1.Issues and Solutions for Symptom Efficacy Evaluation in the Big Data Era of Traditional Chinese Medicine

Emphasizing symptom efficacy is an important manifestation of the personalized diagnosis and treatment of traditional Chinese medicine (TCM). However, in current clinical practice of TCM, there are challenges such as diverse symptom expressions, difficulty in standardization, inconsistent evaluation standards for symptom efficacy, lack of universal quantitative methods, and complexity in collecting complete and accurate symptom information. These issues hinder the full and effective utilization of symptom information. Addressing the current research status and existing problems of symptom terminology standardization, quantification and grading of symptom efficacy, and collection of symptom information, this paper proposes methodological strategies for effectively recording and utilizing TCM symptom efficacy information in the era of big data. These strategies include collecting TCM symptom information based on patient reporting, standardizing the evaluation of TCM symptom efficacy from measurement scales and evaluation dimensions, integrating TCM symptom efficacy evaluation into clinical diagnosis and treatment processes, and utilizing artificial intelligence technology to acquire and process TCM symptom efficacy information. TCM symptom efficacy evaluation strategies based on patient perspectives and artificial intelligence technology will help fully explore the value of data elements, promote the objective demonstration of the specific efficacy of TCM, and facilitate the discovery of effective patterns.
2.Clinical effect of Sijunzi decoction combined with Fangji Huangqi decoction added and decreased in treating chronic heart failure patients with reduced ejection fraction and Qi deficiency,blood stasis and phlegm retention syndrome

Objective To observe the clinical effect of Sijunzi decoction and Fangji Huangqi decoction combined with Sacubitril Valsartan Sodium Tablets in treating chronic heart failure patients with reduced ejec-tion fraction and Qi deficiency,blood stasis and phlegm retention.Methods Ninety-four inpatients with chro-nic heart failure treated in this hospital from January 2021 to December 2022 were selected as the study sub-jects and divided into the Sijunzi Decoction and Fangji Huangqi Decoction combined with Sacubitril Valsartan Sodium Tablets group (the Chinese medicine combined group) and Sacubitril Valsartan Sodium Tablets group (the control group) according to the random number table method.Both groups were treated with standard therapy,and the Chinese medicine combined group added Sijunzi Decoction and Fangji Huangqi Decoction on this basis.Both groups were continuously treated for 8 weeks.The mean systolic blood pressure,left ventricu-lar end diastolic diameter (LVEDD),left ventricular ejection fraction (LVEF),TCM syndrome score and 6 min walking test (6-MWT) results at 24 h before treatment and after treatment were compared between the two groups.Results The average 24 h systolic blood pressure,score of each TCM syndrome and LVEDD after treatment in the two groups were decreased compared with those before treatment (P<0.05),while LVEF and 6-MWT in the two groups were increased,and the differences were statistically significant (P<0.05). Compared with the control group,the average 24 h systolic blood pressure in the Chinese medicine combined group was higher,each score of TCM syndrome score and LVEDD were lower,LVEF and 6-MWT were high-er,and the differences were statistically significant (P<0.05).Conclusion Sijunzi Decoction and Fangji Huangqi Decoction combined with Sacubitril Valsartan Sodium Tablets has a good effect on the patients with chronic heart failure with reduced ejection fraction and Qi deficiency,blood stasis and phlegm retention,which could effectively increase the systolic blood pressure,improve the cardiac coronary arterial perfusion and the quality of life of the patients,and alleviate the left ventricular remodeling status.
3.Role of SphK/S1P in cardiovascular diseases

Sphingosine-1-phosphate(S1P)is a highly active membrane phospholipid metabolite with important physiological roles in the cardiovascular system.Sphingosine kinase(SphK)is a lipase that is important for main-taining stable S1P levels in vivo.S1P exerts different physiological effects in cardiovascular diseases by catalytically regulating the binding of S1P to G protein-coupled receptors(S1PR1-5)on the cell surface,thus exerting different physiological effects in cardiovascular diseases.For example,they are involved in regulating the development of dis-eases such as atherosclerosis,myocardial ischemia-reperfusion injury,myocardial infarction,heart failure and other diseases,maintaining the barrier function of vascular endothelial cells,protecting the endothelial glycocalyx layer,leukocyte adhesion,inflammatory reactions and other pathophysiologic processes.
4.Research on medical service satisfaction evaluation based on ECSI model——taking the S city public hospital as an example

