Objective To explore the feasibility of using high-definition thoracoscopy to identify sympathetic ganglia during thoracic sympathicotomy for primary palmar hyperhidrosis. Methods The clinical data of patients with primary palmar hyperhidrosis who underwent high-definition thoracoscopic sympathicotomy in Taikang Xianlin Drum Tower Hospital from June to July 2023 were retrospectively analyzed. Intraoperative visualization rates and anatomical variations of sympathetic ganglia were recorded, and the consistency between white-light thoracoscopy and near-infrared fluorescence imaging was compared. Additionally, surgical videos from previous fluorescence-guided procedures were reviewed. Results Finally 100 patients were collected, including 54 females and 46 males, with an average age of (21.92±6.56) years. All patients underwent endoscopic thoracic sympathicotomy at R3 level. The overall intraoperative ganglion visualization rate was 92.5% (740/800), with G2-G5 rates of 95.5% (191/200), 94.0% (188/200), 94.0% (188/200), and 86.5% (173/200), respectively. Ganglion variations occurred in 32.0% (237/740), predominantly at G3 (29.8%) and G4 (42.6%). In 5 indocyanine green-enhanced patients, the concordance rate between white-light and near-infrared fluorescence imaging was 100.0% (38/38). Video analysis of 14 near-infrared fluorescence-guided surgeries demonstrated a 99.1% (107/108) consistency rate. Postoperative palmar hyperhidrosis improvement reached 100.0% (100/100) with no Horner’s syndrome. Conclusion With the wide clinical application of high-definition thoracoscopy, accurate thoracic sympathicotomy has the feasibility of clinical application.

ObjectiveTo construct a risk prediction model for frequent acute exacerbations of chronic obstructive pulmonary disease (COPD) under disease-syndrome combination, thus providing decision support for precise clinical intervention. MethodsA total of 2 029 patients with acute exacerbations of COPD admitted to the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2020 to August 2024 were retrospectively included. These patients were classified into groups of frequent acute exacerbations (≥2 times/year) and infrequent acute exacerbations (<2 times/year) according to the hospitalization times per year. Risk factors were screened by LASSO regression combined with logistic regression, and a nomogram model was constructed. The model performance was assessed based on the area under the curve (AUC), calibration curves, and decision curve analysis (DCA). ResultsThe differences in baseline characteristics between the frequent acute exacerbations group (1 196 cases) and infrequent acute exacerbations group (833 cases) were not statistically significant. LASSO regression combined with multivariate logistic regression screened the following independent risk factors: body mass index (BMI), hospitalization days, number of smoking years, place of residence, use of noninvasive ventilators, oxygen-demanding therapy, liver cirrhosis, use of systemic glucocorticosteroids, and traditional Chinese medicine syndrome (phlegm and stasis obstructing the lung). The nomogram model showed good discrimination and calibration in both the training set (AUC=0.748) and validation set (AUC=0.774). ConclusionThe risk prediction model for frequent acute exacerbations of COPD, integrating traditional Chinese medicine syndrome, constructed in this study has high accuracy. It can provide a scientific basis for early clinical identification of high-risk patients and individualized intervention.

Objective:
To evaluate the intervention effectiveness of co-occurrence and prevention for myopia and obesity among primary and secondary school students, so as to provide a scientific basis for the development of comprehensive intervention measures in myopia and obesity.
Methods:
From September 2022 to September 2023, a cluster random sampling method was used to select 6 primary schools and 6 junior high schools from Tongzhou District, Beijing. Participants were randomly assigned to an intervention group (914 before intervention and 754 after intervention) and a control group (868 before intervention and 652 after intervention), with an expected duration of one academic year. Based on the RE-AIM framework, integrate resources from families, schools, communities, and medical institutions to develop a school-based intervention technology packagefor the co-occurrence and prevention of myopia and obesity in children. The intervention group received intervention according to the comprehensive intervention technology package, while the control group did not receive any intervention measures. Relevant health indicators during the baseline period and after intervention were measured and collected, and groups were compared by Chi quest test, t-test and Wilcoxon rank sum test.
Results:
After intervention, the uncorrected visual acuity of primary and secondary school students in the intervention group (4.79±0.30) and the control group (4.77±0.33) both decreased compared to those before intervention (4.80±0.30, 4.90±0.32) ( t =-7.00,-5.24); the decrease in uncorrected visual acuity in the intervention group was smaller than that in the control group( t =5.33)( P <0.01). After intervention, body mass index, waist circumference, hip circumference, and body fat percentage of primary and secondary school students in the intervention group decreased compared to those before intervention. However, the changes in these indicators were not statistically significant ( t/Z =-0.03, - 0.36,- 0.30,- 0.01, P >0.05); the above indicators in the control group increased compared to those before intervention, but only hip circumference and body fat percentage showed statistically significant changes ( t/Z =2.17, 2.62, P <0.05). After intervention, both the intervention group and the control group showed increases in systolic and diastolic blood pressure compared to those before intervention(intervention group: t =2.16,5.29; control group: t =6.84,5.07); the intervention group had lower systolic and diastolic blood pressure than the control group( t = -5.27 , -2.08)( P <0.05). After intervention, the intervention and the control groups had statistically significant differences in cognitive accuracy(92.48%, 69.33%) in terms of "outdoor exercise can prevent myopia" and "having 5 servings of adult fist sized vegetables and fruits every day" ( χ 2=6.30, 7.86, P <0.05). There was a statistically significant difference in the proportion of primary and secondary school students in the intervention group (40.98%) and the control group (35.43%) for "who did not drink sugary drinks for every day in the past 7 days" ( χ 2=4.32, P <0.05). After intervention, the intervention group and the control group showed increases in "school outdoor activity duration on school days" and "outdoor activity duration on rest days" compared to those before intervention ( t/Z =-13.32,-9.71;- 2.59,-2.69);the behavior rate of "visual acuity measurement frequency at least once every 3 months" in the intervention group (46.68%) and the control group (52.76%) increased compared to those before intervention (36.43%, 44.01%), and the increases in the intervention group were greater than that in the control group ( χ 2=17.52,11.08) ( P <0.05).
Conclusions
Comprehensive intervention measures have significant intervention effects on controlling the occurrence and development of comorbidity of myopia and obesity in children. It could actively promote collaboration and cooperation among families, schools, communities and medical institutions to reduce the occurrence of myopia and obesity among primary and secondary school students.

