1.Effect of hope theory-based pelvic floor exercises on hope level and anal function in patients with prophylactic enterostomy reversal

Objective To analyse the effect of hope theory-based pelvic floor exercises on anal function in patients with prophylactic enterostomy reversal.Methods A total of 104 patients with low rectal cancer and underwent prophylactic enterostomy reversal from March 2021 to March 2023 were selected and randomly divided into a routine group and a trial group,with 52 patients per group.Patients in the routine group received a standard postoperative nursing care plus a pelvic floor exercise program,while the patients in the trial group received a hope theory-based pelvic floor exercise program in addition to the standard postoperative nursing care.The anal function of patients was evaluated at 1 and 3 months after surgery,while the self-care ability and hope level were evaluated before the intervention and at 3 months post-surgery.Results In comparison with the routine group,the patients in trial group demonstrated significantly lower scores of anal function at both 1 month and 3 months after surgery(12.1±2.4 vs.13.9±2.2,t=-3.987,P<0.001;5.8±1.4 vs.7.3±1.7,t=-4.912,P<0.001,respectively).The trial group showed significant increases in the total scores in both self-care ability and hope level after the intervention(26.6±3.5 vs.12.3±1.6,t=26.796,P<0.001;7.4±0.3 vs.2.0±0.4,t=77.881,P<0.001,respectively).Conclusion Hope theory-based pelvic floor exercises can enhance postoperative anal function,self-care ability and hope level in patients undergoing prophylactic enterostomy reversal.
2.Effects of clinical nursing path model on depression and anxiety among rectal cancer patients with permanent intestinal stoma

Objective:To explore the effects of clinical nursing path model on depression and anxiety among rectal cancer patients with stoma.Methods:From January 2017 to December 2018, this study selected 100 rectal cancer patients with stoma in the First Affiliated Hospital of Nanjing Medical University. All of patients were randomly divided into control group and intervention group with the method of random number table, 50 cases in each group. Patients of two groups all received routine nursing. On the basis of routine nursing, observation group carried out the intervention based on the clinical nursing path model. Depression and anxiety before and after intervention was assessed with the Patient Health Questionnaire-9 (PHQ-9) and the Generalized Anxiety Disorder-7 (GAD-7) .Results:Before intervention, there was no statistical difference in the scores of PHQ-9 of patients between two groups ( t=0.116, P=0.908) . After intervention, the score of PHQ-9 of intervention group was lower than that of control group with a statistical difference ( t=6.083, P<0.01) , and lower than that before intervention also with a statistical difference ( t=7.676, P<0.01) . Before intervention, there was no statistical difference in the scores of GAD-7 patients between two groups ( t=0.227, P=0.821) . After intervention, the score of GAD-7 of intervention group was lower than that of control group with a statistical difference ( t=9.249, P<0.01) ; scores of GAD-7 of two groups were lower than that before intervention also with statistical differences ( P<0.01) . Conclusions:Application of clinical nursing path model can effectively improve depression and anxiety of rectal cancer patients with stoma.
3.Comparison of therapeutic effects of different surgery methods on early hypertensive intracerebral hemorrhage in basal ganglia region in elderly patients and their prognostic factors analysis

Objective To explore the therapeutic effects of different surgery methods on early hypertensive intracerebral hemorrhage(HICH)in basal ganglia region in elderly patients and on prognostic factors analysis.Methods 89 elderly patients with early HICH were randomly divided into four groups according to surgery methods and whether their ICP was monitored.Group A(n=21)was given minimally invasive hematoma drainage,group B(n=23)was given small bone window for removal of hematoma,group C(n=21)and group D(n=24)was given ICP monitoring and corresponding management of ICP on the basis of group A and group B,respectively.The changes of intracranial pressure before and after operation,prognosis and post-operative complications were compared.Results The intracranial pressure was significantly decreased at 3rd day,7th day after operation in group C and D as compared with those in group A and B at the same time points(F=11.76,P<0.05),and the score of GCS was also higher in group C and D at 7th day after operation than in group A,B at the same time points(F=4.72,P<0.05).At 14th and 28th day after operation,the score of GCS was higher in group C than in group A and B(F=19.24,P<0.05),and higher in group C than in group D at 28th day after operation(F=22.26,P<0.05).The dosage of mannitol was significantly lower in group C and group D than in group A and group B(F=18.87,P<0.05).The incidence rate of post-operative complications was 14.3% in group C vs.28.6% in group A(P<0.05)and 20.8% in group D vs.47.8% in group B(χ2=7.04,P<0.05).The proportion of a good recovery and a light disability was significantly higher in group C and D(76.2% and 75.0%)than in group A and B(42.9% and 39.1%)respectively(χ2=14.99,all P<0.05).Conclusions Minimally invasive hematoma drainage shows the advantages of small trauma and a few complications for the treatment of elderly patients with early HICH,and its combination with ICP can early change intracranial pressure and further improves the prognosis.
4.Effect of trimetazidine postconditoning on myocardial cell apoptosis in acute ischemia-reperfusion injury in rats

