1.Impact of internationalization at home medical education on clinical medicine undergraduate abilities based on value-added assessment

Objective:To investigate the impact of internationalization at home medical education on clinical medical undergraduate abilities based on value-added assessment, and to provide a reference for deepening education and teaching reform in medical colleges and universities.Methods:The study subjects consisted of undergraduate students enrolled in 2020 at Chongqing Medical University majoring in clinical medicine (five-year program) and clinical medicine (Sino-foreign cooperative education program). The questionnaire in the 2023 China Medical Students Survey, initiated by the National Medical Education Development Center, was used in this study. SPSS 27.0 was used for independent samples t-test and hierarchical linear regression. Results:Compared with the control group, the clinical medical undergraduates who received internationalization at home medical education had higher scores in academic article writing ability [(3.89±0.73) vs. (2.87±0.80)], literature retrieval and information management ability [(3.98±0.64) vs. (3.43±0.65)], and international communication and global health concept [(3.81±0.68) vs. (3.52±0.69)]. Controlling for individual-level student variables, internationalization at home medical education had a positive effect on clinical medicine undergraduate abilities based on value-added assessment ( β=5.003, P<0.001). Conclusions:The internationalization at home medical education in Chongqing Medical University enhances the abilities of clinical medical undergraduates, and provides a reference for the comprehensive improvement of the education quality of medical talents.
2.Optimization of cellulase-assisted ultrasound extraction process for total flavonoids from Plumbago zeylanica and evaluation of their anti-oxidant activity

AIM To optimize the cellulase-assisted ultrasound extraction process for total flavonoids from Plumbago zeylanica L.,and to evaluate their anti-oxidant activity.METHODS With extraction time,liquid-solid ratio,cellulase addition amount,extraction temperature and ultrasonic power as influencing factors,extraction rate of total flavonoids as an evaluation index,the extraction process was optimized by response surface method on the basis of single factor test.Subsequently,The scavenging rates of extract on DPPH,ABTS and OH free radicals were determined.RESULTS The optimal conditions were determined to be 34∶1 for liquid-solid ratio,3%for cellulase addition amount,51 ℃ for extraction temperature,38 min for extraction time,and 400 W for ultrasonic power,the extraction rate of total flavonoids was(33.411±0.97)%.The IC50 values of three free radicals were 0.13,0.042,3.29 mg/mL,respectively.CONCLUSION This reasonable and reliable method can be used for the cellulase-assisted ultrasound extraction of total flavonoids from P.zeylanica with strong anti-oxidant activity.

Recent studies have shown that the physiological homeostasis of oral mucosal cells is regulated by the circadian clock.Dis-ruption or dysfunction of the circadian clock is closely associated with the development of oral squamous cell carcinoma(OSCC).Research based on the circadian clock offers a novel perspective on the pathogenesis and therapeutic strategies for OSCC.However,there is current-ly limited research on this topic,and people generally have insufficient understanding and recognition of the circadian clock.Given the complexity and challenges of circadian clock which is the fourth dimension of medical research,we organize relevant experts based on summarizing the current research results of circadian clock in the pathogenesis and precision diagnosis and treatment of OSCC,combining the scientific principles of the circadian clock's role and their long-term research experience,then summarizes and recommends the con-sensus opinions for the research of circadian clock in the pathogenesis mechanism and precision diagnosis and treatment of human OSCC,with the hope of providing guidance for the basic research and clinical application of circadian clock or circadian rhythm in the pathogene-sis mechanism and precision diagnosis and treatment of oral and maxillofacial squamous cell carcinoma.
4.Construction and practice of a teaching quality assurance system for Chinese-foreign joint education program of clinical medicine: a case study of Chongqing Medical University

Chinese-foreign joint education program of clinical medicine is an important means to achieve the globalization of medical education. Chongqing Medical University and University of Leicester in the UK have jointly established a Chinese-foreign joint education program of clinical medicine to achieve the integration of Chinese and British cultivation concepts, management systems, teaching resources, teacher teams, evaluation systems, and multiculturalism. They have also constructed an internal teaching quality assurance system with the main contents of the improvement of management mechanisms, the formulation of training programs, the construction of teaching staff, the design of syllabuses, the curriculum assessment system, and teaching quality evaluation, as well as an external teaching quality assurance system with the core components of clinical medicine accreditation, Chinese-foreign joint education program evaluations, international quality audits, and professional quality monitoring. Both systems can help to comprehensively improve teaching quality.
5.Relationship between spleen volume and non-alcoholic fatty liver disease by three-dimensional computed tomography reconstruction

