1.Association Between MTHFR C677T Gene Polymorphism and Hypertension, Hyperhomocysteinemia and Hyperlipidemia in Tibet Region

To explore the correlation between MTHFR C677T gene polymorphism and hypertension, hyperhomocysteinemia(Hcy), and hyperlipidemia in the Tibetan population of Tibet. Using a cluster sampling method, participants from high-altitude regions including Ngari Prefecture, Lhasa City, and Nyingchi City in Tibet were enrolled. Differences in MTHFR C677T genotype distribution among individuals with hypertension, HHcy, and hyperlipidemia were analyzed, and multivariate logistic regression was performed to assess the association between these conditions and the TT genotype. A total of 574 eligible subjects were included, with a mean age of 40.64±12.67 years. Males accounted for 46.7%(268/574) and females 53.3%(306/574). Regional distribution was 34.8%(200/574) from Nyingchi City, 33.1%(190/574) from Lhasa City, and 32.1%(184/574) from Ngari Prefecture. Mean systolic and diastolic blood pressures were 117.89±18.98 mm Hg and 79.74±14.88 mm Hg, respectively. The frequency of the TT genotype was significantly higher in the hypertension group than in the non-hypertension group(12.32% The MTHFR C677T TT genotype is significantly associated with hypertension and hyperhomocysteinemia in the Tibetan population, suggesting that this polymorphism may be a genetic risk factor for these diseases in high-altitude regions.
2.Analysis of risk factors in patients with nonvalvular persistent atrial fibrillation complicated with ventricular hypertrophy and construction and validation of prediction model

ObjectiveTo construct a nomogram prediction model for non-valvular persistent atrial fibrillation (PeAF) patients with left ventricular hypertrophy (LVH) , followed by prognostic analysis through follow-up. MethodsThis study retrospectively enrolled 949 patients with newly diagnosed and hospitalized non-valvular PeAF. Among them, 403 patients presented with LVH. The cohort was randomly stratified into a training set (n=665) and a validation set (n=284). Univariate and multivariate Logistic regression analyses were employed to identify independent risk factors for PeAF complicated by LVH. A nomogram prediction model was subsequently constructed and evaluated for discriminative ability, calibration, and clinical utility using receiver operating characteristic (ROC) curve analysis, calibration plots, and decision curve analysis (DCA). ResultsSeven independent risk factors were ultimately identified and included in the prediction model: female sex, hypertension, diabetes, red blood cell distribution width-SD (RDW-SD), body mass index (BMI), left atrial diameter (LAD), and left ventricular ejection fraction (LVEF). The area under the ROC curve (AUC) in the training set was 0.862 (95% CI: 0.834-0.890), and in the validation set, it was 0.870 (95% CI: 0.829-0.911), demonstrating excellent predictive performance. ConclusionIndependent risk factors for LVH in PeAF patients include female, hypertension, diabetes, RDW-SD, BMI, LAD, and LVEF. The prediction model built based on this can help early identification of PeAF patients with high risk of LVH. At the same time, the incidence of major adverse cardiovascular events (MACE) is higher in PeAF patients with LVH. Patients with atrial fibrillation combined with LVH may benefit from catheter ablation.

This paper summarizes Professor WANG Xiuxia's clinical experience in treating hyperprolactinemia using the liver soothing therapy. Professor WANG identifies liver qi stagnation and rebellious chong qi (冲气) as the core pathomechanisms of hyperprolactinemia. Furthermore, liver qi stagnation may transform into fire or lead to pathological changes such as spleen deficiency with phlegm obstruction or kidney deficiency with essence depletion. The treatment strategy centers on soothing the liver, with a modified version of Qinggan Jieyu Decoction (清肝解郁汤) as the base formula. Depending on different syndrome patterns such as liver stagnation transforming into fire, liver stagnation with spleen deficiency, or liver stagnation with kidney deficiency, heat clearing, spleen strengthening, or kidney tonifying herbs are added accordingly. In addition, three paired herb combinations are commonly used for symptom specific treatment, Danggui (Angelica sinensis) with Chuanxiong (Ligusticum chuanxiong), Zelan (Lycopus lucidus) with Yimucao (Leonurus japonicus) , and Jiegeng (Platycodon grandiflorus) with Zisu (Perilla frutescens).
4.Effects of long working hours and shift work on the mental health of community medical workers

