1.Epidemic factors in foodborne parasitic diseases in ethnic minority areas of Guizhou Province from a One Health perspective

This study was aimed at understanding the prevalence and influencing factors of food-borne parasitic diseases in ethnic minority areas of Guizhou Province,to provide a scientific basis for the development of appropriate intervention measures based on the human-animal-environment One Health concept.In 2023,the infection status of the human population,reservoir hosts,intermediate hosts,food-borne parasitic diseases,and related social and environmental factors were investigated in Congjiang County in Qidongnan Miao and Dong Autonomous Prefecture;Luodian County in Qiannan Buyi and Miao Autonomous Prefecture;and Ceheng County in Qianxinan Buyi and Miao Autonomous Prefecture.At least 1 000 individuals were sampled from each county,along with at least 50 insect-protected host samples from each location.Food-borne parasite infections were detected with the modified Kato thick smear method.A questionnaire survey was administered to the population.Detection of food-borne parasitic metacercariae was performed in intermediate host fish through the flaking and digestion method,and in crabs through the pounding and sedimentation method.The chi-square test was used to compare rates,and logistic regression was applied for multivariate analysis.A total of 3 023 questionnaires and fecal samples were collected.Males accounted for 47.50%,females accounted for 52.50%,and members of ethnic minorities accounted for 96.06%.A total of 186 food-borne parasitic infections were identified,and the infection rate was 6.15%.Five insect species were detected,which showed an infection rate of 5.39%.The infection rate of Clonorchis sinensis was 0.33%,that of Taenia was 0.40%,that of Heteroceles was 0.17%,that of Acanthus was 0.17%,and that of Echinostoma was 0.03%.Human infections with Echinostomus colloides and Echinostomia transferoris had not previously been reported in China.Single-factor analysis revealed statistically significant differences in food-borne parasite infections according to various factors,including the consumption of untreated water,raw fish and shrimp,raw pig blood,raw cow gastric juice,and raw pork and beef,as well as raw pig and cow viscera(P<0.05).Multivariate analysis indicated that the risk factors for food-borne parasite infections among residents in minority areas of Guizhou Province included the consumption of raw pig blood(OR=2.841,95%CI:1.809-4.463),raw cow gastric juice(OR=2.122,95%CI:1.297-3.469),and raw fish and shrimp(OR=1.779,95%CI:1.049-3.018).A total of 173 fecal samples of the reservoir host were examined,which showed a rate of food-borne parasite infection of 5.2%.A total of 510 intermediate host fish were examined,which showed a 4.51%positivity rate of encysted metacercaria of Clonorchis sinensis.The crab,pig,and beef samples were not positive.In conclusion,food-borne parasitic infections were prevalent in ethnic minority regions of Guizhou Province,and consumption of raw food were influencing factors.A focus on populations with raw food consumption habits,including raw pig blood,cow gastric juice,fish and shrimp,is essential.Concurrently,monitoring of animal hosts must be strengthened to perform key interventions according to the One Health concept.
2.Practice and reflection on quality control of physical data in population cohort studies

Objective To evaluate and further optimize the quality control measures in the field of queue by comparing and analyzing the quality of physical data before and after optimization.Methods 17 420 physical data collected in 2020 and 12 762 physical data collected in 2021 were used in the natural population cohort study of West China Hospital of Sichuan University.The data in 2020(before optimization)and 2021(after optimization)were evaluated and compared from multiple dimensions such as normalization,completeness,logic,accuracy and consistency.Results Compared to 2020,data quality in 2021 showed a significant improvement.The proportion of abnormal data decreased significantly from 1978(11.35%)to 276(2.16%),and the difference was statistically significant(P<0.05).Specifically,the number of abnormal blood pressure data decreased from 143(0.82%)to 67(0.52%),and waist circumference data decreased significantly from 1777(10.20%)to 113(0.89%).In terms of data integrity and logical evaluation,the performance in 2021 was also generally better than that in 2020,and the difference was also statistically significant(P<0.05).Conclusion It is very important to optimize and adjust the collection process of physical data and realize integrated information system management for improving data quality.