Objective To investigate the association of spleen volume with the risk of non-alcoholic fatty liver disease(NAFLD)as well as their causal relationship.Methods We included 90 NAFLD cases and 47 healthy controls who had received contrast-enhanced computed tomography(CT)scan of the abdomen at the Second Affiliated Hospital of Xi'an Jiaotong University from November 2022 to November 2023.We conducted three-dimensional reconstruction of the spleen through a deep learning network model using a two-stage coarse-to-fine segmentation approach.We compared the two groups using the two-sample t test or Mann-Whitney U test for continuous data and using the chi-square test for categorical data;evaluated the correlation between spleen volume and liver function indicators through Pearson correlation or Spearman rank correlation analyses;determined the factors influencing the development of NAFLD through multivariable Logistic regression analysis;and further assessed the casual relationship between spleen volume and NAFLD using the inverse variance-weighted two-sample Mendelian randomization(IVW-MR)method.Results Spleen volume was significantly larger in NAFLD cases than in controls(272.93±104.16 vs 204.37±81.20 cm3,P<0.001).The Spearman rank correlation analysis showed that spleen volume was positively correlated with the hepatic steatosis index(rs=0.422,P<0.001)and gamma-glutamyl transferase levels(rs=0.211,P=0.047)in patients with NAFLD.The multivariable Logistic regression analysis indicated that spleen volume was an independent risk factor for the development of NAFLD(odds ratio[OR]=1.01,95%confidence interval[CI]:1.00-1.02,P=0.049).The IVW-MR analysis detected a causal relationship between spleen volume and NAFLD(OR=1.16,95%CI:1.05-1.28,P=0.005).Conclusion Increased spleen volume may be a risk factor for the development and progression of NAFLD.Further studies are still needed to investigate the specific mechanism.
6.Therapeutic Efficacy of Yiqi Huoxue Tongmai Decoction Combined with Western Medicine for Pulmonary Hypertension Due to Left Heart Disease and Its Effect on Endothelial Function

Objective To investigate the therapeutic efficacy of Yiqi Huoxue Tongmai Decoction(a modified priscription derived from Buyang Huanwu Decoction)combined with western medicines in the treatment of patients with pulmonary hypertension due to left heart disease(PH-LHD)of qi deficiency and blood stasis syndrome,and to explore its effect on endothelial function.Methods Eighty patients with PH-LHD of qi deficiency and blood stasis syndrome who were admitted to Shangluo Central Hospital from March 2022 to December 2023 were randomly divided into a study group and a control group,with 40 patients in each group.The control group was given routine treatment with western medicine,and the study group was treated with Yiqi Huoxue Tongmai Decoction on the basis of treatment for the control group,three weeks constituting a course of treatment.Before and after intervention,the two groups of patients were observed in the changes of traditional Chinese medicine(TCM)syndrome scores,pulmonary circulation indicators[mean pulmonary arterial pressure(mPAP)and pulmonary capillary wedge pressure(PCWP)],cardiopulmonary exercise testing(CPET)indicators[peak heart rate,peak systolic blood pressure,peak diastolic blood pressure,maximal oxygen uptake(VO2max),peak oxygen pulse],vascular endothelial function indicators[serum nitric oxide(NO)and endothelin 1(ET-1)]and the scores of quality of life.After treatment,the clinical efficacy and incidence of adverse reactions of the patients between the two groups were compared.Results(1)After three-week of intervention,the total effective rate of the study group was 90.00%(36/40),while that of the control group was 72.50%(29/40),and the comparison between the two groups(tested by chi-square test)showed that the efficacy of the study group was significantly superior to that of the control group(P<0.05).(2)After intervention,the primary symptom scores,secondary symptom scores and total TCM syndrome scores of the two groups of patients were all decreased compared with those before intervention(P<0.05),and the decrease of the scores in the study group was significantly superior to that in the control group(P<0.01).(3)After intervention,the levels of pulmonary circulation indicators of mPAP and PCWP in the two groups were all decreased compared with those before intervention(P<0.05),and the decrease in the study group were significantly superior to that in the control group(P<0.01).(4)After intervention,the CPET indicators of peak heart rate,peak systolic blood pressure and peak diastolic blood pressure in the two groups were all decreased(P<0.05),while the VO2max and peak oxygen pulse rate were all increased compared with those before intervention(P<0.05),and the decrease of peak heart rate,peak systolic blood pressure and peak diastolic blood pressure as well as the increase of VO2max and peak oxygen pulse rate in the study group was significantly superior to that in the control group(P<0.05 or P<0.01).(5)After intervention,the vascular endothelial function indicator of serum ET-1 level in the two groups was decreased compared with that before intervention(P<0.05),and the serum NO level was increased compared with that before intervention(P<0.05),and the decrease of serum ET-1 level and the increase of serum NO level in the study group were significantly superior to those in the control group(P<0.01).(6)After intervention,the scores of six dimensions of 36-Item Short-Form Health Survey(SF-36),including mental health,role-emotional,social functioning,role-physical,vitality,and general health in the patients of the two groups were increased compared with those before intervention(P<0.05),and the increase in the study group was significantly superior to that of the control group(P<0.05 or P<0.01).(7)The incidence of adverse reactions in the study group was 20.00%(8/40)and that was 15.00%(6/40)in the control group,and the comparison between the two groups showed that the difference was not statistically significant(P>0.05).Conclusion Yiqi Huoxue Tongmai Decoction combined with western medicines is effective on enhancing the clinical efficacy for the intervention of patients with PH-LHD of qi deficiency and blood stasis syndrome,and can improve the patients'cardiopulmonary function,vascular endothelial function and improve the quality of life,with higher safety.
7.Prognostic value of intra-voxel incoherent motion-diffusion weighted imaging quantitative parameters in predicting prognosis of patients with KRAS mutation non-small cell lung cancer undergoing first-line concurrent chemoradiotherapy

