,IL-18,and TNF-α levels,and down-reg-ulated the expression levels of pancreatic PI3K,AKT,NF-κB-related proteins and mRNA(P＜0.05 or P＜0.01).Conclusion Qingjie Huagong decoction may inhibit the inflammatory response and protect pancreat-ic tissues by regulating the expression of PI3K/AKT/NF-κB signaling pathway.
