1.Construction of a diagnostic prediction model for childhood allergic asthma based on the detection results of specific IgE for airborne allergens

Objective:To construct a diagnostic prediction model for childhood asthma and conduct a preliminary evaluation based on the test results of specific IgE (sIgE) for airborne allergens and in combination with clinical data.Methods:This study is a case-control study. A total of 4 338 cases that completed the sIgE test for airborne allergens in the Allergy Department of Beijing Children′s Hospital Affiliated to Capital Medical University from January to December 2023 were selected as the research subjects. They were divided into the asthma group and the non-asthma group based on the diagnostic information. Age, gender, cough and wheezing symptoms, and the classification results of sIgE concentrations of 15 airborne allergens were collected as the predictor variables of the asthma diagnostic prediction model. Differential analysis and LASSO regression were employed for the screening of predictor variables. The multivariate logistic regression method was applied to construct the nomogram prediction model. The data set was randomly split at a ratio of 7∶3 into a training set (3 036 cases) for constructing the prediction model and a validation set (1 302 cases) for testing the predictive efficacy of the model. The area under the receiver operating characteristic (ROC) curve (AUC), the Hosmer-Lemeshow calibration curve were utilized to assess the discrimination and goodness of fit of the model, and the clinical decision curve (DCA) was adopted to evaluate the clinical application value of the model.Results:Among 4 338 pediatric cases, children aged 0 to <3 years accounted for 10.17% (441 cases), those aged 3 to <6 years accounted for 36.49% (1 583 cases), those aged 6 to <12 years accounted for 46.98% (2 038 cases), and those aged 12 to 18 years accounted for 6.36% (276 cases). Males constituted 65.17% (2 827 cases), and females 34.83% (1 511 cases). The proportion of children without wheezing symptoms was 41.47% (1 799 cases), while those with wheezing symptoms was 58.53% (2 539 cases). The asthma group accounted for 41.77% (1 812 cases), and the non-asthma group for 58.23% (2 526 cases). Statistically significant differences were observed between the asthma group and the non-asthma group in 18 predictive variables including age, gender, wheezing symptoms, d1, d2, e1, e5, g2, g6, m6, t11, t3, t6, w1, w22, w6, wx5, and m3 ( P<0.05). LASSO regression analysis identified six predictor variables: age (calculated in months), cough and wheezing symptoms, and sIgE of four airborne allergens, namely, Dermatophagoides pteronyssinus (d1), Canis familiaris dander (e5), Aspergillus fumigatus (m3), and Artemisia vulgaris pollen (w6).Multifactorial regression analysis revealed that the contribution degrees of the above-mentioned predictor variables to the asthma diagnosis prediction model were ranked as follows: cough and wheezing symptoms ( OR=24.37, P<0.001), m3 ( OR=1.34, P<0.001), d1 ( OR=1.22, P<0.001), e5 ( OR=1.12, P=0.028), w6 ( OR=1.11, P<0.001), and age ( OR=1.01, P<0.001).The AUCs of the nomogram prediction model for the training set and the validation set were 0.853 (95% CI: 0.840-0.866) and 0.838 (95% CI: 0.817-0.860), respectively. The Hosmer-Lemeshow calibration curve indicated a good fit ( P=0.215 for the training set; P=0.352 for the validation set). The DCA of the validation set demonstrated that when the probability threshold for predicting the occurrence of childhood asthma was 8%-92%, the model had the best applicability. Conclusion:By combining age, cough and wheezing symptoms, and sIgE of the four airborne allergens (d1, e5, m3, and w6) selected from 15 airborne allergens, a childhood asthma diagnosis prediction model with good predictive performance and clinical practicability was constructed. It can serve as a simple and convenient tool for accurately identifying asthma and provides a practical basis for the application of artificial intelligence big data analysis models in the prevention, treatment, and management of childhood asthma.
2.Validity and reliability of the Chinese version of the Supervisory Relationship Questionnaire

Objective:To adapt and validate the Chinese version of the Supervisory Relationship Questionnaire(SRQ)and evaluate its psychometric properties among supervisees.Methods:Item analysis,exploratory factor anal-ysis(EFA)and confirmatory factor analysis(CFA)were conducted on data from 888 supervisees recruited via on-line platform.Test-retest reliability over a four-week interval was examined in a randomly selected subset of 70 su-pervisees.An additional sample of 367 supervisees completed the Supervisory Relationship Scale(SRS)to evaluate criterion validity.Results:The revised SRQ contained 65 items and 6 factors.Both exploratory factor analysis and confirmatory factor analysis supported six-factor structure(safe base,structure,commitment,reflective education,role model and formative feedback),explaining 66.52%of the variance,with factor loadings of the items ranging from 0.58 to 0.83.The revised SRQ had six factors with good fit indices(x2/df=1.31,GFI=0.81,NFI=0.87,CFI=0.97,TLI=0.90,IFI=0.91,RMSEA=0.03,SRMR=0.05).The scores of the revised SRQ were positively correlated with the scores of SRS(ICC=0.96,P<0.01).The Cronbach α coefficient of the revised SRQ was 0.97,and the test-retest reliability coefficient(ICC)was 0.74.Conclusion:The SRQ-C demonstrates sound psy-chometric properties,including structural validity,criterion-related validity,internal consistency,and temporal stabil-ity.It is a reliable and valid instrument for assessing supervisory relationships from the supervisee perspective.
3.Association between maternal periconceptional supplementation of folic acid or multiple micronutrients containing folic acid and perinatal mortality rate