Objective To explore the prognostic value of quantitative parameters of intra-voxel incoherent motion-diffusion weighted imaging(IVIM-DWI)in predicting the prognosis of patients with Kirsten rats arcomaviral oncogene homolog(KRAS)mutation non-small cell lung cancer(NSCLC)undergoing first-line concurrent chemoradiotherapy.Methods A total of 139 patients with KRAS-mutated NSCLC were selected as the research subjects,and all of them received first-line concurrent chemoradiotherapy.Based on follow-up results(with all-cause mortality within one year defined as the endpoint event),patients were divided into favorable prognosis group and poor prognosis group.General data and IVIM-DWI quantitative parameters[apparent diffusion coefficient(ADC),diffusion coefficient(D),pseudo-diffusion coefficient(D*)and perfusion fraction(f)]were compared be-tween the two groups.Cox regression analysis was employed to identify factors influencing poor prognosis in patients with KRAS-mutated NSCLC.Receiver operating characteristic(ROC)curves were plotted to evaluate the prognostic predictive value of IVIM-DWI quantitative parameters.Kaplan-Meier(K-M)survival curves and the Log-rank test were used to analyze the relationships between IVIM-DWI quantitative parameters and prognosis in patients with KRAS-mutated NSCLC.Results Among the 139 patients,four were lost during follow-up,resulting in a survival rate of 69.63%(94/135).The favorable prognosis group comprised 94 patients,while the poor prognosis group included 41 pa-tients.There were statistically significant differences in the types of KRAS mutations,the best thera-peutic effect after treatment,and smoking history between the two groups(P<0.05).After adjus-ting for confounding factors,ADC,D,D* and f values were identified as influencing factors for poor prognosis in patients with KRAS-mutated NSCLC(P<0.05).The area under the curve(AUC)for the combined prediction of prognosis using ADC,D,D* and f values was 0.933,which was higher than the AUC for each parameter alone.Patients with low ADC,D,D* and f values exhibited sig-nificantly lower survival rates compared to those with high values(P<0.05).Conclusion IVIM-DWI quantitative parameters,including ADC,D,D* and f values,are influencing factors for the prognosis of first-line concurrent chemoradiotherapy in patients with KRAS-mutated NSCLC.The combined predictive performance of these parameters is high,offering guidance for clinical diagnosis and treatment and potentially improving disease prognosis.
8.Exploration and practice of local internationalization and enhanced virtual blended learning model in the context of new medical education

Since 2020, with the Sino-foreign cooperative education of clinical medicine as the pilot project, Chongqing Medical University (CQMU) has integrated its high-quality resources with those from University of Leicester (UoL) and explored the practice of enhanced virtual blended learning model from the aspects of self-learning before class, practical application during class, and consolidation and improvement after class. In addition, the implementation of this model was ensured from the aspects of teacher-student communication, platform construction, and optimization of evaluation mechanism, so as to replace the traditional teaching model and promote the implementation of local internationalization strategy. At present, preliminary results of student training have been achieved, and the project has passed the clinical medicine professional certification of the Ministry of Education and CQMU-UoL interim review.

10.Clinical study on the warmed iodophor for preoperative disinfection of laparoscopic hysterectomy in elderly

Objective To determine the clinical effect of warmed iodophor for preoperative disinfection in elderly patients undergoing laparoscopic hysterectomy.Methods A total of 84 elderly patients who underwent laparoscopic hysterectomy were selected and randomly divided into the room temperature group(disinfected with iodophor at a temperature of 24℃)and the warmed group(disinfected with iodophor at a temperature of 39℃),with 42 cases in each group.The disinfection qualification status,incision infection status,nasopharyngeal temperature,awakening time,adverse reactions,intraoperative blood loss,incision drainage volume,and hidden blood loss of patients in the two groups were compared.Results There was no statistically significant difference in the disinfection qualification rate or incision infection rate between the two groups(P>0.05).At 10 minutes,30 minutes,60 minutes after disinfection and the end of the operation,the nasopharyngeal temperatures of patients in the warmed group were significantly higher than those in the room temperature group(P<0.05).The awakening time of patients in the warmed group was significantly shorter than that in the room temperature group(P<0.05),and the incidences of intraoperative hypothermia and shivering reaction after awakening were significantly lower than those in the room temperature group(P<0.05).There was no statistically significant difference in the intraoperative blood loss or incision drainage volume of patients between the two groups(P>0.05),while the hidden blood loss of patients in the room temperature group was significantly higher than that in the warmed group(P<0.05).Conclusion The iodophor at a temperature of 39℃for preoperative disinfection in elderly patients undergoing laparoscopic hysterectomy can not only ensure the disinfection effect and surgical safety,but also effectively maintain intraoperative body temperature stability,promote postoperative awakening,and reduce hidden blood loss.
