1.Banxia Xiexin Tang Ameliorates Cognitive Dysfunction in Rat Model of Vascular Dementia via AGE/RAGE Pathway

ObjectiveTo explore the mechanism by which Banxia Xiexin Tang (BXT) regulates the advanced glycation end products (AGE)/receptor for advanced glycation end products (RAGE) signaling pathway to reduce neuroinflammatory responses and ameliorate cognitive dysfunction in the rat model of vascular dementia (VD). MethodsThe components of BXT were detected by ultra performance liquid chromatography-quadrupole -orbitrap-tandem mass spectrometry(UPLC-Q-Orbitrap-MS/MS), and the core components and key action pathways were screened out by network pharmacology and molecular docking. Sixty SPF-grade male SD rats were randomly allocated into the sham and modeling groups by the random number table method. The VD model was replicated by the modified bilateral occlusion of the common carotid arteries (2-VO) method. The successfully modeled rats were randomly allocated into the model, low-, medium-, and high-dose (3.748 5, 7.497, 14.994 g·kg-1) BXT (BXT-L, BXT-M, and BXT-H), and nimodipine (NMP, 0.002 7 g·kg-1) groups according to the random number table method. The rats in the drug intervention groups were administrated with corresponding drugs by gavage, and the sham and model groups received the same amount of normal saline for 14 consecutive days. The Morris water maze, Y-maze, and new object recognition experiments were conducted to evaluate the cognitive dysfunction of rats. Hematoxylin-eosin (HE) staining was used to evaluate the histopathological changes of the hippocampal tissue in rats. The mRNA levels of AGE, RAGE, and phosphorylated nuclear factor-kappa B p65 (p-NF-κB p65) in the hippocampal tissue of rats were determined by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). The expression of related proteins in the AGE/RAGE pathway in the hippocampal tissue of rats was determined by Western blot and immunohistochemistry (IHC). The levels of neurotransmitters and inflammatory mediators in the rat serum were measured by enzyme-linked immunosorbent assay (ELISA). ResultsThe chemical components of BXT were detected by UPLC-Q-Orbitrap-MS/MS. Network pharmacology and molecular docking identified the AGE/RAGE pathway as the key pathway. The results of the water maze, Y maze, and novel object recognition tests showed that compared with the sham group, the model group demonstrated prolonged successful latency and decreases in number of platform crossings, alternation rate, number of entries into the new arm, preference index, and discrimination index (P0.01). Compared with the model group, the BXT-H and BXT-M groups showed shortened successful latency (P0.01) and increases in number of platform crossings (P0.05), alternation rate (P0.01), number of entries into the new arm (P0.05), preference index (P0.01), and discrimination index (P0.01). HE results showed that compared with the sham group, the cells of model rats were loosely and disorderly arranged, and the nuclei were condensed. Compared with the model group, the pathological changes of the hippocampus in the BXT group were mitigated. Real-time PCR results showed that compared with the sham group, the model group presented up-regulated mRNA levels of AGE, RAGE, and p-NF-κB p65 in the hippocampus (P0.01), and compared with the model group, the BXT-H and BXT-M groups showcased down-regulated mRNA levels of AGE, RAGE, and p-NF-κB p65 (P0.01). Western blot results showed that compared with the sham group, the model group presented up-regulated expression of AGE, RAGE, p-NF-κB p65, and tumor necrosis factor-α (TNF-α) (P0.05), and compared with the model group, the BXT-H group presented down-regulated expression of AGE, RAGE, p-NF-κB p65, and TNF-α (P0.05). IHC results showed that compared with the sham group, the model group had increased expression of RAGE (P0.01), and compared with the model group, the BXT-H and BXT-M groups had reduced expression of RAGE (P0.01). ELISA results showed that compared with the sham group, the model group exhibited elevated levels of TNF-α and Interleukin-1β (IL-1β) and declined levels of acetylcholine (ACh) and dopamine (DA) in the serum (P0.01). Compared with the model group, the BXT-L, BXT-M, and BXT-H groups showed lowered levels of TNF-α and IL-1β in the serum (P0.05) and elevated levels of ACh and DA (P0.05). ConclusionBXT may ameliorate cognitive dysfunction in the rat model of VD by down-regulating the AGE/RAGE signaling pathway, reducing neuroinflammatory responses, and regulating neurotransmitter levels.