Objective To investigate the effect of trimetazidine postconditoning on myocardial cell apoptosis in acute ischemia-reperfusion injury in rats.Methods 40 healthy adult Wistar rats were randomly assigned to Sham-operated group,MIRI group(I/R group),trimetazidine group and ischemic postconditioning group(IPOC group).The left anterior descending coronary artery of rat was ligated and the acute myocardial ischemia-reperfusion injury model was established.After that,ultrastructural changes of apoptotic myocardial cells were observed under electron microscope,and apoptotic cells were detected under optical microscope.The expressions of the Bcl-2,caspase-3 apoptosis related proteins were determined.Results ① The degree of ultrastructural injury of apoptotic myocardial cells was mild and basically the same in trimetazidine group versus IPOC group,and had a significant improvement as compared with I/R group.② The degree of myocardial apoptosis was basically the same in trimetazidine versus IPOC group,and lighter than in I/R group.③ Bcl-2 protein expression levels were(61.9 ± 2.9) %,(21.4 ± 3.2) %,(46.8 ± 3.5) % and (49.7 ± 3.1) %,and caspase-3 protein expression levels were(38.1±4.5) %,(72.8±5.7) %,(47.2±5.1) % and(39.8 ±4.6)% in Sham-operated group,I/R group,trimetazidine group and IPOC group respectively.Compared with I/R group,the expression of Bcl-2 protein was increased obviously,while the expression of easpase-3 protein was decreased in trimetazidine group and IPOC group.But there were no significant differences in the protein levels of Bcl-2 and caspase-3 between trimetazidine group and IPOC group(both P>0.05).Conclusions Trimetazidine postconditoning can inhibit the myocardial cell apoptosis in acute ischemia-reperfusion injury in rats,and has a protective effect on myocardial cells.
5.Protective effects of panax quinquefolium 20s-protopanaxtriol saponins on ventricular remodeling in rats with pressure overloaded hypertrophic myocardium

ObjectiveTo investigate the protective effects of panax quinquefolium 20s-protopanaxtriol saponins (PQTS) on ventricular remodeling in rats with pressure overloaded hypertrophic myocardium.Methods Wister rats were randomly divided into operation,model,positive captopril,and low,moderate,high PQTS groups.The model of pressure overload-induced ventricular remodeling was established through the method of rat's abdominal aorta deligation.After 6 weeks of PQTS treatment ( 12.5,25.0 and 50.0 mg · kg-1 · d-1,i.p),myocardial morphological and hemodynamic parameters were determined.The content of malondialdehyde (MDA) and activity of superoxide dismutase (SOD)in serum,and the concentrations of prostacycline (PGI2),thromboxane A2 (TXA2),endothelium (ET) and angiotensin Ⅱ( Ang Ⅱ ) in plasma were also determined.ResultsCompared with remodeling group,PQTS could inhibit myocardial pathological changes,decrease significantly ventricular weight and cardiac coefficient,increase significantly systolic blood pressure,diastolic blood pressure,mean arterial pressure,left ventricular systolic pressure and the maximum left ventricular pressure rising and dropping rates(dp/dtmax),reduce the heart rate and left ventricular end-diastolic pressure in ventricular remodeling rats.PQTS could also decrease the content of MDA and enhance significantly activity of SOD in serum.In addition,PQTS could decline the contents of ET,Ang Ⅱ and TXA2 in plasma while increase significantly the content of PGI2 in plasma and PGI2/TXA2 ratio(P<0.05 or P<0.01).ConclusionsPQTS has protective effects on ventricular remodeling through improving systolic and diastolic function in ventricular remodeling rats,increasing anti-oxidase activity,reducing the damage of free radicals and vasoactive substance onmyocardium,and correcting disequilibrium of PGI2/TXA2 in ventricular remodeling rats.
6.Comparative study of woman false positive rate in exercise treadmill before or after menopause