4.In vitro and in vivo antimicrobial effects of the antiparasitic drug tiliquinol to Enterococcus faecalis

Objective:To investigate the antimicrobial activity of the antiparasitic drug tiliquinol to Enterococcus faecalis. Methods:From 2023 to 2024, the standard Enterococcus faecalis strain ATCC 29212 and 6 clinical isolates in the Laboratory Department of the Affiliated Changsha Hospital of Xiangya School of Medicine were selected as the subject of this investigation. The sensibility of tiliquinol against E. faecalis was assessed using broth microdilution method, time-killing curves, and kirby-bauer test; the drug resistance induction ability of tiliquinol was detected by continuous induction of resistance and one-step resistance test. The antimicrobial ability of tiliquinol was determined by the biofilm combined laser confocal microscopy. Skin subcutaneous abscess model of E. faecalis infection was established to evaluate the in vivo antibacterial activity of tiliquinol. Paired t-tests and analysis of variance were used for comparisons between two groups and among multiple groups respectively. Results:The minimum inhibitory concentration (MIC) and minimum bactericidal concentration of tiliquinol against E. faecalis ATCC 29212 were both 2 mg/L. Kirby-bauer test showed obvious antimicrobial activity of tiliquinol against E. faecalis. The time-killing curves showed that the subinhibitory concentration of tiliquinol could effectively inhibit bacterial proliferation. Fifteen-day continuous treatment with tiliquinol showed no drug resistance mutation; tiliquinol treatment at 8×MIC for four hours caused reduced count of viable bacteria from (12.01±1.14) lg CFU/ml to (7.72±0.94) lg CFU/ml in ATCC 29212 stain ( t=3.64; P<0.05), while tiliquinol at 2×MIC significantly inhibited the formation of ATCC 29212 biofilm, which reduced from (1.73±0.27) to (0.18±0.14) ( t=8.77, P<0.05); tiliquinol at 4×MIC also significantly cleared the formed biofilm, which reduced from (0.52±0.03) to (0.40±0.06) ( t=3.07, P<0.05). Utilizing the skin subcutaneous abscess model revealed significant antibacterial effects of tiliquinol treatment, specifically, and compared with the control group, the viable bacterial loads in the 30 mg/kg tiliquinol treatment group decreased by more than 1 lg CFU/ml ( t=4.099, P<0.05). Conclusion:Tiliquinol exhibits substantial antibacterial efficacy against E. faecalis both in vitro and in vivo.
5.Electrocardiographic prediction parameters for life-threatening arrhythmic events in congenital long QT syndrome patients

Objective:To analyze the electrocardiogram (ECG) data of congenital long QT syndrome (LQTS) patients, and to identify the ECG parameters for prediction of life-threatening arrhythmic events (LAEs).Methods:This cohort study enrolled patients diagnosed with congenital LQTS at the Department of Cardiology, Beijing Tsinghua Changgung Hospital from September 2014 to May 2023. Baseline clinical and ECG data were collected. Patients were followed with LAEs as the primary endpoint. Based on the occurrence of LAEs, patients were divided into two groups: the event group and the event-free group. Cox regression analysis was used to identify independent predictors of LAEs in LQTS patients.Results:A total of 293 patients diagnosed with congenital LQTS were included, aged 32.5 (19.0, 41.8) years, including 201 females (68.6%). Sixty-six patients experienced LAEs and 227 patients did not. Compared to the event-free group, the event group had a younger onset age (13.0 (5.5, 20.5) years vs. 26.0 (13.0, 35.0) years), a slower heart rate (69.0 (59.5, 76.5) beats/min vs. 77.0 (67.0, 88.0) beats/min), a higher proportion with family history of sudden cardiac death (30.3% vs. 14.5%), as well as longer QT intervals (500.0 (467.0, 594.0) ms vs. 428.0 (402.0, 470.0) ms) and QTc intervals (544.0 (502.5, 589.0) ms vs. 489.0 (480.0, 504.0) ms). Additionally, the event group had higher peak T-wave alternans value (65.0 (42.5, 85.3) μV vs. 44.0 (36.0, 54.0) μV), a higher proportion of patients with documented torsades de pointes (TdP) or ventricular fibrillation (VF) on 24-hour Holter monitoring (39.3% vs. 4.9%), and higher rates of pharmacological treatment (100.0% vs. 9.7%) and device therapy or left cardiac sympathetic denervation (45.5% vs. 2.2%) (all P<0.05). Multivariate Cox regression analysis identified that the heart rate<60 beats/min ( HR=2.0, 95% CI: 1.0-3.7) and QTc interval ≥500 ms ( HR=2.9, 95% CI: 1.5-5.6) on 12-lead ECG, as well as peak T-wave alternans value ≥55.5 μV ( HR=3.2, 95% CI: 1.3-7.8) and documented TdP or VF ( HR=2.0, 95% CI: 1.1-3.7) on 24-hour Holter monitoring were independent predictors of LAEs in LQTS patients (all P<0.05). Conclusion:Heart rate <60 beats/min and QTc interval ≥500 ms on 12-lead ECG, along with peak T-wave alternans value ≥55.5 μV and documented TdP or VF on 24-hour Holter monitoring, have been identified as independent predictors of LAEs in patients with LQTS. These ECG parameters may serve as valuable early indicators of sudden cardiac death in LQTS patients.