Objective To explore the association of working hours and shift work with occupational stress, fatigue accumulation, and depressive symptoms among primary community medical workers. Methods A total of 516 medical workers from five community medical service centers in Pudong New Area, Shanghai City, were selected as the research subjects using the convenience sampling method. The Core Scale of Occupational Stress Measurement, the Workers' Fatigue Accumulation Self-diagnosis Questionnaire, and the Patient Health Questionnaire were used to assess research subjects' occupational stress, fatigue accumulation, and depressive symptoms, respectively. Results Long working hours (>40 hours/week) were reported by 50.4% of workers among the research subjects, while shift works were reported by 16.9% of the workers. The detection rates of occupational stress, fatigue accumulation, and depressive symptoms were 26.6%, 41.7%, and 30.8%, respectively. Multivariate logistic regression analysis result revealed that, after adjusting for confounders such as age, sex, and education level, longer working hours were associated with higher risks of occupational stress, fatigue accumulation, and depressive symptoms (all P<0.05). Shift workers in community medical centers had higher risks of occupational stress, fatigue accumulation, and depressive symptoms compared with non-shift workers (all P<0.05). Conclusion Long working hours and shift work could increase the risks of occupational stress, fatigue accumulation, and depressive symptoms among community medical workers.
5.MRI subtraction technique for evaluating efficacy of systemic therapy for advanced hepatocellular carcinoma and predicting prognosis after combining with surgery

Objective To explore the value of MRI subtraction technique(ST)for evaluating the efficacy of systemic therapy for advanced hepatocellular carcinoma(HCC)and predicting prognosis after combining with surgery.Methods Totally 35 patients with 39 HCC lesions who received systemic therapy+radical resection were retrospectively collected.Based on preoperative MRI,tumor activity ratio(recorded as tumor activityST)was obtained with ST,while tumor activity value(recorded as tumor activitypathology)was obtained through postoperative pathology,and their correlation was analyzed.The patients were regularly followed up after surgery,and the survival data were recorded.Receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of tumor activityST for predicting patients'survival status.Then the patients were divided into survival benefit group and no survival benefit group according to the cut-off value,and survival analysis was conducted.Results Tumor activityST was positively correlated with tumor activitypathology(r=0.900,P<0.001).The median follow-up time was 32.93 months,during which 8 patients died,and the median survival time was 29.9 months.The area under the curve(AUC)of tumor activityST for predicting patients'survival status was 0.67,and the cut-off value was 0.36.Thirty patients with tumor activityST<0.36 were enrolled in survival benefit group,while 5 patients≥0.36 were collected in no survival benefit group.The overall survival in survival benefit group was longer than that in no survival benefit group(P<0.001).Conclusion MRI ST could be used to non-invasively evaluate the efficacy of systemic therapy for advanced HCC and predict prognosis after combining with surgery.
6.Effect of N-methyl-D-aspartic acid receptor in secondary visual cortex on methamphetamine-associated contextual learning and memory

OBJECTIVE To investigate the effect of N-methyl-D-aspartic acid(NMDA)receptor in the secondary visual cortex(V2)on methamphetamine-associated contextual learning and memory.METHODS With male C57BL/6J mice as subjects and using the mouse conditioned place preference(CPP)experiment,the scores of CPP were observed after microinjection of NMDA receptor selective antagonist D-AP5(0.5 μg per side)into the bilateral V2 during the formation phase,single microinjec-tion of D-AP5(0.5 μg per side)into the bilateral V2 prior to the expression test,and methamphetamine(0.5 mg·kg-1,ip)-induced reactivation test and methamphetamine-associated contextual-induced reacti-vation test,to evaluate the effect of NMDA receptors on the formation,expression and reinstatement of methamphetamine-induced CPP.RESULTS Compared with the control group,microinjection of D-AP5(0.5 μg per side)into the bilateral V2 during the formation phase did not have significant inhibitory effect on CPP scores,nor did single microinjection of D-AP5(0.5 μg per side)into the bilateral V2 prior to the expression test,single microinjection of D-AP5(0.5 μg per side)into the bilateral V2 prior to metham-phetamine(0.5 mg·kg-1,ip)-induced reactivation test or single microinjection of D-AP5(0.5 μg per side)into the bilateral V2 prior to methamphetamine-associated contextual-induced reactivation test.CONCLU-SION The NMDA receptor in the V2 is not involved in the formation,expression and reactivation of meth-amphetamine-associated contextual learning and memory.
7.Experience of treatment of HIV infection complicated with gastrointestinal complicationsbased on Li Fazhi syndrome differentiation and treatment

