1.Research on the Standardization System of Mineral Medicines

Objective On the basis of systematically summarizing and organizing the existing standards of mineral drugs in traditional Chinese medicine,this study aims to construct the top-level framework of the mineral drug standard system and promote the standardization process of the mineral drug industry.Methods A comprehensive search and analysis of the standards for 87 kinds of mineral drugs were conducted on authoritative platforms such as the National Standard Information Public Service Platform,China Standard Service Network,and Traditional Chinese Medicine Standard and Guideline Information Service.By delving into the theoretical foundation and scientific methods,combined with practical research,this study uses analogy reference,systematic decomposition,and process analysis to design the top-level framework of the mineral drug standard system.Results Through comprehensive analysis and scientific design,this study summarizes the construction process and methods of the mineral drug standard system,clarifies the hierarchical structure and classification principles of the system,proposes the compilation path,and forms a standard system covering the entire industry chain of mineral drugs,including raw minerals,product categories,and circulation categories.Conclusion This study constructs a full industry chain process framework for the standard system of mineral drugs,providing a clear perspective for the research of mineral drugs.It can more comprehensively display the current state of development of mineral drug standards in our country,laying a foundation for the research on the standardization of mineral drugs.
2.The relationship between blood pressure variability and short-term neurological prognosis in patients with aneurysmal subarachnoid hemorrhage

Objective The relationship between blood pressure variability(BPV)and short-term neurological prognosis in patients with aneurysmal subarachnoid hemorrhage(aSAH)was investigated.Methods The study conducted a retrospective analysis of clinical data from aSAH patients who underwent surgical treatment and were admitted to the ICU at Beijing Tiantan Hospital,Capital Medical University,from January 2023 to April 2024.BPV was quantitively assessed by calculating the standard deviation(SD),successive variation(SV),coefficient of variation(CV),and range of mean blood pressure(MBP).Patients were divided into two group based on discharge GOS scores:good prognosis[Glasgow Outcome Scale(GOS)4-5]and poor prognosis(GOS 1-3)groups.Comparative analyses were performed to evaluate differences in BPV metrics between the two groups,followed by multivariable logistic regression modeling to adjust for potential confounding factors and elucidate the association between BPV and clinical prognosis.Results A total of 150 patients were included,with 59 in the poor prognosis group and 91 in the good prognosis group.The poor prognosis group exhibited significantly elevated levels of MBP-SD[(9.85±3.20)mmHg vs.(8.04±2.31)mmHg,P<0.001],MBP-SV[(10.37±3.85)mmHg vs.(8.07±2.33)mmHg,P<0.001],MBP-CV(10.00±3.30%vs.8.19±2.33%,P<0.001),and MBP-range[(39.60±13.56)mmHg vs.(32.44±9.78)mmHg,P<0.05]compared to the good prognosis group.Cohen’s d values indicated moderate effect sizes for BPV differences(0.65,0.72,0.63,and 0.61,respectively).Multivariable logistic regression showed that MBP-SD(OR=1.22,95%CI:1.08-1.39,P=0.002)and MBP-SV(OR=1.19,95%CI:1.05-1.35,P=0.007)were independently associated with poor prognosis.Conclusion Elevated MBP-SD and MBP-SV within the first 24 hours postoperative period are independent predictors of unfavorable short-term neurological outcomes in aSAH patients.
3.Analysis of the predictive value of bispectral index dynamic changes for hypotension after laparoscopic surgery in elderly patients with primary liver cancer

