1.Correlation between asthma and nocturia in women: an analysis based on NHANES database from 2005 to 2018

Objective: To analyze the relationship between asthma and nocturia in women based on the National Health and Nutrition Examination Survey (NHANES) database from 2005 to 2018,so as to provide reference for the prevention and treatment of female nocturia. Methods: Female respondents aged ≥20 years with nocturia or asthma were selected from the 2005-2018 NHANES database.Those with both diabetes stroke and obstructive sleep apnea syndrome were excluded.A weighted analysis was conducted using a complex sampling design.The association between asthma and nocturia in women was evaluated with univariate analysis,propensity score matching (PSM),and multivariate logistic regression models. Results: A total of 14 718 respondents were selected,of whom 1426 (9.7%) were diagnosed with asthma,and 4664 (31.7%) with nocturia.There is a significant correlation between asthma and nocturia (χ
=39.846,P<0.01). Age,body mass index (BMI),smoking and race were also associated with nocturia (P<0.01).Multivariate logistic regression analysis showed that,the age,BMI,smoking,race and asthma were correlated with the risk of nocturia,before PSM matching (P<0.05).To eliminate confounding bias,PSM was applied,and generalized linear mixed model analysis after matching showed that the risk of nocturia remained high in asthma patients (OR=1.540,95% CI:1.320-1.800,P<0.01). Conclusion: Asthma is associated with nocturia in women,indicating that it may be an important risk factor for female nocturia.

3.Expression and significance of CD4+T cell subtypes and their associated inflammatory factors in NE-FE-COPD patients

Objective To investigate the expression of CD4+T cell subtypes and related inflammatory factors in patients with neutrophil-predominant frequent acute exacerbation chronic obstructive pulmonary disease(NE-FE-COPD).Methods COPD patients who were treated in the hospital from March 2019 to March 2021 were selected as the research objects.According to different phenotypes,they were divided into the infrequent exacerbator COPD group(IECOPD group,n=11),the eosinophilic dominant frequent acute plus severe COPD group(Eos-FE-COPD group,n=13),and the neutrophil dominant frequent exacerbator COPD group(NE-FE-COPD group,n=15).Patients with normal lung function and smoking history>10 packs/year were the control group(CTRL group,n=9).Bronchoalveolar lavage fluid(BALF)was collected from each group,and the expression of CD4+T cell subtypes and inflammatory factors were detected by flow cytometry.The correlation between BALF and lung function and the frequency of acute exacerbation was ana-lyzed.CD4+CD28nullT cells and CD4+CD28+T cells were co-cultured with human airway epithelial cells(hAECs)and divided into co-culture group and Control group.The damage of hAECs was observed by immu-nofluorescence staining,and the mRNA and protein expression levels of ZO-1 and occludin were detected by RT-qPCR and Western blot.Results The proportion of CD4+CD28nullT cells and IL-1β level in BALF in the NE-FE-COPD group were higher than those in the CTRL group,the IE-COPD group,and the Eos-FE-COPD group,and the difference was statistically significant(P<0.05).The proportion of CD4+CD28nullT cells and IL-1βlevel were negatively correlated with lung function(P<0.05),and positively correlated with acute ex-acerbation frequency(P<0.05).Compared with the Control group,hAECs tight junctions were damaged in the co-culture group,and mRNA and protein expression levels of ZO-1 and occludin decreased,with statistical significance(P<0.05).Conclusion CD4+CD28nullT cells and IL-1β may be involved in the occurrence and de-velopment of NE-FE-COPD.
4.Progress on obesity-induced hypothalamic GnRH neuron dysfunction

The declining fertility has emerged as a significant concern impacting national welfare, with obesity identified as a major contributing factor. In the hypothalamus, gonadotropin-releasing hormone(GnRH) neurons release GnRH, which regulates the secretion of estrogen and testosterone via the hypothalamic-pituitary-gonadal(HPG) axis, thereby influencing sexual development and fertility. The proper function of GnRH neurons is crucial for maintaining normal sexual development and fertility. Research indicates that obesity can impact GnRH neuron function through several pathways, such as disrupting pulsatile secretion core kisspeptin neurons, causing imbalance in energy sensing and metabolic factor secretion, and triggering hypothalamic inflammation. These pathways directly or indirectly influence GnRH neuron function in the hypothalamus, which ultimately affects sexual development and impairs fertility. Therefore, this review consolidates recent advancements in understanding the mechanisms by which obesity induces dysfunction in hypothalamic GnRH neurons, offering new perspectives for early clinical interventions aimed at enhancing fertility outcomes in obese individuals.
5.Analysis of DMD gene variants in a single center