The core pathology of human immunodeficiency virus(HIV)is progressive impairment of CD4+T lym-phocyte function.Since the digestive tract is an important target organ of HIV infection,it is susceptible to oppor-tunistic infections.Although highly active antiretroviral therapy(HAART)can effectively inhibit viral replication and reduce the risk of infection,long-term use of HAART is prone to cause adverse reactions of the digestive tract,further weaken the patient's immune function,leading to a vicious circle of"virus invagination-immune im-balance-reinfection".Based on the theory of"integrating disease and syndrome differentiation with the ability to distinguish cold and heat",Professor Li Fazhi proposes that the essence of HIV infection complicated with gastro-intestinal complications is"the inversion of epidemic virus and the stagnation of qi apparatus",and emphases the importance of"through"to reconcile the deficiency and excess of cold and heat.This article summarizes the clinical pathway of treating HIV-infected digestive tract complications(such as infectious diarrhea,fungal esophagitis,etc.)with the use of meridian prescription from the perspective of infection immunology.
8.Stenotrophomonas maltophilia induces RAW 264.7 inflammation by activating ferroptosis

The objective of this study was to investigate whether Stenotrophomonas maltophilia(S.maltophilia)induces ferroptosis,a form of iron-dependent cell death,leading to an inflamma-tory response in RAW 264.7 macrophages by elevating oxidative stress levels.RAW 264.7 cells were stimulated with varying concentrations of S.maltophilia.The concentrations of TNF-αand IL-1β were quantified using ELISA kits to assess the impact of S.maltophilia on the inflammatory response in RAW 264.7 cells.The activities of glutathione(GSH)and malondialdehyde(MDA)levels were measured using GSH and MDA assay kits to evaluate changes in oxidative stress.West-ern blot analysis was employed to detect the expression levels of COX-2,xCT,GPX4,and other proteins involved in ferroptosis signaling pathways,thereby investigating the effect of S.malto-philia on ferroptosis in RAW 264.7 cells.The results demonstrated that S.maltophilia induced concentration-dependent increases in inflammation and oxidative stress in RAW 264.7 cells,up-regulated the expression of COX-2 protein and down-regulated the expression of xCT and GPX4.Pretreatment with the ROS inhibitor N-acetylcysteine(NAC)significantly mitigated the S.malto-philia-induced oxidative stress and ferroptosis signaling activation,thereby alleviating the inflam-matory response.Furthermore,treatment with the ferroptosis inhibitor Fer-1 directly suppressed the activation of the ferroptosis signaling pathway and reversed the inflammation induced by S.maltophilia.These findings suggest that S.maltophilia triggers inflammation in RAW 264.7 cells by activating the ferroptosis signaling pathway via an increase in oxidative stress levels.

The acquired immune deficiency syndrome(AIDS)-induced fever is mainly characterized by long-term,recurrent and irregular symptom,which poses a major threat to the quality of life of the AIDS patients.Tradition-al Chinese Medicine(TCM)has offered an approach of treatment determination based on syndrome differentiation and formula corresponding to syndrome on basis of the idea of'same disease,different therapies'.The earliest or-igin of the idea of'same disease,different therapies' can be traced back to'Huang Di Nei Jing',it plays a signifi-cant role in the theory of TCM after continuous enrichment and development by ancient medical families.The indi-vidualized therapeutic regimen is formulated based on the etiology,pathogenesis and physical condition(such as qi-insufficiency constitution,phlegm-dampness constitution),emphasizing the vital role of'treatment determina-tion based on syndrome differentiation'in the treatment process,highlighting the importance of personnel experi-ence of doctors,and showing the precise effect of TCM on treatment of AIDS-induced fever.
10.Investigation on the teaching effect and demand of Public Health Skills Training course

Objective:To understand the teaching effect and demand of the graduating students of the School of Public Health on the Public Health Skills Training course.Methods:A questionnaire survey was conducted among students of grade 2018 majoring in Preventive Medicine and Maternal and Child Health Care Medicine and students of grade 2019 majoring in Food Hygiene and Nutrition and Health Inspection and Quarantine of Anhui Medical University.Multivariate Logistic regression analysis was used to explore the influencing factors of course effectiveness and demand.Results:Among the 212 students who participated in the survey,89.62%of the students said that they could master the learning of Public Health Skills Training course,and 84.91%of the students believed that it had practical application significance for their future work and life.The results of multivariate Logistic regression analysis showed that students who were satisfied with the curriculum(compared with dissatisfaction/average)and thought that the curriculum difficulty was average/easy(compared with difficult)had higher curriculum mastery.Students who thought that the time of the course should be moved backward(compared with the appropriate time)and had no intention to participate in the National Public Health Skills Competition(compared with had intention to participate)for college students thought that the study of the course had no practical application significance for their future work and life.Conclusion:Strengthen the teaching of public health skills practical training courses,improve students'satisfaction with the courses,and find teaching methods suitable for students'understanding can improve students'mastery of the courses and help students better improve their practical and analytical abilities.