Objective To analyze the predictive value of bispectral index(BIS)dynamic changes for hypotension after laparoscopic surgery in elderly patients with primary liver cancer.Methods 197 elderly patients with primary liver cancer who underwent laparoscopic surgery in the hospital from November 2021 to July 2024 were selected as the research subjects,and all patients received general anesthesia,and their perioperative BIS was monitored.The patients were divided into a hypotension group and a non hypotension group based on the occur-rence of hypotension after laparoscopic surgery.The general information and the BIS at 5 minutes before anesthesia induction(T0),5 minutes after anesthesia induction(T1),30 minutes after the start of surgery(T2)and the end of surgery(T3)in the two groups were compared.The influencing factors of hypotension after laparoscopic surgery in elderly patients with primary liver cancer were analyzed through Logistic regression analysis,and the predictive value of BIS for hypotension after laparoscopic surgery in elderly patients with primary liver cancer were analyzed through receiver operating characteristic(ROC)curve.Results 51 patients were experienced hypotension after laparoscopic surgery among 197 elderly patients with primary lung cancer,and the incidence rate was 25.89%.The proportion of hypertension and operative time in the hypotension group were higher than those in the non hypoten-sion group(P<0.05),while the levels of albumin(ALB)and BIS at T2 and T3 were lower than those in the non hypertension group(P<0.05).logistic regression analysis showed that hypertension,long operative time,low ALB level,low BIS value at T2 and low BIS value at T3 were all risk factors for hypotension after laparoscopic surgery in elderly patients with primary liver cancer(P<0.05).The sensitivity,specificity and area under curve(AUC)of BIS combined prediction of hypotension after laparoscopic surgery in elderly patients with primary liver cancer at T2 and T3 were 92.16%,78.08%and 0.902%respectively,and the sensitivity and AUC were higher than those single prediction(P<0.05),but the specificity was similar to that of single prediction.Conclusion The predictive value of BIS at 30 minutes after the start of surgery and the end of surgery for hypotension after laparoscopic surgery in elderly patients with primary liver cancer are good,and the joint prediction of this two can further enhance the predictive value.
4.Surveillance report on healthcare associated infections in a tertiary children's hospital from 2019 to 2023

OBJECTIVE To get the current situation of healthcare associated infection(HAI)and implement targe-ted management through analyzing the trends of HAI incidence rate,the distribution of clinical departments,the sites of HAI and the composition of pathogens in a children's hospital in a five-year period.METHODS Data on HAI cases were collected from all inpatients in Children's Hospital of Soochow University from 2019 to 2023 through the real-time monitoring system of XINGLIN Healthcare acquired Infection Surveillance;Cochran-Armit-age test was used to check for the significant changes of distribution of HAI cases.RESULTS A total of 383,376 hospitalized patients were monitored from 2019 to 2023,of which HAI occurred on 6670 cases and 7209 case-times with an incidence rate of 1.74%and an incidence case rate of 1.88%.The top five departments of HAI inci-dence rates were cardiac,neonatal,surgical and pediatric intensive care unit and department of hematology.HAI mainly occurred in blood and alimentary system,upper and lower respiratory tract.A total of 2668 strains of path-ogenic bacteria were isolated,of which 1346 strains were gram-negative bacteria,1140 strains were gram-positive bacteria and 182 strains were fungi.The top three gram-negative bacteria were Klebsiella pneumoniae,Escherich-ia coli and Pseudomonas aeruginosa;the top three gram-negative pathogens were Streptococcus,Staphylococcus aureus and Staphylococcus epidermidis;and the top three fungi were Candida albicans,Candida parapsilosis and Candida tropicalis.CONCLUSIONS The HAI incidence rate of this hospital steadily declines in the past five years,however the same changes are not observed in the departments with high incidence of HAI.Attention should be paid to the raising bloodstream infections and detection rate of S.epidermidis.
5.Summary of the best evidence for the prevention and management of perioperative gastrointestinal dysfunction in patients with colorectal cancer

Objective:To systematically retrieve, extract and summarize the best evidence regarding the prevention and management of perioperative gastrointestinal dysfunction in patients with colorectal cancer, and to provide an evidence-based basis for clinical nursing practice.Methods:All evidence on the prevention and management of perioperative gastrointestinal dysfunction in colorectal cancer patients in various domestic and international databases and websites such as UpToDate, China national knowledge infrastructure, etc. were searched according to the "6S" pyramid model, including clinical decisions, guidelines, expert consensus, systematic evaluations, and randomized controlled trials from the time of database construction to August 15, 2024, on a top-to-bottom basis. Literature screening, quality assessment, and evidence extraction were performed independently by 2 nursing researchers.Results:A total of 19 documents were finally included, included 4 guidelines, 3 expert consensus, 7 systematic evaluations, 1 Meta-analysis, and 4 randomized controlled trials, and 35 pieces of evidence were summarized in 6 areas, including screening and assessment, build a multidisciplinary team, preoperative preparation, intraoperative monitoring, postoperative care, and evaluation of outcomes.Conclusions:The evidence summarized in this study is both scientifically rigorous and practically applicable, and can be used by healthcare professionals to design care plans for colorectal cancer patients that accelerate the recovery of gastrointestinal function, thereby promoting evidence-based clinical nursing practice.
6.Analysis of the predictive value of bispectral index dynamic changes for hypotension after laparoscopic surgery in elderly patients with primary liver cancer