Objective:To describe the prevalence of perinatal death in Tongzhou District of Beijing,and to estimate the association between maternal periconceptional supplementation of folic acid or multiple micronutrients containing folic acid and perinatal mortality rate.Methods:A retrospective cohort study was conducted based on the maternal and child care system in Tongzhou District of Beijing.The subjects were 94 490 perinatal who were born during January 2013 to December 2018.The information on perina-tal outcomes and maternal periconceptional supplementation of folic acid or multiple micronutrients contai-ning folic acid were collected.The Poisson log-linear model was used to estimate the association between maternal periconceptional folic acid or multiple micronutrients supplementation and perinatal mortality rate.Results:The overall perinatal mortality rate was 2.71‰.The perinatal mortality rates for maternal nutrients supplementation containing folic acid and no supplementation during periconceptional period were 2.63‰ and 3.43‰,respectively,and the difference in rates was not statistically significant[crude risk ratio(cRR)=0.77,95%CI:0.54-1.14].After adjusting for potential confounding factors in-cluding ethnicity,age,education level,occupation,household registration,parity,numbers of fetuses,gestational age,pregnant with assisted reproductive technology,delivery year and pre-pregnancy body mass index,the rates remained not statistically significant[adjusted risk ratio(aRR)=0.93,95%CI:0.77-1.13].The perinatal mortality rates were 2.23‰ and 2.99‰ for pure folic acid and multi-nutrients supplements containing folic acid,respectively,and the difference in rates was statistically sig-nificant(cRR=1.34,95%CI:1.02-1.76).The rates difference remained statistically significant after adjusting for potential confounders(aRR=1.31,95%CI:1.06-1.62).Additionally,the peri-natal mortality rates differences among the non-supplementation group and the supplementation group with variate timing of initiation(pre-conception or post-conception)or frequency of supplementation(low-frequency or high-frequency)were not statistically significant,regardless of adjusting for confounders.Conclusion:The overall perinatal mortality rate was lower than the national average level in Tongzhou District of Beijing.Maternal periconceptional supplementation of pure folic acid or micronutrients contai-ning folic acid had no impact on perinatal mortality.The association between maternal periconceptional supplementation of multiple micronutrients containing folic acid and perinatal mortality rate need further research.
4.A convergent mixed-methods study on the maternal influenza vaccination willingness among pregnant and postpartum women in Shanghai and Liaoning Province

ObjectiveTo investigate the current status and related factors of maternal influenza vaccination willingness among pregnant and postpartum women in Shanghai and Liaoning Province, China, and to explore the facilitators and barriers affecting vaccination uptake, so as to provide references for future practices in promoting maternal influenza immunization in China. MethodsA convergent mixed-methods research was conducted. From January to March 2024, a questionnaire survey was conducted among women attending prenatal and postnatal care at 7 medical institutions in Shanghai and Dalian, Liaoning Province, which aimed to assess pregnant women’s knowledge about influenza vaccine and their willingness to vaccination during pregnancy, as well as to identify the related factors. In addition, purposive sampling method was used to conduct in-depth interviews with pregnant women and perinatal healthcare service providers to explore their perspectives on influenza vaccination during pregnancy, including the reasons for their willingness or unwillingness to receive ( or recommend) the vaccine, and the relevant facilitators and barriers to vaccination. ResultsA total of 366 pregnant and postpartum women participated in the questionnaire survey, and 9.56% (35/366) of them were willing to receive the influenza vaccine during pregnancy. The results of multivariate logistic stepwise regression analyses showed that primipara (aOR=0.158, 95%CI: 0.037‒0.671, P=0.012), family members’ support for influenza vaccination during pregnancy (aOR=0.015, 95%CI: 0.003‒0.082, P<0.001) were associated with higher willingness to receive influenza vaccine during pregnancy. Absence of influenza infection during pregnancy (aOR=5.383, 95%CI: 1.801‒16.092, P<0.001), and lack of knowledge regarding influenza vaccination during pregnancy (aOR=11.294, 95%CI: 3.593‒35.496, P<0.01) were associated with lower willingness to receive influenza vaccine during pregnancy. Qualitative findings indicated that the facilitators to vaccination willingness among pregnant and postpartum women included the recommendation of healthcare service providers, adequate knowledge of influenza vaccine information and family members’ support for vaccination. Conversely, the barriers to vaccination willingness included low recommendation from the healthcare service providers, lack of knowledge about the safety of influenza vaccine during pregnancy and inadequate attention to influenza and influenza vaccine. ConclusionThe willingness to receive influenza vaccination among pregnant and postpartum women in Shanghai and Liaoning Province is relatively low. It is recommended that China should promptly improve the evidence-based system for the safety and efficacy of influenza vaccines for pregnant and postpartum women, along with an establishment of the mechanism for addressing adverse reactions. Furthermore, it is essential to enhance educational outreach to pregnant and postpartum women, their families, and healthcare service providers, thereby increasing the accessibility of information regarding influenza vaccination, which are expected to enhance the willingness of pregnant and postpartum women to receive the vaccine.
5.Research on the alleviation of podocyte injury in lupus nephritis by proscillaridin A and its mechanism