3.Factors affecting benefit finding among young and middle-aged patients with type 2 diabetes mellitus

Objective:
To investigate the current status of benefit finding among young and middle-aged patients with type 2 diabetes mellitus (T2DM) and analyze its influencing factors, so as to provide a reference for improving the level of benefit finding in this population.
Methods:
From November 2022 to May 2023, young and middle-aged patients with T2DM aged 18-59 years hospitalized in the endocrinology departments of 2 tertiary hospitals in Hengyang City, Hunan Province were selected as survey subjects by a convenience sampling method. Basic demographic information was collected using a general questionnaire survey. Benefit finding, resourcefulness, and stigma were evaluated using the Benefit Finding Scale, the Chinese Version of the Resourcefulness Scale, and the Type 2 Diabetes Stigma Assessment Scale, respectively. A multiple linear regression model was used to analyze the influencing factors of benefit finding among young and middle-aged patients with T2DM.
Results:
A total of 305 young and middle-aged patients with T2DM were investigated, including 222 males (72.79%) and 83 females (27.21%). There were 231 cases aged 45-59 years, accounting for 75.74%. The scores for benefit finding, resourcefulness, and stigma were (42.86±6.06), (75.12±11.30), and (41.20±10.10), respectively. Multiple linear regression analysis showed that young and middle-aged patients with T2DM who were male (β′=0.088), aged 18-<45 years (β′=0.083), absence of diabetes complications (β′=0.124), and had higher resourcefulness scores (β′=0.679) had higher levels of benefit finding, while patients with higher stigma scores (β′=-0.097) had lower levels of benefit finding.
Conclusion
The level of benefit finding among young and middle-aged patients with T2DM was moderate, and was related to gender, age, diabetes complications, resourcefulness, and stigma.
4.Membranous nephropathy with light chain-restricted deposition secondary to small lymphocytic lymphoma: a case report

The patient was a 56-year-old man who presented with abnormal urinalysis findings of over one month’s duration. Serological tests revealed an elevated serum free light chain κ/λ ratio, and immunofixation electrophoresis detected an IgG-κ monoclonal band. Renal biopsy demonstrated membranous nephropathy, with electron-dense deposits observed in the subepithelial space and mesangium on electron microscopy. Immunofluorescence confirmed glomerular deposits of IgG1/IgG3-κ. Based on clinical manifestation, bone marrow flow cytology, and lymph node pathology, the patient was diagnosed with small lymphocytic lymphoma (SLL), and membranous nephropathy with light chain-restricted deposition (IgG1/IgG3-κ) secondary to SLL. After receiving obinutuzumab treatment, the patient’s proteinuria partially remitted, the lymph nodes significantly shrank, and clinical remission was achieved.
5.Application of grading activities in total hip arthroplasty patients

Objective To explore the intervention effect of grading activities on postoperative delirium in patients undergoing total hip arthroplasty(THA).Methods A total of 100 patients who underwent THA at the Third Affiliated Hospital of Sun Yat-sen University from January 2024 to January 2025 were selected as subjects.Using a randomized digital table method,patients were divided into intervention group(n=50)and control group(n=50).The control group received standard postoperative care,while intervention group began implementing grading activities program immediately upon returning to the ward.Delirium incidence,Harris hip function scale(HHS)score,Pittsburgh sleep quality index,average length of hospital stay,average hospitalization costs,and patient satisfaction were compared between two groups.Results Postoperative delirium in intervention group was significantly lower than that in control group(P<0.05).There were statistically significant differences in HHS,pain scores,and joint function scores between two groups.Sleep quality and higher satisfaction rates were better in intervention group than those in control group,while average hospital stays and total hospitalization costs were lower in intervention group than those in control group with statistically significant(P<0.05).Conclusion Early grading activities can prevent the occurrence of postoperative delirium in patients with THA,reduce postoperative pain,improve joint function and sleep quality,and effectively improve patient satisfaction.
6.Develop a risk assessment system for unscheduled rehospitalisation within 3 months in patients of oral endrotracheal intubation

8.Risk factor analysis of hypertrophic cardiomyopathy with atrial fibrillation based on cardiac magnetic resonance