Objective:To explore the prognostic value of artificial intelligence-based P53 and Ki67 detection in stage I non-mucinous adenocarcinoma(INMA)of lung. Methods:A retrospective analysis was made of patients treated by radical surgical resection for INMA of lung in the Department of Thoracic Surgery of Peking University First Hospital from Jan.2015 to Dec.2016,with complete clinicopathological and 5-year follow-up data.Immunohistochemical staining for P53 and Ki67 was performed on all cases and the index of P53 and Ki67 was calculated with the assistance of artificial intelligence(AI).The optimal cut-off values for P53 and Ki67 were determined using X-Tile software, and based on these values, the patients were divided into low-expression and high-expression groups.Pearson chi-square test and Fisher's exact test were used to compare the differences in clinicopathological characteristics between the different groups.Univariate and multivariate Cox regression analyses were performed to assess the impact of various indicators on 5-year overall survival(OS)and disease-free survival (DFS)for stage I INMA.The time-dependent receiver operating characteristic(ROC) curves and the area under the curve(AUC)was used to analyze the predictive performance of P53 and Ki67 for the prognosis of stage I INMA.Results:Among the 191 patients, the median follow-up time was 60(54, 60) months. The index of P53 and Ki67 were 0%-100% and 1.0%-78.0%,respectively. The X-Tile software revealed optimal cut-off values of 62% for P53 and 20% for Ki67.Then the patients were divided into P53 low-expression group (<62%), P53 high-expression (≥62%) group and Ki67 low-expression (<20%)group,Ki67 high-expression group (≥20%). High expression of P53 was associated with male ( χ2=12.45, P<0.001), smoking ( χ2=12.24, P<0.001), pTNM stage ( χ2=16.28, P<0.001), and histological grade ( P<0.001). High expression of Ki67 was associated with male ( χ2=17.33, P<0.01), smoking ( χ2=21.67, P<0.01), and histological grade ( P<0.001). Male ( HR=2.612, 95% CI: 1.173-5.815, P=0.019), smoking ( HR=2.651, 95% CI: 1.246-5.642, P=0.011), high pTNM stage ( HR=3.815, 95% CI: 1.792-8.122, P<0.001), high histological grade ( HR=5.277, 95% CI: 2.400-11.606, P<0.001), high P53 expression ( HR=5.950, 95% CI: 2.792-12.680, P<0.001), and high Ki67 expression ( HR=3.349, 95% CI: 1.554-7.221, P=0.002) were associated with poorer disease-free survival (DFS). Male ( HR=9.050, 95% CI: 1.113-73.586, P=0.039), smoking ( HR=8.428, 95% CI: 1.701-41.765, P=0.009), high histological grade ( HR=6.865, 95% CI: 1.756-26.834, P=0.006), high P53 expression ( HR=16.699, 95% CI: 3.369-82.761, P<0.001), and high Ki67 expression ( HR=7.558, 95% CI: 1.806-31.632, P=0.006) were associated with poorer overall survival. P53 high-expression was identified as an independent risk factor for both DFS ( HR=2.843, 95% CI: 1.192-6.778, P=0.018) and OS( HR=6.909, 95% CI: 1.202-39.720, P=0.030) in stage I INMA patients. The area under the time-dependent ROC curves for predicting 5-year overall survival after surgery were 0.738 for p53, 0.674 for Ki67, 0.638 for pTNM staging, and 0.587 for histological grade. Among these, p53 demonstrated the highest predictive efficacy. Conclusions:AI-assisted interpretation of P53 and Ki67 indices improves test result repeatability. With critical values of 62% and 20%, high P53 and Ki67 expression indicates poor prognosis, while high P53 expression is an independent risk factor for lower OS and DFS, serving as a reference for postoperative adjuvant therapy screening.