Objective:To investigate the protective effect and its mechanism of proscillaridin A (PSD-A) on podocyte injury in lupus nephritis (LN).Methods:Molecular docking and surface plasmon resonance techniques were used to analyze the binding status of PSD-A to signal transducer and activator of transcription 1 (STAT1). The immortalized human podocyte injury model in the lupus group was induced by the serum of systemic lupus erythematosus patients, and the control and PSD-A intervention (2 nmol/L, 4 nmol/L) groups were also set up. Six female 12-week-old C57BL/6 mice were designated as the control group, and 12 female 12-week-old MRL/lpr lupus mice were randomly divided into lupus group and PSD-A intervention group by random number table method. The PSD-A intervention group was intraperitoneally injected with 5 mg/kg PSD-A, once per week for 6 consecutive weeks. While the control group and the lupus group were intraperitoneally injected with the same volume of the solvent without PSD-A. Western blotting and real-time quantitative PCR were employed to detect the relative protein and mRNA expression levels of podocin, STAT1, and interferon-induced protein with tetratricopeptide repeat 1 (IFIT1) in podocytes of each group. Enzyme-linked immunosorbent assay was used to detect the levels of serum anti-double strand DNA antibody and interferon-α in mice. Coomassie brilliant blue was used to detect the urinary protein level. HE, PAS, Masson and PASM staining and transmission electron microscopy were used to observe the pathological changes of renal tissues. Immunohistochemistry was used to examine the protein expression of podocin, STAT1 and IFIT1 in renal tissues.Results:Molecular docking and surface plasmon resonance techniques proved that PSD-A could bind to STAT1 protein and they exhibited a robust binding affinity. The podocyte experiments showed that, compared with the lupus group, the relative expression levels of podocin protein and mRNA in the PSD-A intervention group were upregulated, while the relative expression levels of STAT1 and IFIT1 protein and mRNA were downregulated (all P<0.05). The animal experiments showed that, compared with the lupus group, the serum levels of anti-double strand DNA antibody, interferon-α, and urinary protein in PSD-A intervention group were decreased, the pathological damage of renal tissues was alleviated, and the injury of renal podocytes was reduced. Immunohistochemical staining showed that the relative protein expression levels of STAT1 and IFIT1 of renal tissues in the PSD-A intervention group were lower than those in the lupus group (all P<0.05). Conclusion:PSD-A can play a protective role in podocyte injury in LN, and its mechanism may be related to the inhibition of the STAT1 signaling pathway.

10.Expert consensus on the evaluation and management of dysphagia after oral and maxillofacial tumor surgery

Surgical operation is the main treatment of oral and maxillofacial tumors.Dysphagia is a common postoperative complication.Swal-lowing disorder can not only lead to mis-aspiration,malnutrition,aspiration pneumonia and other serious consequences,but also may cause psychological problems and social communication barriers,affecting the quality of life of the patients.At present,there is no systematic evalua-tion and rehabilitation management plan for the problem of swallowing disorder after oral and maxillofacial tumor surgery in China.Combining the characteristics of postoperative swallowing disorder in patients with oral and maxillofacial tumors,summarizing the clinical experience of ex-perts in the field of tumor and rehabilitation,reviewing and summarizing relevant literature at home and abroad,and through joint discussion and modification,a group of national experts reached this consensus including the core contents of the screening of swallowing disorders,the phased assessment of prognosis and complications,and the implementation plan of comprehensive management such as nutrition management,respiratory management,swallowing function recovery,psychology and nursing during rehabilitation treatment,in order to improve the evalua-tion and rehabilitation of swallowing disorder after oral and maxillofacial tumor surgery in clinic.