9.The immediate brain effect of acupuncture at Fengchi in patients with posterior circulation ischemia vertigo:a resting-state functional MRI study

Objective To investigate the immediate brain effect of acupuncture at Fengchi using amplitude of low-frequency fluctuation(ALFF)and functional connectivity by the resting-state functional magnetic resonance imaging(rs-fMRI)in patients with posterior circulation ischemia vertigo(PCIV).Methods Twenty patients with PCIV were enrolled.The dizziness handicap inventory(DHI)was used to evaluate the severity of vertigo.The patients were randomly divided into acupuncture group and sham acupoint acupuncture group.Rs-fMRI scan was performed before and after acupuncture.MATLAB-based DPABI 6.1 software was used to analyze rs-fMRI data.Correlation analysis was used between the altered ALFF values and DHI scores.The regions of altered ALFF were taken as seeds to analyze functional connectivity.Results Compared with the sham acupoint acupuncture group,the increased ALFF values were mainly located on the left precuneus,left superior frontal gyrus and left caudate nucleus after acupuncture in the acupuncture group;the decreased ALFF values were mainly located on the left cerebellum and right inferior occipital gyrus.The ALFF value of the left superior frontal gyrus was negatively correlated with the DHI score(P=0.04).The increased functional connectivity was mainly found between left precuneus and the right middle frontal gyrus,the right superior frontal gyrus,the decreased functional connectivity was mainly found between left precuneus and the bilateral paracentral lobule and right cerebellum.Conclusion The ALFF value and functional connectivity are different before and after acupuncture,indicating that the vestibular network,visual and motor brain regions functional activities are changed after needling at Fengchi,which may be the brain functional basis of Fengchi for vertigo in PCIV.

Objective To establish an ideal modeling method for diarrhea predominant irritable bowel syndrome(IBS-D)with anxiely and depression in rats,and to provide a basis for the clinical study of IBS-D.Methods 60 rats were used in this study.(1)At first,20 rats were randomly divided into blank,3%acetic acid enema,4%acetic acid enema,and 5%acetic acid enema groups.After the modeling and observation period,the diarrhea status and the degree of colon injury caused by different modeling concentrations were observed by diarrhea related index and colon histopathology.(2)After the optimal modeling concentration was assessed,40 rats were randomly divided into control(a),acetic acid enema(b),acetic acid+binding(c),and acetic acid+binding+tail clip(d)groups and correspondingly treated for 8 days.After the treatments,the general condition,diarrhea-related index,open field test(OFT)score,and colonic histopathology of rats were evaluated.Results(1)Compared with the blank group,the fecal trait score of 4%acetic acid enema group was increased on days 1 to 3 after intervention(P<0.001),and gradually decreased on days 4 to 7 after intervention.After 1 week,there was no significant difference between the fecal trait score and that of the blank group(P>0.05).Body weight was lower(P<0.01),fecal water content was higher(P<0.001).Compared with blank group,body weight of the 5%acetic acid enema group was decreased(P<0.001),the fecal trait score and diarrhea index were increased(P<0.01).No significant difference was found between 3%acetic acid enema and blank groups.The pathological colon tissue showed that,compared with the blank group,the mucosal structure of the 4%acetic acid enema group was complete with a small amount of inflammatory cell infiltration,and the pathological tissue score showed no significant difference(P>0.05),whereas the 5%acetic acid enema had a medium to large amount of inflammatory cell infiltration,and the pathological tissue score was increased(P<0.01).(2)Compared with group a,group b had lower body weight(P<0.001),and higher fecal trait score,fecal water content and diarrhea index(P<0.01).Compared with a and b groups,the body weight of c and d groups was lower(P<0.001),the fecal traits score,fecal water content,and diarrhea index were increased(P<0.01),and the colon running time was decreased(P<0.01).Compared with group c,Fecal water content in group D was higher(P<0.001).In the OFT score,compared with a and b groups,the OFT distance,standing times,and upright times in c and d groups were lower(P<0.05).Compared with c,the OFT distance,standing times,and upright times in d group were lower(P<0.05).The pathological tissue of colon showed that the mucosal structure of the four groups was complete,and there were different degrees of inflammatory cell infiltration.The pathological tissue scores of groups c and d were higher than those of groups a and b(P<0.05).Conclusions The 4%acetic acid concentration is appropriate for IBS-D modeling.After superposition and binding,the IBS-D diarrhea and internal hypersensitivity characteristic state can be better simulated.After superposition of a tail clip,the IBS-D model of liver stagnation and spleen deficiency can be established successfully.