Objective To deeply analyze the problems of public hospitals in medical service satisfaction,construct a more applicable and explanatory service satisfaction evaluation model,and provide reference for the improvement path of patients'medical service satisfaction.Methods Based on the ESCI model,this paper puts forward a research hypothesis,compiles the"Medical Service Satisfaction Evaluation Scale of Public Hospitals in S City",selects S public hospitals as the research object,and evaluates medical service satisfaction from the perspective of patients.Results At present,the main problems affecting the improvement of patients'satisfaction with medical service management in public hospitals in S city are as follows:it is urgent to improve the diagnosis and treatment technology in terms of hardware quality and optimize the environmental hygiene of public hos-pitals,and it is an urgent task to shorten the waiting time for medical treatment in software quality.The score of medical service satisfaction evaluation of S public hospitals was 3.76,and the satisfaction of patients in S public hospitals with their medical serv-ices was at a moderately low level.Conclusion In view of the above problems,it is recommended to strengthen the construction of key specialties,strengthen the construction of talent team,optimize the medical process,open digital linkage services,and strengthen service management,so as to promote the improvement of patients'satisfaction with medical services.
5.Effect of Danhuansan on high glucose-induced vascular endothelial cells injury by activating PINK 1/Parkin signaling pathway

Objective To investigate the effect of Danhuangsan on high glucose-induced vascular endothelial cell injury based on PINK 1/Parkin signaling pathway,and to explore its specific mechanism.Methods Human um-bilical vein endothelial cells were cultured in vitro and randomly divided into control group,growth factor group,Danhuangsan group,high glucose group,high glucose+growth factor group,high glucose+Danhuangsan group,with 3 cases in each group,treated for 48 hours.Cell scratch test was used to detect cell migration rate,and Transwell test was used to detect cell invasion rate.Immunofluorescence was used to detect the expression of anti-apoptotic protein Bcl-2,Beclin-1 and pro-apoptotic protein Bax.Western blot was used to detect the protein expression levels of PINK 1,Parkin and LC 3-Ⅱ.Results Cell scratch test and Transwell test showed that under normal environment and high glucose treatment,Danhuangsan could reduce the cell migration and invasion rate(P<0.05).Immunofluorescence assay showed that under normal environment and high glucose treatment,Danhuang-san up-regulated the expression levels of Bcl-2 and Beclin-1 protein in cells(P<0.05).Western blot results showed that under normal environment and high glucose treatment,Danhuangsan increased the protein expression levels of PINK 1,Parkin and LC 3-Ⅱ in cells and down-regulated the expression levels of Bax protein(P<0.05),and the effects of Danhuang powder were significantly better than those of blank serum and growth factor(P<0.05).Conclusion Danhuangsan can alleviate high glucose-induced endothelial cell injury by activating PINK 1/Parkin pathway,and the mechanism may be related to promoting mitophagy and enhancing the repair of damage.
6.Construction and effect of a multidisciplinary pain management model during perioperative period based on project-achieving quality control circle

Objective:To construct and implement a multidisciplinary pain management model during the perioperative period based on the project-achieving quality control circle, so as to improve the quality of patient pain management during the perioperative period.Methods:Using the convenient sampling, 310 surgical patients from the Department of Gastrointestinal Surgery, Hepatobiliary Surgery, Thoracic Surgery, Urology Surgery and Joint Surgery of Liaocheng People's Hospital from June to July 2020 were taken as the pre-improvement group, and the routine perioperative pain management model was implemented. Starting from August 2020, a project-achieving quality control circle was carried out, following the steps of theme selection, topic clarification, goal setting, formulation of strategies, investigation of the best strategies, implementation of strategies, and confirmation of effectiveness, to implement a multidisciplinary pain management model during the perioperative period. A total of 310 surgical patients admitted to 5 departments from February to March 2021 were included in the improvement group.Results:The implementation rate of multidisciplinary pain management plan, the rate of out-of-bed activity within 24 hours after surgery, the rate of excellent postoperative rehabilitation compliance, and the average sleep score of patients in the improvement group all increased, with statistical differences ( P<0.05). After improvement, the awareness rate of pain knowledge among medical and nursing staff, the accuracy rate of nurses' rest and active pain assessment records, and the score of nurse pain knowledge all increased, and the differences were statistically significant ( P<0.05) . Conclusions:The construction and implementation of a multidisciplinary pain management model during the perioperative period based on the project-achieving quality control circle can effectively improve the quality of pain management for surgical patients, accelerate patient recovery, and improve the pain management of medical and nursing staff.
7.A biomechanical analysis of cannulated screws fixation in a configuration of "axial compression and lateral buttress" in treatment of Pauwels type Ⅱ femoral neck fractures