Objective:To investigate the DMD genetic variants of the Chinese population with Duchenne (DMD) and Becker muscular dystrophies (BMD).Methods:A cross-sectional study was conducted on 2 690 unrelated patients with DMD and BMD aged 0-18 who visited the Genetic and Prenatal Diagnosis Center of the First Affiliated Hospital of Zhengzhou University from January 2005 to February 2022. The clinical data, such as gender, age, clinical manifestations, and address, were collected. Multiplex ligation-dependent probe amplification, next generation sequencing panel, Sanger sequencing, and PCR amplification were used to detect the variants of the DMD gene in the patients, whose clinical information and gene detection results were descriptively analyzed.Results:The 2 690 patients included 2 648 males and 42 females, with an age of 6.0 (4.0, 9.0) years. The serum creatine kinase increased in all patients. Pathogenic DMD gene variants were detected in the 2 618 patients, including 1 875 cases (71.6%) large deletions, 231 cases (8.8%) duplications, and 512 cases (19.6%) small variants. Among the deletion variants, the deletion of 3 exons was the most common, accounting for 15.4% (288/1 875); and hotspot deletion involved exons 45 to 50, accounting for 6.3% (119/1 875). Exon 2 was the most common type duplication region, accounting for 13.0% (30/231). Small variants were distributed in all 79 exons of the DMD gene, with no hotspots. In addition, the 46 small variants were previously unreported.Conclusion:Exon deletion is the most common type of DMD gene variant, followed by small variants and exon duplication.
6.Teaching practice of transformation ability cultivation in the "orthopedic rehabilitation" course based on "case-based+problem-oriented" method

Objective:To explore the practical effects of achieving transformative learning in students based on the "case-based+problem-oriented" teaching method for the course of "orthopedic rehabilitation".Methods:A "case-based+problem-oriented" method was introduced in clinical courses for students in the rehabilitation therapy program at Tianjin Medical University. The students in the class were randomly divided into the "case-based+problem-oriented" teaching group (56 students) and the traditional teaching group (57 students). This study compared students' exam scores for different types of theoretical questions at the end of the semester. A self-filled anonymous questionnaire survey was conducted to evaluate the effectiveness of transformative learning, including the understanding of rehabilitation theory, learning interest, doctor-patient communication ability, disease analysis and judgment ability, and rehabilitation practice ability. SPSS 20.0 was used for t test and χ2 text. Results:The "case-based+problem-oriented" teaching group achieved significantly higher scores in various types of questions and total scores compared with the traditional teaching group [(85.67±4.78) vs. (72.98±6.49), t=8.78, P=0.004)]. Students in the "case-based+problem-oriented" teaching group showed higher interest in transformative learning and a considerably increased awareness of knowledge acquisition and innovative transformation. Students in the "case-based+problem-oriented" teaching group demonstrated higher abilities in theoretical learning, communication, disease exploration, and analytical judgment in this major and course. Conclusions:The "case-based+problem-oriented" clinical transformative teaching method integrates the theoretical teaching, clinical thinking, and medical training, promotes the students' enthusiasm for learning, and improves their proactive learning of knowledge and ability to transform and innovate.
7.Screening of Clinical Efficacy Evaluation Indicators for Traditional Chinese Medicine Injections based on Mixed Method Research

Currently, evidence on the efficacy and risk of traditional Chinese medicine (TCM) injections is seriously insufficient, and it is difficult to evaluate TCM injections comprehensively, truly and objectively using available efficacy evaluation indicators. Therefore, establishing a clinical efficacy evaluation system that can effectively reflect TCM injections is imperative. Core outcome set (COS) has played an important role in screening TCM efficacy evaluation indicators, however, there are still certain problems, such as large differences in efficacy indicators, non-standardization, and lack of featured and specific TCM indicators. Mixed method research (MMR) has the advantages of looking at problems from both quantitative and qualitative perspectives. It is thereby proposed to use COS and MMR integrated model to establish a clinical efficacy evaluation indicator system that not only fully considers the cha-racteristics of TCM injections, but also highlights the efficacy and advantages of TCM injections from the perspective of TCM intervention. Simultaneously, an indicator screening method using MMR to optimize COS research model is formulated, which can provide ideas for the research on efficacy evaluation indicators of TCM injections.
8.A novel biological sources consistency evaluation method reveals high level of biodiversity within wild natural medicine: A case study of Amynthas earthworms as "Guang Dilong".