Objective To analyse the rate of coincidence to suspected coronary artery disease (CAD) patient between positive exercise treadmill testing (TET) and coronary angiography (CAG) test.Method Two hundred and sixty-eight chest pain patients positive in TET were performed CAG test,coronary stenosis exceed 50% was diagnosed CAD. Results One hundred and seventy-six patients (65.67%) was diagnosed by CAG, 105 male and 71 female in them. Ninety-two (34.33%) patients was negative in CAG test. The false positive rate of TET between post-menopause patients (30.12%, 25/83 ) andmale patients (28.08%, 41/146) was no significant difference(P > 0.05 ); the difference of false positive rate between pro-menopause patients (66.67%, 26/39) and male patients (28.08% ,41/146) was statistical significance (P < 0.01 ). Conclusions The positive rate of CAG can be greatly improved by screening the chest pain patients though TET before CAG, if the positive in TET performed CAG only. TET false positive rate of pro-menopause female is significantly higher than post-menopause female or male.
7.A novel compound isolated from the peels of Citrus changshan-huyou Y.B.Chang

Five compounds, huyouyisu (1), vomifoliol (2), isoferulic acid (3), 1,2,3-trihydroxy-phenol (4) and p-hydroxy-phenol (5), were isolated from the peels of Citrus changshan-huyou Y.B.Chang for the first time by utilizing repeated column chromatography on silica gel. Among them, huyouyisu (1) is a new compound. The structure was elucidated by using 1D and 2D spectra.
8.Mechanism of action of PQDS on anti-ventricular remodeling in rats

Objective To observe the mechanism of action of panax quinquefolium diolsaponins (PQDS) on anti-ventricular remodeling in rats. Methods The model of pressure-loading ventricular remodeling was established through the method of rat’s abdominal aorta deligation. The male Wistar rats were divided into 5 groups,including sham operation group,remodeling model group,Benazepril group,and low and high dose groups of PQDS. The rats were treated with PQDS (with dose of 50 and 100 mg?kg-1 i.g) for 6 weeks. The rats in sham operation group and remodeling model group were treated with normal sodium (with dose of 10 mL?kg-1?d-1 i.g) for 6 weeks. The rats in masculine medicine group were treated by Benazepril (with dose of 10 mg?kg-1 i.g) for 6 weeks. After 6 weeks of treatment,myocardial morphological parameters,the content of malondialdehyde (MDA) and activity of superoxide dismutase (SOD) in serum,the contents of prostacycline (PGI2),thromboxane A2 (TXA2),nitric oxide (NO) and angiotensinⅡ (AngⅡ) in plasma were determined. At the same time,the contents of endothelium (ET) in plasma and myocardium were also determined. Results Compared with remodeling model group,the ventricular weight and cardiac coefficient were decreased significantly in the rats treated with PQDS (P0.05). Conclusion PQDS has protective effects on ventricular remodeling through increasing anti-oxidase activity,reducing the damage of free radicals and vasoactive substance on myocardium and correcting disequilibrium of PGI2/TXA2 in ventricular remodeling etc.
9.Isolation and identification of chemical constituents from peels of Citrus changshan-huyou Y.B.Chang

Objective:To study the chemical constituents of the peels collected from Citrus changshan-huyou Y.B.Chang,and further screen the bioactive components as the lead structures.Methods: These compounds were isolated by repeated flash column chromatography on silica gel and Sephadex LH-20.The structures of isolated compounds were elucidated by using IR,EIMS,and NMR analyses.Results: One novel compound along with nine known compounds were obtained and identified as huyoujiasu(Ⅰ),3-hydroxy-4-methoxybenzoic acid(Ⅱ),3,4-dihydroxybenzoic acid(Ⅲ),fatty acid(Ⅳ),gly-ceride(Ⅴ),6,7-dimethoxycoumarin(Ⅵ),6′,7′-dihydroxybergamottin(Ⅶ),daucossterol A(Ⅷ),daucossterol B(Ⅸ) and huyou-triterpenoid(Ⅹ).Conclusion: One new compound,Huyoujiasu,was obtained from these peels by repeated column chromatography,and the other known compounds were isolated from this fruit for the first time as well.
