OBJECTIVE

This study aims to determine the clinical profile and incidence of in-hospital outcomes of patients who underwent coronary artery bypass graft surgery (CABG) under the Philippine Health Insurance Corporation (Philhealth) Z Benefit Package (PZBP).

METHODS

A retrospective descriptive cohort study. A review of medical records was done from July 2017 to October 2023 to collect data and in-hospital outcomes of patients who underwent CABG surgery under Z benefit package.

RESULTS

One hundred twenty-six patients were included in the study. The mean age of patients was 60 years and majority of them were male and came from NCR. Hypertension was the leading cause of comorbidity. The top two most common in-hospital outcomes were hospital-acquired pneumonia and postoperative atrial fibrillation. Most commonly observed caused of increased length in hospitalization was pneumonia. The mean Society of Thoracic Surgeons (STS) (operative mortality) and Euro scores were 1.04 ± 0.82 and 0.88 ± 0.56, respectively. All patients had successful surgical outcome with no mortality and an average length of stay in the hospital of six days.

CONCLUSIONS

This study will serve as a future reference to enhance screening criteria and improved in-hospital outcomes for those who will undergo CABG under Z benefit package. The study also showed insights on clinical profile and in-hospital outcomes of patients who underwent CABG in our own institution and this may give way to a larger scale of study involving multiple centers who also offer the said program.

BACKGROUND: The pattern of the contrast media-induced adverse reactions has not been investigated extensively in Mexico. OBJECTIVE: To estimate the incidence and the degree of severity of the adverse reactions to contrast media, administered for the first time, in hospitalized subjects. METHODS: We studied 99 patients longitudinally on whom computed tomography with contrast media (iopamidol) was carried out. The adverse reactions were identified by clinical examination; subsequently, they were classified as mild, moderate and severe, following the Manual on Contrast Media version 9 guides, and as immediate and nonimmediate. In addition, the vital functions, oxygen saturation, serum creatinine levels and the total number of eosinophils were measured before and after the procedure. RESULTS: The incidence of immediate and nonimmediate adverse reactions was of 26.3% and 10.1%, respectively. The mild immediate reactions were 18 (69.2%), the most common being the sensation of warmth, nausea and pruritus; among the more delayed reactions, nephrotoxicity stood out (5.1%). The serum creatinine median showed no difference either before or after the intravenous injection of contrast media (p = 0.13); in contrast, there was a significant difference in the total number of eosinophils (p ≤ 0.001). The values of high baseline systolic blood pressure and the diminished baseline amounts in pulse oximetry were significantly related with any type of the adverse reactions to contrast media. CONCLUSION: The incidence of the adverse reactions to contrast media was greater with respect to previous reports; the majority of these reactions were of the immediate type and of a mild nature. The risk factors that have mostly been implicated in the adverse reactions to contrast media could not be identified in our cohort.

OBJECTIVE: This study determined the economic burden for nonfatal uncomplicated acute coronary syndrome (ACS) using 100% compliance to certain a) non-invasive or b) invasive and non-invasive diagnostic and therapeutic interventions with class I recommendations in the American College of Cardiology-American Heart Association (ACC-AHA) clinical practice guidelines for ACS in three tertiary hospitals using the societal perspective. It also determined the costs using the patient perspective in the setting of one private tertiary hospital.

METHODS: This study was a cost analysis that included a) costs of patient resources, b) production losses, and c) costs of other resources or sectors, from hospitalization to one month post-discharge for ACS. Several models were constructed due to variations in the costs of diagnostic and therapeutic interventions in the three settings.

RESULTS: Using the societal perspective, one model for non-invasive options yielded the following (costs as of January 31, 2009): hospital A, Php87,014 - 124,799; hospital B, Php75,592 - 96,072; hospital C, Php71,969 - 92,148. Excluding fibrinolytic therapy, the lowest total cost would be Php65,000. However, if coronary angiography was added to the models for hospital C, the cost was Php107,154 - 134,574 (coronary angiography was not available in hospitals A and B). Using the patient perspective, the adjusted mean cost for the model which used the least expensive medication was Php96,421 (Standard Deviation = 34,076).

CONCLUSION: The economic burden for nonfatal uncomplicated ACS may range from Php65,000 - 134,574.

Objective: The objectives of this study are: 1) to determine the prevalence of 3 atherosclerotic diseases (angina, stroke and claudication) in the Philippines; and 2) to determine the prevalence of 5 risk factors for these diseases (dyslipedemia, diabetes, hypertension, obesity and smoking). Methodology: Using a multi-staged cluster sampling methodology, this study evaluated 4,753 adults aged 20 or more, from 2,636 households, 79 provinces and 17 regions. The survey instruments included validated questionnaires, anthropometric measurements, and blood tests. Results: The prevalence rates of angina, stroke and claudication in adult Filipinos were 12.5 percent, 1.9 percent, and 4.2 percent respectively. The prevalence rates of various risk factors were 8.5 percent for hypercholesterolemia (total cholesterol > 240mg/dl), 4.6 percent for diabetes (based on fasting blood sugar >125mg/dl or a previous history of diabetes), and 17.4 percent for hypertension (BP >140/90 mm Hg on 2 visits, or a previous history of hypertension). The prevalence of obesity was 3.2 percent in men and 6.6 percent in women when body mass index (BMI) was used. These rates rose to 12.1 percent and 54.8 percent respectively when waist-hip ratio (WHR) was used. The prevalence of current smoking was 56.3 percent in men, 12.1 percent in women, and 34.8 percent overall. Smoking, by far, was the most common risk factor noted. Discussion: These data on atherosclerosis-related diseases and risk factors from the NNHeS provide clinicians and policy-makers a bird's eye view of the Philippine situation with respect to vascular disease and its risk factors. These data may be used for policy formulation and program prioritization. In addition, the information may serve as baseline measures to evaluate the success or failure of atherosclerosis control programs in the country. A 2nd NNHeS is planned in the year 2008.