Objective To investigate the independent risk factors for hypertrophic cardiomyopathy(HCM)with atrial fibrillation(AF)based on cardiac magnetic resonance(CMR)using logistic regression analysis.Methods We reviewed 80 patients diagnosed with HCM at our hospital between January 2022 and December 2023.Statistical differences in the CMR and clinical parameters between patients with HCM with and without AF were compared.The cut-off value of HCM with AF was obtained by receiver operator characteristic curve,and binary logistic regression analysis was performed on statistically significant variables to identify independent risk factors for HCM in patients with AF.Results Univariate analysis showed that there were significant differences in the type of left ventricular late gadolinium enhancement(LGE),native T1 mapping value of the left ventricular myocardium without LGE,left atrial anteroposterior diameter,number of left ventricular LGE myocardial segments,and LGE in the basal anterior interventricular septum,mid anterior interventricular septum,and mid inferior interventricular septum were between HCM with and without AF(P<0.05).Multivariate analysis revealed that there were significant differences in left ventricular subendocardial LGE(P=0.048,OR=5.3,95%CI:0.642-43.311),native T1 mapping value of left ventricular myocardium without LGE≥1 247 ms(P=0.03,OR=5.7,95%CI:0.734-27.41),and left atrium anteroposterior diameter 50 mm(P=0.013,OR=6.9,95%CI:1.489-31.538)between HCM with and without AF.Conclusion Left ventricular subendocardial LGE,native T1 mapping value≥1 247 ms,and left atrium anteroposterior diameter≥50 mm are independent risk factors for HCM with AF.

To analyze the standard establishment approach and compilation rationale for metallic pharmaceutical packaging standard development in the 2025 edition of the Pharmacopeia of the People's Republic of China.This article systematically explained the background and process of establishing the guiding principles for metallic materials and containers used in pharmaceutical packaging in the Chinese Pharmacopoeia through basic information,relevant domestic and international standards,the establishment of key quality attributes of metallic pharmaceutical packaging materials,and the construction of metallic pharmaceutical packaging material standards.The newly established guidelines,the Pharmacopeia of the People's Republic of China 9625,prioritized product critical quality attributes(CQAs)and real-world applicability.This dual emphasis on rigidity and adaptability enhances drug safety,meets the regulatory requirements,and promotes the globalization and scientific advancement of China's pharmaceutical packaging industry.
10.Analysis of the safe threshold of surgical margin width after conversion therapy for initially unresectable hepatocellular carcinoma

Background and Aims:Conversion therapy offers initially unresectable hepatocellular carcinoma(HCC)patients a chance for curative resection.However,the optimal margin width following conversion remains unclear.This study aimed to evaluate the impact of surgical margin width on prognosis and identify independent prognostic factors in HCC patients undergoing hepatectomy after conversion therapy.Methods:A retrospective analysis was performed on 413 patients with initially unresectable HCC who received conversion therapy and underwent radical resection at Cancer Prevention and Control Center of Sun Yat-sen University between February 2015 and June 2022.According to the intraoperatively measured pathological margin,patients were classified into two groups:tumor margin<1 cm and≥1 cm,and further divided into subgroups with margins of 0 cm,0.1 cm,and>0.1 cm to compare survival differences among groups.The Kaplan-Meier method and Cox proportional hazards model were used to evaluate disease-free survival(DFS),overall survival(OS),and their influencing factors.Results:The 3-year OS and DFS showed no significant difference between the<1 cm and≥1 cm groups(both P>0.05).However,patients with a 0 cm margin had significantly worse OS than those with a 0.1 cm margin(P=0.048).No significant survival difference was observed in OS and DFS between the 0.1 cm and>0.1 cm groups(both P>0.05).Multivariate analysis identified multiple tumors,poor differentiation,and microvascular invasion as independent adverse prognostic factors for both OS and DFS(all P<0.05),whereas targeted therapy was an independent protective factor for DFS(P=0.014).Conclusion:A pathological margin≥0.1 cm provides comparable survival to wider margins and can be considered a safe threshold for HCC patients undergoing hepatectomy after conversion therapy.The conventional 1 cm margin standard offers no additional benefit.Multiple tumors,poor differentiation,and microvascular invasion predict poor prognosis,while targeted and immunotherapy during conversion may improve long-term outcomes.