Objective:To investigate the biomechanical properties of cannulated screws fixation in a configuration of "axial compression and lateral buttress" in the treatment of Pauwels type Ⅱ femoral neck fractures.Methods:Ten specimens of Sawbones artificial femur were first made into models of type Ⅱ femoral neck fracture with a Pauwells angle of 50° and then randomized into 2 equal groups ( n=5). The specimens in the experimental group were subjected to fixation with cannulated screws in a configuration of "axial compression and lateral buttress" in which the axial screw was 8.5 mm in diameter and the lateral screw 6.5 mm in diameter. The specimens in the control group were subjected to conventional fixation with cannulated screws in a configuration of "inverted triangle and parallel compression" in which the 3 screws was 7.3 mm in diameter. Finally, the specimens were placed onto a biomechanical testing machine to determine the parameters of static axial stiffness, displacement under 60 to 600 N load for 5,000 cycles, ultimate load and ultimate stiffness in turn. The 2 groups were compared to find out their differences. Results:The static axial stiffness was (1,492.00 ± 87.86) N/mm, significantly higher than that in the control group [(1,200.22 ± 228.06) N/mm] ( P<0.05). There was no significant difference between the 2 groups in the cyclic load displacement [(0.44 ± 0.01) mm versus (0.57 ± 0.17) mm] ( P>0.05), but the experimental group showed a lower trend. The ultimate load and ultimate stiffness were (4,292.61 ± 804.29) N and (1,623.55 ± 180.94) N/mm in the experimental group and (4,383.64 ± 1,423.24) N and (1,433.77 ± 289.93) N/mm in the control group, showing no significant difference between the 2 groups ( P>0.05). Conclusion:In the treatment of Pauwels type Ⅱ femoral neck fractures, fixation with cannulated screws in a configuration of "axial compression and lateral buttress" may exhibit better biomechanical properties than that in a conventional configuration of "inverted triangle" .
8.Genetic analysis of a case with a supernumerary marker derived from chromosome 9.

OBJECTIVE:
To delineate a small supernumerary marker chromosome (sSMC) derived from chromosome 9 with combined cytogenetic and molecular methods.
METHODS:
For a pregnant woman with fetal ultrasound revealing left ventricular punctate hyperechoic echo, and a high risk for monosomy or partial deletion of chromosome 8, chromosome 9 trisomy, monosomy or partial deletion of chromosome 11 by non-invasive prenatal testing, and an abnormal MOM value revealed by mid-term serum screening, amniocentesis was performed for G banded chromosomal analysis and single nucleotide polymorphism array (SNP-array) assay. Peripheral blood samples of the woman and her spouse were also collected for the above tests. In addition, the woman was further subjected to C banding karyotyping analysis and fluorescence in situ hybridization (FISH) assay.
RESULTS:
The G-banded karyotype of the pregnant women was 47,XX,+mar[20]/46,XX[80], whilst C-banding analysis showed a deep stain in the middle of the sSMC (suggestive of centromeric region) and light stain at both ends (suggestive of euchromatism). FISH combined with DAPI banding analysis using 9pter/9qter probes revealed a karyotype of 47,XX,+mar.ish i(9)(9p10)(9p++)[2]/46,XX[18], whilst SNP-array has revealed a 68.1 Mb duplication in the 9p24.3q13 region. A database search has suggested the duplication to be likely pathogenic. No abnormality was found in her fetus and spouse by karyotyping and SNP-array analysis.
CONCLUSION
Through combined cytogenetic and molecular genetic analysis, a sSMC derived from chromosome 9 was delineated, which has enabled genetic counseling for the couple.

9.Correlation of Ferroptosis and Other Types of Cell Death in Neurodegenerative Diseases.

Ferroptosis is defined as an iron-dependent, non-apoptotic cell death pathway, with specific morphological phenotypes and biochemical changes. There is a growing realization that ferroptosis has significant implications for several neurodegenerative diseases. Even though ferroptosis is different from other forms of programmed death such as apoptosis and autophagic death, they involve a number of common protein molecules. This review focuses on current research on ferroptosis and summarizes the cross-talk among ferroptosis, apoptosis, and autophagy that are implicated in neurodegenerative diseases. We hope that this information provides new ideas for understanding the mechanisms and searching for potential therapeutic approaches and prevention of neurodegenerative diseases.

10.Risk factors of chronic obstructive pulmonary disease complicated by invasive pulmonary aspergillosis

Objective:
To investigate the risk factors of chronic obstructive pulmonary disease (COPD) complicated by invasive pulmonary aspergillosis (IPA), so as to provide insights into prevention of IPA among COPD patients.
Methods:
The COPD patients complicated by IPA hospitalized in Yiwu Central Hospital from 2017 to 2021 were recruited as the case group, while COPD patients without IPA during the same study period served as controls. Participants' general information, laboratory tests, comorbidities and treatments were collected. The risk factors of COPD complicated by IPA were identified using a multivariable logistic regression model.
Results:
There were 30 participants in the case group and 30 in the control group, including 22 men and 8 women in each group and with a mean age of (75.00±10.00) and (74.00±10.00) years, respectively. There were 27 (90.00%) and 19 (63.33%) cases with stage 3 and higher COPD in the case and control groups, and the mean duration of hospital stay in the past one year was (12.89±4.88) and (8.59±3.85) days in the case and control groups, respectively. Multivariable logistic regression analysis identified long duration of hospital stay in the past one year (OR=1.230, 95%CI: 1.011-1.498), stage 3 and higher COPD (OR=18.637, 95%CI: 1.415-245.402) as risk factors of COPD complicated by IPA.
Conclusions
Duration of hospital stay in the past one year and severity of COPD are risk factors for COPD complicated by IPA.