For wild natural medicine, unanticipated biodiversity as species or varieties with similar morphological characteristics and sympatric distribution may co-exist in a single batch of medical materials, which affects the efficacy and safety of clinical medication. DNA barcoding as an effective species identification tool is limited by its low sample throughput nature. In this study, combining DNA mini-barcode, DNA metabarcoding and species delimitation method, a novel biological sources consistency evaluation strategy was proposed, and high level of interspecific and intraspecific variations were observed and validated among 5376 Amynthas samples from 19 sampling points regarded as "Guang Dilong" and 25 batches of proprietary Chinese medicines. Besides Amynthas aspergillum as the authentic source, 8 other Molecular Operational Taxonomic Units (MOTUs) were elucidated. Significantly, even the subgroups within A. aspergillum revealed here differ significantly on chemical compositions and biological activity. Fortunately, this biodiversity could be controlled when the collection was limited to designated areas, as proved by 2796 "decoction pieces" samples. This batch biological identification method should be introduced as a novel concept regarding natural medicine quality control, and to offer guidelines for in-situ conservation and breeding bases construction of wild natural medicine.
9.Preliminary construction of evaluation index system of lumbar puncture teaching for medical students based on Delphi method and analytic hierarchy process

Objective:To construct the evaluation index system of lumbar puncture teaching for medical students, aiming to create a scientific assessment and evaluation method covering theoretical knowledge, skill practice, and professional accomplishment, so as to comprehensively evaluate the teaching effect of lumbar puncture for medical students, and improve the practical ability of clinical skills of medical students more effectively.Methods:The evaluation index scheme of lumbar puncture teaching for medical students was initially formulated through literature review and group discussion, and 20 experts related to clinical front-line work and medicine were invited to revise the scheme by applying Delphi expert consultation and applying analytic hierarchy process to quantify the entries and establish the final index weights at all levels.Results:The valid recovery rate of both rounds of expert consultation questionnaires in this study was 100%. The second round of expert consultation was conducted without changing experts, with an authority factor of 0.88. Kendall's coefficient of harmony was 0.136 and 0.184, respectively. After two rounds of expert consultation, the evaluation index system of lumbar puncture teaching for medical students, including 3 primary indicators (theoretical knowledge, comprehensive clinical ability and professionalism), 7 secondary indicators and 22 tertiary indicators, was initially constructed.Conclusion:The evaluation index system of lumbar puncture teaching for medical students constructed in this study is scientific and credible, which can provide quantitative basis for the training and assessment of medical students, and is of great theoretical and practical significance.
10.Feasibility exploration of disease differentiation based on the Chinese patent medicine instructions

OBJECTIVE To compare the contents of the instructions of domestic and imported Chinese patent medicines as well as those from Hong Kong ,Macao and Taiwan ,and to explore the feasibility of Chinese patent medicines to strengthen the use of medicine according to disease differentiation . METHODS The domestic Chinese patent medicines included in national medical insurance list as well as imported Chinese patent medicines and those from Hong Kong ,Macao and Taiwan regions publicized on the official website of National Medical Products Administration were searched to statistically analyze the information of “functions”and“indications”in the instructions of Chinese patent medicines . RESULTS Finally,1 311 instructions of domestic Chinese patent medicine and 43 of imported Chinese patent regions were screened out . In the “functions”item contained in the instructions of Chinese patent medicines ,94.04% domestic Chinese patent medicines ,87.50% imported ones and those from Hong Kong ,Macao and Taiwan regions were expressed using the terms of traditional Chinese medicine ;4.89% and 12.50% were expressed using the terms of integrated Chinese and western medicine . The “functions” item were only expressed using the terms of western medicine in 1.07% of th e instructions of domestic Chinese patent medicines . The“functions” item was mostly expressed using the terms of integrated Chinese and western medicine in the instructions of imported Chinese patent medicines and those from Hong Kong ,Macao,and Taiwan regions ,but there was no expressed using the terms of western medicine alone . Among the expressions of “functions”item containing the terms of western medicine ,anti-inflammatory medicines accounted for the highest proportion ,and domestic Chinese patent medicines ,imported Chinese patent medicines and ones from Hong Kong ,Macao and Taiwan regions accounted for 37.18% and 75.00%,respectively. In the instructions of domestic Chinese patent medicines ,the“indications”item of which contained syndrome information accounted for 72.39%,and the instructions of imported Chinese patent medicines and those from Hong Kong ,Macao and Taiwan regions which did not include syndromes information accounted for 74.42%;in the instructions of domestic Chinese patent medicines ,imported ones and those from Hong Kong,Macao and Taiwan regions ,the“indications”item of which contained disease information ,accounted for 78.34% and 41.86%,respectively,and the “indications”item of which were mainly expressed using the terms of western medicine diseases , accounted for 64.85% and 73.68%,respectively. CONCLUSIONS Most of the domestic Chinese patent medicines included in the national medical insurance list have the basis for the use of disease differentiation ;most of the imported Chinese patent medicines and those from Hong Kong ,Macao and Taiwan regions have the basis for the use of disease differentiation .
